1.Clinical features of a case of brucellosis complicated with thyroid abscess

Objective:Clinical characteristics and diagnosis and treatment process was reported and analyzed of a patient with brucellosis complicated with thyroid abscess, providing reference for the clinical diagnosis of brucellosis complicated with thyroid abscess.Methods:Clinical medical records of a patient with brucellosis complicated with thyroid abscess who was treated at the General Surgery Department of Yanchi County People's Hospital in Wuzhong City, Ningxia Hui Autonomous Region in November 2021 were collected. The clinical manifestations, blood routine, brucella antibodies, thyroid function, bacterial culture, thyroid ultrasound and other examination results, as well as the diagnosis and treatment process, were comprehensively analyzed. Results:The patient was a male, 61 years old, who presented with a neck mass without typical clinical manifestations of brucellosis. Thyroid ultrasound revealed a space occupying lesion, and the preliminary diagnosis was thyroid cystadenoma. Thyroid right lobe and isthmus resection surgery was performed. During the operation, it was found that some of the thyroid glands were tightly adhered to the cervical blood vessels, so the resection surgery was changed to abscess drainage, and the drainage fluid was purulent and bloody. The bacterial culture result of thyroid purulent fluid (intraoperative puncture fluid and postoperative drainage fluid) was brucella lamblia, and the serum brucella test tube agglutination test titer was 1 ∶ 400 (+++). The patient improved and was discharged after local drainage and anti brucella treatment. Follow up for 4 months showed no abnormalities. Conclusions:Brucellosis which begins with a local infection of the thyroid gland is extremely rare, with no characteristic clinical manifestations, and is prone to misdiagnosis. Timely correction of the surgical plan during the treatment process avoids the removal of the patient's thyroid, which has a certain clinical reference value.
2.Clinical Application of Ultrasound-guided RISS Plane Block for Postoperative Analgesia After Minimally Invasive McKeown Esophagectomy: A Prospective Randomized Controlled Study

To explore the clinical effect, safety and effectiveness of ultrasound-guided rhomboid intercostal and subserratus plane (RISS) block for postoperative analgesia after minimally invasive McKeown esophagectomy (MIE-McKeown), and provide new ideas for the selection of postoperative analgesia programs for minimally invasive esophageal cancer surgery patients. A prospective randomized controlled study design was used to collect data. Patients undergoing MIE-McKeown in the Department of cardiothoracic surgery of Fuling Hospital of Chongqing University from March 2022 to June 2023 were prospectively collected as research objects. They were divided into three groups by random number table method: Group A: continuous RISS plane block+patient controlled intravenous analgesia (PCIA), Group B: single RISS plane block+PCIA, and Group C: control group, simple PCIA. The outcome indicators of each group were recorded and compared: ①Analgesic effect [visual analogue scale (VAS) pain score for rest and cough at 2, 6, 12, 24, and 48 hours after surgery], ②Postoperative use of analgesics [the amount of sufentanil used within 24 hours after surgery, the number of effective presses of the analgesia pump and the number of additional rescue analgesia], ③Adverse reactions during postoperative analgesia [dizziness, lethargy, postoperative nausea and vomiting(PONV), hypotension, respiratory depression, urinary retention, etc.], ④Intraoperative hemodynamic indicators [mean arterial pressure (MAP) and heart rate (HR) at different time points], ⑤Analgesia satisfaction. ①②③ were the primary outcome indicators, and ④⑤were the secondary outcome indicators. A total of 96 patients who met the inclusion and exclusion criteria were enrolled, with 32 cases in each group. Group A patients had lower resting and cough VAS scores at 2, 6, 12, 24, and 48 hours after surgery compared to Group C. Group A had lower resting VAS scores at 2, 24 hours after surgery and lower cough VAS scores at 12, 24 hours after surgery compared to Group B. Group B patients had significantly lower resting VAS scores at 2, 6, 12 hours after surgery and lower cough VAS scores at 2, 6, 12, 24, and 48 hours after surgery compared to Group C (all Ultrasound guided RISS block can provide good postoperative analgesia for MIE-McKeown surgery. As an active exploration of multimodal analgesia, continuous RISS has better analgesic effects. It is safe, effective, and worthy of further clinical promotion and use.
3.Cloning, expression and characterization of a new endo-β-N-acetylglucosaminidase from Streptomyces alfalfae.

Endo-β-N-acetylglucosaminidase is used widely in the glycobiology studies and industries. In this study, a new endo-β-N-acetylglucosaminidase, designated as Endo SA, was cloned from Streptomyces alfalfae ACCC 40021 and expressed in Escherichia coli BL21 (DE3). The purified recombinant Endo SA exhibited the maximum activity at 35 ºC and pH 6.0, good thermo/pH stability and high specific activity (1.0×10⁶ U/mg). It displayed deglycosylation activity towards different protein substrates. These good properties make EndoSA a potential tool enzyme and industrial biocatalyst.

5.Research progress in NADPH oxidase family in cardiovascular diseases.

Nicotinamide adenine dinucleotide phosphate oxidase (NOX) is a major source of reactive oxygen species (ROS) in the cardiovascular system. The family of NOX includes seven isoforms, and expressed in different cardiovascular cell types and cell compartments, modulating multiple functions, such as cell proliferation, migration, differentiation, apoptosis, senescence, and inflammatory responses. The NOX-derived ROS are involved in many processes associated with cardiovascular diseases, such as hypertension, atherosclerosis, diabetic vascular disease, ventricular remodeling after myocardial infarction, and so on.

7.The impact of sleeve gastrectomy on recurrent obese hyperlipidemic pancreatitis

Objective To study the impact of weight loss surgery after 3 months of stable condition through intervention on patients with obese hyperlipidemic acute pancreatitis (HLAP).Methods Ten patients with obese HLAP who underwent laparoscopic sleeve gastrectomy (LSG) at the General Surgery Department of our hospital were followed-up at 1,3,6,12,24 months after surgery.Their weight,BMI,EWL,and blood parameters (TG,LDL-C,HDL-C,TC,FPG,HbA1c,UA,hs-CRP,ESR,and hemorheology indexes) were compared.The impact on postoperative obese HLAP was assessed.Results The weight,BMI,EWL and blood index (TG,LDL-C,HDL-C,TC,FPG,HbA1c,UA,hs-CRP,ESR,and hemorheology indexes) of the patients gradually decreased.The decrease was significantly different from that before surgery (P<0.05),which became stable at 12 months and with no recurrence at 24 months after surgery.Conclusions LSG reduced body weight and improved metabolic status of the patients.It stopped the occurrence of obese HLAP.LSG can be used as an effective intervention for patients with obese HLAP.
8.Relationship between the distribution of spinal nerve posterior ramus and locations of acupoint in low back.

OBJECTIVETo explore the relationship between the distributions of posterior ramus of spinal nerve (PRSN) and locations of acupoint in low back through anatomical observation.
METHODSThe regional anatomy was performed at five corpses to observe the distribution of erector spinae muscle and PRSN in areas ofpoints and back-points in low back.
RESULTSThe T, L, L, Land LPRSN distributed on both sides of the spine; the medial branches of PRSN travelled between spinalis thoracis muscle and longissimus thoracis muscle, while the lateral branches of PRSN travelled between longissimus thoracis muscle and iliocostalis lumborum muscle.
CONCLUSIONS points and back-points in low back are closely associated with PRSN, particularly T, L, L, Land L.
9.Distribution characteristics of the medial branch of the posterior branch of the spinal nerves around the lumbar facet joints and its clinical significance

Objective To examine the distribution characteristics the medial branch of the posterior branch of the spinal nerves around the lumbar facet joints, so as to explore the localization and targeting of the medial branch of the posterior branch in neurectomy.MethodsFive corpses were dissected, and the anatomical relationships between the medial branches of the posterior branch of the spinal nerves and the facet joints were examined.Measured the distances from the lateral port of the bone fiber tube,the articular process of the facet joint and the superior articular process to the spinous processes.Results The medial branch of the posterior branch of the spinal nerve was close to the root of the articular process and the transverse process,which was not easy to be explored and dissected.Meanwhile, it was easily traced after the medial branch of the posterior branch of the spinal nerve ran through the fibrous tube.Conclusion It may improve the safety and effective when the techniques of medial branch block is performed after the medial branch ran through the fibrous tube.
10.The clinical value of C-reactive protein and procalcitonin in early prediction of pancreatic fistula after pancreaticoduodenectomy

Objective To study the relationship of early postoperative CRP and PCT with pancreatic fistula after pancreaticoduodenectomy (PD);to study whether the combination of CRP and PCT can be used as an early predictor of pancreatic fistula;and to determine the optimal cut-off values of CRP and PCT for early diagnosis of pancreatic fistula after PD.Methods Postoperative pancreatic fistula and other complications were recorded in 82 patients after PD carried out at the Anshan Iron and Steel Group General Hospital from January 2013 to May 2017.PCT and CRP were measured on the 1st to the 5th postoperative day (POD).CRP and PCT were compared between the pancreatic fistula group and the non-complication group on pancreatic fistula prediction.The sensitivity,specificity,and combined sensitivity,combined specificity,negative predictive value,positive predictive value and optimal cut-off value were calculated.Results 56 patients developed postoperative complications,including 17 patients with pancreatic fistula,and 39 patients with non-pancreatic fistula.The postoperative CRP and PCT were correlated with postoperative pancreatic fistula and with other complications.The higher the CRP and PCT,the higher the risk of postoperative pancreatic fistula.The ROC curves suggested that CRP and PCT had a high predictive value for pancreatic fistula,especially on POD 2.The optimal cut-off value of CRP was 189.05 mg/L,the sensitivity was 94.1%,the specificity was 81.5%,the positive predictive value was 94.7%,and the negative predictive value was 24.4%.The optimal cut-off value of PCT was 0.89 mg/dl,the sensitivity was 88.2%,the specificity was 84.6%,the positive predictive value was 91.3%,and the negative predictive value was 20.5%.The sensitivity and specificity of combined CRP with PCT on POD 2 in predicting pancreatic fistula after operations were 100% and 69.1%,respectively.Conclusion Combined CRP and PCT had a high early predictive rate in the identification of pancreatic fistula after PD.
