1.Construction of Laboratory Animal Science and Technology Ethics Governance System in China and Its Preliminary Practice in Guangdong Province

The welfare and ethics of laboratory animals are the ethical principles and behavioral norms that need to be followed in conducting animal-based scientific research, breeding and managing laboratory animals, and supervising and regulating such activities. The level of protection of laboratory animal welfare and ethics is closely related to the development of science and technology, which has become a widely recognized international consensus. At present, Guangdong Province is accelerating the construction of a high-level science and technology innovation province and the Guangdong-Hong Kong-Macao Greater Bay Area International Science and Technology Innovation Center. Guangdong Province should rely on its advanced governance capacity in the field of laboratory animal science and technology ethics to promote the high-quality development of its laboratory animal science and technology sector. Based on the management laws, regulations, and institutional mechanisms of laboratory animals in China, this paper explores the optimization of the laboratory animal science and technology ethics governance system, which includes the institutional guarantees, responsibility systems, ethical review and supervision mechanisms, and education and outreach. Through methods such as literature research, questionnaire surveys, and interview investigations, an empirical study of the laboratory animal science and technology ethics governance system in Guangdong Province has been conducted. Analysis of literature and research results shows that Guangdong Province has basically established a laboratory animal management system, collaboration mechanism, supervision mechanism, and education and training system that meet the current requirements of the laboratory animal science and technology ethics governance system in China. However, there are still problems such as an incomplete laboratory animal science and technology ethics supervision mechanism, an underdeveloped operation mechanism of review institutions, insufficient attention paid by laboratory animal units to the ethical review of animal experiments, inconsistent ethical review standards, and a lack of professional ethical education and training for ethics review personnel. Therefore, optimization measures such as improving the laboratory animal science and technology ethics review system, strengthening supervision and inspection, further strengthening the accountability of responsible entities, formulating review norms, and enhancing hierarchical and classified education and training are proposed, to provide a theoretical basis for promoting the normalized and long-term governance of laboratory animal science and technology ethics in Guangdong Province.

Postoperative infection of internal fixation of closed fractures the lower limbs in adults represents a devastating complication, characterized by diagnostic challenges, prolonged treatment duration and high disability rates. Current management of these infections faces multiple challenges, such as difficulties in early accurate diagnosis, and various controversies about the treatment plan, leading to poor overall diagnosis and treatment results. To address these issues, based on evidence-based medicine and principles with emphasis on scientific rigor, clinical applicability and innovation, the Trauma Branch of the Chinese Medical Association, Orthopedic Branch of the Chinese Medical Doctor Association, Orthopedics Branch of the Chinese Medical Association, and Trauma Orthopedics and Polytrauma Group of the Resuscitation and Emergency Committee of the Chinese Medical Doctor Association have collaboratively organized a panel of relevant experts to develop the Guideline for diagnosis and treatment of infection after internal fixation of closed lower limb fractures in adults ( version 2025). The guideline proposed 10 recommendations, aiming to provide a foundation for standardized diagnosis and treatment of postoperative infection in adults with closed lower limb fractures.

For middle-aged and elderly patients with conditions such as spinal fractures and degenerative spinal diseases, spinal internal fixation is a core surgical procedure for reconstructing spinal stability, heavily relying on the biomechanical stability provided by pedicle screw systems. Whereas, these patients are often complicated by osteoporosis that can significantly compromise the stability of the bone-pedicle screw interface, leading to a marked increase in pedicle screw loosening and surgical failure rates. The bone cement-augmented pedicle screw technique, which involves injecting bone cement into the vertebral body or screw trajectory to optimize the mechanical properties of the bone-pedicle screw composite, has been proven to significantly enhance fixation strength and effectively prevent screw-related failures, thereby reducing the incidence of internal fixation failure in high-risk populations undergoing spinal fusion. However, the widespread clinical application of this technique has faced challenges such as inaccurate clinical decision-making (indication and contraindication selection), non-standardized operative practices, and insufficient awareness of complication prevention, resulting in considerable variability in clinical outcomes and even severe complications. To address this, Prof. Luo Fei from First Affiliated Hospital of Army Medical University initiated the project and the Chinese Association Orthopaedic Surgeons organized relevant experts to develop the Evidence-based clinical practice guideline for bone cement-augmented pedicle screw technique ( version 2025), based on current evidence. The guidelines put forward 8 recommendations regarding the clinical value, scope of application, and operational standards of the technique, aiming to provide evidence-based medical support and technical standardization for clinical decision-making.
4.Application status of the model organism zebrafish in radiation medicine research

As a model organism that bridges the gap between cells and traditional mammals,the zebrafish has broad application prospects in radiation medicine research.Among its unique advantages are its characteristics of a high homology with human genes,high fertility,short embryonic period,and transparent and easy to observe embryos,making it an important tool in radiation medicine research.Recently,remarkable progress has been made in the application of zebrafish to investigate low-dose radiation biological effects,radiation therapy,and radiation damage prevention and treatment,key areas of radiation medicine.In this paper,these applications are reviewed;we explore the value of zebrafish in radiation medicine research and provide a reference for experimental research in related fields.
5.Adenovirus mediated Profurin expression improved the plaque stability of ApoE-/-mice

Aim To investigate the effect of adenovirus(ADV)-mediated Profurin(PF)expression on the plaque stability of ApoE-/-mice.Methods ApoE-/-mice were fed with high-fat diet for 8 weeks,and then treated with ADV-mediated PF intervention,followed by high-fat diet for 4 weeks.Aortic roots were isolated for atherosclerotic plaque area analysis and immunohistochemical analysis.Plasma phospholipid transfer protein(PLTP)activity was detec-ted by fluorescence donor essay,plasma total cholesterol(TC)and triglyceride(TG)were measured by enzyme assay kits,and fast protein liquid chromatography was used for lipoprotein profile analysis.Results Compared with the con-trol group,the plasma TC and TG levels,PLTP activity and circulating tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)levels in ADV-PF group were significantly decreased(P<0.01).In the ADV-PF group,there was no significant change in atherosclerotic lesions on the inner surface of the full-length aorta,but the plaque area and lipid area in the aortic root were reduced(P<0.01),the content of macrophages was significantly decreased(P<0.01),and the smooth muscle cells and collagen area were not significantly different.The content of matrix metalloproteinase-9 in plaque was signifi-cantly decreased(P<0.05).Conclusion Overexpression of PF can alleviate atherosclerosis and reduce the levels of circulating inflammatory factors to a certain extent,and effectively improve the plaque stability of ApoE-/-mice.
6.Significance of Serum Sema4D,sST2 and Omentin-1 Detection in Patients with Hypertrophic Cardiomyopathy with Atrial Fibrillation

Objective To explore the significance of detecting serum semaphorin 4D(Sema4D),soluble growth-stimulation expression gene 2 protein(sST2),and Omentin-1 in patients with hypertrophic cardiomyopathy(HCM)accompanied by atrial fibrillation(AF),and to provide reference for clinical diagnosis and treatment.Methods A total of 90 patients with HCM and atrial fibrillation admitted to Tsinghua University Yuquan Hospital(Tsinghua University Integrated Traditional Chinese and Western Medicine Hospital)from January 2022 to December 2023 were selected as the HCM-AF group,and another 90 patients with sinus rhythm HCM in the same stage were selected as the sinus rhythm HCM group.The serum levels of Sema4D,sST2,Omentin-1 and cardiac function indicators[left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD)]were compared between the two groups.The correlation between serum Sema4D,sST2,Omentin-1 and cardiac function indicators,as well as the severity of atrial fibrillation in HCM was analyzed.The predictive value of three indicators for atrial fibrillation in HCM was analyzedand the predictive efficacy of percutaneous radiofrequency ablation(PRFA)for the recurrence of atrial fibrillation in HCM patients.Results The serum levels of Sema4D and sST2 in the HCM-AF group were higher than those in the sinus rhythm HCM group,while the levels of Omentin-1 and LVEF were lower than those in the sinus rhythm HCM group,with statistically significant differences(t=2.517~12.325,all P<0.05).The serum levels of Sema4D and sST2 in the persistent subgroup were higher than those in the paroxysmal subgroup,while the levels of Omentin-1 and LVEF were lower than those in the paroxysmal subgroup,the differences were statistically significant(t=2.635~15.931,all P<0.05).Spearman/Pearson correlation analysis showed that serum Sema4D and sST2 were negatively correlated with LVEF(r=-0.57,-0.609,all P<0.001),while Omentin-1 was positively correlated with LVEF(r=0.593,P<0.001).Serum Sema4D and sST2 were positively correlated with the severity of AF in HCM(r=0.625,0.631,all P<0.001),while Omentin-1 was negatively correlated with the severity of AF in HCM(r=-0.617,P<0.001).The serum levels of Sema4D and sST2 in the recurrence subgroup were higher than those in the non-recurrence subgroup,while the serum level of Omentin-1 was lower(t=12.805,7.049,13.345,all P<0.05).The receiver operator characteristic(ROC)curve results showed that the AUC(95%CI)of serum Sema4D,sST2 and Omentin-1 combined to predict atrial fibrillation in HCM was 0.916(0.865~0.952),higher than the individual predictions of the three(Z=3.621,3.657,0.318,all P<0.05).The AUC(95%CI)of serum Sema4D,sST2 and Omentin-1 combined to predict the recurrence of atrial fibrillation in patients with HCM after radiofrequency ablation was 0.931(0.857~0.973),higher than the individual predictions of the three(Z=4.115,3.657,3.853,all P<0.05).Conclusion The levels of serum Sema4D and sST2 in the serum of patients with HCM complicated with atrial fibrillation were elevated,while the level of Omentin-1 was drcreased.Detecting serum Sema4D,sST2 and Omentin-1 has high performance in predicting the occurrence of atrial fibrillation in patients with HCM and the recurrence of atrial fibrillation after radiofrequency ablation therapy.
7.Developing an admission assessment index system for hospice care in emergency patients

Objective:To develop an admission assessment index system for hospice care tailored to emergency patients.Methods:Using a mixed-methods approach, a preliminary framework was established through literature review and qualitative interviews. From May to September 2024, a two-round Delphi consultation was conducted with 26 experts to screen and refine indicators. The final system was determined using a priority chart method for weight assignment.Results:Twenty-five experts in fields of emergency medicine, emergency nursing, palliative care nursing, palliative care medicine and general practice completed both consultation rounds. The experts ranged in age from 36 to 56 years and had 12 to 36 years of work experience. All held intermediate or higher professional titles. The effective recovery rates for the two rounds of consultation questionnaires were 96.15%(25/26) and 100.00%(25/25), respectively. Expert authority coefficients were 0.891 and 0.911, with Kendall′s coordination coefficients of 0.348 and 0.399 ( P<0.001), and variation coefficients ranging from 0.040 to 0.204 and 0.000 to 0.179. The finalized system comprises 5 primary indicators (weighted coefficients: life-limiting disease status 0.268, symptom control 0.236, patient/familywishes 0.201, social support needs 0.162, medical resource utilization 0.133) and 27 secondary indicators. Conclusion:The index system developed in this study provides a comprehensive and practical tool for identification of emergency patients requiring hospice care, facilitating timely clinical decision-making.
8.Comparison of the efficacy of autologous bone grafting or bioceramic bone grafting combined with locked compression plate internal fixation in the treatment of tibial plateau fractures

Objective:To compare the clinical efficacy of autologous bone grafting or bioceramic bone grafting combined with locking compression plate (LCP) internal fixation in the treatment of Schatzker type II and III tibial plateau fractures.Methods:A retrospective analysis was conducted on 104 patients with Schatzker type II and III tibial plateau fractures who underwent surgical treatment at the Department of Orthopedics, PLA Central Theater Command General Hospital from January 2010 to December 2021. The cohort comprised 55 males and 49 females, with an average age of 49.13±13.80 years (range 18-73 years). All fractures were unilateral: 55 on the left and 49 on the right. According to the Schatzker classification, 59 were Type II and 45 were Type III. Causes of injury included traffic accidents (48 cases), falls from height (3 cases), sprains or falls (45 cases), and other causes (8 cases). During surgery, bioceramic material or allograft bone tissue was implanted into the collapsed tibial plateau region to restore articular surface flatness. Based on graft type, patients were divided into the allograft bone group (63 cases) and the bioceramic group (41 cases). All cases underwent proximal lateral tibial LCP internal fixation. The two groups were compared in terms of operative time, intraoperative blood loss, bone graft volume, length of hospital stay, fracture healing time, and postoperative complications. The Rasmussen radiographic collapse score was used to evaluate fracture reduction, and the Hospital for Special Surgery (HSS) score system was used to assess knee joint function.Results:All patients were followed up for 12(12, 13) months (range 12-16 months). The fracture healing time in the allograft bone group was 13.70±1.36 weeks (range 11-16 weeks), which was significantly shorter than that in the bioceramic group: 14.59±1.73 weeks (range 11-19 weeks) ( t=2.911, P=0.004). The time to full weight-bearing in the allograft group was 15.0(14.0, 17.0) weeks (range 13-23 weeks), which was shorter than the 16.0 (15.5, 18.5) weeks (range 12-24 weeks) in the bioceramic group, showing a statistically significant difference ( Z=-3.019, P=0.002). At 12 months postoperatively, the Rasmussen radiographic collapse score was 1(0, 1) in the bioceramic group, significantly lower than the 1(1, 2) score in the allograft group ( Z=-2.083, P=0.037). No statistically significant differences were observed between the two groups in bone graft volume, surgical duration, intraoperative blood loss, hospitalization duration, or HSS scores at 6 and 12 months postoperatively ( P>0.05). The complication rate was 6.3% (4/63) in the allograft group and 2.4% (1/41) in the bioceramic group, with no statistically significant difference (χ 2=0.830, P=0.362). Conclusions:For Schatzker type II and III tibial plateau fractures, the use of allograft bone or bioceramic combined with LCP internal fixation can achieve good results. Allograft bone has advantages in terms of fracture healing speed and early weight-bearing recovery, while bioceramic is more effective in maintaining joint surface stability.

10.Clinical observation on the treatment of pubic symphysis separation syndrome with the combination of returning and squeezing, patting and pulling manoeuvre and pelvic fabric band fixation

Objective:To evaluate the clinical efficacy of returning, squeezing, patting and pulling orthopaedic manipulation combined with pelvic fabric tape fixation for the treatment of postpartum pubic symphysis separation.Methods:The clinical data of 80 patients with postpartum pubic symphysis separation from June 2015 to March 2019 were retrospectively analyzed, and all of them were given orthopaedic manipulative therapy using return squeezing and patting and pulling, once a week, for a total of 3 times. After the manipulative treatment, the patients were instructed to brake the pelvic fixation straps for not less than 8 h per day, and digital X-ray (DR) pelvic radiographs or ultrasound tests were performed before and after the treatment to measure the distance between the pubic symphysis. VAS scale was used to assess the degree of pain, and the Oswestry dysfunction index (ODI) was used to assess the degree of dysfunction. The clinical efficacy was evaluated.Results:After treatment of 80 patients, 6 showed significant improvement, 69 showed improvement, and 5 showed no improvement, with a total effective rate of 93.8%. Compared with before group, the inter-pubic symphysis distance [(15.09±3.10) mm, (12.01±4.36) mm, (9.64±0.30) mm, (8.18±1.56) mm vs. (19.35±1.08) mm, F=254.64] were significantly smaller at 1 week, 2 weeks, 3 weeks, and 1 month ( P<0.001); VAS scores (2.90±1.24, 1.29±0.88, 0.84±0.43, 0.56±0.32 vs. 6.11±2.93, F=122.60) were significantly lower than before treatment ( P<0.001); ODI (28.09±4.30, 22.01±4.95, 20.64±0.41, 14.18±1.36 vs. 45.43±4.01, F=734.17) were significantly reduced ( P<0.001). Conclusion:Returning, squeezing, patting and pulling orthopaedic manipulation combined with pelvic fabric tape fixation can quickly restore the separation distance of the pubic symphysis, reduce local pain and improve lumbosacral function.
