1.Voice analysis-based machine learning models to diagnose Alzheimer's disease

Objective To identify key acoustic features associated with the progression of Alzheimer's disease(AD)through voice analysis combined with machine learning and feature selection techniques,thereby constructing classification models that serve as candidate tools for the early screening of AD.Methods Voice samples from AD,mild cognitive impairment(MCI)and healthy(HC)elderly individuals were obtained from the NCMMSC2021 AD voice dataset.The voice samples underwent data preprocessing,followed by feature extraction from the eGeMAPS feature set via the OpenSmile toolkit.Classification models were obtained utilizing random forest and support vector machine(SVM)algorithms.Significance testing and feature importance ranking were conducted using Python,and the further selection of the optimal features was performed through sequential forward selection(SFS).The classification performance before and after feature selection was compared and evaluated using accuracy and the area under the receiver operating characteristic curve(AUC).Results The significant acoustic features in the classification models primarily derived from spectral slope,formant,fundamental frequency,and loudness.The optimal classification performance was achieved with the SVM model following SFS feature selection,with recognition accuracies of 0.926(AUC=0.974)for AD/MCI group,0.875(AUC=0.956)for AD/HC group,and 0.879(AUC=0.904)for MCI/HC group.Conclusion SVM model performs better than random forest model,and the use of SFS for feature selection can effectively enhance model performance.Voice analysis has the potential to serve as a valuable supplementary tool for the rapid AD assessment and screening.
2.Adenovirus mediated Profurin expression improved the plaque stability of ApoE-/-mice

Aim To investigate the effect of adenovirus(ADV)-mediated Profurin(PF)expression on the plaque stability of ApoE-/-mice.Methods ApoE-/-mice were fed with high-fat diet for 8 weeks,and then treated with ADV-mediated PF intervention,followed by high-fat diet for 4 weeks.Aortic roots were isolated for atherosclerotic plaque area analysis and immunohistochemical analysis.Plasma phospholipid transfer protein(PLTP)activity was detec-ted by fluorescence donor essay,plasma total cholesterol(TC)and triglyceride(TG)were measured by enzyme assay kits,and fast protein liquid chromatography was used for lipoprotein profile analysis.Results Compared with the con-trol group,the plasma TC and TG levels,PLTP activity and circulating tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)levels in ADV-PF group were significantly decreased(P<0.01).In the ADV-PF group,there was no significant change in atherosclerotic lesions on the inner surface of the full-length aorta,but the plaque area and lipid area in the aortic root were reduced(P<0.01),the content of macrophages was significantly decreased(P<0.01),and the smooth muscle cells and collagen area were not significantly different.The content of matrix metalloproteinase-9 in plaque was signifi-cantly decreased(P<0.05).Conclusion Overexpression of PF can alleviate atherosclerosis and reduce the levels of circulating inflammatory factors to a certain extent,and effectively improve the plaque stability of ApoE-/-mice.
3.Research Progress on the Application of Radiomics in Tumor Microenvironment of Breast Cancer

Tumor microenvironment is the result of dynamic interactions between different cellular and non-cellular components,including cancer cells themselves,extracellular matrix,immune cells and various non immune cells.Microenvironment is closely related to the growth,metastasis and prognosis of breast cancer,and is closely related to the current popular targeted therapy and immunotherapy.Radiomics is a new field of translational medicine based on extracting high-dimensional data from images to obtain reliable models that can be applied to clinical practice.In this review,the latest advances in the application of radiomics to the tumor microenvironment of breast cancer are discussed.
4.Effects of L-T4 treatment in early and mid- to late pregnancy on coagulation function and adverse pregnancy outcomes in patients with gestational hypothyroidism

Objective:To explore the effects of Levothyroxine Sodium (L-T4) treatment on coagulation function and adverse pregnancy outcomes in patients with hypothyroidism during pregnancy in early and mid-to-late pregnancy.Methods:A total of 120 patients with hypothyroidism during pregnancy who were treated by L-T4 in Linyi Central Hospital from Mar. 2021 to Mar. 2024 were included, and their clinical data were retrospectively analyzed. According to the gestational age of the patients at the time of treatment, 64 cases with a gestational age of <12 weeks were included in the early pregnancy group, and 56 cases with a gestational age of ≥12 weeks were included in the mid-to-late pregnancy group. The serum thyroid hormone indexes, coagulation function indexes, adverse pregnancy outcomes, and neuropsychological development of the offspring were compared between the two groups. Statistical analyses were conducted using independent/paired t-tests and chi-square tests.Results:After treatment, there were no significant differences in the thyroid hormone indexes and coagulation function indexes between the early pregnancy group and the middle and late pregnancy group ( P>0.05). The incidence of fetal growth restriction and gestational hypertension in the early pregnancy group was 10.94% (7/64) and 17.19% (11/64), and that in the middle and late pregnancy group was 26.79% (15/56) and 33.93% (19/56), respectively. The incidence in the early pregnancy group was significantly lower than that in the middle and late pregnancy group ( χ2=5.010, 4.464, P<0.05). The developmental quotient (DQ) of the offspring in the early pregnancy group at 6 months of age in various areas of neuropsychological development (social behavior, language, adaptability, fine motor skills and gross motor skills) was significantly higher than that in the middle and late pregnancy group. The DQ of the first trimester group was (97.82±7.25) points, (94.59±6.85) points, (95.87±8.02) points, (96.08±7.25) points, and (98.34±8.07) points. The DQ of the second trimester group was (92.54±7.06) points, (90.45±6.14) points, (91.01±7.51) points, (91.24±7.08) points, and (92.74±7.38) points. The DQ of the first trimester group was significantly higher than that of the second trimester group ( t=4.029, 3.466, 3.411, 3.688, 3.946, P<0.05) . Conclusion:L-T4 treatment in early or mid- to late-pregnancy patients with gestational hypothyroidism can effectively improve their thyroid hormone levels and coagulation function, but treatment in early pregnancy is beneficial to reduce the incidence of adverse pregnancy outcomes such as fetal growth restriction and alleviate the impact of the disease on the neuropsychological development of offspring.

7.Clinical and endoscopic characteristics of 15 cases of rectal benign lymphoid polyps

To investigate the clinical symptoms and endoscopic features of rectal benign lymphoid polyps, a retrospective analysis was conducted on 15 consecutive cases (16 lesions) who underwent endoscopic resection at the First Affiliated Hospital of Shandong First Medical University between April 2015 and June 2023, with postoperative pathological confirmation of rectal benign lymphoid polyps. The results showed that among the 15 patients, 6 were male and 9 were female, with a mean age of 52.6 years. Colonoscopy was performed in 8 patients due to clinical symptoms, among whom 7 presented with abdominal pain. All 16 lesions were submucosal, including 14 solitary and 1 multiple. The lesions were located in the lower rectum ( n=10) and mid-rectum ( n=6), with a mean long diameter of 0.5 cm. Fourteen lesions exhibited regular shape as flat circles or hemispheres. The surface mucosa appeared normal or gray-white in 11 lesions, and varying degrees of capillary dilation were observed in 8 lesions, among which 2 exhibited prominent capillaries resembling a "wheel-shaped" pattern. Endoscopic ultrasonography (EUS) was performed in 10 lesions prior to endoscopic resection: 4 originated from the third layer, 3 from the second layer, and 3 from both the second and third layers. Five lesions appeared as homogeneous hypoechoic masses, 3 were hyperechoic, and 2 showed mixed echogenicity. Therefore, the clinical symptoms of rectal benign lymphoid polyps are non-specific. Endoscopically, the presence of "wheel-shaped" pattern of dilated capillaries may serve as a diagnostic clue. When EUS demonstrates hyperechoic or mixed echogenicity, it could aid in the differential diagnosis from rectal neuroendocrine neoplasms.
