Objective To develop and validate a nomogram model for predicting recurrence after radiofre-quency catheter ablation(RFCA)in patients with atrial fibrillation and heart failure using body surface electrocar-diogram indicators and clinical indicators. Methods We retrospectively analyzed 305 patients with atrial fibrilla-tion complicated with heart failure who underwent RFCA from January 2019 to January 2024.Patients were random-ized into training set(213 cases)and validation set(92 cases)at a ratio of 7:3 and followed up for at least 1 year.Based on the recurrence status, the patients were divided into recurrence group and non-recurrence group, with body surface electrocardiogram indicators and clinical indicators collected.Multivariate logistic regression analysis identified for risk factors for post RFCA recurrence, which were used to construct a nomogram.Model performance was assessed using the area under the receiver operating characteristic curve(AUC),Hosmer-Lemeshow test, calibration curves, and decision curve analysis(DCA).Results Among the 305 patients,84(27.54%)experi-enced recurrence after treatment. In the training set,61 patients had recurrence and 152 did not.No statistical differences were observed between the training set and the validation set(all P>0.05).In the training set,the recurrence group exhibited a higher proportion of persistent atrial fibrillation and significantly higher CHA2DS2-VASc scores, larger left atrial diameter, longer PR interval, and higher levels of NLR and NT-proBNP compared to the non-recurrence group(all P<0.05).Multivariate stepwise regression analysis revealed that high CHA2DS2-VASc score, long left atrial diameter, prolonged PR interval, and high NLR were independent risk factors of recurrence after RFCA(P<0.05)A four-factor prediction model was established as:Ln(P/1-P)=-12.87+0.84*CHA2DS2-VASc score+0.11* left atrial diameter+0.03*PR interval+0.31*NLR.The training and validation models showed AUCs of 0.85(95%CI:0.80~0.91)and 0.85(95%CI:0.76~0.94),respectively,suggesting that the model had good predictive efficiency.Hosmer-Lemeshow test results(χ2=2.43,P=0.965 for the training set;χ2=5.30,P=0.725 for the validation set)confirmed model fit, indicating that the fitted probability value was consistent with the actual probability value.Calibration curves after 1 000 times of Bootstrap repeated sampling showed the bias calibration curves of the training set and the validation set had good consistency with the actual curves, both close to the ideal curve.DCA revealed clinical utility across a wide threshold probability range(0.02~1.0 for the training set;0.04~1.0 for the validation set). Conclusion This nomogram, based on body surface electrocardiogram indicators and clinical indicators, effectively predicts post-RFCA recurrence in atrial fibrillation and heart failure patients, offering a useful tool for early assessment of recurrence risk.

Objective:To investigate whether a distant-image screen display has an inhibitory effect on children's nearwork-induced transient myopia (NITM).Methods:A prospective self-controlled study was conducted. From March 2022 to March 2023, 120 pediatric volunteers, aged 4 to 12 years, with a mean age of (5.0±2.2) years, were recruited at Tianjin Eye Hospital.Subjects with refractive errors underwent tests wearing corrective lenses.Subjects were categorized based on accommodative response into three groups: accommodative lead (16 subjects, 32 eyes), accommodative equivalent (48 subjects, 96 eyes), and accommodative lag (56 subjects, 112 eyes). Subjects were also divided by myopia status into myopic (20 subjects, 40 eyes) and non-myopic (100 subjects, 200 eyes) groups.Using a random number table, the subjects were assigned to first view video images for 30 minutes on either an iPad or a distant light screen display. After a two-hour rest between the two sessions, the viewing modality was switched.Visual acuity, refractive power, lens thickness, NITM degree, and NITM recovery time were recorded before and after viewing videos via both modalities.Visual acuity, lens thickness, NITM degree, and recovery time following video viewing via the two modalities across different accommodation types and refractive types were compared. This study adhered to the Declaration of Helsinki. The study protocol was approved by the Ethics Committee of Tianjin Eye Hospital (No.2022099), and all guardians of the subjects provided informed consent for this study. Results:The NITM degree and recovery time after watching videos on a distant-image screen display were (0.00±0.36)D and (25.33±15.48) seconds, respectively. The NITM degree and recovery time after watching videos on an iPad were (-0.20±0.40)D and (33.33±17.68) seconds, respectively.Compared with watching videos on an iPad, watching videos on a distant-image screen display resulted in better immediate distance vision, lower myopic refractive power, thinner lens thickness, lower NITM degree, and shorter recovery time, with statistically significant differences ( t=-7.688, 7.842, -4.210, 2.331, -2.887; all P<0.05). After viewing the distant-image screen display and iPad, there was no significant difference in the overall comparison of visual acuity change, lens thickness change, NITM degree and NITM recovery time among the accommodative lead, accommodative equivalent and accommodative lag groups ( H=0.584, 4.923, 1.514, 2.634; all P>0.05. H=3.265, 1.884, 1.606, 1.922; all P>0.05), and there was also no significant difference between the myopic and non-myopic groups ( Z=-1.555, -1.700, -0.254, -2.336; all P>0.05. Z=-1.125, -0.446, -1.033, -0.759; all P>0.05). Conclusions:The use of a distant-image screen display can reduce the NITM caused by near viewing and shorten the NITM recovery time, which is effective for children with different refractive states and accommodative types.

The"capitation payment with retained surplus and shared accountability for reasonable overruns"mechanism constitutes a pivotal institutional framework for advancing the high-quality development of County Medical Communities(CMCs).This study addresses two critical operational challenges:identifying the sources of medical insurance fund surplus and optimizing the governance of fund retention processes.Grounded in transaction cost theory,we develop an analytical framework examining the formation of medical insurance fund surplus through the dual lenses of intra-organizational dynamics within CMCs and external medical insurance payment mechanism design.Utilizing Deqing County,Zhejiang Province as an empirical case, this research proposes a five-pronged strategy:Clarifying generation channels of insurance fund surplus, scientifically determining regional medical insurance budgets, implementing bundled payment mechanisms for CMCs,adopting hybrid payment models integrating unified and differentiated approaches, and establishing performance-based incentive systems. These findings elucidate the formative pathways of medical insurance fund surplus while offering theoretical and practical insights for enhancing payment system reforms to support CMC development.

Aim To explore the key components and mechanisms of Lizhong decoction in treating rats with cold-damp diarrhea based on network pharmacology, molecular docking technology and animal experiments. Methods By literature review and database collec-tion,the components of Lizhong decoction, therapeutic targets, and the mapping with diarrhea disease targets were conducted to construct an intersection target pro-tein-protein interaction network for screening core tar-gets,and GO and KEGG pathway enrichment analysis was performed to build an"active component-target-pathway"network,followed by molecular docking vali-dation.Forty-eight rats were randomly divided into the normal control group(K),model group(DG),Lizhong decoction group(LZDG),and Pulsatilla decoction group(BTDG).Subsequently,a rat cold-damp diar-rhea model was established using Senna combined with low-temperature high-humidity environment, and the rats were intervened with Lizhong decoction and Pul-satilla decoction.HE staining was used to detect path-ological changes in intestinal tissue,ELISA was em-ployed to measure the levels of peripheral blood IL-6,IL-10,IL-1 β,and TNF-α,and western blot was used to determine the expression of colon tight junction pro-teins.Results Network pharmacology initially identi-fied 125 compounds in Lizhong decoction,5 186 drug target components,438 disease targets, and 60"drug-disease"shared targets.GO and KEGG enrichment a-nalysis showed that signaling pathways such as IL-17 and TNF were highly enriched.Molecular docking in-dicated that the core components of the drug had good binding activity with corresponding key targets.Liz-hong decoction could effectively improve the clinical symptoms of rats with cold-damp diarrhea, and com-pared with the DG group, the diarrhea rate, diarrhea in-dex,and other related indicators also gradually de-creased to normal levels.Compared with the DG group, the LZDG group showed reduced inflammation levels and a recovery in energy metabolism levels. Conclusion It can regulate targets such as MMP9 and IL-17 signaling pathways through multi-components like Calycosin and formononetin to exert its therapeutic effect on cold-damp diarrhea.

Diterpenoid ferruginol is a key intermediate in biosynthesis of active ingredients such as tanshinone and carnosic acid. However, the traditional process of obtaining ferruginol from plants is often cumbersome and inefficient. In recent years, the increasingly developing gene editing technology has been gradually applied to the heterologous production of natural products, but the production of ferruginol in microbe is still very low, which has become an obstacle to the efficient biosynthesis of downstream chemicals, such as tanshinone. In this study, miltiradiene was produced by integrating the shortened diterpene synthase fusion protein, and the key genes in the MVA pathway were overexpressed to improve the yield of miltiradiene. Under the shake flask fermentation condition, the yield of miltiradiene reached about(113. 12±17. 4)mg·L~(-1). Subsequently, this study integrated the ferruginol synthase Sm CYP76AH1 and Sm CPR1 to reconstruct the ferruginol pathway and thereby realized the heterologous synthesis of ferruginol in Saccharomyces cerevisiae. The study selected the best ferruginol synthase(Il CYP76AH46) from different plants and optimized the expression of pathway genes through redox partner engineering to increase the yield of ferruginol. By increasing the copy number of diterpene synthase, CYP450, and CPR, the yield of ferruginol reached(370. 39± 21. 65) mg·L~(-1) in the shake flask, which was increased by 21. 57-fold compared with that when the initial ferruginol strain JMLT05 was used. Finally, 1 083. 51 mg·L~(-1) ferruginol was obtained by fed-batch fermentation, which is the highest yield of ferruginol from biosynthesis so far. This study provides not only research ideas for other metabolic engineering but also a platform for the construction of cell factories for downstream products.
