1.Effect of Acupuncture at Zusanli Points Combined with Sanhuang Decoction Fumigation on Postoperative Hemorrhoid Wound Healing and Repair

Objective To observe the effect of acupuncture at Zusanli(ST36)points combined with Sanhuang Decoction fumigation for postoperative hemorrhoid wound healing and repair.Methods A total of 110 postoperative hemorrhoidal patients were randomly divided into observation group and control group,55 cases in each group.The control group was treated by Potassium Permanganate Solution for sitz bath,and the observation group was treated by acupuncture at Zusanli points combined with Sanhuang Decoction fumigation on the basis of the control group's treatment.The course of treatment for the two groups covered 14 days.After two weeks of treatment,the clinical efficacy of the two groups were evaluated,and changes in the anal canal resting pressure,anal canal systolic blood pressure,anal maximal squeeze pressure,as well as the wound granulation tissue,wound area,wound effusion,and wound edema,and wound pain scores of the patients were observed before and after the treatment.The changes of levels of tumor necrosis factor α(TNF-α)and interleukin 6(IL-6)in the two groups before and after treatment were compared.Moreover,the total complication rate and wound healing time of the two groups were compared.Results(1)The total effective rate was 96.36%(53/55)in the observation group and 74.55%(41/55)in the control group.The efficacy of the observation group was superior to that of the control group,the difference being statistically significant(P<0.05).(2)After treatment,the anal canal resting pressure,anal canal systolic blood pressure,anal maximal squeeze pressure of patients in the two groups were significantly improved(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.05).(3)After treatment,the wound granulation tissue,wound area,wound effusion,and wound edema,and wound pain scores of patients in the two groups were significantly improved(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.05).(4)After treatment,levels of serum TNF-α and IL-6 were significantly improved in both groups(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,the difference being statistically significant(P<0.05).(5)The total complication rate of the observation group was 7.27%(4/55),and that of the control group was 27.27%(15/55),and the total complication rate of the observation group was significantly lower than that of the control group,the difference being statistically significant(P<0.05).(6)The wound healing time of the observation group was significantly shorter than that of the control group,the difference being statistically significant(P<0.05).Conclusion Acupuncture at Zusanli points combined with Sanhuang Decoction fumigation for postoperative hemorrhoidal patients can effectively relieve their symptoms such as perianal pain and edema,improve the anal function,reduce inflammatory responses and the incidence of complications,promote wound repair.
2.Preventive effect of Vitamin B supplementation on recurrent stroke: a Meta-analysis

Objective To evaluate whether Vitamin B supplementation could prevent ischemic stroke recurrence.Methods Randomized controlled trials (RCTs) observing Vitamin B supplementation in patients with stroke was performed in databases including ScienceDirect, PubMed/Medline, China National Knowledge Infrastructure, Chinese Biomedical Data-Base, Wanfang Database, and VIP Chinese Science and Technology Journal Database to find related studies in English or Chinese published before August 2016. The patients in control group received a placebo or basic therapy without Vitamin B, and those in experimental group was treated with Vitamin B alone or Vitamin B on the basis of conventional treatment. The data were collected by two researchers independently and the quality of studies was assessed by the modified Jadad Scale. The Meta-analysis was performed using Stata 12.0, funnel plot was drawn, and Egger and Begg regressions were used to evaluate the publication bias, and sensitivity was also analyzed. Results Seven RCTs studies were enrolled to analyze with a total number of 9846 stroke patients, 4755 patients in control group, and 5091 in experimental group, respectively. ① Vitamin B supplementation for prevention of recurrent stroke: heterogeneity test results showed a heterogeneity in literatures enrolled (I2 = 62.9%,P = 0.009), and a random effect model was used for Meta-analysis. It was shown that the incidence of recurrent stroke in the experimental group was significantly lower than that in the control group [pooled relative risk(RR) = 0.64, 95% confidence interval (95%CI) = 0.47-0.87], which indicated that the supplementation of Vitamin B could prevent the recurrence of stroke. Cumulative Meta-analysis showed that Vitamin B supplementation exhibited positive effects in the prevention stroke recurrence from 2012. The 95%CI tended to be stable while demonstrating good change trend as sample growing. The publication bias evaluation results showed that the funnel plot was not symmetrical by visual inspection, further quantitative analysis showed thatP value from Egger regression was 0.008, while that from Begg regression was 0.035, bothP < 0.05, suggesting there were some publication bias. The sensitivity analysis showed that the overall results were stable and reliable. ② The effect of Vitamin B supplementation on plasma homocysteine (Hcy) levels in stroke patients: heterogeneity test results showed a heterogeneity in literatures enrolled (I2 = 96.2%,P = 0.000), and a random effect model was used for Meta-analysis. It was shown that compared with control group, the plasma Hcy levels of patients after Vitamin B supplementation in experimental group were significantly decreased [pooled weighted mean difference (WMD) = -6.92, 95%CI = -9.11 to -4.73), indicating that Vitamin B could significantly reduce plasma Hcy levels in stroke patients. Cumulative meta analysis showed that, as time went on, the relevant research samples were increased, 95%CI tended to be stable and the variation tendency was better. The publication bias evaluation results showed that the funnel plot was symmetry by visual inspection, and further quantitative analysis showed that theP value from Egger regression was 0.345, and that from Begg regression was 0.764, bothP > 0.05, which indicating that there was no evidence of publication bias in the study included.Conclusions Vitamin B supplementation was associated with a lower risk of recurrent stroke in stroke patients and could significantly improve the quality of secondary prevention of stroke. Furthermore, supplementation of Vitamin B could reduce plasma Hcy levels in stroke patients which might contribute to its effect in preventing stroke recurrence.
3.Comparison of two analgesia methods in silver needle therapy among patients with myofascitis

ObjectiveTo compare the effect of two analgesia methods in silver needle therapy among patients with myofascitis.Methods Totally 181 eligible patients with myofascitis were collected from January 2015 to March 2016 and were divided into observation group and control group by random number table. Patients in the control group (n=90) were given partial external lidocaine ointment and diclofenac sodium lidocaine hydrochloride intramuscular injection an hour before the treatment. Patients in the observation group (n=91) first received partial external lidocaine ointment an hour before the treatment,then received flurbiprofen axetil intravenous infusion and intravenous drip 15 minutes before the treatment. Visual analogue scale (VAS) was applied to evaluate the pain that before treatment,at the moment of acupuncture,during the treatment,at the moment of pulling out the needles,and a day after the treatment. Moreover,the clinical effect of two groups was also evaluated after a course of treatment.Results The VAS scores at the moment of acupuncture and during the treatment of the observation group were (7.71±0.80) and (6.71±1.17),while the data in the control group were (4.14±1.06) and (3.58±0.84) (P<0.05). There were no significant differences in the VAS scores before treatment,after pulling out the needles,and a day after treatment,and the clinical effect between two groups (P>0.05).Conclusions Flurbiprofen axetil injection can effectively reduce the pain in silver needle therapy among patients with myofascitis and can also improve patients′ compliance.
4.Effectiveness analysis of fine nursing care on quality of nursing of critically ill patients with stroke

Objective To investigate the effectiveness analysis of fine nursing care on quality of nursing of critically ill patients with stroke. Methods The control group who came from the neurology department of our hospital between February and April in 2015 were given the routine care. The test group who came from the neurology department of our hospital between May and July in 2015 were given the fine nursing care. The quality of nursing and satisfaction of the relatives of patients were compared. Results In the test group, unqualified rates of nursing quality was 8.8%(81/924) and satisfaction of the relatives of patient was 90.9% (229/252); In the control group, unqualified rates of nursing quality was 22.5%(183/814) and satisfaction of the relatives of patient was 82.0%(182/222).There were significant differences between two teams (χ2=63.191, 8.096, P<0.01 or 0.05). Conclusions Fine nursing care can significantly improve the quality of nursing, improve the nurse-patient relationship and improve satisfaction of the relatives of patients.
5.Detection of a fetus with paternally derived 2q37.3 microdeletion and 20p13p12.2 microduplication using whole genome microarray technology.

OBJECTIVETo perform prenatal diagnosis for a fetus with multiple malformations.
METHODSThe fetus was subjected to routine karyotyping and whole genome microarray analysis. The parents were subjected to high-resolution chromosome analysis.
RESULTSFetal ultrasound at 28+4 weeks has indicated intrauterine growth restriction, left kidney agenesis, right kidney dysplasia, ventricular septal defect, and polyhydramnios. Chromosomal analysis showed that the fetus has a karyotype of 46,XY,der(2),der(20), t(2;20)(q37.3;p12.2), t(5;15) (q12.2;q25) pat. SNP array analysis confirmed that the fetus has a 5.283 Mb deletion at 2q37.3 and a 11.641 Mb duplication at 20p13p12.2. High-resolution chromosome analysis suggested that the father has a karyotype of 46,XY,t(2;20)(q37.3;p12.2),t(5;15)(q12.2;q25), while the mother has a normal karyotype.
CONCLUSIONThe abnormal phenotype of the fetus may be attributed to a 2q37.3 microdeletion and a 20p13p12.2 microduplication. The father has carried a complex translocation involving four chromosomes. To increase the chance for successful pregnancy, genetic diagnosis and/or assisted reproductive technology are warranted.

6.Genetic and prenatal diagnosis of a pregnant women with mental retardation.

OBJECTIVETo conduct genetic testing and prenatal diagnosis for a pregnant women with growth retardation, severe mental retardation, and a history of adverse pregnancies.
METHODSG-banded chromosome analysis, fluorescence in situ hybridization (FISH), and whole genome DNA microarray were used to analyze the patient and her fetus.
RESULTSThe women was found to be a chimera containing two cell lines with 47 and 46 chromosomes, respectively. Both have involved deletion of 18q21.2q23. FISH analysis suggested that the cell line containing 47 chromosomes has harbored a chromosome marker derived from chromosome 15. The marker has contained chromosome 15p involving the SNRPN locus and part of 15q, which gave rise to a karyotype of 47,XX,del18q21.3,+ish mar D15Z1+ SNRPN+[82]/46,XX,del18q21.3[18]. Whole genome DNA microarray confirmed that a 3.044 Mb fragment from 15q11.2q12 was duplicated, which involved NIPA1, SNRPN and other 17 OMIM genes. Duplication of this region has been characterized by low mental retardation, autism, developmental delay. Meanwhile, there was a 17.992 Mb deletion at 18q21.33q23, which contained 39 OMIM genes including TNFRSF11A and PHLPP1. This fragment was characterized by mental retardation, developmental delay, short stature, and cleft palate. Whole genome microarray analysis confirmed that there was a 17.9 Mb deletion at 18q21.33q23, which has been implemented with mental retardation, general growth retardation, short stature, and cleft palate. After genetic counseling, the family decided to terminate the pregnancy at 21st week.
CONCLUSIONCombined chromosome karyotyping, FISH, and whole genome DNA microarray can determine the origin of marker chromosomes and facilitate delineation of its correlation with the clinical phenotype.

7.Effect of quality control circle activity on improving Parkinson’ s disease patients with depression

Objective To explore the effect of quality control circle activity on depression and activities of daily living of Parkinson ’ s disease with depression .Methods Totals of 80 patients of Parkinson ’ s disease with depression were divided into treatment group and control group with 40 cases in each group according to the time of admission.Treatment group took part in quality control circle activity for 3 months, two groups were obliged to attend a constant activity with all patients per month , received collective health education .Hamilton depression scale ( HAMD) and Activities of Daily Living ( ADL) were used to measure depression degree and activities of daily living of patients before and after the intervention .Results Three months after the intervention, HAMD score of patients in treatment group was reduced to (8.80 ±2.36), that in control group was (19.47 ±1.85),and the difference was statistically significant (t=22.50,P<0.01).ADL score of patients in treatment group was increased to (89.14 ±2.25), while that score of patients in control group was (58.32 ±2.21), and significantly difference was found between two groups (t =61.81,P <0.01). Conclusions Quality control circle activity can improve the effect of health education through continuously giving the patients and caregivers health education , and establishing an effective social support system . Meanwhile , it can improve the patients ’ depression condition and self management ability , so as to improve the quality of their life .
