1.Allogeneic lung transplantation in miniature pigs and postoperative monitoring

Objective To explore the feasibility and reference value of allogeneic lung transplantation and postoperative monitoring in miniature pigs for lung transplantation research. Methods Two miniature pigs (R1 and R2) underwent left lung allogeneic transplantation. Complement-dependent cytotoxicity tests and blood cross-matching were performed before surgery. The main operative times and partial pressure of arterial oxygen (PaO2) after opening the pulmonary artery were recorded during surgery. Postoperatively, routine blood tests, biochemical blood indicators and inflammatory factors were detected, and pathological examinations of multiple organs were conducted. Results The complement-dependent cytotoxicity test showed that the survival rate of lymphocytes between donors and recipients was 42.5%-47.3%, and no agglutination reaction occurred in the cross-matching. The first warm ischemia times of D1 and D2 were 17 min and 10 min, respectively, and the cold ischemia times were 246 min and 216 min, respectively. Ultimately, R1 and R2 survived for 1.5 h and 104 h, respectively. Postoperatively, in R1, albumin (ALB) and globulin (GLB) decreased, and alanine aminotransferase increased; in R2, ALB, GLB and aspartate aminotransferase all increased. Urea nitrogen and serum creatinine increased in both recipients. Pathological results showed that in R1, the transplanted lung had partial consolidation with inflammatory cell infiltration, and multiple organs were congested and damaged. In R2, the transplanted lung had severe necrosis with fibrosis, and multiple organs had mild to moderate damage. The expression levels of interleukin-1β and interleukin-6 increased in the transplanted lungs. Conclusions The allogeneic lung transplantation model in miniature pigs may systematically evaluate immunological compatibility, intraoperative function and postoperative organ damage. The data obtained may provide technical references for subsequent lung transplantation research.
2.Progress in preclinical studies of xenogeneic lung transplantation and single-center technical experience

Lung transplantation is the ultimate therapeutic option for end-stage pulmonary diseases such as interstitial pneumonia, chronic obstructive pulmonary disease and pneumoconiosis. Currently, the shortage of allogeneic lung donors significantly limits the opportunity for end-stage lung disease patients to receive lung transplantation. In recent years, with the rapid development of biomedical engineering technologies, especially the major breakthroughs in genetic modification and cloning, xenogeneic lung transplantation has shown important potential for clinical translation. Among them, genetically modified pigs have become the most promising xenogeneic lung source due to the close similarity of organ size and physiological characteristics to humans, and the ability to perform targeted gene knockouts (such as α-Gal antigen knockout) to reduce the occurrence of hyperacute rejection. This article focuses on the research progress of porcine xenogeneic lung transplantation, systematically reviews the latest achievements and challenges in animal experiments and human trials, and introduces the technical experience accumulated by Shenzhen Third People's Hospital in the porcine-to-monkey xenogeneic lung transplantation model, in the hope of providing practical references for future research in this field.
3.Rapid health technology assessment of tirzepatide for diabetes mellitus type 2 and long-term weight management

OBJECTIVE To evaluate the efficacy, safety and cost-effectiveness of tirzepatide for diabetes mellitus type 2 (T2DM) and long-term weight management, and provide evidence-based basis for clinical drug treatment and health insurance policy formulation. METHODS Computer searches were conducted in Embase, PubMed, the Cochrane Library, CNKI and health technology assessment (HTA) official website from their inception to October 1st 2024 to collect HTA report, systematic review/ meta-analysis and pharmacoeconomic study on tirzepatide for the treatment of T2DM or for weight management. After data extraction and quality evaluation, descriptive analysis was performed on the research results. RESULTS Totally 18 papers were included, including 14 systematic reviews/meta-analyses and 4 pharmacoeconomics studies, and no HTA report was retrieved. In terms of efficacy, most results showed that the tirzepatide 10 mg and 15 mg were significantly better than other glucagon-like peptide-1 (GLP-1) receptor agonists in reducing glycosylated hemoglobin, body weight, and waist circumference (P<0.05). In terms of safety, compared with other GLP-1 receptor agonists, tirzepatide did not increase the incidence of gastrointestinal-related adverse events (AE), the incidence of AE of grade ≥3, or the incidence of severe hypoglycemia (P>0.05). However, tirzepatide 15 mg may significantly increased the incidence of hypoglycemia and the rate of discontinuation due to adverse reactions (P< 0.05). In terms of cost-effectiveness, based on the background of foreign pharmacoeconomic studies, tirzepatide was more cost- effective compared to semaglutide and liraglutide in the treatment of T2DM or for weight management. CONCLUSIONS Tirzepatide at doses of 10 mg and 15 mg has good efficacy and safety for the treatment of T2DM and for long-term weight management. However, when using the 15 mg dose of tirzepatide, close monitoring is required due to the risk of hypoglycemia and discontinuation due to adverse reactions it may pose. Based on pharmacoeconomic studies conducted abroad results, tirzepatide exhibits economic advantages.
4.Epidemic characteristics of major natural focus diseases and vector-borne infectious diseases in Yantai City of Shandong Province from 2019 to 2023

Objective:To investigate the epidemic characteristics and trends of major natural focus diseases and vector-borne infectious diseases in Yantai City, and provide a basis for formulating reasonable prevention and control strategies.Methods:The descriptive analysis was conducted on the epidemic data of natural focal diseases and vector-borne infectious diseases in Yantai City collected from the Infectious Disease Surveillance and Reporting Management System, a subsystem of the China Disease Control and Prevention Information System, covering the period from January 1, 2019, to December 31, 2023.Results:From 2019 to 2023, a total of 2 746 cases of 12 kinds of natural focus diseases, vector-borne infectious diseases and zoonosis were reported in Yantai City. The top four diseases reported in incidence rate were severe fever with thrombocytopenia syndrome (SFTS, 3.13/100 000), brucellosis (2.57/100 000), hemorrhagic fever with renal syndrome (HFRS, 1.09/100 000), and scrub typhus (0.73/100 000). There were statistically significant differences in the reported incidence rate of the four major natural focus diseases and vector-borne infectious diseases between different years (χ 2 = 92.55, 34.88, 132.33, 39.95, P < 0.001). The four major natural focal diseases and vector-borne infectious diseases had reported cases throughout the year. SFTS cases peaked in June, accounting for 28.80% (320/1 111). Brucellosis cases were predominantly reported from March to August, accounting for 65.54% (599/914). HFRS cases peaked in November, accounting for 38.66% (150/388), while scrub typhus cases peaked in October, accounting for 69.88% (181/259). The top three districts/cities with the highest incidence of the four major natural focus diseases and vector-borne infectious diseases were Laizhou City (585 cases), Zhaoyuan City (377 cases), and Haiyang City (358 cases). The incidence rates of SFTS in women was higher than that in men (χ 2 = 13.92, P < 0.001), and the incidences rates of brucellosis and HFRS in men were higher than those in women (χ 2 = 123.09, 73.32, P < 0.001). The cases of the four diseases were mainly farmers, accounting for 76.06% (845/1 111), 78.12% (714/914), 81.44% (316/388), and 85.71% (222/259), respectively. Conclusions:From 2019 to 2023, the reported incidence rate of the four major natural focus diseases and vector-borne infectious diseases in Yantai City presents an overall upward trend, with obvious seasonal, regional and demographic characteristics. Targeted prevention and control measures should be developed for high-risk areas and populations based on the characteristics of different diseases.
5.A double-center study on the value of D-dimer levels in the diagnosis of thromboembolism in children with severe Mycoplasma pneumoniae pneumonia

Objective:To evaluate the value of D-dimer(D-D) levels in the diagnosis of thromboembolism in children with severe Mycoplasma pneumoniae pneumonia (SMPP).Methods:A case control study was conducted on 51 SMPP patients admitted to Qingdao Women and Children′s Hospital Affiliated to Qingdao University and Jining First People′s Hospital from January 1, 2021 to August 1, 2024.They were divided into a thrombus group (19 cases) and a non-thrombus group (32 cases) according to whether they had thromboembolism.The characteristics of the cases were analyzed.Logistic regression was used to analyze the relationship between increased D-D levels and the risk of thrombosis.The value of D-D levels in predicting thromboembolism in SMPP patients was evaluated by the receiver operating characteristic curve.Results:Of the 19 patients with thrombosis, 11 had with pulmonary thrombosis, 6 had cerebrovascular thrombosis, 3 had heart thrombosis, 1 had spleen artery thrombosis, and 3 had multiple site thrombosis.Compared with the non-thrombus group, the thrombus group had higher D-D levels[(18.5±4.9) mg/L vs.(3.1±0.8) mg/L, t=3.118, P=0.006], higher C-reactive protein levels[25.5(15.5, 92.7) mg/L vs.7.9(3.9, 21.4) mg/L, Z=3.292, P=0.001], higher lactate dehydrogenase levels[484(351, 743) U/L vs.347(285, 396) U/L, Z=2.770, P=0.006] and a higher proportion of pleural effusion[63.1%(12/19) vs.25.0%(8/32), χ2=7.282, P=0.009].Increased D-D levels were an independent risk factor for thromboembolism in SMPP patients ( P=0.005, OR=1.254, 95% CI: 1.069-1.472).When the D-D level was used for predicting thromboembolism in SMPP patients, its cut-off value was 4.46 mg/L, its Youden index was 0.707, its area under the curve was 0.893(95% CI: 0.807-0.979), its sensitivity was 89.5%, its specificity was 81.2%, and its negative predictive value was 92.9%. Conclusions:D-D levels have high value in predicting thromboembolism in SMPP patients, and it can help timely identify patients at high risk.

8.Epidemic characteristics of scrub typhus in Yantai City of Shandong Province from 2006 to 2022

Objective:To study the epidemic characteristics of scrub typhus in Yantai City from 2006 to 2022, and provide a scientific basis for prevention and control of scrub typhus in Yantai City.Methods:In the subsystem "Infectious Disease Monitoring System" of the "China Disease Prevention and Control Information System", data on the outbreak of scrub typhus in Yantai City from January 1, 2006 to December 31, 2022 were collected and processed using software such as SPSS 23.0 for descriptive statistics.Results:A total of 599 scrub typhus cases were reported in Yantai City from 2006 to 2022, with an average annual reported incidence of 0.51/100 000. The reported incidence showed an upward trend year by year from 2006 to 2014 (χ 2trend = 144.72, P < 0.001), and reached a peak in 2014 (1.04/100 000). There was another small peak in 2019 (0.91/100 000). The incidence of scrub typhus showed a clear seasonal unimodal distribution, with October being the peak, accounting for 72.62% (435/599). In addition to Changdao County, other 12 cities (districts) reported scrub typhus cases. The top 3 cities with average annual reported incidence were Zhaoyuan City (1.58/100 000), Laizhou City (1.47/100 000) and Laiyang City (0.75/100 000). In all reported cases, the male to female ratio was 1.00∶1.07 (289∶310). The cases were mainly concentrated in the age range of 50 to 79, accounting for 74.46% (446/599). There was a statistically significant difference in the number of patients in different age groups in different years ( H = 38.71, P = 0.001). Farmers accounted for 88.65% (531/599) of the affected population. Conclusions:The incidence of scrub typhus in Yantai City shows an increasing trend and is widely distributed. It is recommended to strengthen the diagnosis, monitoring and infection management of scrub typhus.
9.Gallic Acid Ameliorates Cognitive Impairment Caused by Sleep Deprivation through Antioxidant Effect

Sleep deprivation (SD) has a profound impact on the central nervous system, resulting in an array of mood disorders, including depression and anxiety. Despite this, the dynamic alterations in neuronal activity during sleep deprivation have not been extensively investigated. While some researchers propose that sleep deprivation diminishes neuronal activity, thereby leading to depression. Others argue that short-term sleep deprivation enhances neuronal activity and dendritic spine density, potentially yielding antidepressant effects. In this study, a two-photon microscope was utilized to examine the calcium transients of anterior cingulate cortex (ACC) neurons in awake SD mice in vivo at 24-hour intervals. It was observed that SD reduced the frequency and amplitude of Ca2+ transients while increasing the proportions of inactive neurons. Following the cessation of sleep deprivation, neuronal calcium transients demonstrated a gradual recovery. Moreover, whole-cell patch-clamp recordings revealed a significant decrease in the frequency of spontaneous excitatory post-synaptic current (sEPSC) after SD. The investigation also assessed several oxidative stress parameters, finding that sleep deprivation substantially elevated the level of malondialdehyde (MDA), while simultaneously decreasing the expression of Nuclear Factor erythroid 2-Related Factor 2 (Nrf2) and activities of Superoxide dismutase (SOD) in the ACC. Importantly, the administration of gallic acid (GA) notably mitigated the decline of calcium transients in ACC neurons. GA was also shown to alleviate oxidative stress in the brain and improve cognitive impairment caused by sleep deprivation. These findings indicate that the calcium transients of ACC neurons experience a continuous decline during sleep deprivation, a process that is reversible. GA may serve as a potential candidate agent for the prevention and treatment of cognitive impairment induced by sleep deprivation.
10.Spiritual health status and influencing factors of patients with lung cancer during perioperative period

Objective:To explore the influencing factors by investigating the spiritual health status of patients with lung cancer during perioperative period.Methods:A total of 208 perioperative patients who underwent radical resection of lung cancer in Cancer Hospital Chinese Academy of Medical Sciences from September 2020 to September 2021 were selected as research objects by the convenient sampling method. The General Data Questionnaire, Chinese version of Functional Assessment of Chronic illness Therapy-Spiritual (FACIT-SP-12) and Family APGAR Index (APGAR) were used to investigate the patients. Pearson correlation analysis was used to explore the correlation between spiritual health and family caring. Pearson correlation analysis was used to explore the correlation between spiritual health and family caring. Multiple linear regression analysis was used to investigate the influencing factors of spiritual health in patients with lung cancer during perioperative period. A total of 208 questionnaires were sent out and 200 were effectively received, with the effective recovery of 96.15%.Results:The total score of FACIT-SP-12 of 200 patients with perioperative lung cancer was (35.59±8.16) , indicating a moderate spiritual health level. The main factors affecting spiritual health of perioperative lung cancer patients were the degree of family care, marital status and payment of medical expenses ( P<0.05) . Conclusions:The spiritual health of perioperative lung cancer patients is in the middle level, and the spiritual health of patients with different family caring degrees, marital status and medical payment method is different. Clinical medical workers can carry out targeted nursing intervention according to individual conditions of patients to improve their quality of life.
