1.Intravitreal ranibizumab injection combined with subthreshold micropulse yellow laser in the treatment of diabetic macular edema

AIM:To evaluate the efficacy of intravitreal ranibizumab injection(IVR)combined with subthreshold micropulse(STMP)in the treatment of diabetic macular edema(DME).METHODS: Retrospective study. A total of 98 DME patients(98 eyes)admitted to our hospital from March 2022 to March 2023 were enrolled and divided into two groups based on treatment methods: the control group(49 eyes)received STMP yellow laser therapy alone, while the study group(49 eyes)underwent combined IVR and STMP yellow laser therapy. Comparisons were made between the two groups regarding best corrected visual acuity(BCVA), retinal neovascularization(RNV)leakage area, parafoveal macular thickness(PMT), foveal macular thickness(FMT), central retinal thickness(CRT), and foveal avascular zone(FAZ)area, quality of life was assessed using the Chinese-version low vision quality of life questionnaire(CLVQOL), and complication rates were recorded. Additionally, serum levels of nitric oxide synthase(NOS)and vascular endothelial growth factor(VEGF)were measured before and after treatment in both groups.RESULTS: At 3 mo after treatment, both groups showed improved BCVA compared to baseline, with reduced RNV leakage area, PMT, FMT, CRT, FAZ, and serum levels of VEGF, while serum NOS levels and all CLVQOL domain scores were higher than pre-treatment(all P<0.05). Furthermore, the study group demonstrated superior outcomes in all these parameters compared to the control group(all P<0.05), and no ocular or systemic complications occurred in any patient.CONCLUSION: IVR combined with STMP yellow laser for DME improves visual acuity, reduces RNV leakage area, PMT, FMT, CRT, and FAZ, modulates serum NOS and VEGF levels, enhances quality of life, and demonstrates good safety.
2.Application of standing-sitting Baduanjin exercise in postoperative rehabilitation of breast cancer patients

Objective To explore the effect of standing-sitting Baduanjin exercise in postoperative rehabilitation of breast cancer patients. Methods Totally 70 cases of patients with breast cancer postoperation were randomly assigned to the control group and the experimental group. Patients in the control group received routine postoperative nursing, while patients in the experimental group adopted standing-sitting Baduanjin exercise. Fatigue condition and functional recovery of ill limb were compared through evaluation by BFI and functional assessment of ill limb.Results The total BFI score of fatigue in the experimental group was (3.29± 0.42) points 1 month after operation, and was (3.28±0.45) points 3 months after operation. They were all superior than the score of control group (t=-4.007,-3.996;P<0.01). Limb function of 96.8% patients were completely recovered in the experimental group 1 month after operation, while just 75.8% patients in the control group. 3 months after operation, 100% patients were completely recovered in the experimental group, and in the control group, it was 81.8% (χ2=4.233,4.264;P<0.05).Conclusions Suitability standing-sitting Baduanjin exercise can relieve the fatigue symptom of patients with breast cancer postoperation, and delay the aggravation of symptoms during chemotherapy.
3.Application of Orem self-care theory in the mental nursing of children with acute leukemia

Objective To explore the clinical application value of Orem self-care theory in the mental nursing of children with acute leukemia .Methods Fifty-five children with acute leukemia were randomly divided into the control group ( n=27 ) and the experimental group ( n=28 ) according to single and double number .The control group received the routine nursing , and the experimental group received the nursing intervention according to Orem self -care theory on the basis of the routine nursing .The changes of mental status and the nursing satisfaction were evaluated and compared between two groups through the Symptom Checklis-90 ( SCL-90 ) , Hamilton Anxiety Scale ( HAMA ) , and Hamilton Depression Scale ( HAMD ) . Results The anxiety and depression were mainly dominated in children with leukemia .One week after the intervention, the score of HAMA in the experimental group decreased from (18 ±6)to (12 ±5),and was lower than (16 ±3)in the control group; the score of HAMD in the experimental group decreased from (23 ±8) to (17 ±3), and was lower than (20 ±4) in the control group, and the differences were statistically significant (t=3.50, 2.39, 2.68, 1.87, respectively;P<0.05).The nursing satisfaction in the experimental group was 96.4%, and was higher than 85.2%in the control group, and the difference was statistically significant (χ2 =3.47, P<0.05).Conclusions The application of Orem self-care theory in children with acute leukemia can obviously alleviate their anxiety and depression , and improve clinical nursing satisfaction .
4.Clinical analysis of 330 patients with primry aldosteronism

The clinical data of 330 patients with primary aldosteronism (PA) from January 2006 to March 2010 were retrospectively analyzed. The prevalence of 1, 2, and 3 stage hypertension in these subjects was 3.64%,20. 91%, and 75.45 %, respectively. Of all PA patients, 89.09% were young adults and 81.82% were overweight or obese. There was a marked preponderance of male patients in the overweight or obese group ( P<0. 01 ). The incidence of hypokalemia was 32. 12%. The concentration of serum potassium was not associated with the disease course. Logistic regression showed that the concentration of plasma aldosterone was an independent risk factor of hypokalemia in PA patients( P<0. 01 ). 79. 09% PA patients presented the plasma aldosterone level over 12 ng/dl and the renin activity level of less than 1 ng · ml-1 · h-1. The aldosterone-to-rennin activity ratio was >20 in 94.24% of the patients with PA.
