1.Optimization of Extraction Process for Yunpi Huatan Tongqiao Decoction

Objective Establish the fingerprint of the group and optimize the extraction process of the Yunpi Huatan Tongqiao decoction.Methods The macroscopic characterization and similarity analysis of the extract fingerprint were carried out by applying the total statistical moment method.With baicalin,wogonin,rosmarinic acid,liquiritin,glycyrrhizic acid and paste yield as key quality attributes,and extraction time,water addition and extraction times as key process parameters,the optimal extraction process was selected by AHP-independent weight method,and the process was verified.Results Establishing a total statistical moment similarity evaluation method,and the average statistical moment similarity of the total amount of fingerprints of different extraction methods was 0.8807.The ANOVA results of the process evaluation model function are displayed that P<0.0001,R2=0.9904,indicating that the model was statistically significant.The best extraction process was determined as follows:adding 10-fold volume of water in the whole prescription,extracting for 1.5 h and extracting twice.Conclusion The total statistical moment analysis method that conforms to the fingerprint characteristics of the fingerprint of the extract was established,which was stable and reliable,which provided a reference basis for the quality control of the whole process of the subsequent process research.
2.Intravascular ultrasound assisted endovascular therapy for renal artery stenosis

Objective To observe the value of intravascular ultrasound(IVUS)for assisting endovascular therapy for renal artery stenosis(RAS).Methods Thirty patients with RAS who underwent endovascular therapy were retrospectively analyzed.Parameters of renal artery and plaques in RAS segment measured with CT angiography(CTA)and IVUS before treatment were compared.Bland-Altman diagram was performed to evaluate the consistency of lumen cross-sectional stenosis rate and plaque eccentricity index between CTA and IVUS.The stent parameters measured with IVUS were recorded immediately after implantation of balloon-expandable covered stents.Results Before treatment,the minimum lumen diameter,lumen cross-sectional stenosis rate and stenotic segment length of IVUS were all larger,while maximum lumen diameter and lumen eccentricity index of IVUS were both smaller than those of CTA(all P<0.05).No significant difference of plaque eccentricity index,plaque type nor stenosis distal remodeling was found between CTA and IVUS(all P>0.05).The average difference between IVUS and CTA for evaluating lumen cross-sectional stenosis rate and plaque eccentricity index was-0.020(-0.096,0.050)and-0.020(-0.130,0.091),respectively.The consistency of IVUS and CTA for evaluating plaque eccentricity index was better than that of lumen cross-sectional stenosis rate.The stent symmetry,stent eccentricity index,stent expansion coefficient and stenosis coverage rate immediately after implantation measured with IVUS was(82.69±14.61)%,(1.54±9.16)%,(99.81±10.70)%and 100%,respectively.Among 30 cases,2 cases(2/30,6.67%)underwent postdilation since poor stent apposition.Conclusion IVUS could assist evaluating lumen and plaque parameters of stenotic renal arteries,guiding stent release and real-timely monitoring the effect of endovascular therapy.
3.Effect of oral immunotherapy with donor human milk on feeding tolerance in very low birth weight premature infants

Objective:To investigate the effect of oral immunotherapy with donor human milk on feeding tolerance in premature infants with very low birth weight.Methods:A total of 94 extremely low birth weight preterm infants admitted to the neonatal Intensive Care Unit of Cangzhou People's Hospital from August 2022 to February 2024 were selected, These infants were randomized at 1:1 into 2 groups (47 cases each) using random number method. The control group were given oral 0.9% sodium chloride (oral drip), while the experimental group were given donor human milk (oral drip). General information were collected, including gender, delivery method, twins or not, small for gestational age or not, birth weight, gestational age, intestinal feeding of dairy products, and so on. Total intestinal feeding time, length of hospital stay and feeding intolerance were compared between groups.Results:The total intestinal feeding time and length of hospital stay were significantly shorter in the experimental group compared with the control group ( P<0.05). The incidence of feeding intolerance in the experimental group was also lower than that in the control group ( P<0.05). Conclusion:Oral immunotherapy with donor human milk can improve the feeding tolerance in premature infants with very low birth weight and shorten the hospital stay.
4.Design and implementation of epidemiological survey on dementia in community residents in Tongliao City

Objective:To describe the detail sampling design,weighting,instruments,filed procedures and quality control methods of the epidemiological survey on dementia among community residents in Tongliao City.Methods:A three-stage disproportionate probability sampling design was used to investigate the inhabitants aged 65 years and over in Tongliao City,Inner Mongolia Autonomous Region.The 10/66 Dementia Research Group(10/66 DRG)assessment instruments were used to diagnose dementia,using computer-assisted personal interview mode in the selected older people.Comprehensive quality control methods were implemented throughout the field-work.Results:A total of 166 villages or communities were sampled from nine counties or districts in Tongliao Cit-y.Totally 4 345 older people were interviewed with 96.2%response rate.By calculating sampling design weights,non-response adjustment weights and post-stratification adjustment weights,these weights were multiplied and per-formed trimming adjustment and standardization adjustment to generate final weights.The 171 interviewers were well-trained and qualified to carry out filed interview.Quality control methods included computer data check,audio record check,and telephone check in order to ensure the quality of the survey.Conclusion:This survey is imple-mented using a rigorous sampling design and timely quality control methods,and uses the 10/66 DRG assessment instruments with satisfactory international validity and reliability as survey instruments,which has international cross-cultural comparability.It provides a valid and feasible methodology of epidemiological survey on dementia for further studies in different regions in China.
5.Value of ECG P-wave parameters combined with serum adiponectin and VF in predicting recurrence of AF after radiofrequency ablation

Objective To investigate the value of electrocardiogra(ECG)P-wave parameters com-bined with serum adiponectin and visfatin(VF)in predicting recurrence of atrial fibrillation(AF)after radiofrequency ablation.Methods A total of 120 AF patients(AF group)admitted to our hospital from March 2021 to March 2023 were enrolled,and divided into a recurrence group(39 cases)and a non-recurrence group(81 cases)according to the postoperative condition.Another 120 healthy individuals who took physical examination in the same period were enrolled and served as control group.All subjects underwent ECG.ELISA was used to detect adiponectin and VF levels.Logistic regression analysis was used to identify the influencing factors of postoperative recur-rence in AF patients.ROC curve was plotted to analyze the predictive value of P-wave parameters combined with serum adiponectin and VF for postoperative recurrence in AF patients.The P-wave parameters included P-wave duration(PWD),maximum PWD(Pmax),minimum PWD,and P-wave dispersion(Pd).Results PWD,Pmax and Pd were significantly decreased in AF patients after operation(P<0.01).Serum adiponectin level was obviously lower and that of VF was nota-bly higher in the AF group than the control group(6.15±1.58 μg/L vs 11.43±2.26 μg/L,12.05±2.30 μg/L vs 6.23±1.02 μg/L,P<0.01).The recurrence group had remarkably higher preopera-tive PWD,Pmax and Pd,and lower adiponectin and higher VF levels than the non-recurrence group(P<0.01).Multivariate logistic regression analysis showed that PWD,Pmax,Pd,and VF were risk factors,and adiponectin was a protective factor for postoperative recurrence in AF pa-tients(P<0.05,P<0.01).ROC curve analysis indicated that PWD,Pmax,Pd,adiponectin and VF combined together to detect postoperative recurrence in AF patients had an AUC value of 0.972,which was higher than that of above indicators alone(Z=2.574,Z=2.567,Z=2.543,Z=2.554,Z=2.586,P<0.05).Conclusion ECG P-wave parameters as well as elevated serum VF and re-duced serum adiponectin are influencing factors for postoperative recurrence in AF patients,and their combined detection may promote the predictive value for postoperative recurrence in them.
6.Correlation between oral health status and frailty of elderly people in Beijing community

Objective:To understand the oral health and frailty status of the elderly in the community in Beijing and analyze the correlation between the two, so as to provide a reference for the frailty management of the elderly in the community.Methods:This study was a cross-sectional study. Using the multi-stage stratified sampling method, a total of 241 community elderly people in 9 communities in Beijing from July to December 2021 were selected as the research objects. They were investigated using the general information questionnaire, Mini-Nutritional Assessment (MNA) and the Fried Frailty Phenotype. Univariate analysis and ordinal logistic regression analysis were used to explore the influencing factors of frailty among the elderly in the community. A total of 260 questionnaires were distributed in this study and 241 valid questionnaires were recovered, with an effective recovery rate of 92.6%.Results:Among the 241 community elders, 115 (47.7%) were not frail, 92 (38.2%) were pre-frail and 34 (14.1%) were frail. Ordinal Logistic regression analysis showed that the number of teeth of 0-9, 10-19, dry mouth and incomplete or unrepaired restoration of missing teeth were risk factors for frailty among the elderly in the community ( P<0.05) . Conclusions:From the perspective of oral health, this study further analyzes the risk factors of frailty in the elderly in the community. Medical institutions and elderly care institutions at all levels can use oral health status as a screening item for the frailty risk of the elderly in the community, providing new ideas for the prevention and intervention of frailty in the community.
7.Advances in the prevention of mother-to-child transmission of hepatitis B virus

Mother-to-child transmission (MTCT) of hepatitis B virus (HBV) represents the most common mode by which children acquire HBV infection. For pregnant women with high viral load and positive hepatitis B e antigen (HBeAg), antiviral therapy during late pregnancy combined with timely and standardized inoculation of neonatal hepatitis B immunoglobulin and hepatitis B vaccine can minimize the possibility of MTCT of HBV. This review focuses on using and withdrawing antiviral drugs during pregnancy, managing postpartum hepatitis, and breastfeeding issues to further optimize the combined immunization and antiviral treatment strategies and seek the optimal solution to preventing MTCT of HBV.
8.Validation of VR in pediatric anesthesia emergency during standardized residency training

Objective:To explore the effect of virtual reality (VR) on the emergency management of pediatric anesthesia.Methods:Four kinds of perioperative critical events were selected as VR training content, and the training objects were 60 residents who took part in the standardized residency training in Ninth People's Hospital of Shanghai Jiao Tong University School of Medicine. Through theoretical examination and questionnaire survey, the training effect and the anxiety of residents after critical events were evaluated. The data were input into Graphpad Prism 5 software, and the quantitative data conforming to normal distribution were expressed as (mean ± standard deviation), and compared by nonparametric test and t test of two independent samples between groups. Qualitative data were expressed in terms of rate, and the 2× k trend chi-square test was used to compare the scores of the two groups. Results:All the residents took part in the theoretical assessment of the management of pediatric anesthesia emergencies. Compared with the offline theoretical teaching, the average scores of the students who underwent VR training were higher, and the incidence of anxiety was significantly reduced.Conclusion:VR simulation teaching in the department of anesthesiology can fully arouse the enthusiasm of residents and make them overcome the fear of dealing with pediatric anesthesia emergencies.
9.Study on the Effects of Tenuifolin on Brain Mitochondrial Autophagy in AD Model Mice Based on PINK 1/Parkin Signaling Pathway

OBJECTIVE:To investig ate the effects of tenuifolin (TEN)on brain mitochondrial autophagy in Aizheimer ’s disease(AD)model mice. METHODS :Totally 50 male APP/PS1 double transgenic mice were randomly divided into model group,TEN medium-dose+ 3-MA group [TEN 40 mg/(kg·d)+autophagy inhibitor 3-MA 30 mg/(kg·d)] and TEN low-dose , medium-dose and high-dose groups [ 20,40,80 mg/(kg·d)],with 10 mice in each group. In addition ,10 wild-type homologous mice were included in normal control group. Administration groups were intragastrically given corresponding drug solution ;normal control group and model group were intragastrically given 0.3% sodium carboxymethyl cellulose solution ,once a day ,0.01 mL/g, for consecutive 3 months. After last administration ,positive expression [measured by integrated optical density (IOD)] of microtubule associated protein 1 light chain 3(LC3)in neuron was detected ;mRNA expressions of LC3,ubiquitin-binding protein p62,Cathepsin D ,Rab7,phosphatase and tensin homolog deleted on chromosome ten gene-induced putative kinase 1(PINK1) and E 3 ligase(Parkin)as well as protein expressions of LC 3,p62,PINK1 and Parkin were detected in brain mitochondria. RESULTS:Compared with normal control group ,IOD value of LC 3 in neuron as well as mRNA and protein expressions of LC 3, p62,PINK1 and Parkin in brain mitochondria were all increased significantly in model group (P<0.05 or P<0.01),while mRNA expressions of Cathepsin D and Rab 7 were decreased significantly (P<0.05 or P<0.01). Compared wit h model group ,IOD values of LC 3(except for TEN low-dose and medium-dose groups ) in neuron ,mRNA expressions of LC 3,Cathepsin D ,Rab7, PINK1(except for TEN low-dose group )and Parkin (except for TEN low-dose group ) in brain mitochondria as well as protein expressions of LC 3 (except for TEN medium-dose group),PINK1(except fo r TEN high-dose group decreased significantly)and Parkin (except for TEN low-dose group decreased significantly )were increased significantly in TEN low-dose , medium-dose and high-dose groups (P<0.05 or P<0.01);mRNA(except for TEN low-dose group )and protein expressions of p62 were decreased significantly (P<0.05 or P<0.01). Compared with TEN medium-dose group ,the changes of above indexes were inhibited significantly in TEN medium-dose + 3-MA group (P<0.05 or P<0.01). CONCLUSIONS :TEN can induce mitophagy in brain tissue of AD model mice by activating PINK 1/Parkin signaling pathway and improve lysosome function.
10.Best evidence summary for prevention of pressure injury in adult patients with acute respiratory distress syndrome and mechanical ventilation in prone position

Objective:To retrieve, evaluate and summarize the evidence for the prevention of pressure injury in adult patients with acute respiratory distress syndrome (ARDS) and mechanical ventilation in prone position, to provide a basis for clinical nurses in the management of pressure injury in patients with mechanical ventilation in the prone position of ARDS.Method:UpToDate, BMJ Best Practice, Guidelines International Network, National Guideline Clearinghouse, National Institute for Health and Clinical Excellence, Joanna Briggs Institute (JBI) Evidence-based Health Care Centre, Cochrane Library, Medlive, EMBASE, PubMed, BMJ, CNKI, Wanfang Database and SinoMed were systematically retrieved to search evidence on the prevention of pressure injury in adults with ARDS and mechanical ventilation in prone position, including clinical decision-making, guidelines, evidence summary, systematic evaluation, Meta-analysis, RCT and quasi-experimental studies. The search time limit was from December 1, 2010 to December 1, 2020. The literature quality was evaluated by 4 personnel, and the included literature was extracted and summarized by 2 personnel.Result:Finally, 7 articles were included, including 1 clinical decision, 3 guidelines, 2 RCTs and 1 quasi-experimental study. A total of 12 pieces of evidence were extracted from 5 aspects, such as evaluation, intervention measures, supervision, professional knowledge training of nursing staff and principles of prevention and compliance.Conclusions:The best evidence for the prevention of pressure injury in adult ARDS patients with prone mechanical ventilation summarized in this study is convenient for clinical nurses to conduct more targeted and scientific evaluation and care of pressure injury for patients.
