1.Related factors of cognitive impairment in middle-aged and old-aged patients with type 2 diabetes mellitus

Objective:To investigate the related factors of cognitive impairment in middle-aged and old-aged patients with type 2 diabetes mellitus(T2DM).Methods:A total of 970 patients with T2DM(585 middle-aged group and 385 old-aged group)were selected from residents of a large community in Beijing from September to December 2018.The Mini-Mental State Examination(MMSE)was used to assess the cognitive func-tion.Multivariate logistic regression was used to analyze the related factors.Results:The detection rates of cognitive impairment were 12.0%and 13.5%in middle-aged and old-aged patients with T2DM,respectively.Among mid-dle-aged patients with T2DM,work(OR=0.22,95%CI:0.03-0.77)and education at the junior college or un-dergraduate level and above(OR=0.18,95%CI:0.04-0.55)were protective factors for cognitive impair-ment.Myocardial infarction(OR=4.13,95%CI:1.26-13.63)was a risk factor for cognitive impairment.Among old-aged patients with T2DM,drinking tea 1-2 times a week(OR=0.11,95%CI:0.01-0.58)and education at the junior college or undergraduate level and above(OR=0.19,95%CI:0.05-0.54)were protective factors for cognitive impairment.Stroke(OR=3.64,95%CI:1.55-8.39)and good sleep self-assessment(OR=2.75,95%CI:1.13-7.35)were risk factors for cognitive impairment.Conclusion:Cognitive impairment in middle-aged pa-tients with T2DM is related to work,education level and myocardial infarction,and cognitive impairment in old-aged patients with T2DM is related to lifestyle,education level and stroke.

Objective:To retrieve, evaluate and summarize the best evidence for the management of oral intake in stroke patients with dysphagia.Methods:A systematic search was conducted in Chinese and English databases and subject-specific professional websites for clinical decisions, best practices, guidelines, systematic reviews, expert consensuses and evidence summaries on the management of oral intake in stroke patients with dysphagia. The retrieval time limit was from the establishment of the database to March 31, 2024. The included literatures were screened, quality-evaluated, and evidence was extracted and summarized.Results:A total of 16 articles were included, including three clinical decisions, four guidelines, four expert consensuses, four evidence summaries and one systematic review. A total of 28 pieces of evidence were summarized from eight aspects, namely, swallowing function screening, nutritional risk screening, feeding management, oral medication management, water drinking management, oral care, aspiration management, and training and education.Conclusions:This study systematically summarizes the best evidence for the management of oral intake in stroke patients with dysphagia, providing scientific guidance and strategic support for the improvement of patients' long-term quality of life.

5.Analysis and Recommendations on the Current Status of Pharmaceutical Management in County Medical Communities in Hubei Province

Objective This study aims to assess the current status of pharmaceutical management in county medical communities in Hubei province,and provide recommendations for the homogenization,standardization,and regulation of pharmaceutical management in these communities.It also intends to offer decision-making support for health administrative departments,and provide reference experiences for management in other regions.Methods The current status of pharmaceutical management in county medical communities in Hubei province was conducted through a questionnaire survey and field research.Existing problems were analyzed,key management areas were identified,and reasonable recommendations were proposed.Results Pharmaceutical management in county medical communities has significant shortcomings in organizational structure,system construction,personnel allocation,key link control,and the leading unit's outreach capabilities.These deficiencies are not aligned with the high-quality development of pharmacy in the new era.Conclusions It is recommended that county medical communities should establish a comprehensive pharmaceutical management quality control system.This can be achieved by improving organizational management,strengthening talent development,enhancing core systems,setting monitoring indicators,and increasing outreach capabilities.Additionally,evaluation standards for the quality control system of pharmaceutical management should be established to enhance management capabilities through scientific assessment and positive feedback.
6.Analysis of neuroendoscopic or microscopic transsphenoidal approach in the treatment of pituitary tumors of different grades and the measures to reduce the common postoperative complications

Objective:To explore the clinical effect of endoscopic assisted or microscopical transsphenoidal surgery in the treatment of different grades of invasive pituitary tumors and the measures to reduce complications.Methods:From January 2016 to June 2019, 87 patients with pituitary tumor admitted to the neurosurgery of Tangshan People′s Hospital of Hebei Province were selected as the study objects for retrospective analysis. Among them, 42 patients were included in the endoscopic group and 45 patients were included in the endoscopic group. To compare the related indexes and clinical effects of two different surgical methods, and to analyze the treatment strategies to reduce the common postoperative complications.Results:The amount of intraoperative bleeding, the time needed for operation and the hospital stay of patients in endoscopic group were significantly less or shorter than those in microscopical group(intraoperative bleeding volume: (66.19±9.55) ml and (79.78±11.68) ml; operation time: (93.93±9.53) min and (103.33±9.23) min; hospitalization time: (7.71±1.53) d and (10.16±1.33) d). The difference was statistically significant (T values were 5.92, 4.67, 7.94, all P< 0.01). There was no significant difference between the two groups ( P= 0.577). There were 2 cases (4.8%) of cerebrospinal fluid leakage in endoscopic group and 12 cases (26.7%) in microscopical group. The difference between the two groups was statistically significant(χ 2=5.67, P< 0.05). There were 7 cases (16.7%) of diabetes insipidus in endoscopic group and 22 cases (48.9%) in microscopical group. The difference between the two groups was statistically significant (χ 2=5.19, P< 0.05). Conclusion:The curative effect of endoscopic transsphenoidal resection of pituitary tumor is similar to that of microscopical resection of pituitary tumor. The patients recover quickly, have fewer complications, and have a short hospital stay.
7.Expression and significance of Beclin 1 and LC3 in non small cell lung cancer

Objective To investigate the expression of beclin1 and LC3 in non small cell lung cancer and explore their clinical significance. Methods Immunohistochemistry was applied to confirm expression of Beclin1 and LC3 in 66 samples of NSCC and 52 samples of adjacent tissues. Clinical pathological features were analyzed. Results Beclin1 and LC3 expression were lower in cancer tissues than that in adjacent cancer tissues ,and the dif-ference was statistically significant(46.9%vs 75%,53%vs 80.7%,P<0.05). The expression of Beclin1 and LC3 was down-regulated with TNM staging rising and tissue differentiation decreasing. With tumor enlarging ,Beclin1 down-regulated,and the difference is statistically significant(P<0.05). Expression of Beclin1 was positively cor-related with LC3 expression(r=0.31,P<0.05). Conclusion The down-regulation of autophagy-related protein Beclinl and LC3 might be involved in the genesis and development of non small lung cancer.
8.Effects of maternal rats exposed to chronic unpredictable stress before pregnancy on the postpartum behavior and neurobiology of themselves and their progency

Objective To determine the effects of maternal rats exposed to chronic unpredictable stress before pregnancy on the behavior and neurobiology of the mother and their offspring.Methods Two-month Virgin female Sprague-Dawley(SD) rats were applied to study.Females were housed with sexually experienced males (ratio 3:1) for propagation after 21-day chronic unpredictable stress.The behaviors of rats and their two-month-offspring were tested.The two-month rats were injected intraperitoneally with BrdU,then the brains were removed and 20 μm frozen sections were used to detect the neurogenesis of hippocampus.Results 1.Open field test:after lactation,CUS + Pre group ((720.52 ± 238.99) cm),CUS group ((995.62 ± 191.77) cm),CON group ((464.95 ±210.11)cm) and CON + Pre group((740.76 ± 281.48)cm) had statistic difference in total distance(P < 0.05).The total distance((1281.10 ±247.89)cm) and border distance((1153.20 ±238.19)cm) in female two-monthprogeny were higher than these in male two-month-progeny ((1074.70 ± 263.35) cm,(957.28 ± 82.94) cm) in CUS + Pre group (P < 0.05).2.Sucrose consumption test:after lactation,the sucrose intake ((13.00 ± 3.46) g)and sucrose consumption percentage((87.00 ±9.01)%) in CUS + Pre group were higher than that in CUS group ((8.13 ± 3.36) g,(79.06 ± 6.45) %,P < 0.05).The sucrose intake ((12.43 ± 3.31) g) and sucrose consumption percentage((86.90 ± 5.80)%) in CON group were higher than that in CUS group ((8.13 ± 3.36) g,(79.06 ± 6.45) %,P < 0.05).The sucrose intake ((14.71 ± 4.39) g) and sucrose consumption percentage ((91.54 ± 1.89) %) in CON + Pre group were significantly higher than those in CUS group ((8.13 ± 3.36) g,(79.06 ± 6.45) %) (P < 0.01).3.Immunohistochemistry:there was no statistic difference on the new neuron in dentate gyrus of hippocampus in both female and male two-month-progeny of CUS group and CON group((1.18 ±0.37) cells,(1.24 ± 0.41) cells,(1.38 ± 0.47) cells,(1.41 ± 0.35) cells) (P > 0.05).Conclusion The 21d CUS before pregnancy induce the anxiety-like behavior and depressive-like behavior in maternal rats,and lactation can attenuate influence of stress to protect maternal rats.There is no effect on the behavior and cell proliferation of hippocampus in adult progeny by chronic unpredictable stress exposure before pregnancy.However,there is the difference of anxiety-like behavior in both female and male two-month-progeny.
9.Effect of antidepressant intervention on mood state of at or around the time of radiation therapy

Objective To explore the effect of antidepressant intervention on mood state of at or around the time of radiation therapy. Methods 62 hospitalized patients with cancer receiving radiotherapy were divided into two groups by the order of the time when they accept the radiotherapy which called intervention group ( antidepressant fluoxetine group) and control group. And the two groups patients filled out Hamilton Anxiety Scale( HAMA )and Hamilton Depression Scale (HAMD) at or around the time of radiation therapy to investigate the effects of fluoxetine on anxiety and depression. Results The scores of HAMA and HAMD showed that there was no differences between the intervention group and the control group before the radiotherapy. There was significant difference during the radiotherapy ( 6.41 ± 2.30 ) vs ( 9.29 ± 5.62 ); ( 8.80 ± 3. 048 ) vs ( 12.22 ± 8.32 ) and two weeks after the end of the radiotherapy ( 4.90 ± 1.71 ) vs ( 12.32 ± 7.24 ); ( 6.83 ± 2.47 ) vs ( 16.09 ± 8.61 ) between antidepressant fluoxetine group and control group(P < 0.05 ). Conclusion More attention should be paid to mental health and antidepressant can alleviate anxiety and depression of the cancer patients'at or around the time of radiotherapy.
10.Blood Sugar Meter′s Blood Sampling Gun Contaminated by Hepatitis B-type Virus and Its Disinfection: Present Situation Discussion

OBJECTIVE To understand the portable blood sugar meter′s blood sampling gun contaminated by hepatitis B-type virus,and to discuss the effect of two kinds clinical commonly used disinfection methods. METHODS The blood sampling gun of blood sugar meter was checked before and after 75% alcohol or 1% chlorine-containing disinfectant disinfection. RESULTS Before disinfection of the gun their were 35 samples from 47 lotions in disinfection with positive HBV-DNA,between two kinds of disinfectants were significant difference. CONCLUSIONS The blood sugar meter′s blood sampling gun contaminated by hepatitis B-type virus is seriously,75% alcohol is no use to disinfect the gun.It is suggested to supply two blood sampling guns per one blood sugar meter and use 1% chlorine-containing disinfectant for its disinfection of gun after its use in order to avoid the cross infection.
