This article systematically analyzed the historical evolution of the origin, scientific name, producing area, quality evaluation, harvesting and processing, and other aspects of Quisqualis Fructus by consulting the ancient materia medica, medical books, prescription books, local literature and combining with the modern literature and standards, summarized and explored the development rules of its medicinal properties and efficacy along with their underlying causes, in order to provide support for the development and utilization of famous classical formulas containing this herb. According to the textual research, Shijunzi was first recorded as Liuqiuzi in Nanfang Caomuzhuang of the Jin dynasty, and the name of Shijunzi was first used in Kaibao Bencao of the Song dynasty, which has been consistently used throughout subsequent dynasties, and there were also aliases such as Junziren, Sijunzi, and Dujilizi. The mainstream source of Quisqualis Fructus used in the past dynasties has been the dried mature fruits of Quisqualis indica, a plant belonging to the family Combretaceae. In modern times, its variety Q. indica var. villosa has also been recorded as the medicinal material of Quisqualis Fructus. In 2007, the Flora of China(English edition) designated Q. indica var. villosa as a synonym of Q. indica. Today, the accepted name of Shijunzi is updated to Combretum indicum. According to ancient herbal records, the producing areas of Quisqualis Fructus were Guangdong, Hong Kong, Macao, Guangxi, Hainan, Sichuan and Fujian, and then gradually expanded to Yunnan, Taiwan, Jiangxi and Guizhou. Since the Song dynasty, two major production regions have gradually emerged in Sichuan, Chongqing and Fujian. Currently, it is primarily cultivated in Chongqing, Guangxi and other areas, with Chongqing yielding the highest output. Since modern times, superior quality has been defined by large size, a purple-black surface, plump grains, and a yellowish-white kernel. According to ancient herbal records, the harvesting period of Quisqualis Fructus was the July and August of the lunar calendar, mostly used raw after shelling or with the shell intact, it underwent processing methods such as cleaning, slicing, mixing, steaming, roasting, stewing, and frying. Currently, the harvesting period is autumn, followed by sun-drying or low-heat drying, with processing methods including cleaning, stir-frying, and stewing. In ancient and modern literature, the records of the properties, functions and indications of Quisqualis Fructus are basically the same, that is, sweet in taste, warm in nature, predominantly non-toxic, belonging to the spleen and stomach meridians. It possesses effects of insecticide, decontamination and invigorating spleen for ascariasis, enterobiasis, abdominal pain due to worm accumulation and infantile malnutrition. The contraindications for use primarily include avoiding consumption by individuals without parasitic infestations, limiting use for those with spleen-stomach deficiency-cold, refraining from drinking hot tea during medication, and avoiding excessive intake. Based on the textual research, it is suggested that the dried mature fruits of Q. indica should be used as the medicinal material for the development of famous classical formulas containing Quisqualis Fructus. Processing methods may be chosen according to prescription requirements, and the raw products is recommended for medicinal use if not specified.

Adrenocortical carcinoma(ACC)is a rare and highly aggressive malignant tumor. Currently, mitotane is the first-line treatment. However, reports on neoadjuvant therapy for ACC using mitotane combined with immune checkpoint inhibitors remain scarce. This article reports a case of ACC. The patient was asymptomatic, and a right adrenal mass was detected during examination. Diagnostic imaging and endocrine evaluation confirmed the diagnosis of ACC. Due to the large tumor size, radical resection was initially considered unfeasible. After 7 months of mitotane therapy and two courses of tislelizumab, significant tumor shrinkage was achieved, allowing for successful open resection of the large right adrenal tumor combined with right nephrectomy. Postoperative histopathological examination confirmed the diagnosis of ACC. During the 3-month postoperative follow-up,no evidence of recurrence or metastasis was observed.

Objective:To develop and validate a deep learning-based multimodal model integrating clinical and radiomic features for predicting acute kidney injury(AKI)risk after partial nephrectomy. Methods:A retrospective analysis was conducted on 416 patients who underwent partial nephrectomy at Zhongshan Hospital,Fudan University from January 2023 to January 2025. The cohort included 100 AKI patients[defined by a ≥ 25% reduction in postoperative evaluated glomerular filtration rate(eGFR)within 48 hours sustained for >24 hours]and 316 non-AKI patients(1∶3 ratio, randomly matched with 16 additional cases for redundancy). Clinical and radiomic features were extracted from preoperative contrast-enhanced CT scans using PyRadiomics. Demographics included 259 males and 158 females, with a median age of 57(49,65)years,body mass index of(24.1 ± 3.3)kg/m2,preoperative eGFR of(88.5 ± 18.3)ml/(min·1.73 m2),postoperative eGFR(48-hour)of(76.0 ± 21.9)ml/(min·1.73 m2),Zhongshan Score(ZSscore)of 7.34 ± 2.01,and R.E.N.A.L. score of 7.50 ± 1.71. All tumors were T 1a stage. Patients were divided into training(n = 312)and test(n = 104)sets(3∶1 ratio). A clinical model was constructed via multivariate logistic regression, while radiomic and combined(clinical + radiomic)models utilized an artificial neural network(ANN)with 1 input layer,5 hidden layers,1 output layer, and 10 5 training epochs. Model performance was evaluated by using receiver operating characteristic(ROC)curves and area under the curve(AUC),and was compared to the Martini model. Feature contributions were interpreted via SHapley Additive exPlanations(SHAP). Results:In the test set,the results of multivariate logistic regression showed that patient’s weight, preoperative eGFR,R.E.N.A.L. score, surgical approach, and operation time were risk factors for AKI( P < 0.05). The AUC of the clinical feature prediction model constructed based on the above factors was 0.852(95% CI 0.775?0.929). In the test set,the AUC of the Martini model was 0.725(95% CI 0.565?0.791). The radiomic model, trained on 1 315 imaging features, achieved an AUC of 0.898(95% CI 0.804?0.993)with 94.2%(98/104)accuracy. The combined clinical and radiomic model, integrating 1 315 radiomic features and clinical features, demonstrated superior performance with an AUC of 0.946(95% CI 0.887?1.000)and 96.2%(100/104)accuracy,outperforming both the clinical model( P = 0.03)and the Martini model( P < 0.01). SHAP analysis identified the top five predictors in the combined model:ZSscore(SHAP value:0.78),long-run low gray-level emphasis(SHAP value:0.61),run-length non-uniformity(SHAP value:0.58),size-zone non-uniformity(SHAP value:0.46),and gray-level co-occurrence matrix joint energy(SHAP value:0.36). Conclusions:The deep learning-based multimodal model integrating clinical and radiomic features accurately predicts AKI risk after partial nephrectomy, offering a novel strategy for preoperative risk stratification and personalized intervention.

Objective:To compare the effect of two hemostatic drugs, tranexamic acid and hemocoagulase, on postoperative bleeding in adult patients with Kashin-Beck disease undergoing total knee arthroplasty. Methods:A retrospective analysis was conducted to select 80 adult patients with Kashin-Beck disease who underwent unilateral primary total knee arthroplasty at Linyou County Hospital from January 2021 to March 2023. According to the inclusion and exclusion criteria, 19 cases with chronic anemia, long-term oral anticoagulants implanted with cardiac stents, and hematological diseases were excluded. Among the remaining 61 adult patients, 24 cases with preoperative intravenous application combined with intraoperative local application of tranexamic acid in articular cavity were selected as the experimental group, and 37 cases with preoperative intravenous application combined with intraoperative local application of hemocoagulase in articular cavity were selected as the control group. The preoperative general condition, postoperative drainage volume, total perioperative blood loss, occult blood loss, and hemoglobin levels at different time points before and after surgery were compared between the two groups of patients. Results:There was no statistically significant difference in age, gender distribution, body mass index, preoperative hemoglobin level, preoperative prothrombin time, and preoperative activated partial thromboplastin time between the two groups of patients ( P > 0.05). The total perioperative blood loss [(1 027.78 ± 472.71) ml], drainage volume 48 h after surgery [(336.67 ± 112.74) ml] in the experimental group were all lower than those in the control group [(1 390.39 ± 454.01), (498.65 ± 187.57) ml], and the differences were statistically significant ( P < 0.05). The hemoglobin levels on the second and fifth day after surgery were significantly higher than those in the control group, and the differences were statistically significant ( P < 0.05). Conclusion:For total knee arthroplasty for adult patients with Kashin-Beck disease, preoperative intravenous application combined with intraoperative local application of tranexamic acid in articular cavity is superior to hemocoagulase, which can effectively reduce total perioperative blood loss and postoperative drainage volume, and is worthy of clinical promotion.
