1.Early prediction and warning of MODS following major trauma via identification of cytokine storm: A prospective cohort study.

PURPOSE:
Early mortality in major trauma has decreased, but MODS remains a leading cause of poor outcomes, driven by trauma-induced cytokine storms that exacerbate injuries and organ damage.
METHODS:
This prospective cohort study included 79 major trauma patients (ISS >15) treated in the National Center for Trauma Medicine, Peking University People's Hospital, from September 1, 2021, to July 31, 2023. Patients (1) with ISS >15 (according to AIS 2015), (2) aged 15-80 years, (3) admitted within 6 h of injury, (4) having no prior treatment before admission, were included. Exclusion criteria were (1) GCS score <9 or AIS score ≥3 for TBI, (2) confirmed infection, infectious disease, or high infection risk, (3) pregnancy, (4) severe primary diseases affecting survival, (5) recent use of immunosuppressive or cytotoxic drugs within the past 6 months, (6) psychiatric patients, (7) participation in other clinical trials within the past 30 days, (8) patients with incomplete data or missing blood samples. Admission serum inflammatory cytokines and pathophysiological data were analyzed to develop machine learning models predicting MODS within 7 days. LR, DR, RF, SVM, NB, and XGBoost were evaluated based on the area under the AUROC. The SHAP method was used to interpret results.
RESULTS:
This study enrolled 79 patients with major trauma, and the median (Q1, Q3) age was 51 (35, 59) years (52 males, 65.8%). The inflammatory cytokine data were collected for all participants. Among these patients, 35 (44.3%) developed MODS, and 44 (55.7%) did not. Additionally, 2 patients (2.5%) from the MODS group succumbed. The logistic regression model showed strong performance in predicting MODS. Ten key cytokines, IL-18, Eotaxin, MCP-4, IP-10, CXCL12, MIP-3α, MCP-1, IL-1RA, Cystatin C, and MRP8/14 were identified as critical to the trauma-induced cytokine storm and MODS development. Early elevation of these cytokines achieved high predictive accuracy, with an AUROC of 0.887 (95% CI 0.813-0.976).
CONCLUSION
Trauma-induced cytokine storms are strongly associated with MODS. Early identification of inflammatory cytokine changes enables better prediction and timely interventions to improve outcomes.

2.Preliminary study on the construction of an echocardiogram image quality control system based on artificial intelligence

Object:To explore the feasibility of using artificial intelligence for quality control of echocardiographic images.Methods:Retrospectively,5 000 two-dimensional echocardiographic video images within the period from 2021 to 2023 were randomly retrieved from the echocardiography database of Nanjing Drum Tower Hospital,Affiliated Hospital of Medical School,Nanjing University. Among these selected images,1 559 of them were apical views. The physician team formulated the scoring rules,which specifically included four scoring criteria:gain,scaling ratio,cardiac axis angle,and structure. Subsequently,the data were labeled with view classification and image quality scores. The labeled data were further partitioned into the training set( n = 643),the validation set( n = 276),and the test set( n = 640). The training and validation sets were utilized for constructing the models for view classification and quality assessment,while the test set was employed to verify the models' effectiveness. The view classification module was implemented using the SlowFast model,and the quality assessment module involved algorithms such as ResNet,Video Swin Transformer,SSD,and U-Net. Results:The average accuracy,precision,recall rate and F1 score of the classification model in identifying each apical view were 0.987 1,0.983 0,0.987 1 and 0.984 9 respectively,and the inference time was(333.4 ± 105.4)ms. The average accuracies of the quality assessment module in terms of gain,scaling ratio,cardiac axis angle and display of main structures were 0.915 1,0.928 2,0.938 7 and 0.965 6 respectively,and the overall scoring accuracy was 0.912 7.Conclusions:The echocardiogram quality control system developed in this research can effectively classify and evaluate the quality of two-dimensional images of the apical views in echocardiograms. Moreover,it guarantees the objectivity,timeliness and high-efficiency of quality control,which has reference value for the establishment of the echocardiogram quality control system.
3.Effects of tri(2-chloroethyl)phosphate on reproductive function of male mice:Based on non-targeted metabolomics

Objective To investigate the effects of organophosphate flame retardant tri(2-chloroethyl)phosphate(TCPP)on reproductive function in male mice by the approach of non-targeted metabolomics.Methods A total of twelve 6-week-old SPF male CD-1 mice were randomly divided into control group and TCPP group,with 6 mice in each group.TCPP group was given TCPP(at the dose of 100 mg/kg/d)intragastrically,and control group was intragastrically adminis-trated with the same amount of corn oil(solvent control).After 6 weeks,the mice were killed,and the sperm were isolated from the epididymis.The sperm concentration and viability were analyzed.Testicular tissue sections were stained with he-matoxylin,and ki67 expression was detected by immunohistochemistry.Non-targeted metabolomics was used to detect the difference of metabolites in testicular tissue between the two groups,and to analyze the difference of metabolites and related pathway changes between the two groups.Results Compared with control group,the total sperm motility of mice in TCPP treatment group was significantly decreased(P<0.05).HE staining showed irregular arrangement of spermatogenic tubule supporting cell layer in TCPP treatment group.The expression level of ki67 in testicular tissue of mice treated with TCPP was significantly decreased(P<0.05).Non-targeted metabolomics detected 266 up-regulated metabolites with statistical difference.And 554 down-regulated metabolites with statistical difference,among which the largest difference multiples were organic acids and amino acid metabolites.The pathways with the highest concentration of differential metabolites in-cluded purine metabolism,nucleotide metabolism,amino acid metabolism,cofactor synthesis,etc.,which were mainly re-lated to basic cell life activities,pathophysiology and chemical carcinogenesis.Conclusion TCPP can significantly re-duce sperm motility and ki67 expression in mice at the dosage of 100 mg/kg/d,which might be related to its influence on key metabolic pathways such as purine,amino acid and pyruvate.
4.Quantitative evaluation of left ventricular remodeling in left bundle branch block induced cardiomyopathy by multimodal imaging

Objective:To quantitatively evaluate the effects of left bundle branch block(LBBB)on left ventricular structure,function and myocardial perfusion using left ventricular pressure-strain loop and single photon emission computed tomography(SPECT),and to investigate the relationship between myocardial work,myocardial perfusion and pathological changes of left ventricular remodeling in left bundle branch block induced cardiomyopathy(LBBB-CM).Methods:Fourteen male beagle dogs were selected,and the main trunk of the left bundle branch was ablated to create an LBBB dog model. Electrocardiogram(ECG),transesophageal echocardiography and arterial blood pressure data of LBBB dogs were collected before and 12 months after the ablation of left bundle branch trunk. Global and segmental myocardial work parameters were obtained by left ventricular pressure-strain loop. The differences of above parameters between baseline and 12 months after the ablation of left bundle branch were compared. SPECT was performed in LBBB dogs 12 months after the creation of LBBB. The hearts were harvested for anatomy observation and histopathological analysis in LBBB dogs and another 7 male beagle dogs(normal control group)matched by age and weight. The correlation between myocardial perfusion(percentage of regional tracer uptake)and myocardial work parameters,myocardial fibrosis in LBBB dogs were analyzed.Results:Compared with baseline,the left ventricular end-diastolic volume of 12 months after the ablation increased[(20.78 ± 5.32)ml vs(26.71 ± 7.94)ml, P = 0.003],left ventricular ejection fraction decreased[(59.17 ± 5.67)% vs(47.69 ± 5.45)%, P<0.001];left ventricular global/segmental longitudinal strain,global/segmental constructive work and global/segmental work efficiency decreased(all P<0.05),left ventricular global/segmental wasted work increased(all P<0.001). Heterogenous perfusion defect was observed in LBBB dogs by SPECT,compared with lateral wall segments,the percentage of regional tracer uptake of septum was decreased(all P<0.05). Gross anatomical and myocardial pathological changes were manifested as cardiomegaly,flaky or focal grayish thickening of endocardium,cardiomyocyte degeneration and fibrosis. Compared with normal control group,the collagen fiber volume fraction(CVF)in all segmental endocardium and partial segmental myocardium of LBBB dogs were significantly increased(all P<0.05). Percentage of regional tracer uptake was positively correlated with segmental myocardial work(SMW)and segmental myocardial efficiency(SWE)( r s = 0.49,0.31;both P<0.001),and negatively correlated with CVF and segmental wasted work(SWW)( r s = -0.51,-0.49;both P<0.001). Conclusions:Isolated LBBB is not benign,which can result in left ventricular remodeling,decreased cardiac constructive function,abnormal myocardial perfusion,endocardial fibrosis and myocardial fibrosis.The parameters of myocardial work assecsed by echocardiograpgy and myocardial perfusion,as non-invasive examination,can to some extent reflect the degree of left ventricular remodeling in LBBB-CM.

8.Prescription investigation for potential adverse drug interactions based on pharmacokinetics of gefitinib and establishment of review rules

OBJECTIVE To analyze the potential adverse drug interactions based on pharmacokinetics(PK-pADIs)of gefitinib,and establish its corresponding prescription review rules.METHODS Outpatient prescriptions of gefitinib combination therapy in our hospital from January 1,2022 to November 30,2024 were collected through rational drug software system.PK-pADIs present in the prescriptions were identified based on the Drugs.com? drug interactions database.The specific combination drugs and cases of PK-pADIs were statistically analyzed,and prescription review rules were established according to the severity classification of PK-pADIs.RESULTS&CONCLUSIONS A total of 217 prescriptions of gefitinib combination therapy were enrolled.Among them,28 prescriptions(12.90%),involving a total of 28 patients,had 29 cases of PK-pADIs,with respiratory medicine prescriptions(22 prescriptions)being the main type.The combination drugs included proton pump inhibitors(13 cases),strong cytochrome P450 3A4(CYP3A4)inhibitors(7 cases),H2 receptor antagonists(4 cases),CYP3A4 inducers(3 cases),and CYP2D6 substrates(2 cases).The severity classifications for these interactions were severe,moderate,severe,moderate and moderate,respectively.Based on the above severity classification of PK-pADIs,four prescription review rules had been established as follows:when gefitinib was combined with acid-suppressing drugs,it should be subject to"manual review";when gefitinib was combined with dexamethasone,metoprolol,or strong CYP3A4 inhibitors,an"alert"should be triggered,and the physician should be informed via an alert box to strengthen the monitoring of relevant indicators.Clinical pharmacists need to conduct in-depth training on knowledge related to gefitinib drug interactions in key clinical departments such as respiratory medicine.They should strengthen the monitoring and guidance of rational drug use for patients who are on long-term gefitinib therapy,and promptly identify and intervene in PK-pADIs,thereby enhancing the rationality,safety,and effectiveness of clinical drug use.
9.Analysis of clarithromycin potential adverse drug interactions and establishment of refined prescription pre-review rules

OBJECTIVE To analyze the potential adverse drug interactions(pADIs)of clarithromycin,and establish refined prescription pre-review rules.METHODS Outpatient prescriptions of clarithromycin in combination with other drugs were collected from January 1,2024 to June 30,2024 through hospital information system of the Third People's Hospital of Bengbu.pADIs were identified and their risk severities were graded according to Lexicomp and Micromedex databases.Then,refined prescription pre-review rules for clarithromycin pADIs-related drugs were established according to the identification and risk level results.RESULTS Among 3 046 clarithromycin combined drug prescriptions,946 cases of pADIs occurred in 812 prescriptions.There were 6,415 and 525 cases classified as"contraindicated","major"and"moderate",respectively.The combination drugs with"contraindicated"levels were tamsulosin,rupatadine,domperidone and ticagrelor,while those with"major"levels were mainly theophylline,dexamethasone and amlodipine.Accordingly,26 refined rules were established,including 4 items of"warning information→prescription interception",11 items of"warning information→prescription double signature"and 11 items of"attention information→prescription approval".CONCLUSIONS There are"contraindicated"and"major"risks associated with clarithromycin and its combination drugs in the hospital,and refined prescription pre-review rules for clarithromycin combined drug prescription have been established successfully.
10.Deep learning models semi-automatic training system for quality control of transthoracic echocardiography

Objective To explore the value of deep learning(DL)models semi-automatic training system for automatic optimization of clinical image quality control of transthoracic echocardiography(TTE).Methods Totally 1 250 TTE videos from 402 patients were retrospectively collected,including 490 apical four chamber(A4C),310 parasternal long axis view of left ventricle(PLAX)and 450 parasternal short axis view of great vessel(PSAXGv).The videos were divided into development set(245 A4C,155 PLAX,225 PSAXGV),semi-automated training set(98 A4C,62 PLAX,90 PSAXGV)and test set(147 A4C,93 PLAX,135 PSAXGV)at the ratio of 5:2:3.Based on development set and semi-automatic training set,DL model of quality control was semi-automatically iteratively optimized,and a semi-automatic training system was constructed,then the efficacy of DL models for recognizing TTE views and assessing imaging quality of TTE were verified in test set.Results After optimization,the overall accuracy,precision,recall,and F1 score of DL models for recognizing TTE views in test set improved from 97.33%,97.26%,97.26%and 97.26%to 99.73%,99.65%,99.77%and 99.71%,respectively,while the overall accuracy for assessing A4C,PLAX and PSAXGV TTE as standard views in test set improved from 89.12%,83.87%and 90.37%to 93.20%,90.32%and 93.33%,respectively.Conclusion The developed DL models semi-automatic training system could improve the efficiency of clinical imaging quality control of TTE and increase iteration speed.
