1.Early prediction and warning of MODS following major trauma via identification of cytokine storm: A prospective cohort study.

PURPOSE:
Early mortality in major trauma has decreased, but MODS remains a leading cause of poor outcomes, driven by trauma-induced cytokine storms that exacerbate injuries and organ damage.
METHODS:
This prospective cohort study included 79 major trauma patients (ISS >15) treated in the National Center for Trauma Medicine, Peking University People's Hospital, from September 1, 2021, to July 31, 2023. Patients (1) with ISS >15 (according to AIS 2015), (2) aged 15-80 years, (3) admitted within 6 h of injury, (4) having no prior treatment before admission, were included. Exclusion criteria were (1) GCS score <9 or AIS score ≥3 for TBI, (2) confirmed infection, infectious disease, or high infection risk, (3) pregnancy, (4) severe primary diseases affecting survival, (5) recent use of immunosuppressive or cytotoxic drugs within the past 6 months, (6) psychiatric patients, (7) participation in other clinical trials within the past 30 days, (8) patients with incomplete data or missing blood samples. Admission serum inflammatory cytokines and pathophysiological data were analyzed to develop machine learning models predicting MODS within 7 days. LR, DR, RF, SVM, NB, and XGBoost were evaluated based on the area under the AUROC. The SHAP method was used to interpret results.
RESULTS:
This study enrolled 79 patients with major trauma, and the median (Q1, Q3) age was 51 (35, 59) years (52 males, 65.8%). The inflammatory cytokine data were collected for all participants. Among these patients, 35 (44.3%) developed MODS, and 44 (55.7%) did not. Additionally, 2 patients (2.5%) from the MODS group succumbed. The logistic regression model showed strong performance in predicting MODS. Ten key cytokines, IL-18, Eotaxin, MCP-4, IP-10, CXCL12, MIP-3α, MCP-1, IL-1RA, Cystatin C, and MRP8/14 were identified as critical to the trauma-induced cytokine storm and MODS development. Early elevation of these cytokines achieved high predictive accuracy, with an AUROC of 0.887 (95% CI 0.813-0.976).
CONCLUSION
Trauma-induced cytokine storms are strongly associated with MODS. Early identification of inflammatory cytokine changes enables better prediction and timely interventions to improve outcomes.

2.Effects of tri(2-chloroethyl)phosphate on reproductive function of male mice:Based on non-targeted metabolomics

Objective To investigate the effects of organophosphate flame retardant tri(2-chloroethyl)phosphate(TCPP)on reproductive function in male mice by the approach of non-targeted metabolomics.Methods A total of twelve 6-week-old SPF male CD-1 mice were randomly divided into control group and TCPP group,with 6 mice in each group.TCPP group was given TCPP(at the dose of 100 mg/kg/d)intragastrically,and control group was intragastrically adminis-trated with the same amount of corn oil(solvent control).After 6 weeks,the mice were killed,and the sperm were isolated from the epididymis.The sperm concentration and viability were analyzed.Testicular tissue sections were stained with he-matoxylin,and ki67 expression was detected by immunohistochemistry.Non-targeted metabolomics was used to detect the difference of metabolites in testicular tissue between the two groups,and to analyze the difference of metabolites and related pathway changes between the two groups.Results Compared with control group,the total sperm motility of mice in TCPP treatment group was significantly decreased(P<0.05).HE staining showed irregular arrangement of spermatogenic tubule supporting cell layer in TCPP treatment group.The expression level of ki67 in testicular tissue of mice treated with TCPP was significantly decreased(P<0.05).Non-targeted metabolomics detected 266 up-regulated metabolites with statistical difference.And 554 down-regulated metabolites with statistical difference,among which the largest difference multiples were organic acids and amino acid metabolites.The pathways with the highest concentration of differential metabolites in-cluded purine metabolism,nucleotide metabolism,amino acid metabolism,cofactor synthesis,etc.,which were mainly re-lated to basic cell life activities,pathophysiology and chemical carcinogenesis.Conclusion TCPP can significantly re-duce sperm motility and ki67 expression in mice at the dosage of 100 mg/kg/d,which might be related to its influence on key metabolic pathways such as purine,amino acid and pyruvate.
3.Right heart contrast-enhanced echocardiographic diagnosis of a left superior vena cava aneurysm

Permanent left superior vena cava aneurysm is a rare mediastinal venous lesion,the majority of the literature is in the form of case reports. In this article,a 50-year-old female with an incidental permanent left superior vena cava aneurysm discovered on echocardiography in Nanjing Gulou Hospital,and the right heart contrast-enhanced echocardiographic played an important role in the process of clear diagnosis. The patient was observed without intervention. No complications were attributable to the permanent left superior vena cava aneurysm during follow-up.

Stiff person syndrome (SPS) is a rare neuro-immune disorder characterized by exaggerated startle responses and muscle spasms in response to minor stimuli. SPS is often misdiagnosed as panic disorder or agoraphobia. This paper reported a case of a patient with SPS presenting symptoms similar to panic attack. The patient had been misdiagnosed as dystonia, anxiety disorder and panic attack. The patient was finally correctly diagnosed through multiple disciplinary team discussions. This study integrates a comprehensive understanding of symptomatology and patient-reported experiences with targeted neurological evaluations and supplementary diagnostic tools to improve the accuracy of etiological diagnoses. This study aims to enhance the ability of psychiatrists to recognize somatic diseases presenting with psychiatric symptoms and to improve clinical diagnostic and treatment levels.

6.Application of polymyxin B in treatment of patients with severe infections and distribution of plasma concentration

OBJECTIVE To analyze the clinical characteristics of the patients with severe infections who were treated with polymyxin B and compare the plasma concentration of polymyxin B among the patients with severe infections with different creatinine clearance rates(Ccr).METHODS The clinical data were collected from 152 patients with severe infections who were treated with intravenous polymyxin B in intensive care unit(ICU)of Zhongda Hospital Affiliated to Southeast University from Jan.2021 to Mar.2023.The trough concentration(Cmin),median concen-tration(C1/2t)and peak concentration(Cmax)of polymyxin B were determined after 5 doses were completed.Based of the area under the curve(AUC)of drug concentration of polymyxin B(AUC0~24h)combined with the Ccr level[the ≤30 to<60 ml/min group(n=40),the 60 to<130ml/min group(n=79),and the ≥ 130ml/min group(n=33)],the AUC0~24h of polymyxin B were calculated,and the influence of Ccr on the plasma concentration was observed.RESULTS Among the 152 patients with severe infections,118 were male,and 34 were female,with the age ranging between 20 and 90 years old;the patients with high blood pressure accounted for 14.47%(22/152),the patients with diabetes mellitus 7.24%(11/152).Polymyxin B is primarily used in clinical settings for the treatment of pulmonary infections and bloodstream infections caused by carbapenem-resistant Acinetobacter baumannii and Klebsiella pneumoniae.The median initial dose and the median maintenance dose were 1.35(1.00,1.67)mg/kg q12 h and 1.07(0.83,1.25)mg/kg q12 h,respectively.The median AUC0~24h of polymyxin B was 76.57(54.65,106.47)μg·h/ml among the 152 patients,and the patients with AUC0~24 h ranging between 50 and 100 μg·h/ml accounted for 53.95%(82/152).Although the median AUC0~24h of polymyxin B of all the three groups ranged between 50 and 100)μg·h/ml,there were significant differences in Cmin,C1/2t,Cmax and AUC0~24h among the three groups(P<0.05).In addition,there were significant differences in the AUC0~24h of polymyxin B less than 50 μg·h/ml,ranging between 50 and 100 μg·h/ml and more than 100 μg·h/ml among the three groups of patients(x2=21.632,P<0.001).CONCLUSION Therapeutic drug monitoring(TDM)is nec-essary for the patients with severe infection who receive the polymyxin B for treatment,especially for the patients with Ccr ≤30 to<60 ml/min and Ccr ≥130 ml/min.

10.Analysis of hospital infectious disease prevention and control policies from the perspective of public health risk governance

Objective:To quantitatively analyze the hospital infectious disease prevention and control policy texts in China, providing reference for optimizing the formulation of hospital infectious disease prevention and control policies and enhancing the public health risk management capabilities of infectious diseases.Methods:With the " hospital" " epidemic" " public health" " risk management" as keywords, relevant policy documents issued from October 2017 to December 2023 were searched on the official websites of the State Council and the National Health Commission. Based on the perspective of public health risk governance for infectious diseases, the content analysis method was used to quantitatively analyze policy texts from three dimensions: policy tools (supply oriented, demand oriented, and environmental oriented), policy objects (hospitals and medical staff as internal objects, grassroots medical and health institutions, disease prevention and control institutions, and health administrative authorities as external objects), and risk management lifecycle (risk identification, risk assessment, risk control, and risk management effectiveness evaluation).Results:A total of 98 hospital infectious disease prevention and control policies were included, with a total of 539 codes obtained. Among them, there were 134, 179, and 226 policy items for supply, demand, and environment oriented, respectively. There were 239, 25, 24, 26, and 225 policy items for hospitals, medical staffs, grassroots medical and health institutions, disease prevention and control institutions, and health administrative authorities, respectively. In the stages of risk identification, risk assessment, risk control, and risk management effectiveness evaluation, there were 117, 34, 326, and 62 policy items, respectively. Three dimensional cross analysis revealed that in the risk identification stage, environmental policy tools were often used to strengthen the responsibilities of hospitals (29/117) and health administrative authorities (23/117); The main policy tools used in the risk assessment stage were demand based tools for the health administrative authorities (6/34); During the risk control stage, three types of policy tools were used to propose requirements for each policy object; The evaluation stage of risk management effectiveness mainly used environmental policy tools to regulate the main responsibility of health administrative authorities (33/62), which was not reflected in grassroots medical and health institutions, medical staffs, and disease prevention and control institutions.Conclusions:In China′s hospital infectious disease prevention and control policies, there were differences in the design and application of various policy tools and risk management policies at different stages, and the division of responsibility for policy objects still needed to be optimized. Policy makers should make reasonable use of policy tools, strengthen the full lifecycle management and policy coverage of infectious disease risks from the perspective of public health governance, clarify the division of responsibilities among various policy objects, and improve the hospital′s ability to manage infectious disease public health risks.
