1.Study on the liver function protective effect and mechanism of ethanol extract from Atractylodes lancea

Objective Based on the network pharmacology and animal experiments,to investigate the protective effect and possible molecular mechanism of ethanol extract from Atractylodes lancea on liver function in mice with liver fibrosis induced by bile duct ligation.Methods The main active ingredients atractylodin,atractylenolide Ⅰ,Ⅱ and Ⅱ from Atractylodes lancea were selected,which had been verified by literature and experiments,and the targets of these active ingredients were obtained through the SwissTargetPrediction database.The liver fibrosis disease targets were obtained through On-line Mendelian Inheritance in Man (OMIM),DisGeNET and GeneCards databases.The targets were added to the Wei Sheng Xin platform to find the intersection target for Atractylodes lancea in treating liver fibrosis.Cytoscape 3.10.1 was used to construct the "drug-component-target-disease" network diagram and protein-protein interaction core target network diagram.GO functional enrichment analysis and KEGG pathway analysis were performed,and molecular docking was performed between active components and core targets.Liver fibrosis was induced in mice by bile duct ligation,and liver function markers were measured.Results A total of 91 corresponding targets of atractylodin,atractylenolide Ⅰ,Ⅱ and Ⅲ and 9296 liver fibrosis disease targets were obtained,including 74 intersecting targets and 31 core targets.KEGG enrichment analysis showed that the main signaling pathways involved included inflammatory pathways such as epidermal growth factor receptor (EGFR) and phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt).Molecular docking results showed that the active ingredients had strong binding activity with the core target protein.The results of animal experiments showed that,compared with the sham surgery group,the model group displayed notable,the liver index,spleen index,activity of serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST),degree of liver fibrosis,mRNA and protein expression of α-smooth muscle actin (α-SMA) and recombinant collagen type Ⅰ alpha 1 (COL1A1),and mRNA of recombinant collagen type Ⅳ alpha 2 (COL4A2) were significantly increased,and the thymus index was sigficantly decreased (P<0.05);compared with the model group,the liver injury of mice in the Atractylodes lancea administration group reduced liver injury,its liver index,spleen index,activity of serum ALT and AST,degree of liver fibrosis,mRNA and protein expression of α-SMA and COL1A1,and mRNA of COL4A2 were significantly decreased,and the thymus index was sigficantly increased (P<0.05).Conclusion Atractylodes lancea can improve liver function and alleviate tissue pathological damage in mice with liver fibrosis,which may be related to activating pathways such as PI3K/Akt,inhibiting oxidative stress and inflammatory reactions,and intervening in liver fibrosis.
2.Design of Vibration Signal Acquisition System for Artificial Knee Joint Based on Dynamic Simulation

Objective The stress variations during the wear process of an artificial knee joint were studied.Then,a signal acquisition system was designed to capture the vibration signals induced by the wear of knee joint prosthesis.The aim was to provide new technical means for online wear monitoring of the artificial knee joint.Methods To effectively collect vibration signals,the optimal installation position of the vibration sensors was determined by analyzing the dynamic model of the knee joint prosthesis during motion and identifying the main distribution areas of the tibial insert contact stress.The dynamic model of the femoral prosthesis was solved using Lagrangian equations.The torque variation curve of the femoral prosthesis was obtained to validate the effectiveness of finite element analysis.The signals collected by the vibration sensors installed at different positions in the friction wear experiments and the surface morphology in different areas were compared to verify the effectiveness of the acquisition system design and finite element analysis results.Results The stress concentration regions of the tibial pad under four degrees of freedom(flexion,internal and external rotation,anterior-posterior displacement,and up-and-down displacement)were obtained based on a dynamic simulation.A stress concentration was evident in the middle and posterior regions of the tibial pad.A vibration signal with a higher amplitude was collected when the vibration sensor was installed at the rear end of the tibial pad.This aided the vibration feature extraction of the knee joint prosthesis.Conclusions The vibration signal acquisition system designed based on the dynamic simulation analysis effectively collected the vibration signals generated by the artificial knee joint during the wear process.This study provides an important means for evaluating the wear mechanisms of artificial knee joints and monitoring their full-life health status.

4.The influence of patellofemoral joint degeneration on outcome of Oxford medial unicompartmental knee arthroplasty

Objective:To explore the influence of lateral patellofemoral joint degeneration on the treatment of anteromedial osteoarthritis of knee joint by Oxford medial unicompartmental knee arthroplasty.Methods:The clinical data of 73 patients (73 knees) with knee osteoarthritis underwent unicompartmental knee arthroplasty at Department of Orthopaedic Surgery, Xuanwu Hospital, Capital Medical University from March 2016 to December 2017 were analysed respectively.There were 18 males and 55 females, aged (68.6±7.5) years(range: 53 to 89 years).The lateral patellofemoral joints of patients were evaluated by Ahlback grading system. Patients with Ahlback 0 andⅠ were in the non degenerative group (37 cases), and those with Ahlback Ⅱ and above were in the degenerative group (36 cases). Hospital for special surgery knee score(HSS) and the Western Ontario and McMaster Universities(WOMAC) osteoarthritis index, as well as the condition of kneeling, sit to stand movement, up stair and down stair were recorded. The data before and after operation were compared by paired sample t test, and the data between groups were compared by independent sample t test. χ 2 test was used for counting data. Pearson correlation analysis was used to compare the correlation between ahlback score, HSS and WOMAC osteoarthritis index. Results:The follow-up time was (35.1±6.6) months (range: 25 to 47 months).The knee function of the patients improved significantly after operation.The HSS score increased from 57.7±11.8 preoperative to 81.8±7.8 postoperative ( t=16.64, P=0.00) and WOMAC osteoarthritis index decreased from 48.9±13.4 preoperative to 15.6±8.8 postoperative ( t=20.48, P=0.00). There was no statistical difference in the change of HSS between the degenerative group and the non-degenerative group before and after surgery(27.5±12.2 vs. 22.5±12.3, t=-1.65, P=0.10) as well as the change of WOMAC osteoarthritis index(31.8±14.0 vs. 36.4±13.7, t=-1.35, P=0.18), but the lateral patellofemoral joint degeneration was related to inability to complete squats (χ 2=5.17, P=0.04) and sitting up (χ 2=7.22, P=0.01). Conclusion:The degeneration of lateral patellofemoral joint has no effect on the early functional recovery of patients with anteromedial knee osteoarthritis after Oxford medial unicompartmental knee arthroplasty.

6.Efficacy and safety of Chushi pellets in patients with subacute eczema of more dampness less heat type

Objective To evaluate the clinical efficacy and safety of Chushi pellets in patients with subacute eczema of more dampness less heat type.Methods A randomized,double blind,double-dummy and positive parallel-controlled study was performed.Totally,129 patients with subacute eczema were collected at the dermatology clinic in Beijing Hospital of Traditional Chinese Medicine from May 2010 to October 2011,and classified into two groups to be treated with Chushi pellets and placebo capsules (n =66,experiment group) or Shiduqing capsules and placebo watered pills (n =63,control group) for 4 weeks.Follow-up visits were scheduled on day 1 (baseline),and 4 weeks after the beginning of treatment.Clinical endpoints included skin rash area,color,morphology,degree of pruritus and laboratory examination results.The therapeutic effect and safety of Chushi pellets were evaluated.The differences in clinical parameters were assessed by using t test and Wilcoxon rank-sum test between pre-and post-treatment,and by Pearson chi-square test or Fisher's exact test between the experiment and control group.Results Finally,the treatment outcome of 123 patients were assessed for the efficacy of Chushi pellets.At the end of the treatment,the total response rate was 69.3% (43/62) in the experiment group,and 59.0% (36/61) in the control group (x2 =1.43,P > 0.05).The improvement in pruritus was greater in the experiment group than in the control group (x2 =4.05,P < 0.05).Side effects mainly included loose stool and stomach upset,with an incidence of 13.6% (9/66) in the experiment group,and 11.1% (7/63) in the control group.No marked changes were observed in laboratry parameters after treatment in either of the groups.Conclusions Chushi pellets are safe and effective for the treatment of subacute eczema of more dampness less heat type.Compared with Shiduqing capsules,Chushi pellets shows an equivalent overall efficacy but a stronger antipruritic effect.
