Porcine reproductive and respiratory syndrome virus(PRRSV)mainly causes sow abor-tion,stillbirth,mummified fetus and respiratory symptoms in piglets.Since first reported in China in 1996,the virus complexity has increased significantly in more than 20 years of genetic evolution,bringing huge economic losses to the pig industry.In recent years,with the emergence of various PRRSV recombinant virus strains,preventing and controlling this epidemic became increasingly difficult.The purpose of this article is to comprehensively review the genome structure and func-tion of PRRSV,RNA virus recombination mechanism,main types of recombination,and the epi-demic status and recombination for the dominant epidemic PRRSV strains,in order to provide clues for in-depth research on gene recombination of PRRSV,thus providing the theoretical sup-port for formulating scientific prevention and control strategies.
2.Effects of miR-34b-5p on atherosclerosis-associated foam cell formation and inflammatory response by targeting IGFBP1 expression

Objective:To investigate the role of miR-34b-5p in formation of atherosclerosis(AS)-associated foam cells and in-flammation,and its possible mechanism.Methods:Expression levels of miR-34b-5p and insulin-like growth factor binding protein-1(IGFBP1)mRNA in serum of 34 AS patients and 20 healthy controls were detected.RAW264.7 macrophages were treated with oxi-dized low-density lipoprotein(ox-LDL)to construct AS foam cell model,and expression levels of miR-34b-5p and IGFBP1 mRNA in foam cells were detected.Interaction between miR-34b-5p and IGFBP1 was verified by dual luciferase reporter gene assay.miR-34b-5p inhibitor(inhibitor),inhibitor negative control(inhibitor-NC),IGFBP1 siRNA plasmid(si-IGFBP1)and siRNA negative control(si-NC)were separately or co-transfected into RAW264.7 macrophages.Lipid deposition ability of foam cells was observed,and levels of intracellular total cholesterol(TC),IL-1β,IL-6 and TNF-α were detected.Results:Compared with healthy controls,miR-34b-5p was highly expressed in serum of AS patients(P<0.05),while expression of IGFBP1 mRNA was lower(P<0.05).ox-LDL treatment significantly increased expression of miR-34b-5p(P<0.05),while significantly reduced expression of IGFBP1 mRNA in RAW264.7 macrophages(P<0.05).Dual luciferase reporter assay showed that IGFBP1 was the target gene of miR-34b-5p.Compared with inhibi-tor-NC group,expression of IGFBP1 protein in RAW264.7 macrophages in inhibitor group was increased significantly(P<0.05).After ox-LDL induction,miR-34b-5p inhibitor inhibited lipid deposition ability of macrophages,and decreased levels of intracellular TC,IL-1β,IL-6 and TNF-α.Interfering expression of IGFBP1 gene could reverse the intervention effect of miR-34b-5p inhibitor on macro-phages by enhance lipid deposition ability of macrophages,and increase levels of intracellular TC,IL-1β,IL-6 and TNF-α.Conclu-sion:Down-regulation of miR-34b-5p expression can inhibit the formation of AS-associated foam cells and inflammatory response,and the mechanism may be realized by targeting up-regulation of IGFBP1 expression.

The incidence of osteoporotic thoracolumbar vertebral fracture (OTLVF) in the elderly is gradually increasing. The kyphotic deformity caused by various factors has become an important characteristic of OTLVF and has received increasing attention. Its clinical manifestations include pain, delayed nerve damage, sagittal imbalance, etc. Currently, the definition and diagnosis of OTLVF with kyphotic deformity in the elderly are still unclear. Although there are many treatment options, they are controversial. Existing guidelines or consensuses pay little attention to this type of fracture with kyphotic deformity. To this end, the Lumbar Education Working Group of the Spine Branch of the Chinese Medicine Education Association and Editorial Committee of Chinese Journal of Trauma organized the experts in the relevant fields to jointly develop Clinical guidelines for the diagnosis and treatment of osteoporotic thoracolumbar vertebral fractures with kyphotic deformity in the elderly ( version 2024), based on evidence-based medical advancements and the principles of scientificity, practicality, and advanced nature, which provided 18 recommendations to standardize the clinical diagnosis and treatment.
6.Multidisciplinary treatment of locally advanced thyroid cancer:a case report

Differentiated thyroid carcinoma (DTC) is the most common endocrine tumor,characterized by high incidence,low recurrence rate,and low mortality. Locally advanced thyroid cancer (LATC) is rare in clinical practice,with poor prognosis,and is one of the leading causes of death among thyroid cancer patients. In August 2024,the Thyroid Surgery Department of China-Japan Union Hospital of Jilin University admitted a 61-year-old male patient with a history of "neck mass surgery 16 years ago and progressive enlargement with ulceration of the mass over the past 3 months." Physical examination revealed an irregularly shaped mass measuring approximately 12 cm × 12 cm in the left supraclavicular fossa,with a dark red surface,ulceration,bleeding,and necrotic exudate in some areas. After admission,a multidisciplinary team (MDT) consultation was initiated,and the patient underwent his fifth surgery,which included resection of the left cervical mass,left cervical lymphadenectomy,resection of a submental mass,and free flap transplantation. The patient was discharged on postoperative day 10 in good condition. One month after surgery,a follow-up ultrasound at our hospital showed no significant abnormalities in the neck,with good healing of the local skin and survival of the transplanted flap. This article reviews the MDT treatment process of this LATC case and summarizes the characteristics of LATC based on domestic and international literature,providing experience and references for the comprehensive treatment of this disease.
7.Risk prediction models of dangerous behaviors among patients with severe mental disorder in community

BackgroundThe occurrence rate of dangerous behaviors in patients with severe mental disorders is higher than that of the general population. In China, there is limited research on the prediction of dangerous behaviors in community-dwelling patients with severe mental disorders, particularly in terms of predicting models using data mining techniques other than traditional methods. ObjectiveTo explore the influencing factors of dangerous behaviors in community-dwelling patients with severe mental disorders and testing whether the classification decision tree model is superior to the Logistic regression model. MethodsA total of 11 484 community-dwelling patients with severe mental disorders who had complete follow-up records from 2013 to 2022 were selected on December 2023. The data were divided into a training set (n=9 186) and a testing set (n=2 298) in an 8∶2 ratio. Logistic regression and classification decision trees were separately used to establish predictive models in the training set. Model discrimination and calibration were evaluated in the testing set. ResultsDuring the follow-up period, 1 115 cases (9.71%) exhibited dangerous behaviors. Logistic regression results showed that urban residence, poverty, guardianship, intellectual disability, history of dangerous behaviors, impaired insight and positive symptoms were risk factors for dangerous behaviors (OR=1.778, 1.459, 2.719, 1.483, 3.890, 1.423, 2.528, 2.124, P<0.01). Being aged ≥60 years, educated, not requiring prescribed medication and having normal social functioning were protective factors for dangerous behaviors (OR=0.594, 0.824, 0.422, 0.719, P<0.05 or 0.01). The predictive effect in the testing set showed an area under curve (AUC) of 0.729 (95% CI: 0.692~0.766), accuracy of 70.97%, sensitivity of 59.71%, and specificity of 72.05%. The classification decision tree results showed that past dangerous situations, positive symptoms, overall social functioning score, economic status, insight, household registration, disability status and age were the influencing factors for dangerous behaviors. The predictive effect in the testing set showed an AUC of 0.721 (95% CI: 0.705~0.737), accuracy of 68.28%, sensitivity of 64.46%, and specificity of 68.60%. ConclusionThe classification decision tree does not have a greater advantage over the logistic regression model in predicting the risk of dangerous behaviors in patients with severe mental disorders in the community. 9.Topological properties of brain functional networks in adolescents with major depressive disorder

Objective:To investigate the topological properties of whole-brain functional networks in first-episode drug-na?ve adolescents with major depressive disorder (MDD).Methods:Seventy-six first-episode drug-na?ve adolescents with MDD admitted to Department of Neurology, Xiangyang No.1 Hospital Affilated to Hubei University of Medicince from January 2022 to January 2023 were selected as study subjects; 66 gender- and age-matched healthy controls (HCs) were recruited via advertisement. All subjects underwent resting-state functional MRI. The whole-brain functional networks were constructed for each subject; and then, the global topological metrics (global efficiency, local efficiency, clustering coefficient, characteristic path length, normalized clustering coefficient, normalized characteristic path length, and small-worldness properties) and local topological metrics (nodal degree centrality, nodal efficiency and nodal betweenness centrality) of the functional brain networks were analyzed between the two groups using graph-theory methods. Network-based statistics were used to examine between-group differences in functional connectivity strength of whole brain networks. Results:Small-worldness properties were demonstrated in both MDD group and HC group. MDD patients showed significantly higher global efficiency (0.129[0.124, 0.132] vs. 0.131[0.128, 0.133]), significantly lower clustering coefficient and characteristic path length (0.143[0.139, 0.146] vs. 0.139[0.135, 0.144]; 0.457[0.446, 0.734] vs. 0.451[0.440, 0.463]), and significantly increased nodal centralities in the right inferior parietal lobule, bilateral caudate nucleus and bilateral thalamus of brain functional networks compared with HCs ( P<0.05, FDR-corrected). Compared with HCs, MDD patients exhibited obviously lower functional connectivity strength in the orbitofrontal-temporal and anterior cingulate-limbic-temporal circuits. Conclusion:Abnormal alterations of topological properties of the brain functional networks are found in adolescents with MDD, which may be the underlying neuropathologic basis for MDD.
10.Effect of sanguinarine combined with cisplatin on the apoptosis of bladder cancer EJ cells

To investigate the effects of sanguinarine (Sang) combined with cisplatin (Cis) in accelerating the apoptosis of bladder cancer EJ cells, CCK-8 method was used to detect the proliferation of bladder cancer EJ cells treated with different concentrations of Sang with the IC50 values calculated. Annexin V FITC/PI method was used to detect cell apoptosis in the control group, Sang group, Cis group and the combination group. Flow cytometer was used to detect cell cycle arrested. Western blot was used to detect the influence of Bcl-2 expression in the control group, Sang group, Cis group and the combination group. Nude mouse subcutaneous tumor model was constructed to verify that the combination group could accelerate the apoptosis of bladder cancer EJ cells and reduce the side-effects on mice. The safety of the Sang was evaluated by HE staining of vital organs in mice. In vitro, Sang could significantly inhibit the proliferation of EJ cells. Compared with the control group, the number of apoptosis EJ cells in the combination group was significantly increased (P < 0.05), and more cells were arrested in G2/M phase. The expression of Bcl-2 was significantly down-regulated in the combination group (P <0.001). In vivo, compared with the control group, the tumor growth was significantly slower, and a large number of apoptotic cells were inspected (P < 0.05) of the combination group. The side effects of cisplatin were reduced in the combination group. Sang has high biosafety and little side effect. Combined Sang and Cis can increase cell cycle G2/M block, down-regulate Bcl-2 expression, promote cell apoptosis and inhibit tumor growth.
