Postoperative infection of internal fixation of closed fractures the lower limbs in adults represents a devastating complication, characterized by diagnostic challenges, prolonged treatment duration and high disability rates. Current management of these infections faces multiple challenges, such as difficulties in early accurate diagnosis, and various controversies about the treatment plan, leading to poor overall diagnosis and treatment results. To address these issues, based on evidence-based medicine and principles with emphasis on scientific rigor, clinical applicability and innovation, the Trauma Branch of the Chinese Medical Association, Orthopedic Branch of the Chinese Medical Doctor Association, Orthopedics Branch of the Chinese Medical Association, and Trauma Orthopedics and Polytrauma Group of the Resuscitation and Emergency Committee of the Chinese Medical Doctor Association have collaboratively organized a panel of relevant experts to develop the Guideline for diagnosis and treatment of infection after internal fixation of closed lower limb fractures in adults ( version 2025). The guideline proposed 10 recommendations, aiming to provide a foundation for standardized diagnosis and treatment of postoperative infection in adults with closed lower limb fractures.
3.A multicenter randomized controlled study on the effect of acellular dermal matrix combined with hydroconductive dressing on wound healing of diabetic foot

Objective:To investigate the effect of acellular dermal matrix (ADM) combined with hydroconductive dressing (HD) on wound healing of diabetic foot.Methods:In this study, 141 admitted patients with diabetic foot from Jun 2022 to Jun 2024 were randomly divided into three groups: control group, ADM group and ADM+HD group. The clinical data of all patients were collected and the differences of each index were compared.Results:There were 47 cases in each group. After 4 weeks of treatment, there were significant differences in wound area and VAS score among the three groups ( P<0.05). The number of ADM replacement in ADM+HD group was less and the incidence of adverse reactions was lower than that in ADM group ( P<0.05). Conclusion:ADM combined with HD has significant advantages in accelerating wound healing, relieving pain, reducing the number of dressing changes and reducing the incidence of infection.

Objective To investigate the expression of ferritinophagy-related gene ELAV-like RNA binding protein 1(ELAVL1)in multiple myeloma(MM)and elucidate its diagnostic and prognostic value for MM.Methods First,we analyzed ELAVL1 expression level in healthy controls and MM patients using data from the GEO and TCGA databases.Subsequently,bone marrow specimens were collected from 28 newly diagnosed MM patients and 20 healthy controls,and qRT-PCR was employed to validate ELAVL1 expression.The diagnostic and prognostic potential of ELAVL1 was assessed using ROC curve analysis and Kaplan-Meier survival curves.Additionally,univariate and multivariate COX regression analyses were performed to identify independent risk factors for MM prognosis.Finally,KEGG and GO enrichment analyses were performed using the DAVID online platform.Results The level of ELAVL1 expression was significantly higher in newly diagnosed MM patients and refractory/relapsed MM patients than in the healthy controls(P<0.001).Moreover,ELAVL1 expression was positively correlated with the International Staging System(ISS)stage of MM(P<0.01).Furthermore,qRT-PCR validation confirmed that ELAVL1 expression was elevated in the 28 newly diagnosed MM patients compared to the 20 healthy controls(P<0.001).ROC curve analysis demonstrated that ELAVL1 could effectively differentiate between newly diagnosed MM patients,healthy controls,and MGUS patients(P<0.001 and P=0.000 2,respectively).Survival analysis revealed that high ELAVL1 expression was associated with shorter progression-free survival(P=0.0141)and overall survival(P=0.008 0).Univariate and multivariate COX regression analyses identified high ELAVL1 expression as an independent risk factor for poor MM prognosis(P=0.005 0).KEGG analysis suggested that ELAVL1 might be involved in the Hippo and MAPK signaling pathways.Conclusion High ELAVL1 expression in MM may serve as a biomarker for diagnosis and poor prognosis.ELAVL1 may promote MM initiation and progression via the Hippo and MAPK signaling pathways.
5.Faecalibacterium prausnitzii supernatant inhibits liver inflammation by enhancing intestinal barrier function in mice with liver fibrosis

Objective To investigate the effects of the culture supernatant of Faecalibacterium prausnitzii(F.prausnitzii)on intestinal barrier function and liver fibrosis progression in mice.Methods A total of 24 male C57BL/6 mice were randomly assigned to three groups:normal control group(Control),liver fibrosis model group(Model),and model mice+F.prausnitzii supernatant group(Model+S).Hepatic fibrosis was induced by intraperitoneal injection of carbon tetrachloride in both the Model group and the Model+S group over a period of 10 weeks.Following a 2-week modeling period,the mice in the Model+S group were subjected to an 8-week treatment with bacterial supernatant.The colon and liver were analyzed for pathological morphology using HE staining.Additionally,impairment of intestinal barrier function was assessed by periodic acid schiff staining,ELISA,and fluorescein isothiocyanate-dextran(FITC-Dextran)fluorescent probe.Changes in the gut microbiota structure were analyzed with 16S rDNA sequencing.The degree of liver fibrosis was assessed using sirius red staining,and the expression levels of markers associated with intestinal barrier function,liver fibrosis,and inflammation were measured using qPCR and ELISA.Results Compared with the Model group,the severity of intestinal damage and the expressions of pro-inflammatory factors in the Model+S group were decreased.Additionally,there was a significant increase in the expressions of multiple genes related to intestinal barrier function,as well as a decrease in the concentration of FITC-Dextran in peripheral blood and lipopolysaccharide in the liver(all P<0.05),and there was an improvement in the composition of the gut microbiota.Furthermore,while the liver function and extent of liver fibrosis in the Model+S group did not show any significant differences compared to the Model group,there was a noticeable decrease in the infiltration of inflammatory cells within the liver.Additionally,the expressions of pro-inflammatory factors such as Tlr4,Il-6 and Tnf-α,and the concentration of IL-6 in the liver was significantly decreased(all P<0.05).Conclusion The F.prausnitzii supernatant exhibits the potential to enhance intestinal barrier function and rectify gut microbiota imbalance in mice with liver fibrosis,thus reducing hepatic inflammation.However,it does not exert any significant effect on the progression of liver fibrosis.
6.Validation of the Interdependent Happiness Scale in Chinese adults

Objective:To adapt the Interdependent Happiness Scale(IHS)into Chinese(IHS-C)version and evaluate its validity and reliability.Methods:Item analysis and exploratory factor analysis(EFA)were conducted on data from 500 adults collected via Credamo.Confirmatory factor analysis(CFA)and multi-group measurement invariance tests were performed on an additional 600 adults.The Simplified Chinese versions of the Depression-Anxiety-Stress Scale(DASS-21),Satisfaction with Life Scale(SWLS),and Flourishing Scale(FS)were used to assess criterion validity and incremental validity.Furthermore,test-retest reliability over a four-week interval was examined in a sample of 245 adults,collected through three university-wide elective courses and two WeChat groups.Internal consistency was assessed with the total sample(n=1 345).Results:EFA revealed a single common factor,explaining 53.80%of the variance,with factor loadings of the items ranging from 0.63 to 0.84.CFA indi-cated a good fit for the one-factor model(x2/df=1.97,CFI=0.99,TLI=0.98,SRMR=0.03,RMSEA=0.04).The scale demonstrated measurement invariance across gender and identity groups(ΔCFI ≤ 0.01,ΔRMSEA 0.015).The scores of IHS-C were negatively correlated with DASS-21 subscale scores(r=-0.65,-0.48,-0.58,Ps<0.001)and positively correlated with SWLS scores(r=0.82,P<0.001)and FS scores(r=0.84,P<0.001).The Cronbach α for IHS-C was 0.89,and the test-retest reliability was 0.73.Conclusion:The Chinese version of the Interdependent Happiness Scale(IHS-C)is a valid and reliable tool for assessing happiness in adults.
7.Construction of debriefing and reflection training program for undergraduate nursing interns based on competency theory

Objective:To construct a debriefing and reflection training program for nursing undergraduate interns, and to provide a basis for implementing such a program and improve the practical clinical skills of the interns.Methods:Based on competency theory, the draft of the debriefing and reflection training program for undergraduate nursing interns was constructed through literature review and expert conference discussion. Delphi expert consultation was conducted from August to October 2024, and the final version of the program was established based on the experts' opinions. The weight of each indicator was determined using the analytic hierarchy process. Excel 2019 was used for data entry, and SPSS 19.0 was used for data analysis.Results:Two rounds of expert consultation were conducted, with a questionnaire recovery rate of 100.00% for both. The experts consulted in the second round had an authority coefficient of 0.861, a coefficient of variation for each index of 0.048 to 0.237, and a Kendall's concordance coefficient of 0.137 ( P<0.05). The final program included 5 primary indicators, 16 secondary indicators, and 73 tertiary indicators. Conclusions:The debriefing and reflection training program for undergraduate nursing interns proposed in this study has high scientifical validity and reliability. It can provide a reference for debriefing and reflection training of nursing undergraduate interns in China, thereby cultivating qualified clinical nursing talents.
8.Effect of ligation methods of inferior mesenteric artery on preserving left colic artery in lapa-roscopic radical resection of rectal cancer: a prospective randomized controlled study

Objective:To investigate the effect of ligation methods of inferior mesenteric artery (IMA) on preserving left colic artery (LCA) in laparoscopic radical resection of rectal cancer.Methods:The prospective randomized controlled study was conducted. The clinical data of 864 patients who underwent laparoscopic radical resection of rectal cancer at Renji Hospital of Shanghai Jiaotong University School of Medicine from January 2020 to December 2024 were selected. Patients were randomly divided into the low ligation group and high ligation group using a random number table. Patients of the low ligation group underwent laparoscopic radical resection of rectal cancer with preserving LCA by low ligation of IMA and apical lymph node dissection, and patients of the high ligation group underwent laparoscopic radical resection of rectal cancer with traditional high ligation of IMA. Observation indicators: (1) grouping of the enrolled patients; (2) intraoperative conditions; (3) postoperative pathological examination; (4) postoperative recovery. Comparison of measurement data with normal distribution between groups was conducted using the independent sample t test. Comparison of count data between groups was conducted using the chi-square test or Fisher exact probability. Comparison of ordinal data between groups was conducted using the non-parametric test. Results:(1) Grouping of the enrolled patients. A total of 864 patients with rectal cancer who underwent laparoscopic radical resection of rectal cancer were screened for eligibility, including 410 males and 454 females, aged (63±11)years. All 864 patients were randomly divided into the low ligation group and high ligation group, with 432 patients in each group. There was no significant difference in gender, age, body mass index, carcinoembryonic antigen, distance from tumor to anal margin, diabetes, hypertension, neoadjuvant radiochemotherapy, IMA subtypes and IMA length between the two groups ( P>0.05), ensuring comparability. (2) Intraoperative conditions. All patients of the two groups successfully completed surgery, with no errors in blood vessel ligation during operation or conversion to open surgery. There was a significant difference in time of IMA dissection between the low ligation group and high ligation group [(31±11)minutes vs. (28±9)minutes, t=4.39, P<0.05], and there was no significant difference in total operation time, volume of intra-operative blood loss or prophylactic stoma rate between the two groups ( P>0.05). (3) Postopera-tive pathological examination. There was a significant difference in the number of lymph node dissected between the low ligation group and high ligation group (1.8±1.4 vs. 1.5±1.4, t=2.51, P<0.05), and there was no significant difference in tumor diameter, the total number of lymph node dissected, total lymph node positive status, No.253 lymph node positive status, TNM staging between the two groups ( P>0.05). (4) Postoperative recovery. The time to postoperative first flatus and the number of anastomotic leakage of patients in the low ligation group were (74±22)hours and 16 cases, versus (78±20)hours and 31 cases in the high ligation group, respectively, showing significant differences in the above indicators between the two groups ( t=2.52, χ2=5.06, P<0.05). There was no significant difference in the time to postoperative initial liquid food intake, duration of post-operative hospital stay, duration of abdominal drainage tube indwelling, duration of anal tube indwelling, postoperative wound infection, pulmonary infection, intestinal obstruction, or urinary dysfunction between the two groups ( P>0.05). None of patients in the two groups had readmission or death during the postoperative 30 days. Conclusion:Low ligation of IMA in laparoscopic radical resection of rectal cancer can guide precise LCA preservation, which is beneficial for accelerating the recovery of intestinal function and reducing the incidence of anastomotic leakage.
