1.Clinical Study Based on DR Imaging Measurement of Ulnar Coronoid Process

Objective To explore the influence of different factors on the safe screw placement of the ulnar coronoid process based on the DR imaging measurement of the ulnar coronoid process.Methods During the period from July 2020 to November 2021,102 normal adult elbow joint DR films were randomly included from Yunnan Provincial Hospital of Traditional Chinese Medicine.Standard elbow joint DR films were selected as the measurement objects,with the apex of the coronoid process as the vertex and two straight lines parallel to the elbow joint space along both sides of the coronoid process.The length of the line segment from the apex of the coronoid process to the intersection of the radius and ulnar cortex of the ulna was the safe distance to place the nail on the coronal surface of the ulna coronoid process;The standard lateral DR film of the elbow joint was selected as the measurement image,and the apex of the coronoid process was used as a point to draw out two safety lines intersecting with the ulnar cortex at the proximal and distal ends.The length of the safety lines at the proximal and distal ends of the coronoid process was the safe nail placement distance on the sagittal plane of the ulnar coronoid process.The differences in safe nail placement distance between different genders,left and right sides,were compared and the correlation between safe nail placement distances on different cross-sections was analyzed.Results There was a statistically significant difference in the safe placement distance of the ulnar coronoid process between males and females(P<0.05);On the coronal plane,there was a correlation between the safe nail placement distance on the radial and ulnar sides in males;On the sagittal plane(P<0.05),there was a correlation between the safe placement distance of the ulnar coronoid process in women's proximal and distal ends(P<0.05).Conclusion Studying the safe screw placement distance of the ulnar coronoid process is beneficial to guide the clinical placement of screws for coronoid process fractures,the design of new steel plates,and the correction of deformity.
2.Practice and prospect of the green channel management for stroke patients in a hospital

The hospital took the lead in the area to carry out stroke process improvement, including the accountability practice of the president, micro-videos for illness condition description, standardized language communication, CT-room thrombolysis, and interdisciplinary seamless corporation among others. The timespan from admission to thrombolysis ( door to needle time, DNT) has shortened significantly, scoring the median time of 25 minutes and the shortest of 8 minutes. The hospital is also the first in the area to initiate the program to bypass the emergency department and the thrombolytic treatment on the ambulance. Such bypass program keeps the DNT of emergency patients within 15 minutes.
3.Effect of low-dose glucocorticoid on corticosteroid insufficient patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the effect of low-dose glucocorticoid on prognosis of critical illness-related corticosteroid insufficient (CIRCI) patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods A total of 385 eligible patients met the criteria of AECOPD were admitted from January 2010 to December 2012.The AECOPD patients co-morbid with CIRCI screened by an adrenal corticotrophic hormone test within 12 hours after admission were randomly divided into treatment group (n =32) and control group (n =31) for prospective,randomized (random number) and controlled clinical study.Hydrocortison (150mg/d) for treatment group or normal saline instead for control group was injected intravenously for 7 days.The 28-day mortality,shock-free days,length of ICU stay within 28 days and ventilator-free days were evaluated.And the markers of inflammation C-reactive protein,tumor necrosis factor-α,interleukin 6 and procalcitonin were measured before and 7 days after treatment.The variables were analyzed by Student' s t-test,non-parametric statistical test,Chi-square test or KaplanMeier test with SPSS 18.0 statistic software.P < 0.05 was considered statistically significant.Results A cohort of 385 patients with AECOPD was screened,and the prevalence rate of CIRCI was 16.4%.The shock rate was higher in the AECOPD patients co-morbid with CIRCI than that in the AECOPD patients without CIRCI (23.8% vs 8.7%,P <0.01).Compared with the control group,the 28-day mortality was significantly lower in treatment group (2/32 vs 8/31,P < 0.05),and shock-free days within 28 days longer in the treatment group (18.2 ± 9.5 vs 25.8 ± 4.1,P < 0.05).However,there was no difference in the shock rate,days of ICU stay and ventilator-free days between the two groups.After treatment,the levels of infection markers were decreased and obviously lower than those in control group (P < 0.01),such as Creactive protein (13.2 ± 5.5 mg/L vs 8.3 ± 3.1 mg/L for control group; 13.5 ± 5.9 mg/L vs 5.1 ± 2.3mg/L for treatment group),tumor necrosis factor-α (26.1 ± 16.2 μg/L vs 17.5 ± 11.7 μg/L for control group ; 25.0 ± 14.8 μg/L vs 10.4 ± 7.8 μg/L for treatment group) and procalcitonin [3.88 (0.25,8.5) μg/L vs 2.03 (0.15,5.1) μg/L for control group; 3.77 (0.21,8.0) μg/L vs 1.26 (0.10,3.2) μg/L for treatment group],furthermore,the levels of infection markers were decrease more obviously in the treatment group than those in the control group (P < 0.01).Conclusions There was high prevalence rate of CIRCI in the patients with AECOPD in the department of critical medicine,and low-dose glucocorticoid reduced 28-day mortality,shock days and markers of infection and inflammation.

OBJECTIVETo study chemical constituents of contained in roots of Pterospermum heterophyllum.
METHODThe chemical constituents of P. heterophyllum were separated and purified by silica gel, sephadex LH-20, ODS column chromatography and preparative HPLC. Their structures were identified by spectroscopic techniques.
RESULTTen compounds were separated and identified as asperglaucide (1), 2-methoxy4-hydroxyphenol-1-O-beta-D-apiofuranosyl (1-->6)-O-beta-D-glucopyranoside (2), 5, 5'-dimethoxy-9-beta-D-xylopyranosyl-(-) -isolariciresinol(3), (-)-epicatechin (4), eriodictyol (5), taxifolin (6), 3beta-hydroxy-12-en-28-ursolic acid (7), 2beta, 3beta-dihydroxy-12-en-28-oleanolic acid (8), quercetin (9), cholest-4-en-3-one (10).
CONCLUSIONCompounds 1-10 were separated from this plant for the first time.

5.Event-related potentials and the mirror-normal differences of object rotation in first-episode schizophrenia

Objective To explore the brain electrophysiological mechanism of object rotation in first-episode schizophrenia.Methods 30 patients with schizophrenia and 28 normal healthy people,who were from the Center for Mental Disease Control and Prevention,Third Hospital of PLA,took part in the mental rotation tasks,then the incubation period and amplitude of P500,and the wrong number and reaction time were measured.Results Compared with control group ( normal:(494.16 ± 34.68 ) ms,( 9.56 ± 2.54) μV; mirror:(496.51 ± 33.10 ) ms,(6.38 ± 2.41 ) μV),schizophrenia' incubation periods were significantly delayed ( normal:( 571.30 ± 51.21 ) ms;mirror:(573.41 ±39.27) ms) and volatility were significantly lower ( normal:(4.26 ± 1.01 ) μV; mirror:(3.61± 1.21 )μV) in normal and mirror rotation (P<0.05 ).The mirror-normal differences were not significant on the incubation periods of two groups (P > 0.05 ) ; the mirror-normal image differences were not significant on the patient group' volatility (P > 0.05 ) ; the normal volatility was significantly higher than mirror in the control group (P < 0.05).Conclusion Schizophrenia'mental rotation ability is impaired,and mirror-normal differences on mental rotation are disappeared.It can be used as an early-stage clinical auxiliary diagnosis index.
6.The ERP brain topographic map study on mental rotation of depressions

ObjectiveTo explore the change of the event related potential brain topographic map on depression' mental rotation,and to perfect the brain function relation map for depression in space ability.Methods 32 depression and 29 normal healthy people were tested to make mental rotation tasks in the brain ERP system.The distribution of the changing brain topographic map were observed.Results ( 1 ) Compared with the control group ( error rate ( 29±9 ) %,response time ( 604.74 ± 54.39 ) ms,the error rate was significantly higher and response time was significantly longer in depression (error rate( 33 ± 15 )%,response time(755.22 ± 70.18 )ms,P<0.05).(2) Compared with the control group (N100:PZ( -3.78 ± 1.05)μV,CZ( -5.67 ±2.21)μV,P3( -2.34 ±0.59) μV,P4( -2.92 ±0.80) μV ;P500:PZ(7.35 ±2.61 ) μV,CZ(7.65 ± 2.42) μV,P3 (6.53 ±2.11 ) μV,P4 ( 7.29 ± 2.57 ) μV ),the total volatility was significantly lower in depression ( N 100:PZ ( - 0.31 ±0.09)μV,CZ( -2.27 ±0.57)μV,P3( -0.30 ±0.07) μV,P4( -0.33 ±0.08) μV;P500:PZ(6.04 ±2.16)μV,CZ ( 5.92 ± 2.01 ) μV,P3 ( 6.02 ± 2.11 ) μV,P4 (6.01 ± 2.34 ) μV,P < 0.05 ) and the excitability difference of the left and right parietal-occipital lobe was disappeared (P>0.05) ; Compared with the control group,in N100 the normal and mirror excitability was significantly lower,and in P500 the normal excitability was significantly lower,but mirror was significantly higher in depression (P < 0.05 ).Compared with the left and right brain,the normal excitability in the right parietal-occipital lobe was significant higher (P < 0.05 ),but the mirror excitement difference was disappeared in depression (P> 0.05 ),and the normal and mirror excitement in the right parietal-occipital lobe was both significantly higher in normal healthy people (P < 0.05 ).ConclusionDepressed patients; mental rotation ability is impaired.And the negative potential for looking forward to reaction is lower and exist the right advantage hemisphere brain in normal,but mirror advantage hemisphere disappears in depressed patients.This study suggests the brain topographic map of mental rotation ability damaged can be used as the clinical auxiliary diagnosis index.
7.Extraction of AF signal during atrial fibrillation from single-lead ECG based on non-stationary heartbeat series.

The real-time and wireless mobile has become the trend of electrocardiogram (ECG) monitoring system for atrial fibrillation (AF). At present, the ECG with multi-leads (12 leads) is needed by most of AF signal extraction algorithms in order to extract effective AF waves. However, it is not very convenient for patients' movements in a multi-lead ECG monitoring system. Although the traditional template matching method is for single-lead ECG extraction, it is less robust than blind source extraction algorithm, and is affected severely by noise. In view of this,we put forth a new real-time algorithm for extracting AF from the singlelead ECG, using non-stationary heartbeat series during AF to extend dimension (segmentation), and then applying a blind source extraction algorithm to extract the effective AF signal. Experiment results showed that this method could be used to extract AF signal effectively from a single-lead ECG data. Therefore, it is suitable to apply this method to Wireless Monitoring System using single-lead ECG.

8.Study on micro-hardness and wear resistance of human tooth at different depth.

Micro-hardness of human teeth was measured by micro hardnessmentry at different depths i.e. enamal, enamelo-dentinal junction and dentin. A simulated dental friction test was conducted of respectively in these three areas, opposing pure titanium ball under a modified fretting machine. The results showed that micro-hardness is 320.40 +/- 22.77 HV of enamal, 232.26 +/- 32.31 HV of enamelo-dentinal junction, and 61.17 +/- 9.42 HV of dentin (P < 0.05). Wear depth is 17.33 +/- 0.58 microns of enamal, 49.44 +/- 16.47 microns of enamelo-dentinal junction and 95.20 +/- 15.07 microns of dentin(P < 0.05). Wear resistance of enamal is much better than that of dentin's. This preliminary investigation should be very useful in clinical applications.
