1.Research Progress on Treatment of Liver Cancer with Mechanistic Insights into Herbal Medicine: Focus on NF-κB Signaling Pathway

Liver cancer ranks among malignancies with high global incidence and mortality rates, presenting significant therapeutic challenges. The NF-κB signaling pathway plays a pivotal role in pathogenesis, progression, and treatment of liver cancer, where its aberrant activation is intricately linked to tumor cell proliferation and the dysregulation of apoptosis. In recent years, targeting the inhibition of this pathway has become an important research direction in the treatment of liver cancer. Traditional Chinese medicine (TCM), characterized by multicomponent nature and multitarget effects, exhibits unique advantages in the management of liver cancer. In this article, the regulatory mechanisms of the NF-κB signaling pathway in liver cancer are systematically reviewed, and the research progress on how TCM exerts antitumor effects by inhibiting this pathway and inducing apoptosis in liver cancer cells is summarized. This work provides a theoretical basis and new insights for the study of single compounds and compound formulations of TCM against liver cancer.
2.Interventional effect and mechanism of Bifidobacterium in chronic liver disease

Compared with traditional therapies for chronic liver disease (CLD), Bifidobacterium has the characteristics of multi-target intervention, high biosafety, and good host compatibility and provides new strategies for intervention of CLD progression in terms of microecological regulation. Various studies have shown that Bifidobacterium regulates liver homeostasis and exerts a therapeutic effect on CLD by regulating intestinal flora, maintaining antioxidation, promoting energy consumption, alleviating inflammation, improving glycolipid metabolism, and exerting an antitumor effect. This article systematically reviews the studies on Bifidobacterium in the treatment of CLD in China and globally, explores their different mechanisms, and elaborates on the interaction between related signaling pathways (such as the nuclear factor erythroid 2-related factor 2 signaling pathway and the adenosine monophosphate-activated protein kinase signaling pathway) and the liver, in order to provide a basis for probiotic intervention in liver pathology, as well as new ideas for the comprehensive treatment of CLD.
3.Scoping review of medication-related risk factors for falls in older adults

OBJECTIVE To systematically review medication-related risk factors for falls in older adults, to provide references for ensuring medication safety among older adults. METHODS A systematic search was conducted in PubMed, Embase, Web of Science, and CNKI for relevant literature published from database inception to November 1, 2025. Relevant studies on medication-related falls in older adults, both domestic and international, were included. Drug factors influencing falls in older adults were summarized and analyzed. RESULTS A total of 22 studies were included. Four major classes of fall-risk-increasing drugs were identified: psychotropic medications [12 studies, odds ratio (OR) range 1.500-5.790], cardiovascular system drugs (5 studies, OR range 1.236-4.784), analgesics (3 studies, OR range 1.500-4.490), and hypoglycemic agents (3 studies, OR range 2.070-2.751). Additionally, anticholinergic burden (1 study, OR was 2.610) and polypharmacy (7 studies, OR range 2.902-25.897 for the use of ≥4 medications) were identified as significant risk factors for falls. CONCLUSIONS Falls in older adults are significantly associated with psychotropic medications, cardiovascular system drugs, analgesics, and hypoglycemic agents, among which psychotropic medications pose the highest risk. Anticholinergic burden and polypharmacy are also important risk factors. In clinical practice, interventions should be implemented through deprescribing and risk monitoring to effectively reduce the risk of falls in older adults.
4.Effect and mechanism of Biejiajian Pill on subcutaneous xenograft tumor model of hepatocellular carcinoma Huh7 cells

ObjectiveTo investigate the inhibitory effect of Biejiajian Pills (BJJW) on the growth of liver cancer, as well as its potential mechanism in mediating the AMP-activated protein kinase (AMPK)/mammalian target of rapamycin (mTOR) pathway through mitochondrial energy metabolism. MethodsHuman hepatoma Huh7 cells were used to establish a nude mouse model of subcutaneous xenograft tumor. A total of 18 tumor-bearing nude mice were randomly divided into model group, BJJW group (2.2 g/kg), and metformin group (250 mg/kg), and the corresponding drug was given by gavage for 14 consecutive days. Tumor volume and weight were monitored during the experiment; HE staining was used to observe histopathological changes; the levels of reactive oxygen species (ROS) and adenosine triphosphate (ATP) in tumor tissue were measured; immunohistochemistry and Western blotting were used to measure the expression levels of proteins associated with the AMPK/mTOR pathway. A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups, and the Tukey’s test was used for further comparison between two groups; the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups, and the Dunn’s test was used for further comparison between two groups. ResultsCompared with the model group, the BJJW group had a tumor inhibition rate of 45.73%, with significant reductions in both tumor volume and weight (P<0.01). Pathological examination showed that compared with the model group, the BJJW group had a significant reduction in the number of tumor cells and the presence of extensive necrosis. Mechanistic studies showed that compared with the model group, the BJJW group had a significant increase in ROS level (P<0.001) and a significant reduction in ATP level (P<0.001), as well as significant increases in p-AMPK/AMPK ratio (0.81±0.20 vs 0.13±0.04, P<0.01) and p-ULK1/ULK1 ratio (0.69±0.17 vs 0.18±0.13, P<0.01) and a significant reduction in p-mTOR/mTOR ratio (1.34±0.16 vs 3.20±0.62, P<0.01). ConclusionBJJW may inhibit the growth of liver cancer by inducing mitochondrial energy metabolism dysfunction, increasing the level of ROS, reducing the level of ATP, and activating the AMPK/mTOR signaling pathway.
5.Analysis of factors influencing total corneal surgically induced astigmatism following implantable collamer lens implantation

AIM: To analyze the factors influencing total corneal surgically induced astigmatism(SIA)following implantable collamer lens(ICL)implantation.METHODS:This prospective study enrolled 162 patients(162 eyes)who underwent ICL implantation at our hospital between July 2023 and January 2024. Based on preoperative assessment of anticipated postoperative residual astigmatism, different incisions were selected. Superior incision was selected for patients with expected residual astigmatism with the rule in 75 eyes, and temporal incision was selected for patients with expected residual astigmatism againist the rule in 87 eyes. Parameters including total corneal refractive power, incision length, internal ostium-to-visual axis distance, central corneal thickness, preoperative total corneal astigmatism, and corneal diameter were measured using the Pentacam anterior segment analyzer before and at 3 mo after surgery. Postoperative total corneal SIA was calculated based on the changes in total corneal refractive power. Multiple linear regression analysis was performed to assess the influence of the above parameters on postoperative total corneal SIA.RESULTS:A total of 162 cases(162 eyes)that implanted with ICL were included in the analysis, and 8 cases were lost to follow-up, with a loss rate of 4.9%. Eventually 154 cases(154 eyes)completed the research. The superior incision group comprised 72 cases(72 eyes), including 17 males and 55 females, with a mean age of 25.96±6.17 years, while the temporal incision group comprised 82 cases(82 eyes), including 20 males and 62 females, with a mean age of 27.79±6.47 years. No significant difference in postoperative total corneal SIA was observed between the two groups [0.31(0.21, 0.49)D vs. 0.27(0.13, 0.485)D, P=0.159]. Multiple linear regression analysis revealed that internal ostium-to-visual axis distance and preoperative total corneal astigmatism significantly influenced postoperative total corneal SIA in the superior incision group(P=0.001). The regression equation was: postoperative total corneal SIA=0.71-0.381×internal ostium-to-visual axis distance+0.16×preoperative total corneal astigmatism. No significant influencing factors for postoperative total corneal SIA were identified in the temporal incision group.CONCLUSION: During ICL implantation, the magnitude of total corneal SIA is comparable between superior and temporal incisions. For patients receiving a superior incision, the internal ostium-to-visual axis distance and preoperative total corneal astigmatism value can be used to quantitatively predict postoperative total corneal SIA to a certain extent, thereby aiding in the optimization of postoperative visual quality.
6.Investigation of radon activity concentration and dose assessment in subways of Nanning City, China

Objective To investigate the radon activity concentrations in subways of Nanning City and assess the average annual effective doses for subway staff and passengers due to radon exposure. Methods Sixty-three stations across the subway lines 2, 3, and 5 were selected as study sites. Radon activity concentrations were measured using the scintillation counting method with scintillation vials. Results The radon activity concentrations in subway lines 2, 3, and 5 were 7.9-24.4, 12.0-26.2, and 12.6-18.2 Bq/m3, respectively. The average radon activity concentrations for these three lines were (17.4 ± 4.6), (19.1 ± 4.1), and (14.6 ± 1.7) Bq/m3, respectively. Statistical analysis using SPSS 26.0 software revealed a significant difference in radon activity concentrations among these stations (P<0.01). Considering the data in previous research, the average radon activity concentration across all stations in the subway lines of Nanning City was determined to be 17.4 Bq/m3. The estimated average annual effective dose due to radon exposure was 0.131 mSv for subway staff and 0.033 mSv for passengers. Conclusion The radon activity concentrations in the subway lines of Nanning City were significantly lower than the national standard limit (400 Bq/m3). The annual effective doses from radon exposure for both subway staff and passengers were below the limits specified in the Basic Standards for Protection Against Ionizing Radiation and for the Safety of Radiation Sources (GB18871—2002). The health impact of radon and its progeny on subway staff and passengers in the subway lines of Nanning City was extremely low and can be considered negligible.

Ulcerative colitis(UC) is a chronic non-specific inflammatory bowel disease characterized by inflammation and ulceration of the colonic mucosa and submucosa, and its complex pathogenesis involves immune abnormality, oxidative stress and other factors. The nuclear transcription factor E2-related factor 2(Nrf2), encoded by the Nfe212 gene, plays a central role in antioxidant responses. It not only activates various antioxidant response elements such as heme oxygenase-1(HO-1) and quinone oxidoreductase 1(NQO1), but also enhances the activity of glutathione-S-transferase(GST) and superoxide dismutase 1(SOD1), effectively eliminating reactive oxygen species(ROS) accumulated in the body, and mitigating oxidative stress-induced damage to intestinal mucosa. In addition, Nrf2 can reduce the release of inflammatory factors and infiltration of immune cells by regulating immune response, cell apoptosis and autophagy pathways, thereby alleviating intestinal inflammation and promoting the repair and regeneration of damaged mucosa. Based on this, this paper reviews the research progress of Chinese materia medica in the prevention and treatment of UC by modulating the Nrf2 signaling pathway. It deeply explores the physiological role of Nrf2, the molecular mechanism of activation, the protective effect in the pathological process of UC, and how active ingredients in Chinese materia medica regulate the Nrf2 signaling pathway through multiple pathways to exert their potential mechanisms. These studies have revealed in depth that Chinese materia medica can effectively combat oxidative stress by regulating the Nrf2 signaling pathway. It can also play a role in anti-inflammatory, promoting autophagy, inhibiting apoptosis, protecting the intestinal mucosal barrier, and promoting intestinal mucosal repair, providing new ideas and methods for the multi-faceted treatment of UC.
9.Traditional Chinese Medicine in Prevention and Treatment of Depression Based on Ferroptosis Theory: A Review

Ferroptosis is a novel form of programmed cell death that plays a critical role in the body's disease resistance processes. Research has shown that ferroptosis plays a significant role in the pathogenesis of depression. Traditional Chinese medicine (TCM) has unique advantages in the prevention and treatment of depression, and the effective application of ferroptosis theory to guide TCM in the prevention and treatment of depression has become a current research hotspot. This article provides a summary and review of the regulatory mechanisms of ferroptosis, its relationship with depression, and the current research status of TCM in the prevention and treatment of depression based on ferroptosis. It has been found that the occurrence of ferroptosis is related to processes such as iron metabolism disorders, uncontrolled lipid peroxidation, and abnormal amino acid metabolism. Ferroptosis plays an important regulatory role in the onset, progression, and treatment of depression, offering new ideas for therapeutic approaches to the condition. Additionally, research on TCM interventions that regulate ferroptosis to exert antidepressant effects has identified several effective components, such as saikosaponin B2, luteolin, quercetin, gallic acid, catalpol, turmerone, oxypaeoniflorin, and traditional compound formulas such as Dihuang Yinzi and Xiaoyaosan, as well as acupuncture and moxibustion. These interventions have been shown to exert antidepressant effects by regulating ferroptosis, providing scientific evidence and references for further research into ferroptosis theory and its application in guiding the prevention and treatment of depression through TCM.
10.Burden of congenital birth defects in children under five in China from 1990 to 2021 and prediction of future trend.

OBJECTIVES:
To study the incidence and disease burden of congenital birth defects in children under five in China from 1990 to 2021 and to predict the incidence of congenital birth defects in this population from 2022 to 2036, providing a reference for the prevention of congenital birth defects in children.
METHODS:
Using the Global Burden of Disease Study 2021 (GBD 2021) database, the incidence and disability-adjusted life years (DALY) were employed to describe the disease burden. The Joinpoint regression model was used to analyze the trends in incidence and DALY rates of congenital birth defects in children under five. A grey prediction model GM(1,1) was applied to fit the trend of incidence rates of congenital birth defects in this age group and to predict the incidence from 2022 to 2036.
RESULTS:
In 2021, the incidence rate of congenital birth defects among children under five in China was 737.28 per 100 000. Among these, congenital musculoskeletal and limb deformities had the highest incidence rate at 307.15 per 100 000, followed by congenital heart defects (223.53 per 100 000), congenital urinary and genital tract malformations (74.99 per 100 000), and congenital gastrointestinal malformations (62.61 per 100 000). From 1990 to 2021, the incidence rate and DALY rate of congenital birth defects in children under five in China decreased at an average annual rate of 1.73% and 5.42%, respectively. The prediction analysis indicated a decreasing trend in the incidence of congenital birth defects among children under five in China from 2022 to 2036, with the incidence rate dropping from 892.36 per 100 000 in 2022 to 783.35 per 100 000 in 2036.
CONCLUSIONS
The incidence and disease burden of congenital birth defects in children under five in China showed a significant declining trend from 1990 to 2021. It is predicted that this incidence will continue to decrease until 2036.
