1.Prospective study on effect of dual-subject three-step method on postoperative recovery in patients with intradural tumors
Journal of Clinical Medicine in Practice 2025;29(1):102-106
Objective To investigate the effects of the dual-subject three-step method on periop-erative outcomes in patients with intradural tumors. Methods A total of 86 patients with intradural tumors were enrolled in a prospective study and were randomly divided into control group and observa-tion group, with 43 patients per group. The control group received routine perioperative care, while the observation group underwent intervention of the dual-subject three-step method. The recovery process, psychological state[Patient Health Questionnaire-9(PHQ-9)and Generalized Anxiety Dis-order-7(GAD-7)]at different perioperative time points, lumbar spine function scores of Japanese Orthopaedic Association(JOA),and occurrence of complication were observed. Results The obser-vation group exhibited shorter durations for first flatus, first ambulation, recovery of bowel move-ments,and hospital stay compared with the control group(P<0.05).The PHQ-9 and GAD-7 scores in the observation group were lower than those in the control group at 1 day preoperatively,1 day post operatively, and 1 month after discharge(P<0.05).At discharge,1 month after discharge, and 6 months during follow-up,the JOA scores in the observation group were higher than those in the con-trol group(P<0.05).The complication rate in the observation group was lower than that in the con-trol group(6.98%versus 25.58%,P<0.05).Conclusion Application of the dual-subject three-step method in patients with intradural tumors can effectively improve psychophysical stress, reduce complications, improve lumbar spine function, and promote early postoperative recovery.

Objective To confirm the epidemiological characteristics of coronavirus disease COVID-19 reported in Qianjiang city and provide a scientific basis for establishing and implementing effective infection prevention and control. Methods The cases of COVID-19 reported in Qianjiang were retrospectively collected and the epidemiological characteristics, including time, spatial and population distribution, clinical symptoms and exposure history, were analyzed using descriptive epidemiological method. Results A total of 198 confirmed cases of COVID-19, including 102 imported cases, were reported in Qianjiang city by March 31, 2020. Of the 198 patients, 9 died, and 189 were discharged. The male-to-female ratio was 1.2:1. Among the reported patients, most of the patients were aged 30-69. In the early stage, only imported cases were reported, subsequently, imported cases and local cases were prevalent together, and the peak of confirmed cases was appeared on February 15, 2020. Among the clinical types for admitted diagnosis, the common type (67.17%) was dominant, and fever (82.83%) and cough (67.17%) were the predominant clinical manifestations for primary diagnosis. The median time from illness onset to first visit was 3 days, the median time from first visit to confirmed diagnosis was 7 days, and the median interval from illness onset to diagnosis was 11 days. Conclusions With comprehensive measures taken, the pandemic of COVID-19 in Qianjiang have been brought under control. However, it is necessary to be alert to the potential risks brought by the resumption of work, production, education, as well as the outbound imported cases. Various prevention and control measures should continue to be strictly implemented.

Objective To discuss the application effects of case-based learning combined with tutor responsibility system teaching method on orthopedic specialist nurse training. Methods A total of 43 orthopedic specialist nurses who were admitted in 2014 and 2015 received the case-based learning combined with tutor responsibility system teaching method training. After the training, the theoretical examination, the project defense and the Competency Inventory for Registered Nurse(CIRN) were applied to evaluate the efficacy of the training. Results All 43 orthopedic specialist nurses passed the theoretical and clinical examination successfully and had the orthopedic specialist nurse certificate after the defense. The total score after the training was (178.88±25.95), which was significantly higher than the score (158.64±24.97) before the training (t=3.687, P<0.05).Conclusions The case-based learning combined with tutor responsibility system teaching method can improve orthopedic specialist nurses' core competence, clinical ability and research capacity so as to lay the foundation for further specialist nurse training.

Objective To study the effect of allodecedllular sheath extracellular matrix(ASECM) for repairing of the hernia. Methods 12 dogs were randomly divided into two groups:(1)ASECM repairing group (ASECM group), 6 dogs; (2) artificial materia (AM) repairing group (Control group), 6 dogs. Abdominal wall defect of 3cm?5cm area was made on both of right and left abdomen in ASECM group and control group. The defect of left abdomen was not repaired as a self-control. The defect of right abdoment was repaired by ASECM in ASECM group , and by AM in control group . Microscopically, imunohistochemical study and electroscopy were performed 2,8,16 weeks after operation in both ASECM group and control geoup. Results A hernia could be seen on the defect of left abdominal wall in both groups 2 days after operation, but no hernia was found on the defect of right abdominal wall. In ASECM group , microscopy showed that the number of fibroblast cells (FBC) growing in the ASECM increased graduately 2~8 weeks after operation, but decreased 16 weeks after operation, and no inflammatory cells infiltration was seen at any time;electroscopically, the ASECM was filled with clear ranked thin collegen 16 weeks after operation. Contrarily, in control group, microscopy showed that the amount of covering the superficialness of AM increased graduately 2~8 weeks after operation, but not many FCs could be seen inside the AM, and inflammatory cells infiltration was found at every time postoperativly. Electroscopically, FCs grew to cover the superficialness as reaction of host to the foreign body ,and the AM had not merged with host tissues. Immuohistochemical study showed that there were typy I and III collegen presented in the repairing materias. Conclusions ASECM can provide a fram for the host FCs growth, So the ASECM can be mixed together with host tissues to provide a good intensive potency against hernia recurrence, and ASECM might be an ideal tissue-engineering materia for repairing hernia.
