1.Research on medication rules of Chinese medicine in treating chronic kidney disease with spleen and kidney deficiency based on the mutual admiration of spleen and kidney

Objective Based on the theory of mutual admiration of spleen and kidney,this study intends to explore the medication patterns of traditional Chinese medicine for the treatment of spleen-kidney deficiency-type chronic kidney disease(CKD)by using data mining methods and to provide reference for the clinical treatment of spleen-kidney deficiency-type CKD.Methods The literatures included in China National Knowledge Infrastructure,Wanfang Data Knowledge Service Platform,and VIP databases were used as data sources.The literature related to traditional Chinese medicine treatment of spleen-kidney deficiency-type CKD was analyzed by Excel 2021,IBM SPSS Modeler 18.0,IBM SPSS Statistics 27,and systematic clustering analysis and finally visualized by Cytoscape 3.7.2,RStudio.Results A total of 90 prescriptions were included,involving 146 flavors of drugs.The top 5 high-frequency drugs were Huangqi,Fuling,Baizhu,Dahuang,and Danshen.The medicinal properties are mainly mild and warm;The medicinal taste is characterized by sweetness,bitterness,and pungent;The main meridians of drugs are spleen,liver,and kidney meridians.Association rule analysis demonstrated that the commonly used couplet drugs were Huangqi-Fuling and Huangqi-Baizhu;Commonly used corner drugs included Huangqi-Baizhu-Fuling and Huangqi-Dahuang-Fuling.Cluster analysis found that the clustering effect of spleen-kidney deficiency-type CKD treatment drugs in five categories was better.Conclusion The medication rules of traditional Chinese medicine in the treatment of spleen-kidney deficiency-type CKD are preliminarily clarified,which provides a basis for clinical medication and new prescription development.
2.Exploration on medication rules of national patents of traditional Chinese medicine compound for prevention and treatment of urinary tract infection based on R language data mining

Objective To analyze the medication rules of traditional Chinese medicine compound patents for the prevention and treatment of urinary tract infection(UTI)through data mining technology.Methods The patents of traditional Chinese medicine compounds for the prevention and treatment of UTI in patent announcement module of China National Intellectual Property Administration website as data sources.The statistical analysis platforms of Excel 2021,IBM SPSS Modeler 18.0 and IBM SPSS Statistics 27.0 were used for frequency of use,medicinal properties,association rule analysis,and systematic clustering analysis and finally visualized by Cytoscape 3.7.2,RStudio.Results Through screening,a total of 179 compound patents met the inclusion criteria,involving 466 kinds of Chinese materia medica,and the top 5 high-frequency drugs were Pugongying,Huangbai,Gancao,Jinyinhua,Bianxu.The medicinal properties are mainly cold and mild;The medicinal taste is characterized by sweetness,bitterness,and bitterness;The main meridians of drugs are liver,lung and kidney meridians.Common couplet medicines included Qumai-Bianxu,Gancao-Bianxu and the three herb drug combinations included Cheqianzi-Qumai-Bianxu,Gancao-Qumai-Bianxu.A total of 5 high-frequency combinations of traditional Chinese medicine were obtained by cluster analysis.Conclusion This study preliminarily reveals the compatibility and medication rules of traditional Chinese medicine in the treatment of UTI,which provides data support for the optimization of clinical syndrome differentiation and treatment system and the development of new prescriptions.
3.Study on Identification Indexes in adult chest CT.for individualization

Objective To find and select stable and specific identification indexes in chest CT images,to establish mathematical models and provide a systematic and scientific identification method.Methods Medical imaging analysis and processing technology were applied to compare the image indexes such as lung apical shadow,double lung texture,trachea,sternum,thoracic morphology,liver,spleen,interlobular fissure morphology,first rib,aorta and thoracic vertebrae morphology of 600 serial chest CT scans of of the same adults at different periods and 600 scans of different adults.Consistency test(Kappa analysis)was applied to determine the consistency of different identification indexes,and to screen out the image identification indexes that were not easily affected by subjective factors and had high consistency;the cumulative exclusion probability method was applied to calculate the combined identification ability of the observation indexes,and select optimal indexes to establish the identification index system.Results Five indexes-left lung texture,right lung texture,interlobular fissure of the liver,first rib on the left side,and first rib on the right side demonstrated high consistency across age groups and minimal subjective interference.A combination of any three indexes achieved>99.99%discrimination probability for homologous versus non-homologous sources identification.Conclusion The independent or combined use of the indexes of left lung texture,right lung texture,interlobular fissure of the liver,first rib on the left side,and first rib on the right side enables individual identification in adult chest CT under different imaging conditions.
4.Effectiveness of modified reed tube moxibustion based on spleen-kidney mutual assistance theory in patients with CAPD combined with spleen-kidney yang deficiency type insomnia

Objective:To explore the effect of modified reed tube moxibustion based on spleen-kidney mutual assistance theory in patients with continuous ambulatory peritoneal dialysis (CAPD) combined with spleen-kidney yang deficiency type of insomnia.Methods:From September 2020 to December 2022, 64 patients with CAPD combined with insomnia of spleen-kidney yang deficiency type were selected from the Department of Nephrology, Ningbo Municipal Hospital of Traditional Chinese Medicine, Zhejiang Chinese Medical University by the convenience sampling method. A randomized numerical table method was used to divide study subjects into a control group and a study group, with 32 cases in each group. The control group received conventional treatment and auricular needle-embedding therapy; while the study group received traditional treatment and auricular needle-embedding therapy combined with modified reed tube moxibustion. The intervention period lasted three months. Differences in Insomnia Severity Index, Chinese medicine syndrome score, and peritoneal dialysis adequacy were compared between the two groups.Results:After intervention, the Insomnia Severity Index score of the study group was lower than those of the control group, and the difference was statistically significant ( P<0.05). The urea clearance index and creatinine clearance rate of the study group were higher than those of the control group, and the differences were statistically significant (all P<0.05) . Conclusions:Modified reed tube moxibustion based on spleen-kidney mutual assistance theory has a positive effect on improving insomnia symptoms and dialysis adequacy in patients with CAPD combined with insomnia of spleen-kidney yang deficiency type.

9.Clinical characteristics and risk factors of renal function deterioration in children with renal dysplasia complicated with chronic kidney disease

【Objective】 To explore the clinical characteristics and risk factors of renal function deterioration in children with renal dysplasia and chronic kidney disease (CKD), so as to provide a basis for the diagnosis, treatment, and management. 【Methods】 The clinical data of children with renal dysplasia complicated with CKD treated in the Children’s Hospital of Chongqing Medical University during 2012 and 2022 were retrospectively analyzed, including the gender, age of diagnosis, growth index, concomitant malformation and complications. According to the diagnostic criteria and staging standard of KDIGO2020 guidelines, patients with disease deteriorated to CKD stage 4-5 were enrolled into the regression group. Factors affecting the deterioration of renal function were determined with Cox regression analysis. 【Results】 A total of 122 children were involved, including 66 (54.1%) with CKD stag 4-5. There were more boys than girls. Bilateral and unilateral renal dysplasia occurred in 88 (72.13%) and 34 (27.87%) cases, respectively, and 64 (52.46%) cases were complicated with other urinary diseases. There were significant differences in weight, height and body mass index (BMI) among patients with CKD stage 1-5 (P<0.01). The age of onset of CKD <10 years, BMI lower than the 3rd percentile of the same sex and age, bilateral renal dysplasia, and one or more complications of congenital renal and urinary tract abnormalities (CAKUT) were the risk factors of deterioration of renal function (P<0.05). 【Conclusion】 Renal dysplasia complicated with CKD are more common in boys, with high incidence of bilateral renal dysplasia. Bilateral renal dysplasia, age of onset of CKD <10 years, BMI lower than 3% and complications are important influencing factors of renal dysplasia in children with CKD.

In case of ischemic stroke(IS),thrombus in the vascular system can cause ischemia and hypoxia in brain tissue,produce inflammatory cytokines and cause brain tissue damage,while reactive oxygen species during ischemia-reperfusion cause stress injury.Conventional drug administra-tion is limited by the selective permeability of the blood-brain barrier and the low bioavailability of the drug itself,and its therapeutic effect against IS is unsatisfactory.Nanomedicine is expected to bring hope in that it has a unique mechanism of action and can cross the blood-brain barrier to reach the periphery of the infarct,release drugs or therapeutic genes,and exert a therapeutic effect.Nanomedi-cine inhibits platelet aggregation,enhances the efficacy of thrombolytic drugs,dissolves thrombus,increases blood supply to ischemic areas,eliminates reactive oxygen species and weakens injury response by fighting inflammatory cytokines.Loaded therapeutic genes regulate the differentiation pro-cess of neural stem cells,increase the number of neurons,induce the occurrence of blood vessels,and enhance the repair function of brain tissue.Nanomedicines can not only improve pharmacokinetics and pharmacodynamics to achieve more effective drug treatment,but also use nanoimaging technology to achieve real-time monitoring and condition assessment of therapy.
