Objective To summarize the clinical features, treatment and prognosis of patients with primary central nervous system lymphoma(PCNSL)with clonal bone marrow B cells, and to explore the influence on clinical diagnosis and treatment. Methods PCNSL patients with clonal bone marrow B cells diagnosed by flow cytometry between Jan 2020 and Jul 2023 at Huashan Hospital of Fudan University were enrolled. The auxiliary examination data of these patients were collected, including complete blood count, routine biochemistry, bone marrow aspiration and biopsy,contrast-enhanced brain MRI,and whole-body PET-CT.Kaplan-Meier was used to draw the survival curve, and relevant literature was reviewed. Results A total of 223 newly diagnosed PCNSL patients were included,187 of whom completed bone marrow puncture and biopsy evaluation. We found clonal bone marrow B cells in 16 of 187 cases(8.56%)by flow cytometry.2 patients showed B lymphoma involving the bone marrow. All patients received a high-dose methotrexate based chemotherapy. The median progression free survival(PFS)of 16 patients with clonal bone marrow B cells was 11.1 months, and the median PFS of 171 patients with normal bone marrow was 12.6 months. There was no significant difference in the PFS between the two groups. Conclusion PCNSL with clonal bone marrow B cells had no specific clinical features, but bone marrow flow cytometry showed clonal B cells.High-dose methotrexate treatment regimen is effective. There was no significant difference in PFS for PCNSL patients with clonal B cells and normal findings in bone marrow.Clonal B cells in bone marrow may be caused by monoclonal B-cell lymphocytosis(MBL),lymphoma involves the bone marrow and the presence of common precursor cells.Bone marrow examination should be performed in the initial evaluation of suspected PCNSL.

Objective:To investigate the effects of traditional Chinese curve-correcting and rotation-reducing spinal manipulation on biomechanical parameters and factors influencing herniated disc resorption in lumbar disc herniation(LDH).Methods:A retrospective analysis of 51 LDH patients treated between January 2022 and May 2024 was conducted.Lumbosacral parameters(vertebral rotation angle[α],disc angle[β],sacral slope[SS],lumbar lordosis[LL])were measured via MRI before treatment and at final follow-up.Disc resorption was assessed using Michigan State University(MSU)classification.Multivariate logistic regression identified factors associated with resorption.Results:Post-treatment α angle significantly decreased(3.02°→1.86°,P=0.002),while SS(28.4°→30.0°,P<0.001)and LL angles(31.0°→35.12°,P<0.001)increased;Disc resorption occurred in 56.86%(29/51)of patients.Longer disease course(OR=0.79,95%CI:0.69-0.91)and disc calcification(OR=0.03,95%CI:0.00-0.25)were independent inhibitors of resorption(P<0.001).Conclusion:Spinal manipulation restores lumbosacral biomechanics by reducing vertebral rotation and increasing lumbar curvature, with higher resorption rates in patients with short duration(≤6 months),non-calcified discs, and MSU type 2-3 herniations.

Accurate pain assessment is a prerequisite for pain management. However, pain is a relatively subjective feeling greatly affected by the environment, education, and emotional state. Therefore,in diagnosing and treating pain,it is necessary not only to assess the subjective feelings of patients, but also to have stable and reliable objective indicators to evaluate the pain status of different patients. At present, there are many clinical tools for assessing and monitoring pain. This paper introduces the commonly used subjective assessment tools(single-dimensional pain assessment scale,multi-dimensional pain assessment scale)and objective assessment tools(surgical pleth index, analgesia and nociception index, nociceptive level index, and biomarkers),and analyzes their advantages and disadvantages,so as to provide reference for future clinical pain assessment.

Cervical spine health issues not only seriously affect patients' quality of life but also impose a heavy burden on the social healthcare system. Existing guidelines lack sufficient clinical guidance on lifestyle and work habits, such as exercise, posture, daily routine, and diet, making it difficult to meet practical needs. To address this, relying on the China Association of Chinese Medicine, Wangjing Hospital of China Academy of Chinese Medical Sciences took the lead and joined hands with more than ten institutions to form a multidisciplinary guideline development group. For the first time, the group developed the Guidelines for Cervical Spine Health Intervention based on evidence-based medicine methods, strictly following the standardized procedures outlined in the World Health Organization Handbook for Guideline Development and the Guiding Principles for the Formulation/Revision of Clinical Practice Guidelines in China (2022 Edition). This proposal systematically explains the methods and steps for developing the guideline, aiming to make the guideline development process scientific, standardized, and transparent.

Objective:To investigate the nutritional risk status of children in PICU and analyze its influencing factors. Methods:From July 2021 to February 2023,all children aged 1 to 18 years admitted to PICU of Beijing Children's Hospital were investigated by using the pediatric Yorkhill Malnutrition Scoring tool(PYMS)and the clinical data questionnaire. Results:A total of 492 children in PICU were enrolled. The first nutritional risk screening results showed that there were 32 cases of no/low nutritional risk(6.5%),76 cases of medium risk(15.4%),and 384 cases of high risk(78.1%).The incidence of medium/high nutritional risk was as high as 93.5%.The PYMS score of nutritional risk in PICU was(2.61±1.42).The results of multiple linear regression analysis showed that weight, fever time before admission, white blood cells, body mass index, primary diagnosis, father's education, and diet before illness were the main influencing factors of nutritional risk of children in PICU(P<0.05).Conclusion:Children in PICU are in a state of high nutritional risk.It is suggested that children in PICU should carry out nutritional screening in a standardized manner, identify children with high nutritional risk and its influencing factors early. To actively conduct nutritional assessment and nutritional intervention could improve the clinical outcome of children in PICU.

Bismuth iodide(BiOI)with different crystal plane ratios of(110)to(001)was synthesized, and typeⅡheterojunction formed between(001)and(110)crystal planes of BiOI was used to improve the separation efficiency of photogenerated electrons and holes. Then the BiOI(001)/(110)was composited with nitrogen-doped graphene quantum dots(N-GQDs)to prepare a ternary composites, which could enhance the range and intensity of light absorption, and prolonged the lifetime of photogenerated electrons due to the formation of Z-scheme heterojunctions between BiOI and N-GQDs,thereby leading to the excellent photoelectric performance of the BiOI/N-GQDs for generating sensitive photoelectric response signals.A photoelectrochemical sensor for sensitive detection of chlorpyrifos(CPF)was designed with BiOI/N-GQDs-modified FTO electrode as a photocathode. The S and N atoms contained in CPF were coordinated with Bi(Ⅲ)on the surface of BiOI,which reduced the photocurrent of BiOI/N-GQDs.The photocurrent change was linear with logarithm of concentration of CPF in the range of 1.5×10-12-5.0×10-9 mol/L,and the detection limit was 1.5×10-12 mol/L.The sensor was highly sensitive, selective and stable, and could be used for determination of trace CPF in environmental and food samples.
