1.Effects of Total Intravenous Anesthesia and Inhalational Anesthesia on Postoperative Recovery in Patients Undergoing Transsphenoidal Pituitary Surgery:A Systematic Review.

Objective To systematically evaluate the effects of total intravenous anesthesia and inhalational anesthesia on postoperative recovery in patients undergoing transsphenoidal pituitary tumor resection.Methods A comprehensive search was conducted in international biomedical databases including Ovid Medline,Embase,CINAHL(EBSCO),Cochrane Library,and Web of Science,from inception to July 4,2023.Additionally,ClinicalTrials.gov and the World Health Organization International Clinical Trials Registry Platform were searched for ongoing and completed trials.The randomized controlled trials(RCT)comparing total intravenous anesthesia and inhalational anesthesia in patients undergoing transsphenoidal surgery for pituitary tumors were included.The methodological quality of the included studies was evaluated by the Cochrane Collaboration tool.Relevant data were extracted and synthesized for analysis.Results A total of 327 records were identified,of which eight RCTs met the inclusion criteria.Four studies showed that the patients receiving desflurane or sevoflurane anesthesia experienced faster emergence from anesthesia than those receiving propofol.Two studies indicated that patients in the propofol group had lower levels of emergence agitation and a lower incidence of early postoperative nausea and vomiting.The results on postoperative cognitive function were inconsistent across studies.No differences were found between the groups in terms of postoperative complications or overall recovery quality during hospitalization.Conclusions Inhalational anesthesia appears to provide an advantage in promoting faster emergence following transsphenoidal pituitary surgery,whereas total intravenous anesthesia may contribute to smoother and more stable recovery.Further high-quality studies are needed to clarify the effects of different anesthetic techniques on both short- and long-term postoperative recovery.

2.Control status and follow-up of acute attacks in children with bronchial asthma with normal pulmonary ventilation function

Objective To investigate the control status of bronchial asthma(referred to as"asthma")in school-age children with normal pulmonary ventilation function and the occurrence of acute attacks within 1 year of follow-up.Methods A retrospective analysis was conducted on clinical data of 327 children aged 6-14 years with bronchial asthma and normal pulmonary ventilation function from April to September 2021.Based on the measured value of one second rate(FEV1/FVC),the children were divided into the ≥80%group(267 cases)and the<80%group(60 cases).The pulmonary ventilation function,asthma control level,and occurrence of acute attacks within 1 year were compared between the two groups.Results The baseline pulmonary ventilation function in the<80%group was lower than that in the ≥80%group,and the proportion of small airway dysfunction was higher than that in the ≥80%group(P<0.05).After standardized treatment for 1 year,the small airway function indices in the<80%group improved but remained lower than those in the ≥80%group(P<0.05).The rate of incomplete asthma control at baseline was 34.6%(113/327),and the asthma control level in the<80%group was lower than that in the ≥80%group(P<0.05).After standardized treatment for 1 year,the asthma control level in the<80%group remained lower than that in the ≥80%group,and the proportion of acute asthma attacks was higher than that in the ≥80%group(P<0.05).Conclusions Approximately one-third of school-age children with asthma still have incomplete asthma control when their pulmonary ventilation function is normal.Among them,children with measured FEV1/FVC<80%have an increased risk of acute asthma attacks and require close follow-up and strengthened asthma management.
3.Clinical effects of Buyang Huanwu Decoction combined with Erlong Xizhu Acupuncture Method on elderly patients with stroke

AIM To investigate the clinical effects of Buyang Huanwu Decoction combined with Erlong Xizhu Acupuncture Method on elderly patients with stroke.METHODS Ninety-six patients were randomly assigned into control group(48 cases)for 1-month intervention of Erlong Xizhu Acupuncture Method,and observation group(48 cases)for 1-month intervention of both Buyang Huanwu Decoction and Erlong Xizhu Acupuncture Method.The changes in neurological function indices(NIHSS score,NDS score),Hcy,CysC,vascular endothelial function indices(ET-1,NO,vWF),intracranial hemodynamic indices(Vmean,R,DR),hemorheological indices(WBLSV,WBHSV,FIB,HCT,EAI)and prognosis assessment indices(mRS score,BI score)were detected.RESULTS After the treatment,the two groups displayed decreased neurological function indices,Hcy,CysC,ET-1,vWF,R,DR,hemorheological indices,mRS score(P<0.05),and increased NO,Vmean,BI score(P<0.05),especially for the observation group(P<0.05).CONCLUSION For the elderly patients with stroke,Buyang Huanwu Decoction combined with Erlong Xizhu Acupuncture Method can reduce serum Hcy,CysC levels,and improve vascular endothelial functions,intracranial hemodynamics,hemorheological indices,thus improve neurological functions and promote prognosis.
4.Clinical effects of Zhenwu Decoction combined with Danshen Drink on patients with chronic heart failure

AIM To explore the clinical effects of Zhenwu Decoction combined with Danshen Drink on patients with chronic heart failure.METHODS Eighty-six patients were randomly assigned into control group(43 cases)for 4-week intervention of conventional treatment,and observation group(43 cases)for 4-week intervention of Zhenwu Decoction,Danshen Drink and conventional treatment.The changes in cardiac function indices(LVFS,LVEDD,LVEF),serum indices(hs-cTnI,sST2,NT-proBNP,MMP-9),inflammatory factors(IL-6,TNF-α,hs-CRP),platelet aggregation rate,QTcd and incidence of adverse reactions were detected.RESULTS After the treatment,the two groups demonstrated increased LVFS,LVEF(P<0.05),and decreased LVEDD,serum indices,inflammatory factors,platelet aggregation rate,QTcd(P<0.05),especially for the observation group(P<0.05).No significant difference in incidence of adverse reactions was found between the two groups(P>0.05).CONCLUSION For the patients with chronic heart failure,Zhenwu Decoction combined with Danshen Drink can safely and effectively improve hs-cTnI level and platelet aggregation functions.

OBJECTIVE:
Despite the combination of Scutellaria barbata D. Don and Scleromitrion diffusum (Willd.) R.J. Wang (SB-SD) being a recognized Chinese medicinal herbal pair that is commonly used in the treatment of ovarian cancer, there is a poor understanding of their pharmacological mechanisms. This study examines the antitumor properties and potential mechanisms of SB-SD on human ovarian cancer A2780 cells through a multi-omics approach, establishing a pharmacological basis for clinical utilization.
METHODS:
A range of mass ratios and reagents were used in the hot reflux extraction of SB-SD. The inhibitory effect of the SB-SD extracts on A2780 cell proliferation was assessed using the cell-counting kit 8 assay. A zebrafish tumor implantation model was used to evaluate the effects of SB-SD extracts on tumor growth and metastasis in vivo. Transcriptomics and proteomics were used to investigate alterations in biological pathways in A2780 cells after treatment with different concentrations of SB-SD extract. Cell cycle, cell apoptosis, intracellular free iron concentration, intracellular reactive oxygen species (ROS) concentration, malondialdehyde (MDA), and mitochondrial membrane potential were measured. Real-time quantitative reverse transcription polymerase chain reaction and Western blotting were utilized to investigate the effects of heme catabolism and ferritinophagy on ferroptosis induced by SB-SD extract in A2780 cells.
RESULTS:
The 70% ethanol extract of SB-SD (a mass ratio of 4:1) inhibited A2780 cell proliferation significantly with a half maximal inhibitory concentration of 660 μg/mL in a concentration- and time-dependent manner. Moreover, it effectively suppressed tumor growth and metastasis in a zebrafish tumor implantation model. SB-SD extract induced the accumulation of free iron, ROS, MDA, and mitochondrial damage in A2780 cells. The mechanisms might involve the upregulated expression of ferritinophagy-related genes microtubule-associated protein 1 light chain 3, autophagy-related gene 5, and nuclear receptor coactivator 4.
CONCLUSION
SB-SD extract effectively inhibited the development of ovarian cancer both in vitro and in vivo. Its mechanism of action involved inducing ferroptosis by facilitating heme catabolism and ferritinophagy. This herbal pair holds promise as a potential therapeutic option for ovarian cancer treatment and may be utilized in combination with routine treatment to improve the treatment outcomes of ovarian cancer patients. Platelets are reprogrammed by cancer via a process called education, which favors cancer development. The transcriptional profile of tumor-educated platelets (TEPs) is skewed and therefore practicable for cancer detection. This intercontinental, hospital-based, diagnostic study included 761 treatment-naïve inpatients with histologically confirmed adnexal masses and 167 healthy controls from nine medical centers (China, n = 3; Netherlands, n = 5; Poland, n = 1) between September 2016 and May 2019. The main outcomes were the performance of TEPs and their combination with CA125 in two Chinese (VC1 and VC2) and the European (VC3) validation cohorts collectively and independently. Exploratory outcome was the value of TEPs in public pan-cancer platelet transcriptome datasets. The AUCs for TEPs in the combined validation cohort, VC1, VC2, and VC3 were 0.918 (95% CI 0.889-0.948), 0.923 (0.855-0.990), 0.918 (0.872-0.963), and 0.887 (0.813-0.960), respectively. Combination of TEPs and CA125 demonstrated an AUC of 0.922 (0.889-0.955) in the combined validation cohort; 0.955 (0.912-0.997) in VC1; 0.939 (0.901-0.977) in VC2; 0.917 (0.824-1.000) in VC3. For subgroup analysis, TEPs exhibited an AUC of 0.858, 0.859, and 0.920 to detect early-stage, borderline, non-epithelial diseases and 0.899 to discriminate ovarian cancer from endometriosis. TEPs had robustness, compatibility, and universality for preoperative diagnosis of ovarian cancer since it withstood validations in populations of different ethnicities, heterogeneous histological subtypes, and early-stage ovarian cancer. However, these observations warrant prospective validations in a larger population before clinical utilities.

7.Evaluation on the stability of positioning precision of the leaves of accelerator MLC under different gantry angles

Objective:To evaluate the stability of the positioning precisions of the leaves of accelerator multi-leaf collimator(MLC)at different gantry angles,so as to determine whether the positioning precision of leaf is affected by the gantry angle.Methods:The HD 120 MLC of American Varian Edge medical linear accelerator at our hospital was selected,and the Varian Eclipse radiotherapy planning system was used to design test cases.One test plan included 5 dynamic subfields(named PMLCE)was designed,which ray amount was 100MU,and small machine head was kept at 0°position,and the numbers of the positions of corresponding leaves of each control point were respectively-20,-15,-10,-5,0,5,10 and 12 that were 8 in total,and the dose rate was set at 500MU/min.In test,all leaves slid as uniform velocity from one extreme position to the opposite extreme position,and the movement distance of leaves between the adjacent control points was 5 cm.The PMLCE test plan was executed on the EDGE linear accelerator by using the IEC1217 coordinate system,and each test was conducted at eight different gantry angles(0°,45°,90°,135°,180°,225°,270° and 315°),and a total of 8 treatment log files of accelerator were obtained.A program was written by using Matlab2016a(MathWorks,USA)software to read the log files of accelerator,and extract out leaf position data,and calculate the deviation between the actual position and planned position of all leaves,and record a total of 960 data of positioning deviations of 120 leaves at 8 leaves positions which included in each file.The above acquisition scheme of data was performed per three days for 10 times,and then the average value was obtained.There were the data of positioning deviations of a total of 8 groups of HD 120 MLC at different gantry angles.The above 8 groups of data were analyzed as statistical method.Results:Under various angles,the range of the percentage of"leaf positioning deviations≤10 μm"was between 45.7%and 58.3%,and that of"leaf positioning deviations≤30 μm"was between 95.6%and 99.2%.When the gantry angle was at 0° and 180°,the leaf positioning accuracy was relatively higher,and the percentages of"leaf positioning deviations≤10 μm"were respectively 58.3%and 57.9%.The maximum positioning error of the HD 120 MLC did not exceed 43.3 μm.The results of the one-way ANOVA showed that there were significant differences in the average positioning precisions between 0° and each one of 45°,90°,135°,225°,270° and 315°,and between 90° and 180°,and between 135° and 180°,and between 180° and each one of 225°,270° and 315° under the gantry angles were respectively 0°,45°,90°,135°,180°,225°,270° and 315°,and under the observation values of the positioning precision of 8 leaf positions(F=4.32,F=5.67,F=3.98,F=3.85,F=6.14,F=2.87,F=4.89,F=4.56,F=4.12,F=5.01,F=6.77,P<0.05).There were no significant differences in the positioning precision among other gantry angles.Conclusion:The positioning precisions of HD 120 MLC leaf under different gantry angles can be kept at higher level,but the influences of gantry angles on the positioning precision of leaf indeed existed.Therefore,doctors and physicists should fully consider these factors under the design stage of treatment plan,so as to optimize the dose distribution and enhance the treatment outcomes.
8.Brain Metabolic Network Redistribution in Patients with White Matter Hyperintensities on MRI Analyzed with an Individualized Index Derived from 18F-FDG-PET/MRI

Objective:
Whether metabolic redistribution occurs in patients with white matter hyperintensities (WMHs) on magnetic resonance imaging (MRI) is unknown. This study aimed 1) to propose a measure of the brain metabolic network for an individual patient and preliminarily apply it to identify impaired metabolic networks in patients with WMHs, and 2) to explore the clinical and imaging features of metabolic redistribution in patients with WMHs.
Materials and Methods:
This study included 50 patients with WMHs and 70 healthy controls (HCs) who underwent 18F-fluorodeoxyglucose-positron emission tomography/MRI. Various global property parameters according to graph theory and an individual parameter of brain metabolic network called “individual contribution index” were obtained. Parameter values were compared between the WMH and HC groups. The performance of the parameters in discriminating between the two groups was assessed using the area under the receiver operating characteristic curve (AUC). The correlation between the individual contribution index and Fazekas score was assessed, and the interaction between age and individual contribution index was determined. A generalized linear model was fitted with the individual contribution index as the dependent variable and the mean standardized uptake value (SUVmean) of nodes in the whole-brain network or seven classic functional networks as independent variables to determine their association.
Results:
The means ± standard deviations of the individual contribution index were (0.697 ± 10.9) x 10-3 and (0.0967 ± 0.0545) x 10-3 in the WMH and HC groups, respectively (p < 0.001). The AUC of the individual contribution index was 0.864 (95% confidence interval, 0.785–0.943). A positive correlation was identified between the individual contribution index and the Fazekas scores in patients with WMHs (r = 0.57, p < 0.001). Age and individual contribution index demonstrated a significant interaction effect on the Fazekas score. A significant direct association was observed between the individual contribution index and the SUVmean of the limbic network (p < 0.001).
Conclusion
The individual contribution index may demonstrate the redistribution of the brain metabolic network in patients with WMHs.
9.Research progress of Shenling Baizhu San and predictive analysis on its quality markers.

Shenling Baizhu San is a classic prescription for replenishing Qi to invigorate the spleen and dispelling dampness to check diarrhea, which mainly treats the syndrome of spleen deficiency and heavy dampness. With the pharmacological effects of regulating immune system, improving lung function and gastrointestinal function, and resisting oxygen, tumor, and inflammation, Shenling Baizhu San is commonly used in modern clinical practice to treat chronic obstructive pulmonary disease, pulmonary fibrosis, bronchial asthma, irritable bowel syndrome, ulcerative colitis, chronic diarrhea, and diabetic, etc. This paper summarized the chemical constituents, pharmacological effects, and clinical application of Shenling Baizhu San in recent years, and predictively analyzed the quality markers of Shenling Baizhu San according to the "five principles" of Q-marker. The Q-markers of Shenling Baizhu San involved ginsenoside Rg_1, ginsenoside Re, ginsenoside Rb_1, pachymic acid, dehydrotumulosic acid, batatasin Ⅰ, batatasin Ⅲ, diosgenin, liensinine, neferine, luteolin, quercetin, glycerol trioleate, β-sitosterol, platycodin D, glycyrrhizic acid, glycyrrhetinic acid, liquiritin, pipecolinic acid, atractylenolide Ⅰ, atractylenolide Ⅲ, and bornyl acetate, which provided references for the quality control and follow-up research of Shenling Baizhu San.

10.Skin diseases in the Da Qing Diabetes Study: a cross-sectional study.

BACKGROUND:
The prevalence of skin diseases and diabetes mellitus (DM) are prominent around the world. The current scope of knowledge regarding the prevalence of skin diseases and comorbidities with type 2 DM (T2DM) is limited, leading to limited recognition of the correlations between skin diseases and T2DM.
METHODS:
We collected 383 subjects from the Da Qing Diabetes Study during the period from July 9th to September 1st, 2016. The subjects were categorized into three groups: Normal glucose tolerance (NGT), impaired glucose tolerance (IGT), and T2DM. The prevalence and clinical characteristics of skin diseases were recorded and investigated.
RESULTS:
In this cross-sectional study, 383 individuals with ages ranging from 53 to 89-year-old were recruited. The overall prevalence of skin diseases was 93.5%, and 75.7% of individuals had two or more kinds of skin diseases. Additionally, there were 47 kinds of comorbid skin diseases in patients with T2DM, of which eight kinds of skin diseases had a prevalence >10%. The prevalence of skin diseases in NGT, IGT, and T2DM groups were 93.3%, 91.5%, and 96.6%, respectively; stratified analysis by categories showed a statistically significant difference in "disturbances of pigmentation" and "neurological and psychogenic dermatoses". The duration of T2DM also significantly associated with the prevalence of "disturbances of pigmentation" and "neurological and psychogenic dermatoses". Subsequently, the prevalence of "disturbances of pigmentation" was higher in males than females in NGT (P < 0.01) and T2DM (P < 0.01) groups. In addition, the difference in the prevalence of "disturbances of pigmentation" was also significant in NGT and T2DM groups (P < 0.01).
CONCLUSIONS
There was a high prevalence of skin diseases in the Da Qing Diabetes Study. To address the skin diseases in the Da Qing Diabetes Study, increased awareness and intervention measures should be implemented.
