Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

Objective:To analyze the effect of rosuvastatin combined with sodium tanshinone Ⅱ A sulfonate(STS)on blood lipids, cardiac function and serum inflammatory factors in elderly patients with stable angina pectoris(SAP)and hyperlipidemia(HLP).Methods:This randomized controlled study enrolled 140 elderly patients with SAP and HLP admitted in the Third People's Hospital of Liaocheng between December 2021 and December 2023.Patients were divided into control group(n=70,took rosuvastatin orally)and combination group(n=70,treated with ro-suvastatin combined with STS).Both groups were treated for two weeks.Clinical effects, blood lipids, cardiac function and serum inflammatory factors and incidence of adverse reactions were compared between the two groups. Results:Total effective rate of combination group was significantly higher than that of control group(92.86%vs.80.00%,P=0.026).Compared with patients in control group, those in combination group had significant lower triglyceride(TG)[(1.42±0.39)mmol/L vs.(1.69±0.11)mmol/L],total cholesterol(TC)[(4.37±0.69)mmol/L vs.(5.25±0.43)mmol/L]and low density lipoprotein-cholesterol(LDL-C)[(2.62±0.39)mmol/L vs.(3.19±0.38)mmol/L],and significant higher high-density lipoprotein cholesterol(HDL-C)[(1.24±0.31)mmol/L vs.(1.10±0.22)mmol/L](P<0.01 all).Compared with patients in control group, those in combination group had significant higher left ventricular ejection fraction(LVEF)[(59.13±5.46)%vs.(55.97±4.02)%,P<0.001],and significant lower C-reactive protein(CRP)[(2.66±0.68)mg/L vs.(3.50±1.61)mg/L],tumor necrosis factor-α(TNF-α)[(93.34±19.26)pg/ml vs.(111.70±25.00)pg/ml]and interleukin-6(IL-6)[(76.28±23.88)pg/ml vs.(100.64±27.52)pg/ml](P<0.001 all). There was no significant difference in the total incidence rate of adverse reactions between two groups(25.7%vs.14.3%,P=0.091).Conclusion:Rosuvas-tatin combined with STS is effective in the treatment of elderly patients with SAP and HLP.It is superior to rosuvas-tatin alone in terms of improving blood lipids, cardiac function and serum inflammatory factors.

Turning to critical illness is a common stage of various diseases and injuries before death. Patients usually have complex health conditions, while the treatment process involves a wide range of content, along with high requirements for doctor′s professionalism and multi-specialty teamwork, as well as a great demand for time-sensitive treatments. However, this is not matched with critical care professionals and the current state of medical care in China. Telemedicine, which shortens the distance of medical professionals and the gap of disease diagnosis and treatments in various regions through electronic information, can effectively solve the current problem. Therefore, there is an urgent need to develop a standardized, high-quality visualization telemedicine round system .Therefore, experts have been organized to search domestic and foreign literature on telemedicine round for critically ill patients and to form this consensus based on clinical experiences so as to further improve the level of critical care treatments in regions.

Objective:To describes the probability and rate of native liver survival (NLS) in biliary atresia (BA) patients after Kasai portoenterostomy (KPE)over various time periods and analyzes the perioperative factors associated with liver transplantation or death. Methods:A retrospective case-summary.BA patients administrated at the Department of Fetal and Neonatal Surgery in Hunan Children′s Hospital between January 2015 and December 2021.Probability and rate of NLS were calculated by life table.Cox proportional hazards regression model and Logistic model was applied to explore the perioperative factors related to post-Kasai liver transplantation/death.Results:The median age at Kasai surgery was 62 days. The rate of jaundice clearance (JC) was 64.5% within 3 months after Kasai, and 58.3% of the patients had cholangitis. The probability of NLS reached its lowest point in the first 1 year after Kasai (76.2%) and ranged from 93.2% to 98.0% in years 2-8 after Kasai. The rates of NLS in 2 years, 5 years and 8 years were 71.1%, 62.8% and 56.0%, respectively.Cytomegalovirus (CMV) infection before or on the day of Kasai without antiviral treatment can increase the risk of liver transplantation or death[ HR(95% CI): 1.628 (1.081-2.452), P=0.020].Preoperative gamma-glutamyl transferase increased the risk of liver transplantation/death within 1 year after Kasai[ OR(95% CI): 1.001 (1.000-1.001), P=0.021], and early cholangitis was a risk factor for liver transplantation/death within 5 years after Kasai[ OR(95% CI): 1.934 (1.004-3.726), P=0.048].JC within 3 months post-KPE was a protective factor of NLS. Conclusions:The first year after Kasai was the highest risk period for liver transplantation/death, which should be the focus of follow-up management.JC within 3 months after surgery is the protective factor for overall NLS, 1-year NLS and 5-year NLS.

Hemorrhagic shock is a common clinical emergency that can aggravate cell injury after resuscitation. Astrocytes are crucial for the survival of neurons because they regulate the surrounding ionic microenvironment of neurons. Although hemorrhagic shock and resuscitation (HSR) injury can impair cognition, it remains unclear how this insult directly affects astrocytes. In this study, we established an HSR model by bleeding and re-transfusion in mice. The social interaction test and new object recognition test were applied to evaluate post-operative cognitive changes, and the results suggest that mice experience cognitive impairment following exposure to HSR. In the HSR group, the power spectral density of β and γ oscillations decreased, and the coupling of the θ oscillation phase and γ oscillation amplitude was abnormal, which indicated abnormal neuronal oscillation and cognitive impairment after HSR exposure. In brief, cognitive impairment in mice is strongly correlated with Ca2+ signal strength in lateral septum astrocytes following HSR.

Lumbar disc (LD) herniation and aging are prevalent conditions that can result in substantial morbidity. This study aimed to clarify the mechanisms connecting the LD aging and herniation, particularly focusing on cellular senescence and molecular alterations in the nucleus pulposus (NP). We performed a detailed analysis of NP samples from a diverse cohort, including individuals of varying ages and those with diagnosed LD herniation. Our methodology combined histological assessments with single-nucleus RNA sequencing to identify phenotypic and molecular changes related to NP aging and herniation. We discovered that cellular senescence and a decrease in nucleus pulposus progenitor cells (NPPCs) are central to both processes. Additionally, we found an age-related increase in NFAT1 expression that promotes NPPC senescence and contributes to both aging and herniation of LD. This research offers fresh insights into LD aging and its associated pathologies, potentially guiding the development of new therapeutic strategies to target the root causes of LD herniation and aging.
