ObjectiveIn the context of the digital transformation of education, this study aims to construct a triadic interactive teaching model for surgical animal surgery in clinical medicine using modern information technology. It explores the effectiveness of different teaching methods in improving students' practical skills, aseptic awareness, and teamwork abilities, providing a reference for the reform of clinical practice education. MethodsA quasi-experimental research design was adopted. A total of 80 students from the eight-year clinical medicine program at Nanjing University were selected, including the Class of 2020 (control group, n=40) and the Class of 2021 (experimental group, n=40). The control group received traditional teaching methods, while the experimental group implemented the "Teacher-Student-AI" triadic interactive teaching model. This model utilized a smart teaching platform for personalized pre-class preparation , as well as data-driven post-class review and feedback throughout the entire teaching process. The "assessment indicators and scoring criteria for the surgical animal surgery course" were used to evaluate teaching effectiveness, with independent samples t-tests used for statistical analysis. ResultsPre-course assessments revealed no statistically significant differences in baseline theoretical knowledge or practical skills between the two groups (P>0.05). Upon completion of the course, the experimental group achieved higher scores than the control group across three key dimensions: practical skills (47.98±1.34 vs 46.92±2.51, P=0.022), aseptic awareness (17.84±1.16 vs 16.94±2.29, P=0.029), and teamwork (16.82±1.44 vs 15.95±1.22, P=0.004). However, no statistically significant difference was observed in the scores for humane care awareness between the two groups (8.24±0.70 vs 8.16±0.53, P=0.589). ConclusionThe AI-based triadic interactive teaching model can, to some extent, address the limitations of traditional surgical animal surgery education. It plays a positive role in enhancing medical students' surgical skills, aseptic awareness, and collaborative abilities. This model facilitates the transition from traditional to personalized teaching and offers a practical framework for the digital reform of clinical practice education.

Bovine rotavirus(BRV)is an important pathogen causing diarrhea in calves. To understand the genomic charac-teristics and genetic variations in bovine rotavirus, and to further enrich data on the biological characteristics of rotavirus,we aimed to amplify 11 gene segments of the isolated and cultured G6P[1]bovine rotavirus BLL strain, perform whole genome se-quencing,and analyze the molecular characteristics.MEGA7.0 and DNAMAN software were used for homology and typing a-nalysis,and the whole genome phylogenetic tree was constructed to analyze genetic evolution relationships. The complete geno-type of the BLL strain was G6-P[1]-I2-R2-C2-M2-A3-N2-T6-E2-H3.Phylogenetic analysis of the VP7 and VP4 genes of the BLL strain showed that the VP7 gene had the highest homology with RVA/Cow-wt/HB01/China/2021,and the VP4 gene of the BLL strain was in the same branch as RVA/Human-tc/ISR/Ro8059/1995.From the sequence alignment of VP8*amino acids, the sialic acid domain of the BLL strain was found to be similar to that in other P[1]strains,but different from those in other types of strains, except for residue 189,which was the same as that in Ro8059 but different from that in other strains. The results suggested that the BLL strain might potentially infect humans. Therefore, continued monitoring and study of the biological characteristics of this strain are necessary to provide more information and evidence supporting further research on the cross-species transmission of group A rotavirus in China.

Objective:An inceasing number of commercial multi-chambered bag products for parenteral nutrition (PN) have been launched in China, with some being included in the National Medical Insurance Catalogue (NMIC). In this study, the commercially available triple-chambered bag (3CB) products for PN in China were comprehensively analyzed in terms of basic characteristics, applicable population, and costs to guide the rational use of these products in clinical settings. Methods:The marketing information of 3CB products was collected. The inclusion of these products in NMIC and the Catalogue of Selected Drugs for National Centralized Procurement was investigated, along with the collection of manufacturer's instructions and pricing information in Beijing. Two researchers extracted and calculated the information of 3CB products, including energy density, types and levels of each component, glycolipid ratio, osmotic pressure, and price. The applicable population and economy of these products were also compared. Results:As of April 2024, 11 3CB products, including 23 specifications, had been approved in China. Eight products were included by NMIC and 2 by the Catalogue of Selected Drugs for National Centralized Procurement. The energy density, protein content, and lipid content dramatically differed among 3CB products (energy density, 254.0–493.7 kJ/100 ml; protein, 2.2–5.6 g/100ml; lipid, 2–4 g/100 ml; and glycolipid ratio, 62∶38–40∶60). Five products could only be infused via the central veins. The sources of fat emulsions included soybean oil-based long-chain fat-milk, medium-long-chain fat-milk, olive oil-based fat-milk, and fish oil-based fat-milk. All the 3CB products contained 8 essential amino acids and some non-essential amino acids, with or without tyrosine, taurine, aspartic acid, and glutamic acid. The average daily total cost of 3CB PN was 74.78–585.82 yuan. Since Some 3CB products had been listed in NMIC, they were affordable for urban patients covered by Urban Employee Basic Medical Insurance but might be economically unacceptable for rural residents who are insured by the Basic Medical Insurance for Urban and Rural Residents and uninsured patients. Conclusions:A variety of 3CB products have been commercially available in China and can meet the individual PN needs of most patients. Although some products have been listed by NMIC, the economic burden can be heavy for certain patient populations.

ObjectiveTo investigate the mechanisms of mitochondrial dynamics and metabolic reprogramming in the treatment of diabetic nephropathy (DN) by Shenxiao Tongluo prescription via the peroxisome proliferator-activated receptor γ coactivator-1α (PGC-1α)/sirtuin-3 (SIRT3)/hypoxia-inducible factor-1α (HIF-1α) signaling pathway. MethodsSixty-five SD rats were randomized into a sham group (10 rats) and a modeling group (55 rats), and the modeling rats underwent left nephrectomy and intraperitoneal injection of streptozotocin (35 mg·kg-1) to prepare a DN model. After successful modeling, the rats were randomized into model, empagliflozin (10 mg·kg-1), and low-, medium-, and high-dose (7.656, 15.312, 30.624 g·kg-1, respectively) Shenxiao Tongluo prescription groups. The urine microalbumin (UmAlb), blood urea nitrogen (BUN), and serum creatinine (SCr) levels of rats in each group were assessed after continuous gavage for 8 weeks. The corresponding kits were used to measure the levels of lactate, superoxide dismutase (SOD), and malondialdehyde (MDA) in the kidney tissue. Hematoxylin-eosin staining, Masson staining, and periodic acid-Schiff staining were performed to observe the pathological changes in the kidney tissue. Transmission electron microscopy was employed to observe mitochondrial morphology. Immunohistochemistry was employed to determine the expression levels of dynamin-related protein 1 (DRP1) and pyruvate kinase M2 (PKM2) in the kidney tissue. Western blot was adopted to assess the protein levels of PGC-1α, SIRT3, HIF-1α, dynamin-related protein 1 (Drp1), optic atrophy 1 (OPA1), hexokinase 2 (HK2), and pyruvate kinase M2 (PKM2) in the kidney tissue. ResultsCompared with the sham group, the model group showed elevated levels of UmAlb, BUN, SCr, lactate, and MDA, decreased SOD level (P<0.05), glomerular hypertrophy, thickening of the mesangial basement membrane, vacuolar degeneration of renal tubular epithelial cells, and infiltration of renal interstitial inflammatory cells, oval mitochondria with disordered, blurred or disappearing cristae, down-regulated protein levels of PGC-1α, SIRT3, and OPA1, and up-regulated protein levels of HIF-1α, DRP1, HK2, and PKM2 (P<0.05). Compared with the model group, the treatment in all the groups increased the body weight, lowered the levels of GLU, UmAlb, BUN, and MDA, raised the level of SOD, alleviated the pathological damage in the kidney tissue and mitochondrial damage, up-regulated the expression of PGC-1α, SIRT3, and OPA1, and down-regulated the expression of HIF-1α, DRP1, and PKM2 (P<0.05). Empagliflozin and Shenxiao Tongluo prescription at medium and high doses lowered the levels of SCr and lactate and down-regulated the expression of HK2 (P<0.05), which had no statistical significance in the low-dose Shenxiao Tongluo prescription group. ConclusionShenxiao Tongluo prescription may regulate mitochondrial dynamics and metabolic reprogramming by activating the PGC-1α/SIRT3/HIF-1α pathway, thereby alleviating oxidative damage in the kidney tissue and delaying the progression of DN.
