1.Establishment and verification of ultra high performance liquid chromatography and tandem mass spectrometry for determination of receptor binding domain protein content of virus-like particle SARS-CoV-2 vaccine

Objective To establish an ultra high performance liquid chromatography and tandem mass spectrometry(UHPLCMS/MS) method for determination of the protein content of the receptor binding domain(RBD) of virus-like particle(VLP)SARS-CoV-2 vaccine, and to validate and preliminarily apply it, so as to provide technical support for the quality control.Methods The characteristic peptide of RBD protein of SARS-CoV-2 vaccine was screened by high resolution mass spectrometer, and the content of RBD protein was determined by UHPLC-MS/MS. The separation was performed on a Thermo Fisher Hypersil GOLD C18(100 mm × 2. 1 mm, 3 μm) using the mobile phase A consisting of 0. 1% formic acid aqueous solution and mobile phase B consisting of 0. 1% acetonitrile formate solution at a flow rate of 0. 3 mL/min, via the gradient elution,0-3 min,2% B;3-10 min,2%→50% B;10-10.1 min,50%→90% B;10.1-12 min,90% B;12-12.1 min,90%→2% B; 12. 1-15 min, 2% B. The column temperature was 40 ℃ and the sample size was 1 μL. The specificity, stability,precision, linearity, limit of quantitation(LOQ), recovery and repeatability of the method were verified. Six batches of VLP SARS-CoV-2 vaccine bulk solution and six batches of finished products were detected by the method, and the correlation between the method and the method by amino acid analyzer was analyzed.Results The blank solution had no interference with the determination of characteristic peptide content. The test product solution showed good stability within 24 h at room temperature. The relative standard deviation(RSD) of the peak area of the characteristic peptide was 1. 0% after six consecutive injections of the characteristic peptide reference solution. The standard curve had a good linear relationship at the characteristic peptide concentration ranged from 0. 008 954 to 0. 358 200 μg/mL, and the LOQ was 0. 008 954 μg/mL. The RSDs of recovery rates of characteristic peptide in bulk solution and vaccine samples were all less than 10%. The contents of six batches of vaccine bulk solution and six batches of vaccine samples were 0. 169 40-0. 196 60 mg/mL and 0. 014 35-0. 020 50 mg/mL, respectively. The deviation of RBD protein content in VLP SARS-CoV-2 vaccine bulk solution determined by amino acid analyzer and characteristic peptide method was 5. 3%, and the determination results were basically the same.Conclusion The established UHPLC-MS/MS method is simple to operate with strong specificity and accurate quantitation,and can be used for the determination of RBD protein content in VLP SARS-CoV-2 vaccine.
2.Reliability and validity test of the Chinese version of the Urinary Incontinence Awareness and Attitude Scale

Objective To translate the Urinary Incontinence Awareness and Attitude Scale(URINAS)and test its reliability and validity.Methods The Chinese version of the URINAS was developed by Brislin's translation model for translation,back translation,cultural adaptation,and pilot investigation.A convenience sampling method was used to select 384 urinary incontinence patients who visited a tertiary hospital in Nanchang,Jiangxi Province from June 2024 to October 2024 for investigation,in order to evaluate the reliability and validity of the scale.Results The Chinese version of the URINAS consisted of 5 dimensions with 26 entries.The Cronbach's alpha coefficient of this scale was 0.843;the folded half reliability was 0.917;the retest reliability was 0.852.The content validity of the scale at the level of the entries ranged from 0.846 to 1.000,and that at the level of the scale was 0.979.A total of 5 metrics were extracted by exploratory factor analysis,and the cumulative variance contribution rate of 74.286%.The results of the validation factor analysis showed a chi-square/degree of freedom of 2.268,a root mean square error of approximation of 0.064,a standardized fit index of 0.916,a Tucker-Lewis index of 0.923,a comparative fit index of 0.906,and a goodness-of-fit index of 0.922.Conclusion The URINAS has good reliability and validity,and can better reflect the level of patients' cognition and attitude towards urinary incontinence,thus providing theoretical basis for the development of corresponding intervention programs for such patients.
3.A study of health needs profiling of potential service users in community health service organisations

Objective:To investigate the distribution of health needs and healthcare workforce demands among potential service users in community health service institurions (CHSIs) and establish a comprehensive health needs profile for targeted service development.Methods:This is a cross-sectional study. A questionnaire survey (55 items for adults and 39 items for minors) was conducted among 564 residents aged 3-65 in Dapuqiao Subdistrict, Shanghai from March to May 2024. A health sketch model was developed, encompassing five dimensions (intake, basal metabolism, consumption, influencing factors, and somatosensory perception). Health issues were mapped to standardized healthcare services, and annual standardized workloads (SW) were calculated to quantify human resource demands. Results:Among 534 valid responses (94.68% response rate), 355 from the adults and 179 from minors, the top health issues for adults were irregular diet, nutritional imbalance, obesity, somatic fatigue, and disordered eating; for minors, they were nutritional imbalance, medication overuse, insufficient physical activity, picky eating, and anorexia. The total annual SW required to address these needs was 3 906 807.93. Among them, the total SW for each age group was as follows: the total annual SW demand for children aged between 3 and 5 years old was 24 108.90, for children aged between 6 and 11 years old was 141 355.70, for adolescents aged between 12 and 17 years old was 29 813.74, 223 475.50 for 18-24 year olds, 476 426.53 for 25-34 year olds, 1 221 105.84 for 35-44 year olds, 812 200.22 for 45-54 year olds; and 978 321.50 for 55-65 year olds. Workforce allocation analysis revealed a total demand for 139.84 standardized personnel, with rehabilitation therapists (38.16), general practitioners (35.69), and nurses (27.34) being the top three required roles.Conclusions:Health needs and workforce demands vary significantly across age groups. This study provides a data-driven framework for optimizing community health services through stratified health needs profiling and precision resource allocation. Future strategies should prioritize tailored interventions and workforce planning to bridge service gaps and enhance health outcomes.
4.Human resource allocation in community health service center: a case study of discrepancy between supply and demand based on standardized workload

Objective:To investigate the descrepancy between current supply and demand based on standardiized workload for rational human resource allocation in community health service centers.Methods:A retrospective study was conducted in Dapuqiao Community Health Service Center of Shanghai Huangpu District from January 1st to December 3lst 2023. The current status of demand and supply were analyzed using the 2023 data of the institution. The standardised workload was measured according to the potential basic medical and public health service needs based on the disease profile and population groups in the community. The discrepancy between the current supply and theoretical demand was analyzed.Results:A total of 36 634 patients paid 277 262 visits in outpatient clinics of Dapuqiao Community Health Service Center in 2023. There are total 161 employees in the center, 124 of whom are female (77.02%). The actual calibrated workload of basic medical care and public health service in the center calculated on fee-based medical service items in 2023 was 2 888 425.61 and 455 898.35, respectively; while the theoretically calibrated workload was 5 416 492.42 and 711 230.94, respectively. The center employed 131 medical and health workers currently, calibrated as 90.67 employees based on current demand, while it should be 183.74 calibrated on the theoretical demand, there was a certain discrepancy between the current staffing and the standard staffing based on the standardized workload calculation.Conclusion:At present, the actual standardized workload of basic medical and public health services in Dapuqiao Community Health Service Center still has room for improvement, but there is a certain gap between the current human resource allocation and the demand.
5.Value of using ultrasound features to improve the Ovarian-Adnexal Image Reporting and Data System Category 4 in the benign-malignant differential diagnosis of ovarian-adnexal masses

Objective:To explore the value of ultrasound features modified version 2022 of the Ovarian-Adnexal Imaging Reporting and Data System(O-RADS)Category 4 in the differential diagnosis of benign and malignant ovarian-adnexal tumors.Methods:Retrospective analysis was conducted in 501 cases with ovarian masses classified into 4 categories according to the 2022 version of O-RADS who were collected from 4 clinical centers[the Second Afliated Hospital of Harbin Medical University(188 cases),Zhejiang Provincial People's Hospital(146 cases),Sichuan Provincial Maternity and Child Health Care Hospital(90 cases),and Fuling Hospital of Chongqing University(77 cases)]from January 2018 to July 2024 with concomitant surgical resection.The 424 cases from 3 of the clinical centers(the Second Hospital of Harbin Medical University,Zhejiang Provincial People's Hospital,and Sichuan Maternal and Child Health Hospital)were randomly divided into a training group(339 cases)and an internal validation group(85 cases)according to an 8∶2 randomization,while the cases from the other clinical center(Fuling Hospital of Chongqing University)were selected as the external validation group(77 cases),and the pathological diagnosis was used as the “gold standard”.Univariate and multifactorial logistic regression analyses were performed on the ultrasound characteristics of the training group to screen the independent predictors associated with ovarian carcinogenesis,and to formulate the stratification rules for the 4 types of masses in O-RADS. The ROC curve of this stratification method was plotted and the area under the curve(AUC)was calculated,and it was validated in the internal validation group and the external validation group;and the diagnostic accuracy was compared with that of the 2022 version of O-RADS.Results:Univariate logistic analysis showed that cysts with solid components,≥ 4 papillary projections,smooth inner wall of the cyst,color flow score ≥ 3 points,and acoustic shadowing were independent predictors of ovarian cancer(all P < 0.05);while multifactorial logistic analysis showed that cysts with a solid component and a color flow score ≥3 points were independent risk factors of ovarian cancer(all P < 0.05),and smooth cyst walls and acoustic shadows were independent protective factors(all P < 0.05).The diagnostic accuracies of the modified training group,internal validation group,and external validation group were 73.7%,68.2%,70.1%,respectively,which were significantly higher than the diagnostic accuracies of the 2022 version of the O-RADS(38.9%,37.6%,33.8%)(all P < 0.05).The diagnostic sensitivity,specificity and AUC of the training group were 0.871,0.652,0.762,respectively,while the internal validation group were 0.844,0.585,0.714,and 0.846,0.627,0.737 in the external validation group. Conclusions:Improvement of the 2022 version of O-RADS category 4 using ultrasound features may improve the identification of benign and malignant ovarian-adnexal tumors.
6.Proton pump inhibitor pantoprazole promotes colonization of Helicobacter pylori Sydney strain 1 in the mouse stomach

Objective To explore method for improving the colonization efficiency of Helicobacter pylori(Hp)in the mouse stomach and to determine if the proton pump inhibitor(PPI)pantoprazole can act as a colonization adjuvant to enhance Hp colonization,with the aim of providing an effective tool for establishing an Hp infection mouse model.Methods The Hp Sydney strain 1(SS1)was introduced and solid plate and liquid culture systems were established.The effects of different doses of pantoprazole on gastric acid secretion in mice were compared.The impact of Hp inoculation,alone or combined with pantoprazole pretreatment,on Hp colonization efficiency was analyzed using rapid urease tests,bacterial plate cultures,and TaqMan quantitative polymerase chain reaction.Results PPI pretreatment inhibited gastric acid secretion and promoted Hp colonization in the mouse stomach,to some extent.Conclusions PPI can serve as colonization adjuvants to enhanc e the efficiency of constructing Hp infection mouse models.
7.Contributing factors to intervention strategies for elevated blood pressure during medical selection of Air Force flying cadets

Objective:To investigate the contributing factors to elevated blood pressure in borderline cases during medical selection for recruitment of Air Force flying cadets in order to enhance the accuracy of selection.Methods:Blood pressure was measured among 2 350 male high school graduates in the 2022 re-selection phase of medical selection of Air Force flying cadets. None of the participants had a family history of hypertension according to previous health checkups. Identified through blood pressure measurement, subjects with borderline hypertension were assigned to an intervention group (self-intervention with personalized correction plans) and a control group (self-intervention alone) using a random number table. Standardized blood pressure measurements and comprehensive medical history reviews were performed to compare pre- and post-intervention outcomes across the 2 groups, followed by an investigation into the causative mechanisms of elevated blood pressure.Results:Among the 102 cases of borderline hypertension (51 per group) identified, primary contributing factors included the white-coat phenomenon (41.2%), pre-examination physical activity (17.6%), pre-examination medications (3.9%) and poor sleep quality (35.3%). No significant differences in systolic blood pressure (SBP) or diastolic blood pressure (DBP) were observed between the 2 groups at baseline (both P>0.05). After interventions, the intervention group showed significantly lower SBP ( t=3.13, P=0.002) and DBP ( t=7.68, P<0.001) than the control group. Both groups exhibited reductions in SBP and DBP from baseline ( t=6.63, 8.97, 4.13, 2.03, P<0.001, <0.001, <0.001, =0.043). The percentage of students with normal blood pressure was 96.1% (49/51) in the intervention group and 78.4% (40/51) in the control group. Conclusions:Transient blood pressure elevation in selection settings primarily stems from the white-coat phenomenon, physical exertion, medications and sleep disturbances. Standardizing blood pressure measurement protocols and addressing transient factors can help avoid unwarranted disqualifications and ensure the accuracy of selection.

Objective To translate the nursing time management scale(NTMS)into Chinese and test its reliability and validity,hence to provide a tool for evaluating the time management among nurses in China.Methods Authorised by the author on June 30th,2023,the English version of NTMS was translated into a Chinese version through forward translation,reverse translation,cross-cultural debugging and pre-test by using Brislin translation model.Between September 2023 and January 2024,a convenience sampling method was employed to select a total of 409 nurses as the test subjects from five hospitals of Grade IIA and above in Dongguan.Then the reliability and validity of the translated version of NTMS were tested.Results All 409 nurses completed the tests.The Chinese version of NTMS included three dimensions comprising 15 items:6 in nursing work planning and goal setting,3 in organisation of nursing work and 6 in coordination of nursing work.It was found that the mean scale content validity index(S-CVI/Ave)was 0.920,and the item-level CVI(I-CVI)ranged from 0.840 to 1.000.The overall Cronbach's α coefficient of the scale was 0.894,with 0.700 for the split-half reliability coefficient of and 0.942 for the retest reliability.The Cronbach's α coefficient of each of the 3 dimensions ranged from 0.781 to 0.901,with the split-half reliability coefficient from 0.823 to 0.870 and the retest reliability from 0.692 to 0.965.In the exploratory factor analysis,the KMO value was 0.904.The statistic test of Bartlett's sphericity was 3941.503(P<0.05),with the factor load range of each entry from 0.629 to 0.838.The cumulative variance contribution rate was 67.672%.Conclusion The translated Chinese version of the nursing time management scale demonstrates good reliability and validity.It can be applied as an effective evaluation tool in the nursing time management.
