Multiple cranial neuropathies are complex neurological disorders that present significant treatment challenges due to their intricate and multifaceted underlying causes. Persistent trigeminal artery variant (PTAV) is an unusual anastomosis that connects the internal carotid artery to the cerebellar artery, bypassing the basilar artery. The incidence of PTAVs is approximately 0.18% and neurovascular compression is rarely reported. Patients with schwannomatosis typically have multiple schwannomas without vestibular nerve involvement, which may develop in the spinal nerve roots and, less commonly, in the cranial nerves. We report the case of a 53-year-old female with trigeminal neuralgia associated with PTAV and schwannomatosis in the abducens and lower cranial nerves as dual etiologies for multiple cranial neuropathies.

Heart failure (HF) is a major cause of mortality and morbidity in South Korea, imposing substantial physical, emotional, and financial burdens on patients and society. Despite the high burden of symptom and complex care needs of HF patients, palliative care and hospice services remain underutilized in South Korea due to cultural, institutional, and knowledge-related barriers. This position statement from the Korean Society of Heart Failure emphasizes the need for integrating palliative and hospice care into HF management to improve quality of life and support holistic care for patients and their families. By clarifying the role of palliative care in HF and proposing practical referral criteria, this position statement aims to bridge the gap between HF and palliative care services in South Korea, ultimately improving patient-centered outcomes and aligning treatment with the goals and values of HF patients.

Background: With increasing life expectancy, the number of older adults with multiple chronic conditions requiring complex medication regimens is growing, raising concerns about polypharmacy and potentially inappropriate medication (PIM) use. This study investigated trends in polypharmacy and PIM use among 66-year-olds in South Korea from 2012 to 2021, considering participant characteristics, to inform interventions and policies. Methods: A repeated cross-sectional study was conducted using the National Health Insurance Services database covering approximately 97% of Koreans. We included 3,397,044 individuals aged 66 who underwent the National Screening Program for Transitional Ages between 2012 and 2021. Polypharmacy was defined as the use of ≥5 medications for ≥90 days annually, hyper-polypharmacy as ≥10 medications, and PIM use as ≥1 PIM for ≥28 days. Trends were analyzed by sex, frailty, comorbidity, income, insurance type, and residence. Results: Polypharmacy prevalence increased from 32.0% in 2012 to 35.4% in 2021, and hyper-polypharmacy also rose. PIM use slightly decreased from 55.7% to 53.7%. Higher rates of polypharmacy and PIM use were observed among rural residents, medical aid beneficiaries, and those with lower income. Despite improvements in comorbidity and frailty, socioeconomic disparities widened, particularly among medical aid beneficiaries. Frequently prescribed PIMs included NSAIDs, PPIs, muscle relaxants, and anxiolytics/hypnotics. Conclusion While PIM use slightly decreased over the study period, it remained above 50%, and polypharmacy prevalence increased among older adults in Korea. Socioeconomic disparities in medication use persist, highlighting the need for targeted interventions and policies to promote safe medication use among vulnerable groups.

We aimed to identify myocardial tissue–based proteomic characteristics and the underlying molecular pathophysiology in non-ischemic cardiomyopathy with different etiologies. Methods: Comparative extensive proteomic analysis of the myocardium was performed in nine patients with biopsy-proven non-ischemic cardiomyopathies (3 dilated cardiomyopathy [DCM], 2 hypertrophic cardiomyopathy [HCM], and 4 myocarditis) as well as five controls using tandem mass tags combined with liquid chromatography–mass spectrometry.Differential protein expression analysis, Gene Ontology (GO) analysis, and Ingenuity Pathway Analysis (IPA) were performed to identify proteomic differences and molecular mechanisms in each cardiomyopathy type compared to the control. Proteomic characteristics were further evaluated in accordance with clinical and pathological findings. Results: The principal component analysis score plot showed that the controls, DCM, and HCM clustered well. However, myocarditis samples exhibited scattered distribution. IPA revealed the downregulation of oxidative phosphorylation and upregulation of the sirtuin signaling pathway in both DCM and HCM. Various inflammatory pathways were upregulated in myocarditis with the downregulation of Rho GDP dissociation inhibitors. The molecular pathophysiology identified by extensive proteomic analysis represented the clinical and pathological properties of each cardiomyopathy with abundant proteomes. Conclusions Different etiologies of non-ischemic cardiomyopathies in advanced HF exhibit distinct proteomic expression despite shared pathologic findings. The benefit of tailored management strategies considering the different proteomic expressions in non-ischemic advanced HF requires further investigation.

In periodontitis, NETosis acts asa crucial defense mechanism, characterized by the expression of citrullinated histones. This study aims to evaluate the impact ofage and smoking on the expression of citrullinated histones in patients with periodontitis and the feasibility of using saliva for their detection. Materials and Methods: A total of 19 patients participated in the experiment. Among them, 15 were grouped accord-ing to smoking status and age, with 5 patients assigned to each group. After clinical and radiographic examinations, blood sampleswere collected from the subjects, and serum was separated. For 4 subjects, both blood and saliva samples were collected. Thelevels of citrullinated histones in the samples were quantified using an ELISA kit, and the results were compared and statisticallyanalyzed. Results: The concentration of citrullinated histones was significantly higher in younger individuals (P < 0.05). Although the expression of citrullinated histones was higher in the smoking group compared to the non-smoking group, the difference was not statistically significant. In patients with periodontitis, the levels of citrullinated histones detected in saliva were comparable to those found in serum. Conclusion Age may be potential risk factors influencing the expression of citrullinated histones in patients with periodontitis. Additionally, saliva, which is easier to collect, could be used as a viable sample for detecting citrullinated histones in periodontitis patients.
