1.Traditional Chinese medicine compound in treating idiopathic membranous nephropathy network Meta-analysis

Objective To evaluate the clinical effectiveness and safety of different traditional Chinese medicine(TCM)prescriptions combined with conventional western drugs to improve idiopathic membranous nephropathy(IMN)based on the Chinese medicine evidence system.Methods A computerized search of databases was conducted to collect randomized controlled trials(RCTs)of TCM compound formulas combined with conventional western medicines for the treatment of IMN,with a search timeframe from the establishment of the database to October 2023.Thirty-five RCTs involving six interventions and containing 2518 IMN patients were finally included.The quality of the included literature was evaluated.Reticulated Meta-analysis was performed,and surface under the cumulative ranking(SUCRA)curve was used to rank the clinical efficacy and safety of the six interventions,aiming to provide an evidence-based basis for the rational clinical selection of prescriptions for treating patients with IMN.Results The formula of tonifying deficiency and eliminating stasis combined with conventional western medicine showed significant effects in lowering 24-hour urine protein quantification(SUCRA=95.3%)and total cholesterol level(SUCRA=98.2%).The formula of tonifying deficiency,eliminating stasis,purging heat,and dispelling dampness combined with conventional western medicine was effective in lowering blood creatinine(SUCRA=80.4%)and increasing serum albumin level(SUCRA=72.9%).The formula of tonifying deficiency and clearing heat and dampness combined with conventional western medicine was more effective in lowering M-type phospholipase A2 receptor antibody titer(SUCRA=86.0%).Conclusion In terms of safety,the adverse effects of the TCM compound formula combined with conventional western drugs were overall less than those of conventional western drugs alone.
2.Study on the TCM Syndrome Distribution Law of Patients with Henoch-Schonlein Purpura Nephritis in Shaanxi Area

Objective To explore the syndrome characteristics of Henoch-Schonlein purpura nephritis in Shaanxi area;To provide reference for TCM clinical syndrome differentiation and treatment.Methods Clinical data on patients diagnosed with Henoch-Schonlein purpura nephritis who received outpatient or inpatient treatment at five hospitals in Shanxi area including Hospital of Shaanxi University of Chinese Medicine and Yulin Traditional Chinese Medicine Hospital etc from August 2020 to August 2022 were under retrospective collection.TCM syndrome distribution law in patients with Henoch-Schonlein purpura nephritis in Shaanxi area was concluded through frequency statistics,clustering analysis,principal component analysis,and complex network analysis.Results Totally 143 patients were included,involving 64 symptoms.3 groups of syndromes were obtained after clustering analysis,and principal component analysis was performed for each group of syndromes.Using complex network analysis and combining the results of clustering analysis and principal component analysis,classification groups for each group of main symptoms and comorbidities were formed,and their main syndrome classification was determined:qi-yin deficiency syndrome,accounting for 47.55%;spleen deficiency and dampness,accounting for 34.97%;wind-heat attacking the exterior,accounting for 17.48%.Conclusion The syndromes of Henoch-Schonlein purpura nephritis in Shaanxi area are mainly divided into three types:qi-yin deficiency,spleen deficiency and dampness and wind-heat attacking the exterior.Among them,qi-yin deficiency is the most common.
3.Exploration of Deng Gaopi's Diagnostic and Therapeutic Approach for Cesarean Scar Pregnancy

Cesarean scar pregnancy(CSP)is a special type of ectopic pregnancy(EP)that poses a serious threat to women's reproductive health.Professor Deng Gaopi,a renowned traditional Chinese medicine(TCM)expert of Guangdong Province,has extensive experience in the diagnosis and treatment of EP.He believes that the key pathogenesis of EP is due to the obstruction of blood stasis in the lower abdomen and the disharmony of chong-ren meridians,with the core pathogenic factor of blood stasis and with the disease location in the uterus.As a special type of EP,CSP shares the similar pathogenesis to that of EP.The occurrence of CSP is attributed to the obstruction of qi,blood and body fluids induced by insufficient healthy qi after cesarean delivery,and then phlegm-turbidity and blood stasis congealing in the chong-ren meridians and uterus,resulting in blood vessel stasis and collateral damage,which causes the fertilized egg to implant in the scar.The treatment of CSP should start from the pathogenesis of phlegm blended with blood stasis and stasis developing into masses,with the main therapeutic methods of supplementing qi,activating blood,resolving stasis,and removing masses.The foundational prescriptions include Huayu Xiaozheng Granules(composed of Salviae Miltiorrhizae Radix et Rhizoma,Paeoniae Radix Rubra,Persicae Semen,Achyranthis Bidentatae Radix,Moutan Cortex,Trichosanthis Radix,Arnebiae Radix,etc.)and Lichong Decoction(composed of Astragali Radix,Codonopsis Radix,Atractylodis Macrocephalae Rhizoma,Dioscoreae Rhizoma,Sparganii Rhizoma,Curcumae Rhizoma,Anemarrhenae Rhizoma,Trichosanthis Radix,Galli Gigerii Endothelium Corneum,etc.).Based on the relationship between qi and blood,i.e.,the enrichment of qi promoting the generation of blood,and the movement of qi promoting the removal of stasis,he emphasizes the importance of treatment based on qi-blood differentiation.Additionally,he is skillful in using genuine Lingnan medicinal herbs such as Microctis Folium and Guangdong Pheretima based on regional characteristics and following the principle of individualization.In terms of preventing CSP and its complications,Professor Deng advocates for strengthening the spleen and promoting granulation to fundamentally eliminate to the endometrial damage caused by cesarean section.For complications caused by damp-heat accumulation in the lower energizer which results from the obstruction of blood stasis in the uterus,heat-clearing,dampness-removing,and blood-activating actions should be enhanced in the clinical prescriptions for treatment.

Talents are the main force for the development of traditional Chinese medicine(TCM), and the construction of TCM talents and the reformation of talent evaluation system are essential to promote the inheritance and innovation of TCM. At present, we are still exploring and developing in the fields of the formulation, implementation and evaluation indicators of TCM talent evaluation system. However, there are shortcomings and difficulties. For instance, insufficient stratification in the evaluation, excessive emphasis on the quantity of achievements, neglecting the quality of the achievements and the actual contribution, imperfect assessment indicators, and the weak characteristics of TCM. Therefore, national ministries and commissions have jointly issued a document requesting to break the four only and set a new standard, in order to promote the construction of a scientific and technological talent evaluation system oriented by innovation value, ability and contribution. For the evaluation of TCM clinical talents, China Association for Science and Technology commissioned China Association of Chinese Medicine to build the China Clinical Cases Library of TCM(CCCL-TCM), which aims at collecting the most authoritative and representative TCM clinical cases and exploring the advantages of applying clinical cases as masterpiece of achievement in TCM clinical talents evaluation. CCCL-TCM can promote the construction of a talent evaluation system that is more in line with the development characteristics of TCM industry, and to carry out relevant pilot in TCM colleges and institutions across the country in order to promote the reformation of TCM talent evaluation system.
7.Prevention and Treatment of Diabetic Kidney Disease with Traditional Chinese Medicine Based on Diabetic Tubulopathy

Diabetic kidney disease(DKD)is one of the common chronic complications of diabetes mellitus(DM),the primary cause of chronic kidney disease(CKD),including end-stage renal disease(ESRD),and an important risk factor for early death in diabetic patients.As an important starting point of DKD,diabetic tubulopathy(DT)is important for the early prevention and treatment of DKD.It is found that traditional Chinese medicine can effectively delay the course of DKD through the active prevention and treatment of DT,and achieve early prevention,early treatment and good prognosis.The mechanism is mainly to improve and protect the damaged renal tubules in patients in various periods through different targets and pathways,so as to reduce kidney injury,improve the clinical symptoms of patients and delay the course of the disease.This paper reviews the mechanism of renal tubular injury in diabetic kidney disease and the progress of traditional Chinese medicine in the prevention and treatment of diabetic kidney disease based on diabetic tubulopathy in recent years.

[Objective]To analyze the meaning of"body pain is like being punished with a rod"in Synopsis of the Golden Chamber·Yin Yang Pestilences,so as to better understand the meaning of Zhongjing.[Methods]By using the research method of combining the handed-down literature with the textual research of unearthed cultural relics,combining with the perspective of character and history,and there is one proved case attached.[Results]"Body pain is like being punished with a rod"is a vivid expression that people describe their personal experience."Rod"can be understood as"anything that can be held is called a rod"."Being punished with a rod"is divided into three cases including skin is swollen and bruised;damage occurs;dead by being punished.Pain may be caused by trauma(skin and muscle)or internal injury(viscera injury).Shengma Biejia Decoction is mainly used to detoxify and disperse evil,promote blood circulation,and give consideration to both Qi and blood.[Conclusion]In the era of Zhongjing,"body pain is like being punished with a rod"has a certain degree of differentiation,which is reflected in the differentiation of the way of applying the rod and the painful feeling.This paper discusses the concept of"body pain is like being punished with a rod"by combining literature and cultural relics research,so that readers can intuitively understand the meaning of Zhongjing in the social background at that time,and also provides a new way to learn Synopsis of the Golden Chamber.
9.Traditional Chinese Medicine Intervention in Membranous Nephropathy Based on Podocyte Injury: A Review

Membranous nephropathy, an autoimmune kidney disease with glomerular podocyte injury as the core mechanism, is a common pathological type of adult nephrotic syndrome and has become the main cause of end-stage renal disease in China. Podocytes are terminally differentiated cells that play an important role in maintaining the structural and functional stability of glomeruli and are molecular barriers for protein filtration. Glomerular filtration membrane injury induced by podocyte injury is an important cause of massive proteinuria. Persistent or aggravated proteinuria may prolong the course of membranous nephropathy. It is believed that podocyte destruction in membranous nephropathy is mainly related to oxidative stress, autophagy dysregulation, abnormal expression of podocyte marker proteins, chronic inflammation, epithelial-mesenchymal cell transdifferentiation, and so on. At present, western medicine mostly uses immunosuppressants and hormones for treatment according to its pathological stage, but there are certain adverse reactions. Traditional Chinese medicine (TCM) has made some achievements in the prevention and treatment of membranous nephropathy. In recent years, studies have found that many Chinese medicines can affect the occurrence and development of membranous nephropathy in different links by acting on multiple targets in the human body, with manifest advantages. This paper overviewed the podocyte injury mechanism in membranous nephropathy and summarized the treatment of membranous nephropathy with Chinese medicine monomers, compounds, and Chinese patent medicines in intervening related target pathways, aiming to provide a basis for the clinical treatment, basic research, and targeted drug development of TCM against membranous nephropathy.
