1.Application and management status of midline catheters in 1 954 hospitals

Objective:To investigate the application and management status of midline catheters in 1 954 hospitals, providing a basis for optimizing intravenous therapy nursing practices.Methods:This study used convenience sampling. From November 2023, members of the Intravenous Therapy Nursing Professional Committee of the Chinese Nursing Association selected 1 954 hospitals across various regions of China. Questionnaire on the Current Status of Intravenous Therapy in Hospitals at All Levels designed by the committee, based on literature review and expert discussions, was used to collect data on intravenous therapy practices in different hospitals. Multiple response analysis was applied to analysis the results of multiple-choice questions, where response numbers represent the total number of times each option was selected, and response rates refer to the proportion of selected times for each option out of all selected responses.Results:A total of 1 954 questionnaires were distributed across 31 provinces/municipalities/autonomous regions, and 1 954 valid questionnaires were returned, achieving a 100.0% valid response rate. Among the hospitals surveyed, 844 used midline catheters. Regarding the skin disinfection area for midline catheter insertion, the highest response rate was for a range of>20 cm. The highest response rate for catheter insertion techniques was ultrasound-guided Seldinger puncture. The highest response rate for maintenance interval was once a week. The top three responses for nursing documentation related to midline catheters were informed consent for intubation, puncture record, and maintenance record. The most frequently chosen processes were catheter placement, maintenance, removal, and complication management processes. The qualification for midline catheter intravenous therapy specialist nurses was mostly obtained through specialized nurse training, followed by hospital-based and department-based training.Conclusions:The application of midline catheters has rapidly developed but still reveals some deficiencies, including the choice of puncture tools, infection control, and catheter maintenance. It is recommended to improve nursing documentation and management processes related to midline catheters, establish industry standards suitable for China's national conditions, and strengthen and standardize the specialized training of intravenous therapy nurses to promote the healthy development of intravenous therapy in China.

Objective:To evaluate the clinical equivalence between a domestic calcipotriol/betamethasone dipropionate ointment and the originator product in the treatment of stable plaque psoriasis.Methods:A multicenter, randomized, double-blind, three-arm, parallel-group, active- and placebo-controlled study was conducted, and 449 patients aged 18 - 65 years with stable plaque psoriasis were enrolled from 25 hospitals (such as the First Affiliated Hospital of China Medical University). Eligible patients had a baseline physician's global assessment (PGA) score of ≥ 3 points, baseline body surface area (BSA) involvement of 5% - 30%, and a target lesion psoriasis area and severity index (TL-PASI) for plaque elevation of ≥ 3 points. Participants were randomly assigned in a 2:2:1 ratio to the test group ( n = 179), reference group ( n = 180), and placebo group ( n = 90), and applied the domestic calcipotriol/betamethasone dipropionate ointment, originator product, and ointment base respectively, once daily in the evening for 4 weeks. Efficacy and safety were assessed at weeks 1, 2, and 4. The primary efficacy endpoints were the treatment success rates and clinical success rates in each group at week 4. The per-protocol set (PPS) was used for the primary efficacy analysis, and the intention-to-treat (ITT) set for supplementary efficacy analysis. Equivalence between the test and reference preparations was tested using the Cochran-Mantel-Haenszel method adjusted for randomization strata. Superiority of the test and reference preparations over the placebo was also tested. Measurement data were compared among the 3 groups using analysis of variance or non-parametric tests, while treatment success rates, clinical success rates, and incidence rates of adverse reactions were compared using the chi-square test. Results:The ITT, PPS, and safety sets included 447, 420, and 448 patients, respectively. In the ITT set, patients were aged 43.6 ± 12.8 years, including 320 (71.6%) males and 127 (28.4%) females, and the disease duration was 11.21 ± 9.05 years; 316 (70.7%) had a PGA score of 3 points and 131 (29.3%) had a PGA score of 4 - 5 points. No significant differences in the baseline characteristics (including age, sex, disease duration and disease severity) were observed among the 3 groups (all P > 0.05). Based on the PPS analysis, the treatment success rates were 57.9% (99/171) in the test group, 50.3% (86/171) in the reference group, and 7.7% (6/78) in the placebo group, and the clinical success rates were 57.9% (99/171), 50.3% (86/171), and 10.3% (8/78), respectively; both the test and reference groups were superior to the placebo group in both treatment and clinical success rates (all P < 0.001) ; the rate differences for treatment success (90% confidence interval [ CI]: -1.3% - 16.4%) and clinical success (90% CI: -1.3% - 16.3%) between the test and reference groups were entirely within the pre-defined equivalence margin (-20% - 20%). Subgroup analyses by baseline PGA scores: for patients with a baseline PGA score of 3 points, the treatment success rates in the test, reference, and placebo groups were 60.8% (73/120), 52.1% (62/119), and 11.1% (6/54), respectively, and the corresponding clinical success rates were 61.7% (74/120), 53.8% (64/119), and 13% (7/54), respectively; the test and reference groups did not differ significantly in treatment or clinical success rates (both P > 0.05), but both showed higher success rates than the placebo group (all P < 0.001) ; the results of statistical comparisons among the 3 groups in patients with a baseline PGA score of 4 - 5 points were consistent with those observed in patients with a baseline PGA score of 3 points. The percentage reductions in PGA and TL-PASI scores from baseline to weeks 1, 2, and 4 showed significant differences among the 3 groups, which were significantly higher in the test and reference groups than in the placebo group (all P < 0.001), but did not differ between the test and reference groups (all P > 0.05). The primary adverse reactions were local skin reactions, such as pruritus, pain, and erythema. The incidence rates of adverse reactions were 8.9% (16/179) in the test group, 7.3% (13/179) in the reference group, and 7.8% (7/90) in the placebo group, with no significant difference among the 3 groups ( P > 0.05) . Conclusions:The domestic calcipotriol/betamethasone dipropionate ointment demonstrated clinical equivalence to the originator product in the treatment of stable plaque psoriasis, and the two agents exhibited comparable efficacy for patients with varying degrees of disease severity, and were comparable in the speed and degree of clinical improvement, with similar favorable safety profiles.
3.Relationship between MTHFR C677T gene polymorphism and peripheral edema after cerebral hemorrhage

Objective To investigate the relationship between MTHFR C677T gene polymorphism and peripheral edema after intracerebral hemorrhage.Methods Two hundred and sixteen patients with hypertensive intracerebral hemorrhage were divided into edema enlargement group and edema non-enlargement group according to whether cerebral edema enlargement occurred.Clinical data of the patients were collected,and the MTHFR C677T genotype was detected.Results The level of homocysteine(Hey),vascular endothelial growth factor(VEGF)level at 72 h after admission,blood loss and incidence of venous thrombosis of lower extremity in edema enlargement group were significantly increased than those in edema non-enlargement group(P<0.05-0.01).The frequency of TT genotype of MTHFR C677T in edema enlargement group was significantly higher than that of CT genotype,and the frequency of CT genotype was significantly higher than that of the frequency of CC genotype(all P<0.05).There were significant differences in MTHFR C677T genotype and allele frequency between edema enlargement group and edema non-enlargement group(X2=70.668,P<0.001;x2=84.139,P<0.001).Multivariate Logistic regression analysis showed that VEGF level at 72 h after admission,MTHFR C677T CT genotype and TT genotype were independent risk factors for cerebral hemorrhage edema enlargement(OR=1.013,95%CI:1.007-1.019,P<0.001;OR=5.534,95%CI:2.426-12.625,P<0.001;OR=33.088,95%CI:9.012-121.479,P<0.001).After adjusting for other factors,the OR of MTHFR C677CT to MTHFR C677CC was 5.53(95%CI:2.426-12.625,P<0.05),and the OR of MTHFR C677TT to MTHFR C677CC was 33.09(95%CI:9.012-121.479,P<0.05).The edema growth rate of TT genotype,CT genotype and CC genotype at MTHFR C677T site was significantly different(F=3.204,P=0.044).There was no significant difference in VEGF level among TT genotype,CT genotype and CC genotype of MTHFR C677T site at admission(F=0.978,P=0.379),but there was significant difference in VEGF level at 72 h after admission(F=5.819,P=0.007).Conclusion MTHFR C677T gene polymorphism is associated with secondary edema enlargement after intracerebral hemorrhage.
4.Effectiveness of three-dimensional printing-assisted vascularized fibular graft for repairing metatarsal defects.

OBJECITVE:
To investigate the effectiveness of three-dimensional (3D) printing-assisted vascularized fibular graft for repairing metatarsal defects.
METHODS:
Between November 2021 and February 2024, 11 patients with varying degrees of metatarsal defects caused by trauma were treated. There were 10 males and 1 female, aged 22-67 years, with a mean age of 51.2 years. The defect locations were as follows: the first metatarsal in 4 cases, the fifth metatarsal in 2 cases, the first and the second metatarsals in 1 case, the first to third metatarsals in 1 case, the third and the fourth metatarsals in 1 case, the third to fifth metatarsals in 1 case, and the first to fifth metatarsals in 1 case. The preoperative American Orthopaedic Foot & Ankle Society (AOFAS) score was 67.0 (48.5, 72.5). Based on 3D-printed bilateral feet models and mirrored healthy-side foot arch angles for preoperative planning and design, the vascularized fibular graft was performed to repair the metatarsal defects. At last follow-up, the medial and lateral longitudinal arches of bilateral feet were measured on weight-bearing X-ray films, and functional assessment was conducted using the AOFAS score.
RESULTS:
All operations were successfully completed, with an operation time ranging from 180 to 465 minutes (mean, 246.8 minutes). All incisions healed by first intention, with no occurrence of osteomyelitis. All patients were followed up 6-22 months (mean, 10 months). X-ray film reviews showed bone graft healing in all cases, with a healing time of 3-6 months (mean, 5 months). All patients underwent internal fixator removal at 6-12 months after operation. At last follow-up, no significant difference was observed in the medial and lateral longitudinal arches between the healthy and affected feet ( P>0.05). The AOFAS score of the affected foot was 78.0 (73.5, 84.0), showing a significant improvement compared to the preoperative score ( P<0.05). The effectiveness was rated as excellent in 1 case, good in 7 cases, fair in 2 cases, and poor in 1 case. Linear scarring remained at the donor site, with no functional impairment in adjacent joint movement.
CONCLUSION
3D printing-assisted vascularized fibular graft for repairing metatarsal defects can effectively restore the physiological angle of the foot arch, facilitate the recovery of weight-bearing alignment, promote good bone healing, and yield satisfactory clinical outcomes.

5.Relationship between serum Cc16 and CD62E expression levels and the severity of neonatal respiratory distress syndrome and pulmonary ultrasound scores

Objective To analyze the relationship between serum levels of Clara cell protein 16(Cc16),E-selectin(CD62E),the severity of neonatal respiratory distress syndrome(NRDS)and lung ultrasound scores.Methods A total of 200 neonates with NRDS were selected in this study.The severity of the disease was assessed using oxygenation index(P/F).The neonates were divided into three groups:the mild group(46 cases),the moderate group(65 cases)and the severe group(89 cases).The serum levels of Cc16 and CD62E were compared between the three groups.In addition,based on the lung ultrasound scores,neonates were divided into the low-score group(score<12 points,n=127),the medium-score group(score 12-24 points,n=51)and the high-score group(score>24 points,n=22).The receiver operating characteristic(ROC)curve was used to evaluate the predictive efficacy of the combined detection of serum Cc16,CD62E and lung ultrasound score for severe NRDS.Results Compared with the mild group,the serum levels of Cc16 and CD62E,and lung ultrasound scores were higher in the moderate group and the severe group(P<0.05).Compared with the low lung ultrasound score group,the serum levels of Cc16 and CD62E were higher in the moderate lung ultrasound score group and the high lung ultrasound score groups(P<0.05).Cc16 and CD62E were positively correlated with the severity of NRDS(rs=0.679 and 0.680,P<0.01),and they were also positively correlated with lung ultrasound scores(r=0.692 and 0.685,P<0.01).ROC analysis showed that the combined detection of Cc16,CD62E and lung ultrasound scores had a relatively high diagnostic value for severe NRDS(P<0.05).Conclusion The serum levels of Cc16 and CD62E in NRDS neonates are positively correlated with disease severity and lung ultrasound scores.
6.Analysis of Abnormal Hemoglobin Phenotypes and Gene Mutation in Diabetes Screening Population in Xi'an Region

Objective To investigate the mutational spectrum and phenotype of abnormal hemoglobin in the diabetes screening population in Xi'an Region. Methods A total of 30356 patients from the Second Affiliated Hospital of Xi'an Jiaotong University were collected and measured for HbA1c by high-performance liquid chromatography (HPLC) from January 2022 to December 2022. Both complete blood count and hemoglobin analysis were carried out when HPLC suggested an abnormal peak. Abnormal hemoglobins were identified by Sanger sequencing. Results In this study,the rate of abnormal hemoglobin in the diabetes screening population of Xi'an region was 0.096% (28/30356). A total of 7 types of abnormal hemoglobins were detected:Hb E (n=10),G-Coushatta (n=6),Hb G-Taibei(n=6),Hb Q-Thailand (n=2),Hb North Manchester (n=2),Hb Ottawa (n=1) and Hb Queens (n=1). One case was Hb G-Taipei combined with IVS Ⅱ-659_664 (-GCAATA),the first report in the world. The abnormal hemoglobin phenotypes were normal except for seven cases of Hb E. Except for Hb G-Coushatta and Hb Ottawa,which did not interfere with the glycation test,the other five abnormal hemoglobins affected the glycation test results. Conclusion There is a certain prevalence rate of abnormal hemoglobin in the diabetes screaning population of Xi'an,mainly Hb E,Hb G-Taibei and Hb G-Coushatta. Most abnormal hemoglobins present normal phenotypes,but they could interfere with glycation test results.

Objective:Guided by the requirements for discipline construction in high education, this study aims to plan the layout of disciplinary construction scientifically, examine the shortcomings in integrating Party building work with discipline development within universities, and further propose innovative approaches pertinently to facilitate a deep and seamless integration between Party building activities and discipline advancement.Methods:Through a combination method of literature review and case analysis, this research explored the intricate relationship between Party building work and discipline development in universities, as well as the current status of Party building work guiding discipline development. Taking the practice of a medical college's party branch in promoting discipline development through Party building work as an example, innovative strategies for Party building work to lead discipline development were proposed.Results:Currently, there was room for improvement in terms of awareness and understanding of party building work leading discipline development in universities, as well as in the integration mechanisms and working mechanisms. Party branches of a medical college had integrated party building into their routine work, merging high-quality Party branch work with discipline development by strengthening political awareness, improving Party building mechanisms, promoting education through Party building initiatives, and establishing new integration platforms, thereby exploring practical pathways for their fusion.Conclusions:Party building work is complementary and mutually facilitative to academic development. Guided by the spirit of scientists and through important approaches such as ″forming a development alliance through their combination″″forging high-level teams for party building and academic leadership talents″ and ″advancing publicity to empower and enhance the quality and efficiency of grassroots party building work″ can be effectively strengthened the organic integration of party building and academic development. This holds practical significance for improving educational quality, cultivating high-quality talents, and driving the sustainable development of the university.
8.Establishment of near-infrared spectroscopy quantitative models for moisture and index components in Alismatis Rhizoma decoction pieces

AIM To establish the near-infrared spectroscopy quantitative models for moisture,23-acetylalismol B and 23-acetylalismol C in Alismatis Rhizoma decoction pieces.METHODS The near-infrared spectroscopy(NIRS)data were collected in 95 batches of decoction pieces,after which drying method was adopted in the content determination of moisture,HPLC was applied to determining the contents of 23-acetylalismol B and 23-acetylalismol C,the quantitative models were established by partial least squares method combined with feature extraction algorithms.RESULTS The model training determination coefficients were 0.952 6,0.958 1 and 0.920 8,along with the prediction determination coefficients of 0.930 0,0.905 2 and 0.906 4,the residual prediction deviations(PRD)of 4.00,3.58 and 3.46,and the root mean square error ratios of prediction values to calibration values(RMSEP/RMSEC)of 1.15,1.11 and 1.06,respectively.CONCLUSION The quantitative models based on NIRS exhibit good prediction effects,which can be used for the rapid quality detection of Alismatis Rhizoma decoction pieces.

Objective:To assess the efficacy and safety profile of antaitasvir phosphate combined with yiqibuvir in the treatment of chronic hepatitis C (CHC) of various genotypes, without cirrhosis or with compensated cirrhosis.Methods:394 cases with CHC from 22 centers were collected from October 2021 to April 2023. They were randomly assigned to receive either the experimental drugs (antaitasvir phosphate 100 mg+yiqibuvir 600 mg) or placebo treatment in a 3∶1 ratio. The patients were administered drugs once a day for 12 consecutive weeks, and then followed up for 24 weeks after treatment cessation. All subjects were unblinded at the four-week follow-up following drug discontinuation, with the experimental drug group continuing to complete subsequent post-discontinuation follow-up. The placebo group was switched to receive the experimental drugs for a repeated 12-week treatment period and followed up for another 24 weeks after discontinuation of the drug (placebo delayed treatment phase).The sustained virologic response rate (SVR12) was observed for subjects in the double-blind phase and the placebo delayed-treatment phase at 12 weeks after treatment cessation.Virological resistance analysis was performed on subjects who failed treatment. 10.Application of corticomuscular coherence in the field of rehabilitation medicine:a bibliometric analysis

Objective To analyze the research status,development trends and hotspots in the field of corticomuscular coherence(CMC).Methods Relevant literatures on CMC from 1999 to 2024 were retrieved from the Web of Science Core Collection.CiteSpace 6.3R1 was ued to evaluate publications across multiple dimensions,including publication volume,countries/regions,institutions,authors,cited journals,cited references and keywords.Research hotspots and fron-tier dynamics were identified through keyword co-occurrence,clustering and bursting.Results A total of 447 articles were included.The overall publication trend for CMC research showed a fluctuating up-ward trajectory,peaking in 2023.China led in publication volume but exhibited relatively low centrality and inter-national collaboration.Germany led the way in academic influence.Aalto University and the University of Lon-don served as academic hubs for CMC research.The most influential research team,led by Mathieu Bourgui-gnon,focused on the physiological mechanisms and motor control applications of CMC.The Journal of Physiolo-gy-London was identified as the most impactful journal in the CMC field.CMC research spaned from molecular mechanisms to system-level behavior,delving into the interdisciplinary nexus of neuroscience and motor science while gradually extending to foundational studies in psychology and biology.High-interest keywords included synchronization,motor cortex and precision grip task.Current research hotspots encompassed connectivity,reor-ganization and activation.Future research was predicted to focus on advancements in CMC technology,interven-tions for neurological disorders,brain-muscle interaction mechanisms and signal processing.Conclusion CMC research has rapidly progressed in recent years,becoming a pivotal direction in motor function assess-ment and neurorehabilitation.Future studies should focus on optimizing CMC analytical techniques to enhance their application in brain-computer interfaces and personalized rehabilitation.Additionally,interdisciplinary col-laboration and international exchange should be strengthened to improve research quality and global impact.
