1.Fetal development of chromogranin A-positive gastrointestinal endocrine cells revisited: a histological study using human fetuses

Initial gastrointestinal endocrine cells (GIECs) likely appear at the proximal and distal sites of abdominal intestines and may take a close topographical relation with neural elements in the gut. We examined immunohistochemically-stained sections from 10 fetuses at approximately 8–18 weeks of gestational age (36–155 mm of crown-rump length). Irrespective of whether physiological herniation was present (early 5 specimens) or absent (the other 5), the duodenum and jejunum had well-developed mucosa with villi containing abundant flask-like chromogranin-positive cells. In the earlier 5 specimens, the rectum, standing up to a level of the umbilicus, had a lumen and villi with a few positive cells, but the colon carried neither the lumen or chromogranin-positive cells. The initial GIECs seemed to appear in the basal payer of the epithelium at the distal and proximal foci depending on double pathways of neural crest cell migration. Less number of the colic chromograninpositive cells, more than 5-times difference in density relative to small intestine, was seen in the larger 5 specimens. The appearance of GIECs was delayed at the anal transitional zone (a border area between the columnar and squamous epithelia).The reactivity of neuronal nitric oxide synthase was restricted in the myenteric plexus, whereas clusters of slender calretininpositive cells existed in the lamina propria or core of villi in the duodenum and colon. Relatively small, round or oval positive cells were also seen in the basal layer of the columnar epithelium. Therefore, calretinin-positive cells might exist closely to GIECs in the developing villi.

Neck epaxial muscles, which are differentiated for suspending the head, occupy a large space posterior to the cervical lordosis. Limited information exists regarding developmental process that determines the muscle fiber direction and bony attachment of neck epaxial muscles. We examined histological sections of 28 human fetuses aged approximately 7–18 weeks (crown-rump length, 20–150 mm). In place of the underdeveloped lordosis, the transverse process of cervical vertebrae was shifted anteriorly at the cervicothoracic junction. The semispinalis and longissimus were distinguished by the direction of muscle fibers connecting between the surface aponeurosis and transverse process. The semispinalis capitis and splenius capitis had a bulky anterior margin without bony attachments. The obliquus capitis inferior continued to both the rectus capitis posterior major and the semispinalis cervicis, but the obliquus capitis superior was consistently independent. Muscle attachments to the scapula were quite different from the final morphology: 1) the levator and rhomboidei usually extended inferiorly along the developing scapula beyond the inferior angle and 2) the splenius capitis or semispinalis cervicis rarely issued an aberrant bundle attaching to the scapula. The scaleni, rhomboidei, levator scapulae, iliocostalis and longissimus were arranged in parallel from the anteromedial to the posterolateral planes and together formed a thick oblique muscle bundle originating from the cervical transverse process and running toward the upper thoracic vertebra and ribcage. The transient oblique muscle bundle seen in early fetuses seemed to provide the so-called intermediate axial muscle between the epaxial-hypaxial muscles: a concept postulated in recent molecular neurology and embryology.
3.Disease Risk Factors Related to Lifestyle Changes Among Vietnamese Workers in Japan: A Qualitative Descriptive Study

Objective We investigated lifestyle changes and disease risk factors in Vietnamese workers after migrating to Japan.Methods From January to March 2022, semi-structured interviews were conducted with 15 Vietnamese workers in Japan regarding their daily habits, focusing on diet and environmental changes. Data were analyzed qualitatively. Results Six men and nine women (average age: 25.5 years) participated. Their residence status included Technical Intern Training (nine individuals), Designated Activities (two), and Engineer/Specialist in Humanities/International Services (four). Their periods of stay ranged from six months to eight years. We extracted 31 main categories (and 100 subcategories) from the interviews that indicated lifestyle changes from Vietnam to Japan. These changes included switching to an imbalanced diet, less fruit intake, greater snack intake, lower alcohol intake, and decreased physical activity after moving to Japan. Participants cited busy schedules (and therefore less time) as the primary reason for dietary changes and reduced physical activity in Japan. Additionally, stress levels rose significantly after migration, with some participants attributing that increase to lifestyle changes. Furthermore, despite reporting no major illnesses in Vietnam, health checkups after living in Japan identified disease risk factors, such as weight changes and negative health conditions (e.g., dyslipidemia, hypertension, liver dysfunction, and anemia). Conclusions Our findings suggest that lifestyle changes are related to post-migration abnormalities in health screening outcomes, including weight gain and dyslipidemia. Recognizing and providing early support for these changes are important in preventing the onset or worsening of noncommunicable diseases among migrant workers.
4.Early Orthotic Treatment and Occupational Therapy Improved Boutonnière Deformity in Rheumatoid Arthritis:A Case Report

Since hand deformities in rheumatoid arthritis (RA) are relatively not very painful, the worsening of the deformities often goes unnoticed and the functional impairment progresses irreversibly. Herein, we report a case of boutonnière deformity of the left middle and ring fingers treated with rehabilitation since an early stage. The patient was a 58-year-old woman who was referred to our hospital due to joint pain in the fingers and feet, following which a diagnosis of RA as made;however, she could not be administered methotrexate due to complications. PIP joint deformity of the left middle and ring fingers developed later. The middle finger was in -50-degree extension and difficult to correct passively, while the ring finger was in -35-degree extension and correctable. Surgery for the left middle finger was proposed based on the diagnosis of boutonnière deformity;however, consent was not obtained. After a steroid injection in the painful middle finger, she was managed using a Capener splint and ROM exercises with finger stretching. The symptoms improved five months following the rehabilitation intervention. Nalebuff et al. classified the severity of the boutonnière deformity based on the limited PIP joint extension and recommended treatment accordingly. In this case, surgical treatment was believed to be required;however, since the patient refused surgery, conservative treatment was chosen. Although the extension was severe, there was little joint destruction, due to which the symptoms improved with early and active intervention. Orthotic treatment and occupational therapy were effective in improving ADL.

Previous studies of midterm fetuses indicated that a cartilaginous fabella appeared to be embedded in the plantaris (PL), and was fused with the gastrocnemius lateral head (GL). We re-examined the topographical anatomy of the fabella or its analogue (a tight fibrous mass) originating in the GL and/or PL by evaluating histological sections of the unilateral knees of 15 late-term fetuses. Regardless of whether the cartilaginous fabella was present (6 fetuses) or absent (9 fetuses), the origins of the PL and GL muscles each had three parts. In each fetus, the fabella or its analogue was embedded in a thick common tendinous origin of the GL and PL. PL1 (whose origin is similar to that of the adult PL) originated from the femoral condyle immediately above the common tendon; PL2 originated from the posteromedial aspect of the fabella or its analogue; and PL3 originated from the inferior aspect of the fabella or its analogue. The muscle fibers of PL1, PL2, and PL3 joined to provide a thick plantaris. GL1 (which is adjacent to PL2) originated from the common tendon in the superior side of the fabella or its analogue and GL2 originated from the inferior side of the fabella or its analogue. GL1 and GL2 joined to provide a thick bundle, whereas GL3 (located far below the fabella or its analogue) originated from the posterior surface aponeurosis.Therefore, drastic reconstruction at these muscle origins was necessary during development. Due to the strong mechanical stress from the GL and the space-occupying effect of the muscle, we hypothesize that PL2 and PL3 are degraded or absorbed into the GL1 and GL2 during the postnatal period, so that the remaining PL1 was likely the remaining PL in adults.

STUDY DESIGN: Retrospective case series. PURPOSE: The purpose of this study was to examine changes in the ligamentum flavum thickness and remodeling of the spinal canal after anterior fusion during a 10-year follow-up. OVERVIEW OF LITERATURE: Extreme lateral interbody fusion provides minimally invasive treatment of the lumbar spine; this anterior fusion without direct posterior decompression, so-called indirect decompression, can achieve pain relief. Anterior fusion may restore disc height, stretch the flexure of the ligamentum flavum, and increase the spinal canal diameter. However, changes in the ligamentum flavum thickness and remodeling of the spinal canal after anterior fusion during a long follow-up have not yet been reported. METHODS: We evaluated 10 patients with L4 spondylolisthesis who underwent stand-alone anterior interbody fusion using the iliac crest bone. Magnetic resonance imaging was performed 10 years after surgery. The cross-sectional area (CSA) of the dural sac and the ligamentum flavum at L1–2 to L5–S1 was calculated using a Picture Archiving and Communication System. RESULTS: Spinal fusion with correction loss (average, 4.75 mm anterior slip) was achieved in all patients 10 years postsurgery. The average CSAs of the dural sac and the ligamentum flavum at L1–2 to L5–S1 were 150 mm² and 78 mm², respectively. The average CSA of the ligamentum flavum at L4–5 (30 mm²) (fusion level) was significantly less than that at L1–2 to L3–4 or L5–S1. Although patients had an average anterior slip of 4.75 mm, the average CSA of the dural sac at L4–5 was significantly larger than at the other levels. CONCLUSIONS: Spinal stability induced a lumbar ligamentum flavum change and a sustained remodeling of the spinal canal, which may explain the long-term pain relief after indirect decompression fusion surgery.

PURPOSE: The pathophysiology of discogenic low back pain is not fully understood. Tetrodotoxin-sensitive voltage-gated sodium (NaV) channels are associated with primary sensory nerve transmission, and the NaV1.7 channel has emerged as an analgesic target. Previously, we found increased NaV1.7 expression in dorsal root ganglion (DRG) neurons innervating injured discs. This study aimed to examine the effect of blocking NaV1.7 on sensory nerves after disc injury. MATERIALS AND METHODS: Rat DRG neurons innervating the L5/6 disc were labeled with Fluoro-Gold (FG) neurotracer. Twenty-four rats underwent intervertebral disc puncture (puncture group) and 12 rats underwent sham surgery (non-puncture group). The injury group was divided into a saline infusion group (puncture+saline group) and a NaV1.7 inhibition group, injected with anti-NaV1.7 antibody (puncture+anti-NaV1.7 group); n=12 per group. Seven and 14 days post-surgery, L1 to L6 DRGs were harvested and immunostained for calcitonin gene-related peptide (CGRP) (an inflammatory pain marker), and the proportion of CGRP-immunoreactive (IR) DRG neurons of all FG-positive neurons was evaluated. RESULTS: The ratio of CGRP-IR DRG neurons to total FG-labeled neurons in the puncture+saline group significantly increased at 7 and 14 days, compared with the non-puncture group, respectively (p<0.05). Application of anti-NaV1.7 into the disc significantly decreased the ratio of CGRP-IR DRG neurons to total FG-labeled neurons after disc puncture at 7 and 14 days (40% and 37%, respectively; p<0.05). CONCLUSION: NaV1.7 antibody suppressed CGRP expression in disc DRG neurons. Anti-NaV1.7 antibody is a potential therapeutic target for pain control in patients with lumbar disc degeneration.
