1.Anti-inflammatory Effect of 25-OH Vitamin D in Neonatal Infectious Pneumonia by Modulating the TGF-β-mediated YAP/TAZ Nuclear Translocation

Objective To investigate the anti-inflammatory effect of 25-OH vitamin D(25-OH-VD)on neonatal infectious pneumonia(NIP)and its mechanism.Methods A total of 65 children with NIP admitted to Chengdu Women and Children's Central Hospital from January 2022 to January 2023 were selected.According to the severity of the disease,they were divided into mild group(n=34)and severe group(n=31),and 60 healthy neonates in the same period were selected as the control group.Serum 25-OH-VD,interleukin-2(IL-2),interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α)and IL-1β levels were measured by ELISA.The expressions of vitamin D receptor(VDR),transforming growth factor-β(TGF-β)/nuolea Yes-associated protein(n-YAP)and nuclear transcriptional coactivator PDZ-binding motif(n-TAZ)pathway related proteins in peripheral blood mononuclear cells were detected by Western blot.NIP in vitro models were established by lipopolysaccharide(LPS)stimulation of human lung epithelial cells,which were divided into control group,LPS group and LPS+VD group.Cell viability was detected by CCK-8 assay.The mRNA expressions of recombinant cytochrome P450 27B1(CYP27B1)and VDR in each group were detected by qRT-PCR.The number of YAP positive nuclei in each group was detected by immunocytochemistry.The nuclear translocation of YAP/TAZ complex in each group was detected by immunofluorescence.The expression of inflammatory factor-related proteins in each group was detected by Western blot.Results Compared with the control group,the serum levels of IL-2(1.91±0.18 μ g/L,2.63±0.27 μg/L vs 1.05±0.12 μg/L),IFN-γ(1.73±0.13 μg/L,2.18±0.19 μg/L vs 1.03±0.07 μg/L),TNF-α(1.79±0.08 μg/L,2.38±0.13 μg/L vs 0.97±0.04 μg/L),IL-1 β(2.18±0.07 μg/L,2.59±0.11μg/L vs 0.96±0.02 μg/L),TGF-β(1.67±0.21,2.43±0.42 vs 1.02±0.04),n-YAP(2.08±0.11,4.23±0.37 vs 0.99±0.02)and n-TAZ(2.47±0.42,4.21±0.58 vs 1.03±0.05)proteins of the children in the mild and severe groups were gradually increased,but the content of 25-OH-VD(12.57±2.21 μg/L,7.85±2.03 μg/L vs 16.76±1.02 μg/L)and the expression of VDR(0.73±0.09,0.51±0.06 vs 1.03±0.08)proteins were gradually decreased,and the differences were significant(F=18.983~56.782,all P<0.001).Cell experiment results showed that cellular survival rate was lower at 12 h(76.23%±0.73%vs 116.72%±2.14%),24 h(57.23%±0.94%vs 125.76%±1.67%)and 48 h(41.23%±0.56%vs 138.56%±1.35%)in the LPS group compared with the control group(t=10.342,26.562,37.821),but the rate of cytosolic n-YAP positivity(47.35±3.47 vs 12.46±1.34),the rate of nuclear translocation of the YAP/TAZ complex(2.56%±0.32%vs 1.01%±0.06%)(t=46.362,26.921),the protein expression levels of IL-2(2.03±0.09vs 1.03±0.08),IFN-γ(2.07±0.21 vs 1.02±0.04),TNF-α(2.18±0.11 vs 0.99±0.02)and IL-1β(3.17±0.42 vs 1.03±0.05)were up-regulated(t=28.341,26.713,31.235,47.823),with significant differences(all P<0.001),while there was no significant difference in the mRNA expression of CYP27B1 and VDR(t=0.872,0.786,all P>0.05).Compared with the LPS group,the cell survival rate at 12h(85.23%±0.36%),24h(79.82%±0.63%)and 48h(76.28%±0.72%)and the mRNA expression of CYP27B1(4.42±0.14)and VDR(5.13±0.56)were elevated in the LPS+VD group,while the nucleus n-YAP positivity(24.41±3.23),nuclear translocation of the YAP/TAZ complex(1.47%±0.26%),IL-2(1.21±0.06),IFN-γ(1.13±0.42),TNF-α(1.03±0.37)and IL-1β(1.61±0.58)protein levels were down-regulated,and the differences were significant(t=7.263,19.892,23.145,27.872,26.982,14.762,13.623,18.273,25.314,27.873,22.134,all P<0.0 1).Conclusion The serum 25-OH-VD level is related to the severity of NIP.Exogenous VD supplementation may play an anti-inflammatory role in reducing NIP injury by regulating the TGF-β-mediated YAP/TAZ nuclear translocation mechanism.

Objective To observe the application effect of check-in supervision intervention based on Wechat APP in minimally invasive cerebrovascular intervention.Methods Patients treated with minimally invasive cerebrovascular intervention in our hospital were selected as study objects.Ac-cording to the implementation time of the check-in supervision intervention based on Wechat APP,55 patients admitted before the implementation were included in routine group,and 55 patients admitted after the implementation were included in Wechat group.Neurological function recovery[National In-stitute of Health Stroke Scale(NIHSS)and Modified Edinburgh-Scandinavian Stroke Scale(MESSS)],self-management ability[Scale of Health Self-management Skill for Adults(AHSMARS-Ⅱ)],medica-tion compliance,exercise compliance,quality of life[general quality of life inventory(GQOLI-74)]and nursing satisfaction were compared between the two groups.Results After intervention,NIHSS and MESS scores in the Wechat group were significantly lower than those in the conventional group,AHSMARE-Ⅱ scores,medication compliance,exercise compliance,GQOLI-74 scores and total nursing satisfaction were significantly higher than those in the conventional group(P<0.05).Conclusion The check-in supervision intervention mode based on Wechat APP can effectively improve the quality of home service for patients with minimally invasive cerebrovascular intervention after surgery,and improve pa-tients'self-management ability and compliance.

4.Status and influencing factors of paralytic ileus in patients with adolescent idiopathic scoliosis

Objective:To explore the occurrence and influencing factors of paralytic ileus (PI) in patients with adolescent idiopathic scoliosis (AIS), so as to provide references and basis for the formulation of clinical nursing intervention measures.Methods:Using the convenient sampling method, a total of 93 AIS patients who were hospitalized in Spine Center of Peking Union Medical College Hospital from January 2019 to June 2020 were selected as the research objects. The general information, disease and operation related information of patients were collected, and the incidence of PI was recorded. Binomial Logistic regression was used to analyze the influencing factors of PI occurrence in AIS patients after surgery.Results:PI occurred in 41.9% (39/93) patients, and the incidence of anorexia, abdominal distention, nausea, vomiting and abdominal pain were 66.7% (26/39), 61.5% (24/39), 59.0% (23/39), 30.8% (12/39) and 20.5% (8/39), respectively. Binomial Logistic regression analysis showed that straight leg elevation exercise ( OR=3.582, P=0.023), postoperative first feeding time ( OR=1.209, P=0.027), proton pump inhibitors ( OR=6.820, P=0.002) and osteotomy ( OR=5.516, P=0.007) were the influencing factors of PI occurrence after AIS. Conclusions:The incidence of PI is higher in AIS patients after surgery and the accompanying symptoms are mainly anorexia, abdominal distension and nausea. Straight leg elevation exercise, time of first postoperative feeding, proton pump inhibitors and osteotomy are the influencing factors of postoperative PI in AIS patients.
5.Enzalutamide combining with ADT as neoadjuvant therapy for prostate cancer: a case report

Traditional endocrine drugs, such as bicalutamide, are the first choice for neoadjuvant therapy of prostate cancer. There are few reports on the use of new endocrine drugs in neoadjuvant therapy in China. The patient, male, 63 years old. He was admitted to the hospital for the finding of prostate space occupying. Blood PSA 53.50 ng / ml. Prostate MRI suggested that the prostate lesion broke through the left capsule, the left seminal vesicle gland was invaded, and the bladder wall was invaded. Bone scan suggested that: the left 8th posterior costal branch radioactivity was limited and increased. Prostate adenocarcinoma was diagnosed by puncture, Gleason score 4 + 4 = 8 points, and stage T 4bN 1M 1. The patient was treated with goserelin combined with enzalutamide for 3 months, and PSMA-PET CT: prostate size was normal, no significant 68Ga PSMA uptake was increased, no abnormally high Ga PSMA uptake in bones. The patient was treated with enzalutamide combined with ADT as neoadjuvant endocrine therapy, winning surgical conditions for the patient to undergo surgical resection.
6.Effects of family-centered educational intervention on disease cognition and quality of life in patients undergoing surgery for adolescent idiopathic scoliosis

Objective:To explore the effects of family-centered educational intervention on disease cognition and quality of life in patients undergoing surgery for adolescent idiopathic scoliosis (AIS) .Methods:A total of 122 patients diagnosed with AIS at the Ward Department of Orthopedics in a ClassⅢ Garde A hospital in Beijing from September 2015 to December 2018 were selected by convenient sampling, and divided into the control group ( n=62) and the observation group ( n=60) according to the admission time. Patients in the control group received routine health education, while patients in the observation group received family-centered educational intervention on this basis. The self-designed disease knowledge questionnaire and the Chinese version of Scoliosis Research Society Outcomes Questionnaire (SRS-22) were used to evaluate the disease knowledge and quality of life in the two groups of patients upon admission, 3 months and 6 months after discharge. Results:There was no statistically significant difference in the scores of disease knowledge between the two groups upon admission, 3 months and 6 months after discharge ( P>0.05) . Repeated measures analysis of variance showed no statistically significant difference in the interaction effect and between-group effect of the two groups' scores at 3 the time points ( P>0.05) , whereas the difference in the time effect was statistically significant ( P<0.05) . Three months after discharge, the total score of SRS-22 in the control group was (4.14±0.36) , while that in the observation group was (4.32±0.30) , and the difference was statistically significant ( t=2.964; P<0.05) . Conclusions:Family-centered educational intervention can appropriately improve the quality of life in patients with AIS and help them recover from illness.
7.Effects of family-centered health education on parental disease management abilities in patients with adolescent idiopathic scoliosis

Objective:To explore the application effect of family-centered health education on parents of patients with adolescent idiopathic scoliosis.Methods:From September 2015 to December 2018, parents of patients who were diagnosed with adolescent idiopathic scoliosis in the Department of Orthopedics of a Class Ⅲ Grade A hospital in Beijing were selected as the research objects by convenience sampling method. According to the order of admission time, they were divided into the control group with 71 cases and the observation group with 67 cases. Parents in the control group were given routine health education while parents in the observation group were given family-centered health education on basis of the control group. The Chinese version of the State-Trait Anxiety Inventory and the Family Management Measure Scale were used to assess anxiety status and disease management abilities of parents, respectively.Results:There was no statistically significant difference in anxiety level of parents between the two groups ( P> 0.05) . At 3 and 6 months after discharge, the scores of parental disease management ability in the observation group were (45.89±4.18) and (41.84±3.02) , which were higher than (42.43±5.28) and (40.25±4.20) in the control group. The differences were statistically significant ( P<0.05) . The scores of parental disease management difficulties in the observation group were (28.20±4.71) and (32.98±6.16) , which were lower than (33.75±6.42) and (36.97±5.60) of the control group. The differences were statistically significant ( P<0.05) . Conclusions:Family-centered health education can improve parental disease management abilities of patients with adolescent idiopathic scoliosis after discharge, which is worthy of clinical reference and application.
8.Research on the cardiovascular function evaluation system based on noninvasive detection indices.

Based on the noninvasive detection indeices and fuzzy mathematics method, this paper studied the noninvasive, convenient and economical cardiovascular health assessment system. The health evaluation index of cardiovascular function was built based on the internationally recognized risk factors of cardiovascular disease and the noninvasive detection index. The weight of 12 indexes was completed by the analytic hierarchy process, and the consistency test was passed. The membership function, evaluation matrix and evaluation model were built by fuzzy mathematics. The introducted methods enhanced the scientificity of the evaluation system. Through the Kappa consistency test, McNemer statistical results ( = 0.995 > 0.05) and Kappa values (Kappa = 0.616, < 0.001) suggest that the comprehensive evaluation results of model in this paper are relatively consistent with the clinical, which is of certain scientific significance for the early detection of cardiovascular diseases.

9.Clinical, endoscopic and histopathological features of eosinophilic gastroenteritis in 76 children

Objective To analyze the clinical, endoscopic and histopathological features of eosinophilic gastroenteritis ( EG) in children. Methods A retrospective study of 76 children with EG was performed to analyze clinical symptoms, laboratory and imaging results, endoscopic and pathological features, status of Helicobacter pylori ( H. pylori) infection, treatment and outcomes. Results The main clinical symptoms were abdominal pain in 55. 3%(42/76) cases, vomiting in 39. 5% (30/76) cases and hematochezia in 38. 2% cases( 29/76) . The hemoglobin level decreased significantly in 34 cases ( 44. 7%, 34/76). Peripheral blood eosinophil (EOS) count increased significantly in 9 cases (11. 8%,9/76) and EOS percentage increased significantly in 13 cases(17. 1%,13/76). Total serum IgE elevated in 32 cases ( 54. 2%, 32/59 ) . There were also 18 cases ( 36. 7%, 18/49 ) positive in serum allergen?specific immunoglobulin E ( sIgE) test and 25 cases ( 32. 9%,25/76) positive in fecal occult blood test. Among 51 cases of abdominal ultrasound examination, there were 7 cases of ascites, 4 cases of pelvis fluid and 3 cases of intestinal wall change. Endoscopic examination in 76 cases showed 63 cases ( 82. 9%) of mucosal hyperemia/edema,20 cases ( 26. 3%) of ulceration, 17 cases ( 22. 4%) of erosion, 11 cases ( 14. 5%) of nodularity or hyperplasia and 9 cases ( 11. 8%) of normal mucosa. The pathological examination showed mucosal inflammation with a large number of EOS infiltration(≥20 per HPF).There were 12 cases(15. 8%, 12/76) of H. pylori infection. Among the 76 cases, clinical symptoms improved significantly in 74 patients after treatment with dietary allergen avoidance, anti?allergy medications, antacids, montelukast and corticosteroid, and the total efficacy was 97. 4%. The efficacy of dietary allergen avoidance, anti?allergy medications, antacids and montelukast was 93. 8%( 61/65 ) . The efficacy of corticosteroid was 86. 7%(13/15). Conclusion The clinical manifestations and endoscopic characteristics of EG in children lack specificity. In terms of diagnosis, the elevated total serum IgE and the positive sIgE test may be taken as reference for the diagnosis of EG. The definite diagnosis is based on pathological examination ( EOS infiltration≥20 per HPF).While in terms of treatment, dietary allergen exclusion, anti?allergy medications, antacids and montelukast are highly effective, which can be taken as the first option. There is no need of corticosteroid as routine therapy.
