1.The predict value of CT angiography combined with CT perfusion parameters and serum biology for recurrent stroke events

Objective To investigate the predict value of imaging parameters in computed tomography perfusion(CTP)combined with computed tomography angiography(CTA)examination and serum biomarkers for recurrent stroke events at three-month and one-year follow-ups.Methods A total of 136 patients with cerebral infarction diagnosed for the first time were included in the retrospective study.These patients received CTA+CTP one-stop examination and serum biomarkers testing,followed by three-month and one-year follow-ups for the occurrence of recurrent stroke events.Recurrent stroke events were defined as ischemic stroke,retinal infarction,intracerebral hemorrhage,subarachnoid hemorrhage,and death.Results The recurrent stroke events rate was 23.5%(32 cases)and 36.8%(50 cases)at three-month and one-year follow-ups,respectively.Ischemic penumbra(IP)volume[odds ratio(OR)=1.010,P=0.029]and modified Rankin scale(mRS)score at discharge(OR=1.388,P=0.008)were independent predictors for recurrent stroke events at the three-month follow-up,so were lipoprotein(a)[Lp(a)](OR=1.002,P=0.044),vascular stenosis severity(OR=1.489,P=0.029),and mRS score at discharge(OR=1.282,P=0.038)at the one-year follow-up.Conclusion Among patients with stroke diagnosed for the first time,IP volume,Lp(a),vascular stenosis severity and mRS score at discharge are the most powerful predictors of recurrent stroke events at three-month and one-year follow-ups.

Pyridaben is a broad-spectrum acaricide widely used in agriculture, accidental or self-administration of large doses of pyridaben can cause multiple organ failure in patients. Due to its damage to multiple organs and no specific antidote, the mortality rate is high. This paper reports two patients who took a large amount of pyridaben, developed severe metabolic acidosis, hyperlactatemia, toxic encephalopathy, and liver, kidney, heart and digestive tract damage. After timely gastric lavage, catharsis, organ support andblood purification treatment, the condition improved and discharged. It is expected to provide clinical ideas for the treatment of pyridaben poisoning.
3.Diagnostic value of high-resolution MRI for nerve root compression in lumbar intervertebral foramen stenosis

Objective To explore the correlation value of high-resolution MRI on the cause,location and degree of nerve root com-pression and clinical symptoms of lumbar intervertebral foramen stenosis.Methods Patients with sciatic neuralgia underwent con-ventional MRI examination of the lumbar spine before treatment.MRI thin layer(1 mm)intervertebral disc axial scan was performed on patients with suspected L4/L5 and L5/S1 intervertebral foramen stenosis,and the value of conventional MRI and high-resolution MRI on intervertebral foramen stenosis location,etiology and Elisabeth typing was compared and analyzed.Intervertebral foramen mor-phometric indicators,nerve root length and short diameter were measured on high-resolution MRI post-processed images and their correlation with visual analogue scale(VAS)scores of clinical symptoms was analyzed.Results Sixty-one patients were effectively diagnosed with intervertebral foramen stenosis and nerve root compression after surgery or conservative treatment.The diagnosis rate of conventional MRI for the cause of intervertebral foramen stenosis was 75.4%,and the diagnosis rate of high-resolution MRI post-processed was 91.8%.The Elisabeth typing of lumbar intervertebral foramen stenosis was positively correlated with clinical symp-toms,while the nerve root short diameter and peri-nerve fat space area were negatively correlated with VAS score.Conclusion High-resolution MRI is significantly better than conventional MRI for the diagnosis of nerve root compression in intervertebral fora-men stenosis.It visually shows the location,cause and degree of nerve root compression,which can better guide the clinical and accu-rate minimally invasive surgery.
4.MRI study of cortical structure in adolescents with major depression disorder

Objective To explore the abnormal brain regions in morphology and its correlation with clinical symptoms of adolescents with major depression disorder.Methods Forty adolescents patients with major depression disorder were enrolled as the case group,and 30 healthy volunteers were recruited as the control group(age,gender,education level and handedness matched with the case group).Depressive symptoms in both groups were assessed by the 17-item Hamilton depression scale(HAMD-17).3D-T1 WI imaging data were acquired by GE 3.0T MR for the two groups.Freesurfer software was used to analyze the cortical thickness(CT)and surface area(SA)values of whole brain of the two groups.Two-sample independent t-test statistical analysis was used to analyze the differences of the values of the whole brain CT,SA between the two groups.Pearson correlation analysis was used to analyze the correlation between abnormal CT,SA values and HAMD-17 score in case group.P<0.05 was considered statistically significant.Results There were no statistically significant differences in gender,age,education level,origin of students and interests between the two groups(P>0.05),while there was statistically significant difference in HAMD-17 score between the two groups(P<0.05).The average course of disease in the case group was(14.84+2.71)months,and both groups were right-handed.Brain CT value increased in right medial orbitofrontal lobe and decreased in left anterior central gyrus in case group.There was no difference in the value of SA between the two groups(P>0.05).Pearson correlation analysis showed that the CT value of right medial orbitofrontal lobe was positively correlated with HAMD-17 score.Conclusion It is abnormal for the CT of partial brain regions of adolescents with major depression disorder,but the SA hadn't change significantly.The right medial orbitofrontal lobe has important significance on adolescent depression symptoms.

Pulmonary blast injury has become the main type of trauma in modern warfare, characterized by externally mild injuries but internally severe injuries, rapid disease progression, and a high rate of early death. The injury is complicated in clinical practice, often with multiple and compound injuries. Currently, there is a lack of effective protective materials, accurate injury detection instrument and portable monitoring and transportation equipment, standardized clinical treatment guidelines in various medical centers, and evidence-based guidelines at home and abroad, resulting in a high mortality in clinlcal practice. Therefore, the Trauma Branch of Chinese Medical Association and the Editorial Committee of Chinese Journal of Trauma organized military and civilian experts in related fields such as thoracic surgery and traumatic surgery to jointly develop the Clinical treatment guideline for pulmonary blast injury ( version 2023) by combining evidence for effectiveness and clinical first-line treatment experience. This guideline provided 16 recommended opinions surrounding definition, characteristics, pre-hospital diagnosis and treatment, and in-hospital treatment of pulmonary blast injury, hoping to provide a basis for the clinical treatment in hospitals at different levels.
7.Current status of hospital infection management in psychiatric hospitals in the Ningxia Hui Autonomous Region

ObjectiveTo investigate current status of hospital infection management in psychiatric hospitals in the Ningxia Hui Autonomous Region, so as to provide references for improving the level of hospital infection management of psychiatric hospitals. MethodsIn December 2020, on-site supervision was conducted on hospital infection management in all 9 psychiatric hospitals in the Ningxia Hui Autonomous Region, meantime, the self-compiled questionnaire on hospital infection management status was used for investigation. ResultsAmong the selected hospitals, nine (100.00%) psychiatric hospitals had the main hospital leaders in charge of hospital infection management, five (55.56%) hospitals had established a hospital infection management committee, six (66.67%) hospitals had established an independent hospital infection management department, and one (11.11%) hospital had developed all 13 systems mentioned in the questionnaire related to hospital infection management and job responsibilities. In terms of hospital infection management staff, there were 23 staff members in the nine psychiatric hospitals, including 3 in the specialty (13.04%) and 20 in the part-time setting (86.96%). The score of the implementation of the basic system of hospital infection management in nine hospitals was (3.28±2.22). ConclusionThe system specification related to hospital infection management in the Ningxia Hui Autonomous Region psychiatric hospitals needs to be improved and further strengthened, the professionalism of hospital infection management personnel needs to be improved.
8.Risk factors of uveitis in ankylosing spondylitis

Objective:To investigate the risk factors of uveitis in ankylosing spondylitis (AS).Methods:This retrospective study included 206 patients with AS who visited the department of rheumatology and immunology of People's Hospital of Xinjiang Uygur Autonomous Region between January, 2018 and December, 2018. Those patients with uveitis were enrolled in the uveitis group. AS patients without uveitis were included in the non uveitis group as control. The basic clinical data, laboratory indexes and imaging data were analyzed retrospectively by t-test, Wilcoxon rank sum test, Chi square test and binary logistic regression. Results:Thirty-seven patients with uveitis and 169 patients without uveitis were included. Compared with the non uveitis group, patients with uveitis were older in age [(40±11) years vs (36±11) years, t=-2.06, P<0.05], longer in disease duration [10(5, 16) years vs 5(2, 10) years, Z=-3.16, P<0.01], more peripheral arthritis [51.4%(19/37) vs 32.5%(55/169), χ2=4.66, P<0.05] and peripheral enthesitis [40.5%(15/37) vs 11.8%(20/169), χ2=17.34, P<0.01], and higher human leukocyte antigen (HLA)-B27 positive rate [100%(37/37) vs 85.8%(145/169), χ2=5.95, P=0.01]. However, there were no significant difference in gender, race, cervical tenderness, smoking history and volume, a positive family history of uveitis, a positive family history of AS and BMI. There was no significant difference in bloodplatelet (PLT), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR). By the binary logistic regression analysis, we found that peripheral enthesitis [ OR(95% CI)=4.289(1.832, 10.040), P<0.01], and longer disease duration [ OR(95% CI)=1.072(1.014, 1.134), P<0.05] were independently related to AS related uveitis. Conclusion:This study suggests that the risk of uveitis is increased in AS patients with longer disease duration and peripheral enthesitis.

Objective:To investigate the clinical features and prognosis of extranodal nasal-type NK/T-cell lymphoma of the extra-upper aerodigestive tract (extra-UADT NKTCL).Methods:The clinical data of 159 patients with extra-UADT NKTCL from the China Lymphoma Collaborative Group (CLCG) database between November 2001 and December 2015 were retrospectively analyzed. Kaplan-Meier survival analysis and Log-rank test were used to evaluate the prognosis. The Cox regression model is used for multi-factor analysis.Results:Extra-UADT NKTCL commonly occurs in skin and soft tissues (106/159, 66.7%) and gastrointestinal tract (31/159, 19.5%). The incidences of elevated lactate dehydrogenase (LDH) and Ann Arbor Ⅲ~Ⅳ stage were 47.8% (76/159) and 64.2% (102/159), respectively. The 3-year overall survival (OS) and progression-free survival (PFS) rates were 43.6% and 27.9%, respectively. The corresponding OS rates of primary skin/soft tissue site and gastrointestinal tract site were 41.0% and 59.4% ( P=0.281), while the PFS rates were 24.8% and 48.3%, respectively ( P=0.109). Combined modality treatment improved the 3-year OS of all the patients (58.4% vs 33.9%, P=0.001) and 3-year PFS (40.7% vs 20.7%, P=0.008) when compared with chemotherapy alone. LDH elevation, Ann Arbor synthesising and ≥2 junction external bits were intrusive as independent risk factors for total survival ( P<0.05), LDH elevation and ≥2 junction outer bits were intrusive as independent risk factors for progressionless survival( P<0.05). The distant extranodal dissemination was the primary failure patterns. Conclusions:Extra-UADT NKTCL appears to have distinct clinical characteristics and poor outcome. Compared with chemotherapy alone, combined modality treatment may improve the prognosis of patients with extra-UADT NKTCL.

Objective:To evaluate the prognosis and determine the failure patterns after radiotherapy for low-risk early-stage patients with extranodal NK/T-cell lymphoma, nasal-type (ENKTCL).Methods:A total of 557 patients from 2000—2015 with low-risk early-stage ENKTCL who received radiotherapy (RT) with or without chemotherapy (CT) from China Lymphoma Collaborative Group were retrospectively reviewed. Among them, 427 patients received combined modality therapy, whereas 130 patients received RT alone. Survivals were calculated by Kaplan-Meier method and compared with Log-rank test. Overall survival (OS) was compared with age and sex-matched general Chinese population using expected survival and standardized mortality ratio (SMR). Cox stepwise regression model was used for multivariate analysis.Results:The 5-year OS and progression-free survival (PFS) were 87.2% and 77.2%. The SMR was 3.59 ( P<0.001) at 1 year after treatment, whereas it was 1.50 at 4 years after treatment, without significant difference between ENKTCL group and country-matched general population ( P=0.146). Compared with RT alone, CMT did not result in significantly superior 5-year OS (87.0% vs 87.4%, P=0.961) or PFS (76.1% vs 80.7%, P=0.129). Local failure (11.5%, 64/557) and distant failure (10.8%, 60/557) were the main failure modes, while regional failure was rare (2.9%, 16/557). The 5-year locoregional control rate (LRC) was 87.2% for the whole group, with 89.5% for ≥50 Gy versus 73.7% for <50 Gy ( P<0.001). Radiotherapy dose was an independent factor affecting LRC( P<0.05). Conclusions:Radiotherapy achieves a favorable prognosis in patients with low-risk early-stage ENKTCL. The incidence of either locoregional or distant failure is low. Radiation dose still is an important prognostic factor for LRC.
