1.Research progress on the biological effects of HIF-1α on follicle development and ovulation.

Hypoxia inducible factor-1α (HIF-1α), as a hypoxia inducible factor, affects women's reproductive function by regulating the development and excretion of follicles. HIF-1α induces glycolysis and autophagy in the granule cells by promoting oocyte development, regulating the secretion of related angiogenic factors, and improving follicle maturity. In addition, HIF-1α promotes the process of luteinization of follicular vesicles, maintains luteal function, and finally completes physiological luteal atrophy through cumulative oxidative stress. Dysfunction of HIF-1α will cause a series of pathological consequences, such as angiogenesis defect, energy metabolism abnormality, excessive oxidative stress and dysregulated autophagy and apoptosis, resulting in ovulation problem and infertility. This article summarizes the previous studies on the regulation of follicle development and excretion and maintenance of luteal function and structural atrophy by HIF-1α. We also describe the effective intervention mechanism of related drugs or bioactive ingredients on follicular dysplasia and ovulation disorders through HIF-1α, in order to provide a systematic and in-depth insights for solving ovulation disorder infertility.

2.Effects of short-segment pedicle screw internal fixation surgery combined with hyperbaric oxygen treatment for acute spinal injury on the morphology and function of the spine.

OBJECTIVE:
To explore effect of short-segment pedicle screw internal fixation combined with hyperbaric oxygen in treating acute spinal fractures and its influence on recovery of spinal nerve function.
METHODS:
A total of 96 patients with acute spinal fracture admitted from February 2017 to March 2020 were divided into combined group and control group, with 48 cases in each group. Both groups were treated with short-segment pedicle screw internal fixation. The combined group was given hyperbaric oxygen after surgery. The operation time, surgical blood loss, incision length and other general operation conditions between two groups were recorded. The differences in spinal morphology and function, Ameraican Spinal Injury Assiciation(ASIA) neurological function grade, serum inflammatory factors and ability of daily living activities were observed before and after surgery.
RESULTS:
There was no significant difference in operation time, surgical blood loss, and incision length between combined group and control group(P>0.05). There were no significant differences in anterior height ratio and Cobb angle between two groups before surgery, 1 week and 6 months after surgery(P>0.05). The height ratio of anterior margin of the injured spine was significantly improved in both groups at 1 week and 6 months after surgery compared with preoperative period (P<0.05), and Cobb angle was significantly reduced in both groups compared with preoperative period (P<0.05). There was no statistically significant difference in serum interleukin-6(IL-6), interleukin-8(IL-8), and tumor necrosis factor-α(TNF-α) levels between two groups at 1 d after surgery(P>0.05);the serum IL-6, IL-8, and TNF-α levels of combined group were lower than those of control group at 1 week after surgery (P<0.05). At 6 months after surgery, ASIA neurological function grade of combined group was C grade in 2 cases, D grade in 23 cases, E grade in 22 cases. In control group, 7 cases was grade C, 26 cases was grade D, 13 cases was grade E, and the difference between two groups was statistically significant(P<0.05). The Barthel score of combined group was higher than that of control group at 1 month and 3 months after surgery, and the difference was statistically significant (P<0.05);at 6 months after surgery, there was no significant difference in Barthel score between two groups(P>0.05).
CONCLUSION
Short-segment pedicle screw internal fixation combined with hyperbaric oxygen for the treatment of acute spinal fractures is beneficial to the recovery of spinal nerve function after surgery, and has a certain effect on the early improvement of the patients' activities of daily living.

3.Efficacy of drug coated balloon versus conventional balloon in the treatment of coronary de novo bifurcation lesions: a meta-analysis.

Objective: To systematically evaluate the efficacy of drug coated balloon (DCB) versus conventional balloon in the treatment of coronary de novo bifurcation lesions. Methods: The databases of PubMed, Embase, Cochrane Library, Web of science, CNKI (China National Knowledge Infrastructure), Wanfang database, VIP, China Biology Medicine disc, Chinese clinical trial registry, American clinical trial registry and cardiovascular related websites until September 2020 were retrieved for collecting the randomized controlled trials (RCT) comparing DCB versus conventional balloon in the treatment of coronary de novo bifurcation lesions. The risk of bias of included studies was assessed using the Cochrane risk assessment tool. The meta-analysis was performed by using Revman 5.3 and Stata 14.0 software. Results: Seven RCTs with a total of 613 patients were included in this meta-analysis. Among the included studies, 4 articles reached the low risk of bias, and the other 3 articles reached the medium risk of bias. The results of meta-analysis showed that there was no significant difference in the major adverse cardiac events (RR=0.65, 95%CI 0.39-1.08, P=0.10), myocardial infarction (RR=0.68, 95%CI 0.25-1.80, P=0.43), target lesion revascularization (RR=0.94, 95%CI 0.53-1.67, P=0.83) between DCB group and conventional balloon group. Late lumen loss of side branch was less in the DCB group than that in the conventional balloon group (WMD=-0.25, 95%CI -0.41--0.09, P<0.01) and the risk of side branch restenosis was also lower in the DCB group than that in the conventional balloon group (RR=0.47, 95%CI 0.22-0.98, P<0.05). However, subgroup analysis showed that the conclusions of domestic studies and foreign studies on late lumen loss and side branch restenosis were inconsistent. The meta-analysis based on domestic literature showed that the risk of side branch restenosis after DCB treatment was lower compared with conventional balloon group (RR=0.29, 95%CI 0.15-0.57, P<0.05), while this parameter derived from foreign literatures remained unchanged between two groups (P=0.53). The meta-analysis results of domestic literature showed that late lumen loss in DCB group was less than that in conventional balloon group (WMD=-0.32, 95%CI -0.51--0.13, P<0.05), but this phenomenon was not observed in foreign literatures (P=0.30). Conclusions: The use of DCB in the treatment of coronary de novo bifurcation lesions has the potential to reduce the rate of restenosis and late lumen loss of side branch compared with conventional balloon group. However, due to the limitation on quantity, quality and results of published studies, more high-quality and large scale RCTs are still needed to confirm these findings.

BACKGROUND:
Shenyankangfu Tablet (SYKFT) is a Chinese patent medicine that has been used widely to decrease proteinuria and the progression of chronic kidney disease.
OBJECTIVE:
This trial compared the efficacy and safety of SYKFT, for the control of proteinuria in primary glomerulonephritis patients, against the standard drug, losartan potassium.
DESIGN, SETTING, PARTICIPANTS AND INTERVENTION:
This was a multicenter, double-blind, randomized, controlled clinical trial. Primary glomerulonephritis patients, aged 18-70 years, with blood pressure ≤ 140/90 mmHg, estimated glomerular filtration rate (eGFR) ≥ 45 mL/min per 1.73 m
MAIN OUTCOME MEASURES:
The primary outcome was change in the 24-hour proteinuria level, after 48 weeks of treatment.
RESULTS:
A total of 735 participants were enrolled. The percent decline of urine protein quantification in the SYKFT group after 48 weeks was 8.78% ± 2.56% (P = 0.006) more than that in the losartan 50 mg group, which was 0.51% ± 2.54% (P = 1.000) less than that in the losartan 100 mg group. Compared with the losartan potassium 50 mg group, the SYKFT plus losartan potassium 50 mg group had a 13.39% ± 2.49% (P < 0.001) greater reduction in urine protein level. Compared with the losartan potassium 100 mg group, the SYKFT plus losartan potassium 100 mg group had a 9.77% ± 2.52% (P = 0.001) greater reduction in urine protein. With a superiority threshold of 15%, neither was statistically significant. eGFR, serum creatinine and serum albumin from the baseline did not change statistically significant. The average change in TCM syndrome score between the patients who took SYKFT (-3.00 [-6.00, -2.00]) and who did not take SYKFT (-2.00 [-5.00, 0]) was statistically significant (P = 0.003). No obvious adverse reactions were observed in any group.
CONCLUSION:
SYKFT decreased the proteinuria and improved the TCM syndrome scores of primary glomerulonephritis patients, with no change in the rate of decrease in the eGFR. SYKFT plus losartan potassium therapy decreased proteinuria more than losartan potassium therapy alone.

5.Research Progress of Thalidomide and Its Derivatives in Treatment of Myelodysplastic Syndrome--Review.

Thalidomide and its derivatives have been used in the treatment of myelodysplastic syndrome (MDS) because of their anti-angiogenic and immunomodulatory effects. In recent years, some studies have found that thalidomide and its derivatives not only showed significant efficacy in lower-risk MDS patients with del (5q), but also showed advantages in non-del (5q) MDS patients. In addition, the discovery of its molecular targets and new substrates makes it possible to develop a new generation of immunomodulatory drugs (IMiDs) and to design IMiDs-based proteolysis-targeting chimeras. In this review, the new progress in mechanism and clinical application of thalidomide and its derivatives were summarized briefly, so as to provide a more scientific, reasonable and effective scheme to the treatment of MDS.

OBJECTIVES:
To study the effect of glucose metabolism disorders on the short-term prognosis in neonates with asphyxia.
METHODS:
A retrospective analysis was performed on the medical data of the neonates with asphyxia who were admitted to 52 hospitals in Hubei Province of China from January to December, 2018 and had blood glucose data within 12 hours after birth. Their blood glucose data at 1, 2, 6, and 12 hours after birth (with an allowable time error of 0.5 hour) were recorded. According to the presence or absence of brain injury and/or death during hospitalization, the neonates were divided into a poor prognosis group with 693 neonates and a good prognosis group with 779 neonates. The two groups were compared in the incidence of glucose metabolism disorders within 12 hours after birth and short-term prognosis.
RESULTS:
Compared with the good prognosis group, the poor prognosis group had a significantly higher proportion of neonates from secondary hospitals (48.5% vs 42.6%,
CONCLUSIONS
Recurrent hyperglycemia in neonates with asphyxia may suggest poor short-term prognosis, and it is necessary to strengthen the early monitoring and management of the nervous system in such neonates.

7.Synthesis,Characterization and Anti-tumor Effect Evaluation of Novel Ampelopsin Derivative Poly (γ-glutamic Acid)-ampelopsin Conjugate

OBJECTIVE:To desig n and sy nthesize poly (γ-glutamic acid )-ampelopsin(γ-PGA-AMP),and to characterize it and evaluate its anti-tumor activity in vitro . METHODS :Synthetic product was produced through an esterification reaction between γ-PGA and ampelopsin. The structure of synthetic product was characterized by the UV spectrophotometry ,Fourier transform infrared(FT-IR)spectroscopy,1H-NMR spectra and the quantitative elemental analysis. The content of ampelopsin in synthetic product was determined by UV absorption spectrometry at 292 nm. Using 5-FU as positive control ,MTT assay was used to determine inhibitory effects of γ-PGA-AMP and ampelopsin on human breast cancer cell MCF- 7,human liver cancer cell HepG 2 and human lung cancer cell A 549. The IC 50 was calculated. RESULTS :The results showed that the free 7-hydroxyl group of ampelopsin and the a-carboxyl group of γ-polyglutamic acid had been esterified to obtain γ-PGA-AMP;the yield of γ-PGA-AMP was 55.7%,and the content of ampelopsin was 32.3%. The inhibitory effect of γ-PGA-AMP and ampelopsin on MCF- 7,HepG2 and A 549 cells was obvious. IC 50 of γ-PGA-AMP(to 3 above tumor cells )were 40.19,28.29 and 55.23 μg/mL,those of ampelopsin were 105.30,81.23,130.10 μg/mL,those of 5-FU were 24.72,87.98,30.99 μg/mL,respectively. CONCLUSIONS :γ-PGA-AMP with anti-tumor effect in vitro is synthesized successfully ,and its anti-tumor effect is stronger than that of ampelopsin.
8.Bioinformatic Analysis of Related Genes and Therapeutic Drugs of Asthma

Objective::Bioinformatic analysis was used to compare the gene expression profile between asthma patients and healthy people, and the gene characteristics of asthma were preliminarily identified and the potential mechanism and drugs were revealed. Method::The GSE74986 gene expression profile was downloaded from the gene expression omnibus (GEO) and the differentially expressed genes (DEGs) were analyzed by GEO2R. Then the gene heat map of DEGs was made by Morpheus, and their gene ontology (GO) and Kyoto encyclopedia of genes and genomes (KEGG) analysis were performed by DAVID 6.8. Moreover, the protein-protein interaction (PPI) network and hub genes were constructed by String 10.5. Finally, the significant modules were analyzed by MCODE in Cytoscape 3.6.1, small molecule drugs related to asthma were screened through Coremine Medical. Result::A total of 510 DEGs were screened, including 29 up-regulated genes and 481 down-regulated genes. DEGs were mainly involved in these biological processes and pathways, including chromatin silencing, transcriptional regulation of RNA polymerase Ⅱ promoter, protein transport, messenger RNA (mRNA) processing, RNA splicing, ubiquitin-mediated proteolysis, protein processing in the endoplasmic reticulum, RNA transport, and myeloid differentiation factor (MyD)-dependent Toll-like receptor signaling pathway, platelet activation, nucleotide binding oligomerization domain (NOD)-like receptor signaling pathway and so on. A total of 9 hub genes were obtained, including T-complex protein 1 subunit theta (CCT8), T-complex protein 1 subunit alpha (TCP1), 26S protease regulatory subunit S10B (PSMC6), heat shock protein 90 alpha (HSP90A)A1, cell cycle protein C (CCNC), HSP90AB1, 26S proteasome non-ATPase regulatory subunit 6 (PSMD6), ubiquitin-specific protease 14 (USP14) and eukaryotic translation initiation factor 4E (EIF4E). Two important modules were obtained. The genes in two modules mainly involved these biological process, such as splice, ubiquitin-mediated proteolysis, protein modification, RNA modification and so on. Some potential molecular drugs for the treatment of asthma, such as anisomycin and genistein, have been developed. Conclusion::DEGs and hub genes can contribute to understanding the molecular mechanism of asthma and providing potential therapeutic targets and drugs for the diagnosis and treatment of asthma.
9.Correlation between pulse wave velocity and cardiac autonomic nervous function in patients with type 2 diabetes mellitus and its influencing factors /

Objective :To study correlation between brachial‐ankle pulse wave velocity (baPWV ) and cardiac auto‐nomic nervous function in patients with type 2 diabetes mellitus (T2DM) and its influencing factors .Methods :A to‐tal of 293 T2DM patients ,who hospitalized in our hospital from May 2015 to May 2016 ,were selected .According to baPWV level ,patients were divided into normal baPWV group (n=198 ,≤1400cm/s) and abnormal baPWV group (n=95 ,>1400cm/s).Blood pressure ,heart rate ,blood glucose ,renal function and heart rate variability (HRV) indexes were measured and compared between two groups .Multi‐factor Logistic regression analysis was used to ana‐lyze influencing factors of baPWV .Results :Compared with normal baPWV group , there were significant rise in age ,levels of systolic blood pressure (SBP) ,diastolic blood pressure (DBP) ,central venons pressure (CVP) , pe‐ripheral arterial pressure (PAP ) , percentage of peripheral neuropathy and mean heart rate and significant reduc‐tions in HRV indexes :lgTF (total frequency) ,lgHF (high frequency) ,lgSDNN (standard deviation of normal to normal RR intervals calculated over the 24 h period) and lgrMSSD (root‐mean square of differences between succes‐sive normal to normal intervals) and slgnificant rise in lgSDANN (standard deviation of normal to normal RR inter‐vals in all 5 min segments of the entire recording ) in abnormal baPWV group , P<0. 05 or <0.01 .Multi‐factor Lo‐gistic regression analysis indicated that SBP ,DBP ,PAP ,CVP , heart rate SDANN and peripheral neuropathy were independent risk factors for baPWV (OR=1.031-2. 108 , P<0.05 or <0.01) ,while TF ,HF and rMSSD were its independent protective factors (OR=0. 468-0. 595 , P<0.05 or <0. 01) in T2DM patients .Conclusion :In patients with type 2 diabetes mellitus , abnormal pulse wave velocity is closely correlated with cardiac autonomic nervous function damage ,and it′s affected by blood pressure and blood glucose etc .
10.Preventive effect of sodium bicarbonate combined Vit C on contrast-induced nephropathy in patients with early diabetic nephropathy undergoing PCI

Objective :To explore preventive effect of sodium bicarbonate combined Vit C on contrast-induced nephropathy (CIN) in patients with early diabetic nephropathy (DN) undergoing percutaneous coronary intervention (PCI).Methods : A total of 420 patients with coronary heart disease complicated early DN treated in our department from Sep 2014 to Jul 2016 were selected ,randomly and equally divided into routine PCI group and combined treatment group (received sodium bicarbonate combined Vit C based on routine PCI group ) . Levels of serum creatinine (SCr ) ,glomerular filtration rate (GFR) ,creatinine clearance rate (Ccr) and blood urea nitrogen (BUN) after contrast use and incidence rate of CIN were measured ,recorded and compared between two groups ,and independent risk factors for CIN were analyzed .Results : After contrast use ,compared with routine PCI group ,there were significant reductions in levels of SCr [(110-32 ± 12-35) μmol/L vs.(100-30 ± 9-73) μmol/L] and BUN [ (11-23 ± 2-43) mmol/L vs.(7-16 ± 3-14) mmol/L] ,and significant rise in GFR [ (30-67 ± 9-56) ml·min-1 ·1-73m-2 vs.(37-26 ± 7-69) ml·min-1 ·1-73m-2 ] and Ccr [ (62-31 ± 19-73) ml/min vs.(68-07 ± 21-48) ml/min] in combined treatment group , P<0-01 all ; percentages of SCr rise ≥44-2μmol/L (6-7% vs.1-9%) and SCr increase ≥25% (9-0% vs.2-9%) significantly reduced in combined treatment group , P=0-016 ,0-007- Multi-factor Logistic regression analysis indicated that contrast dose was independent risk factor for CIN in DN patients (OR=1-688 , P=0-001) , while use of sodium bicarbonate combined Vit C was its independent protective factor (OR=0-693 , P=0-021).Conclusion : For patients with early DN undergoing PCI ,sodium bicarbonate combined Vit C can significantly reduce incidence rate of CIN ,effectively protect kidney function ,which is worth extending .
