1.Development of a Diagnostic Scale for Qi-Yin Deficiency with Blood Stasis Syndrome in Diabetic Macrovascular Disease

ObjectiveTo construct a standardized diagnostic scale for Qi-Yin deficiency with blood stasis syndrome in diabetic macrovascular disease. MethodsLiterature related to Qi-Yin deficiency with blood stasis syndrome in diabetic macrovascular disease was retrieved from CNKI, VIP, and Wanfang databases. Diagnostic information from four diagnostic methods was extracted and standardized, with items having a frequency of ≥15 included in the item pool. A three-round Delphi expert consultation was conducted, screening items using support degree, mean score, rank sum, and coefficient of variation. Item weights were determined using analytic hierarchy process (AHP), gactor analysis (FA), and combined weighting method (CWM). The optimal weighting method was selected by comparing the area under the receiver operating characteristic (ROC) curve (AUC). The Youden index was calculated to establish the diagnostic cutoff value, which was proportionally scaled. ResultsA total of 102 studies were included. Thirty-five items were incorporated into the item pool. The authority coefficients for the three Delphi rounds were 0.82, 0.85, and 0.86, with coordination coefficients of 0.648, 0.538, and 0.506, respectively. Fifteen items were retained after screening. ROC curve analysis showed the AUC ranking as FA > CWM > AHP. The maximum Youden index was 0.814, corresponding to a diagnostic cutoff of 8.361 (scaled to 40 points). The final scale adopted a structured diagnostic framework: the symptom dimension requires at least 2 items, and the tongue or pulse dimension requires at least 1 category. ConclusionThis study developed a standardized diagnostic scale for Qi-Yin deficiency with blood stasis syndrome in diabetic macrovascular disease. Core items were screened via the Delphi method, with factor analysis identified as the optimal weighting method through AUC comparison. The diagnostic threshold (40 points) and structured diagnostic framework provide a quantitatively clear, clinically practical tool.
2.Influence of high copper load on neurobehavior and the sirtuin 3/peroxisome proliferator-activated receptor-γ coactivator-1α signaling pathway in rats

Objective To investigate the influence of high copper load on neurobehavior in rats and its possible mechanism of action based on the deacetylase sirtuin 3 (SIRT3)/peroxisome proliferator-activated receptor-γ coactivator-1α (PGC-1α) signaling pathway. Methods A total of 50 adult Sprague-Dawley rats were randomly divided into model group and blank group, with 25 rats in each group. The rats in the model group were fed with 1 g/kg high-copper diet and 0.185% copper sulfate solution (prepared using deionized water) to establish a rat model of high copper load, and after 12 weeks, the shuttle box test and the pole-climbing test were used to observe neurobehavioral changes, while Western blotting and immunofluorescent staining were used to measure the protein expression levels of SIRT3 and PGC-1α in hippocampal tissue. Results The shuttle box test showed that compared with the blank group, the model group had a significant reduction in the number of times of active avoidance reaction after stimulation (P<0.01) and significant increases in the times of active avoidance reaction and passive avoidance reaction (P<0.01);the pole-climbing test showed that compared with the blank group, the model group had a significant increase in pole-climbing time(P<0.01). Western blotting and immunofluorescent staining showed that compared with the blank group, the model group had significant reductions in the protein expression levels of SIRT3 and PGC-1α in hippocampal tissue (P<0.05). Conclusion High copper load has a marked influence on the neurobehavior of rats, possibly by regulating the expression of the SIRT3/PGC-1α signal pathway in hippocampal neurons.
3.Compound Xishu Granules Inhibit Proliferation of Hepatocellular Carcinoma Cells by Regulating Ferroptosis

ObjectiveTo study the mechanism of compound Xishu granules (CXG) in inhibiting the proliferation of hepatocellular carcinoma cells by regulating ferroptosis. MethodsThe transplanted tumor model of human Huh7 was established with nude mice and the successfully modeled mice were randomized into model, Fufang Banmao (0.21 g·kg-1), low-dose (1.87 g·kg-1) CXG, medium-dose (3.74 g·kg-1) CXG, and high-dose (7.49 g·kg-1) CXG groups. Mice were administrated with drinking water or CXG for 28 days, and the body weight and tumor volume were measured every 4 days. Hematoxylin-eosin staining was employed to observe the histopathological changes of tumors. The cell-counting kit-8 (CCK-8) was used to examine the survival rate of Huh7 cells treated with different concentrations (0, 31.25, 62.5, 125, 250, 500, 1 000 mg·L-1) of CXG for 24 h and 48 h. CA-AM, DCFH-DA, and C11-BODIPY581/591 fluorescent probes were used to determine the intracellular levels of ferrous ion (Fe2+), reactive oxygen species (ROS), and lipid peroxide (LPO), respectively. The colorimetric method was employed to measure the levels of glutathione (GSH) and superoxide dismutase (SOD). Western blot was employed to determine the protein levels of glutathione peroxidase 4 (GPX4), transferrin receptor 1 (TFR1), and ferritin heavy chain 1 (FTH1), respectively. ResultsIn the animal experiment, compared with the model group, the drug treatment groups showed reductions in the tumor volume from day 12 (P<0.01). After treatment, the Fufang Banmao and low-, medium-, and high-dose CXG groups had lower tumor volume, relative tumor volume, and tumor weight than the model group (P<0.05), with tumor inhibition rates of 48.99%, 79.93%, 91.38%, and 97.36%, respectively. Moreover, the CXG groups had lower tumor volume and relative tumor volume (P<0.05 in all the three dose groups) and lower tumor weight (P<0.05 in medium-dose and high-dose groups) than the Fufang Banmao group. Compared with the model group, the drug treatment groups showed reduced number of tumor cells, necrotic foci with karyopyknosis, nuclear fragmentation, and nucleolysis, and the high-dose CXG group showed an increase in the proportion of interstitial fibroblasts. In the cell experiment, compared with the blank group, CXG reduced the survival rate of Huh7 cells in a dose-dependent manner after incubation for 24 h and 48 h (P<0.05). Compared with the blank group, the RSL3 group and the low-, medium-, and high-dose CXG groups showed a decrease in the relative fluorescence intensity of CA-AM and increases in the fluorescence intensity of DCFH-DA and fluorescence ratio of C11-BODIPY581/591, which indicated elevations in the levels of Fe2+ (P<0.01), ROS (P<0.05), and LPO (P<0.01), respectively. Compared with the blank group, the RSL3 and low-, medium-, and high-dose CXG groups showed lowered levels of GSH and SOD (P<0.05). In addition, the RSL3 group and the medium- and high-dose CXG groups showed down-regulated expression of GPX4 and FTH1 (P<0.05), and the low- and high-dose CXG groups presented up-regulated expression of TFR1 (P<0.05). ConclusionCXG suppresses the proliferation of hepatocellular carcinoma cells by inducing ferroptosis via downregulating the GSH-GPX4 signaling axis and increasing intracellular Fe2+and LPO levels.
4.Liuwei Dihuang Wan inhibits oxidative stress in premature ovarian failure mice by regulating intestinal microbiota

BACKGROUND:Studies have shown that patients with premature ovarian failure have changes in the structure of intestinal flora and that imbalance of intestinal microbiota may be one of the important mechanisms in the development of premature ovarian failure. OBJECTIVE:To investigate the effect of Liuwei Dihuang Wan on oxidative stress and intestinal microbiota in premature ovarian failure mice induced by cyclophosphamide. METHODS:Forty-five female ICR mice were randomized into three groups:blank group(normal mice),model group(premature ovarian failure mice),and Liuwei Dihuang Wan group.A mouse model of premature ovarian failure was prepared by one-time intraperitoneal injection of cyclophosphamide(120 mg/kg)in the latter two groups.After successful modeling,the Liuwei Dihuang Wan group was intragastrically administered for 28 continuous days,and the other two groups were intragastrically administered with the same amount of normal saline for 28 days.Mouse body mass was recorded weekly and ovarian index was calculated.The development of mouse follicles was observed using hematoxylin-eosin staining.ELISA method was used to detect serum levels of anti-Mullerian hormone,estradiol,follicle stimulating hormone,superoxide dismutase,glutathione peroxidase,and malondialdehyde.Meanwhile,the gut microbiome of all mice was detected through 16S rDNA sequencing. RESULTS AND CONCLUSION:The mice in the model group had loose hair,decreased vigor and grip strength,almost no increase in body mass,and decreased ovarian index.Whereas,the mouse body mass and ovarian index were increased after treatment with Liuwei Dihuang Wan(P<0.05).The estrous cycle of mice in the model group was disorganized;Liuwei Dihuang Wan could restore the estrous cycle and reduce the number of atretic follicles in mice with premature ovarian failure.The serum levels of follicle stimulating hormone and malondialdehyde in the model group significantly increased(P<0.01),while the levels of estradiol,anti-Mullerian hormone,superoxide dismutase,and glutathione peroxidase significantly decreased(P<0.01).Liuwei Dihuang Wan could significantly decrease the serum levels of follicle stimulating hormone and malondialdehyde(P<0.01),and increase the levels of estradiol,anti-Mullerian hormone,superoxide dismutase,and glutathione peroxidase.According to the 16S rDNA sequencing results,Liuwei Dihuang Wan could regulate the abundance and diversity of intestinal microbiota,and increase the relative abundance of beneficial bacteria.KEGG pathway analysis showed that the intestinal microbiota and metabolic pathways,biosynthesis of secondary metabolites,microbial metabolism in different environments,and biosynthesis of amino acids were regulated by Liuwei Dihuang Wan.To conclude,the changes in the structure of intestinal microbiome may be one of the potential mechanisms of Liuwei Dihuang Wan in treating premature ovarian failure.Liuwei Dihuang Wan can regulate the structure of intestinal microbiome,increase the number of beneficial bacteria,reduce the number of harmful bacteria,and thus improve the balance of intestinal microbiota.This regulatory effect helps to reduce oxidative stress levels and further inhibit ovarian oxidative stress in mice with premature ovarian failure.
5.Long-term Outcomes of Endoscopic Radiofrequency Ablation versus Endoscopic Submucosal Dissection for Widespread Superficial Esophageal Squamous Cell Neoplasia

Background/Aims:
Endoscopic radiofrequency ablation (ERFA) is a treatment option for superficial esophageal squamous cell neoplasia (ESCN), with a relatively low risk of stenosis; however, the long-term outcomes remain unclear. We aimed to compare the long-term outcomes of patients with widespread superficial ESCN who underwent endoscopic submucosal dissection (ESD) or ERFA.
Methods:
We retrospectively analyzed the clinical data of patients with superficial ESCN who underwent ESD or ERFA between January 2015 and December 2021. The primary outcome measure was recurrence-free survival.
Results:
Ninety-two and 33 patients with superficial ESCN underwent ESD and ERFA, respectively. The en bloc, R0, and curative resection rates for ESD were 100.0%, 90.2%, and 76.1%, respectively. At 12 months, the complete response rate was comparable between the two groups (94.6% vs 90.9%, p=0.748). During a median follow-up of 66 months, recurrence-free survival was significantly longer in the ESD group than in the ERFA group (p=0.004), while no significant differences in overall survival (p=0.845) and disease-specific survival (p=0.494) were observed.Preoperative diagnosis of intramucosal cancer (adjusted hazard ratio, 5.55; vs high-grade intraepithelial neoplasia) was an independent predictor of recurrence. Significantly fewer patients in the ERFA group experienced stenosis compare to ESD group (15.2% vs 38.0%, p=0.016).
Conclusions
The risk of recurrence was higher for ERFA than ESD for ESCN but overall survival was not affected. The risk of esophageal stenosis was significantly lower for patients who underwent ERFA.
6.Engineering yeast for high-efficiency isoliquiritigenin production via synthetic biology approaches

Background: Isoliquiritigenin, a key pharmacologically active compound derived from the traditional Chinese medicine Glycyrrhizae Radix et Rhizoma, can be further modified into various high-value 5-deoxyflavones, demonstrating significant potential for pharmaceutical development. Currently, the supply of isoliquiritigenin primarily depends on plant extraction. However, heterologous synthesis using microbial cell factories presents a promising alternative, offering a solution to resource limitations caused by the dwindling availability of Glycyrrhiza uralensis. Objective: This study aimed to employ heterologous synthesis in yeast strains for the stable and high-efficiency production of isoliquiritigenin. Methods: First, a stable chassis strain for isoliquiritigenin production was constructed by integrating optimized biosynthetic pathway enzyme genes. A type IV noncatalytic chalcone isomerase-like protein and a synthetic protein scaffold system were employed to enhance the metabolic channeling of key pathway enzymes. Subsequently, yeast metabolism was fine-tuned to balance precursor supply, and cofactor engineering strategies were implemented to increase nicotinamide adenine dinucleotide phosphate hydrogen (NADPH) availability, thereby ensuring the catalytic efficiency of the key enzyme chalcone reductase. Results: The engineered strain Y21-2 achieved a 24.4-fold increase in isoliquiritigenin titer compared to the original strain. Additionally, the proportion of the by-product naringenin chalcone was reduced by 67.8%, marking the first instance in which the ratio of C-5 hydroxylated by-products was minimized to 10.4% during the microbial synthesis of 5-deoxyflavones. Conclusion: This work provides a valuable reference for the efficient and sustainable production of isoliquiritigenin, laying a solid foundation for further pathway optimization and the biotechnological synthesis of other high-value natural 5-deoxyflavones.
7.Bidirectional two-sample Mendelian randomization study on causality between colorectal cancer and sepsis

OBJECTIVE To observe the causal association between colorectal cancer and sepsis by means of bidirec-tional two-sample Mendelian randomization(MR).METHODS The Genome Wide Association Study(GWAS)datasets for colorectal cancer and sepsis were retrieved from the GWAS databases between its establishment and Feb.1,2024.MR was carried out for the colorectal cancer and sepsis interacting as exposure and outcome factors.The single nucleotide polymorhpism(SNPs)that were significantly associated with the exposure factors were screened out by setting P as less than 5.0× 10-8,r2 less than 0.001,the genetic distance 10,000 kb.The SNPs that were remarkably associated with the exposure factors were extracted from the GWAS datasets of the outcome variables,the instrumental variable were finally obtained,the inverse variance weighting(IVW)was taken as the main approach for the causal inference.The level pleiotropy was tested by using MR Egger method and MR-PRESSO,the heterogeneity was tested by IVW method and MR-Egger method,the sensitivity was analyzed by leave-one-out method,and the robustness of the result was tested.RESULTS A total of 30 SNPs were screed out by setting the colorectal cancer as exposure factor and the sepsis as outcome variable(F>10);there was causal as-sociation between the colorectal cancer and the sepsis(OR=28.955,95%CI:1.215 to 690.052,P=0.037).Totally 14 SNPs were screened out by setting the sepsis as exposure factor and the colorectal cancer as treatment variable(F>10),and there was no causal association between the colorectal cancer and the sepsis(OR=0.999,95%CI:0.997 to 1.002,P=0.674).There was no level pleiotropy in the instrumental variables during the two times of MR analysis;there was no heterogeneity in the instrumental variables,and the result of the MR analysis was robust.CONCLUSION There is causal association between the colorectal cancer and the increases of risk of sepsis.But there is no causal association between the sepsis and the increase of risk of colorectal cancer.
8.Predicting axillary lymph node metastasis in invasive breast cancer using machine learning models based on serum biomarkers and other clinical features

Objective:Serum tumor markers(STMs)are important indicators associated with metastasis in patients with breast cancer(BC).This study focuses on predicting the risk of axillary lymph node metastasis(ALNM)in patients with invasive BC in Xinjiang by combining STMs and clinicopathological factors.Methods:Data from 3,360 patients diagnosed with invasive BC and treated at the Affiliated Cancer Hospital of Xinjiang Medical University between 2015 and 2019 were analyzed,focusing on 11 relevant demographic and clinical factors.Five ma-chine learning(ML)algorithms were used to develop predictive models for ALNM.Their performance was compared using metrics such as area under the curve(AUC),accuracy,Kappa value,and Brier score.The best-performing model was then compared with a nomogram based on Logistic regression(LR)to determine the final model.Shapley additive explanations(SHAP)values were used to rank the importance of factors contributing to ALNM.Results:Of the 3,266 patients studied,1,368(41.89%)developed ALNM.Among the five constructed ML models,eXtreme gradient boosting(XGBoost)demonstrated the best predictive performance with an AUC of 0.768,an accuracy of 0.735,and a Kappa value of 0.450.In both the training and validation sets,the XGBoost model outperformed the LR-based nomogram(training set AUC and Brier score:0.822(0.810~0.820)vs.0.742(0.721~0.763),0.170(0.163~0.177)vs.0.197(0.189~0.204);validation set AUC and Brier score:0.769(0.740~0.770)vs.0.747(0.716~0.779),0.190(0.178~0.202)vs.0.195(0.189~0.204)).Therefore,XGBoost was selec-ted as the final predictive model.SHAP analysis identified T stage,age,molecular subtype,and CEA level as the four most influential factors for ALNM prediction.Conclusions:The XGBoost model effectively predicts the risk of ALNM in patients with invasive BC based on STMs and clinicopathological features,outperforming traditional nomograms.SHAP analysis highlighted T stage as the most critical factor influencing ALNM.
9.Dosiomics model for predicting radiation-induced temporal lobe injury in nasopharyngeal carcinoma after intensity-modulated radiotherapy

Objective:To investigate and validate the performance of a dosiomics model that utilized 3D dose distribution to forecast radiation-induced temporal lobe injury (RTLI) in nasopharyngeal carcinoma (NPC) patients following intensity-modulated radiotherapy (IMRT).Methods:Clinical data of 3578 patients diagnosed with NPC admitted to Jiangsu Cancer Hospital from January 2011 to December 2021 were retrospectively analyzed. According to the inclusion and exclusion criteria, 97 NPC patients who developed RTLI were assigned into the case group. A 1:1 propensity score matching (PSM) method was used to match 97 NPC patients without RTLI as the control group. Patients were assigned into the training cohort ( n=135) and the validation cohort ( n=59) at a 7:3 ratio by simple random method. Dosiomics features were extracted from the patients' three-dimensional dose distribution maps. Spearman rho and the least absolute shrinkage and selection operator regression were used to select dosiomics features. Clinical features were collected and screened by univariate and multivariate analyses. Eight machine learning classifiers were then trained to build dosiomics models and clinical models, respectively. The area under the ROC curve (AUC), sensitivity, and specificity were calculated to compare the predictive performance of the dosiomics and clinical models. Multivariate analysis was conducted using logistic regression to assess the influencing factors, while comparisons of the ROC curves between two different models were performed using the DeLong test. Results:A total of 1130 dosiomics features were extracted from the three-dimensional dose distribution maps, and 14 features were retained for model building after feature selection. The model based on the support vector machine (SVM) classifier achieved the highest AUC value of 0.977 (95% CI: 0.949-1.000) in the validation cohort, with an AUC of 1.000 (95% CI: 1.000-1.000) in the training cohort. By conducting univariate and multivariate analyses of the patients' clinical features, 2 clinical features were retained to build the clinical model. The model based on the SVM classifier achieved the optimal AUC value of 0.667 (95% CI: 0.523-0.810) in the validation cohort, with an AUC of 0.804 (95% CI: 0.730-0.878) in the training cohort. DeLong test showed that the difference between the dosiomics and clinical models was statistically significant ( P<0.05). Conclusion:The dosiomics model based on 3D dose distribution yields high predictive performance for RTLI in NPC patients after IMRT, which surpasses the clinical feature model, providing a new approach for early clinical prediction of RTLI.
10.Longitudinal changes of thalamic metabolites assessed by hydrogen proton MR spectroscopy in patients of cervical spondylotic myelopathy following decompression surgery

Objective To access the changes of thalamic metabolites before and 6 months after surgery in patients with cervical spondylotic myelopathy(CSM)using hydrogen proton magnetic resonance spectroscopy(1H-MRS)and to analyze its association with improvement in neurological function.Methods Forty-eight CSM patients(CSM group)who underwent cervical decompression surgery were included,and 33 healthy controls(HC)(HC group)were recruited.All subjects underwent bilateral thalamic 1H-MRS scans before the surgical procedure,and subsequently again 6 months later.Neurological function was assessed pre-operatively and post-operatively(6 months)in all patients with CSM using the modified Japanese Orthopaedic Association(mJOA)score.The changes of mJOA(△mJOA)were employed as an indicator of neurological improvement.The pre-and post-operative thalamic metabolites ratio of N-acetyl aspartate/creatine(NAA/Cr),choline/Cr(Cho/Cr),myo-inositol/Cr(mI/Cr),glutamate and glutamine/Cr(Glx/Cr)were compared in CSM patients and HC.A correlation analysis was conducted to determine the relationship between alterations in pre-and post-operative thalamic metabolites ratio(△NAA/Cr,△Cho/Cr,△mI/Cr,△Glx/Cr)and △mJOA.Results Compared to HC group,patients with CSM group showed significantly lower NAA/Cr(t=-4.988,P<0.001;t=-3.562,P=0.001),Cho/Cr(t=-5.946,P<0.001;t=-2.764,P=0.007)and mI/Cr(t=-3.988,P<0.001;t=-2.079,P=0.041)before and 6 months after surgery.6 months post-operative NAA/Cr(t=-2.805,P=0.007)and mI/Cr(t=-3.285,P=0.003)were increased in CSM groups compared to pre-operative NAA/Cr and mI/Cr.In CSM group,△mI/Cr correlated significantly with △mJOA(r=0.478 2,P=0.000 6).Conclusion There are differences in thalamic metabolites in CSM patients before and after surgery.△mI/Cr is correlated with the improvement of neurological function,which can be used as an imaging parameter to evaluate the neurological function recovery in patients with CSM after surgery.
