Objective Radial artery occlusion(RAO)is one of the common complications following coronary intervention via the traditional radial artery approach. This study aims to evaluate the clinical effectiveness of retrograde recanalization of occluded radial arteries through the distal transradial approach(dTRA)approach. Methods A total of 35 patients with RAO admitted to the cardiovascular department of the Anhui Chest hospital between December 2022 and April 2024,who were scheduled to undergo coronary intervention and had attempted recanalization of RAO via dTRA approach were selected. The primary result was the success rate of recanalizing RAO via dTRA.The secondary results included factors influencing the failure of recanalization via dTRA,postoperative puncture complications, and the patency rate at the 3-month follow-up.Results This study divided the patients into a successful group(29 cases,82.9%)and a failed group(6 cases,17.1%)based on whether the distal radial artery was successfully opened and occluded. The proportion of smoking(100.00%vs.17.24%,P=0.040),history of diabetes(100.00%vs.10.34%,P=0.025),and chronic total occlusion of coronary artery(83.33%vs.17.24%,P=0.030)in the failure group were higher than those in the success group, and the difference was statistically significant. The application rate of balloon tracking assisted technology in the failed group(16.67%vs.58.62%,P=0.045),and the diameter of the radial artery at 3 days after surgery[(1.63±0.13)mm vs.(2.13±0.32)mm,P=0.021]and the peak radial artery blood flow velocity at 3 days postoperatively[(0.10±0.78)m/s vs.(0.50±0.13)m/s,P<0.001]were all lower in the successful group, and the differences were statistically significant.Logistic regression analysis 3 days after surgery showed that chronic complete occlusion of the coronary artery was an independent risk factor for surgical opening failure(OR 0.042,95%CI 0.004-0.438,P=0.008).After 3 months of follow-up,the patency rate of the successful group was 55.2%.Conclusions Retrograde recanalization of RAO via dTRA is safe and feasible, but its long-term patency rate is not high.

OBJECTIVE:
To investigate the therapeutic effect of Xiangshao Granules (XSG) on post-stroke depression (PSD) and explore the underlying mechanisms.
METHODS:
Forty-three C57BL/6J mice were divided into 3 groups: sham (n=15), PSD+vehicle (n=14), and PSD+XSG (n=14) groups according to a random number table. The PSD models were constructed using chronic unpredictable mild stress (CUMS) after middle cerebral artery occlusion (MCAO). The sham group only experienced the same surgical operation, but without MACO and CUMS stimulation. The XSG group received XSG (60 mg/kg per day) by gavage for 4 weeks. The mice in the sham and vehicle groups were given the same volume of 0.9% saline at the same time. The body weight and behavior tests including open field test, sucrose preference test, tail suspension test, and elevated plus-maze test, were used to validate the PSD mouse model. Real-time fluorescence quantitative polymerase chain reaction (RT-qPCR), enzyme-linked immunosorbent assay (ELISA), and immunofluorescence staining were used to evaluate the anti-inflammatory effects of XSG. The potential molecular mechanisms were explored and verified through network pharmacology analysis, Nissl staining, Western blot, ELISA, and RT-qPCR, respectively.
RESULTS:
The body weight and behavior tests showed that MCAO combined with CUMS successfully established the PSD models. XSG alleviated neuronal damage, reduced the expressions of pro-apoptotic proteins Caspase-3 and B-cell lymphoma-2 (BCL-2)-associated X (BAX), and increased the expression of anti-apoptotic protein BCL-2 in PSD mice (P<0.05 or P<0.01). XSG inhibited microglial activation and the expressions of pro-inflammatory cytokines including tumor necrosis factor-α, interleukin (IL)-1 β, and IL-6 via the toll-like receptor 4/nuclear factor kappa-B signaling pathway in PSD mice (P<0.05 or P<0.01). Furthermore, XSG decreased the expression of indoleamine 2,3-dioxygenase1 (IDO1) and increased the concentration of 5-hydroxytryptamine in PSD mice (P<0.05 or P<0.01).
CONCLUSION
XSG could reverse the anxiety/depressionlike behaviors and reduce the neuronal injury in the hippocampus and prefrontal cortex of PSD mice, which may be a potential therapeutic agent for PSD.

Radiochemotherapy-induced oral mucositis (OM) is a common oral complication in patients with tumors following head and neck radiotherapy or chemotherapy. Erosion and ulcers are the main features of OM that seriously affect the quality of life of patients and even the progress of tumor treatment. To date, differences in clinical prevention and treatment plans for OM have been noted among doctors of various specialties, which has increased the uncertainty of treatment effects. On the basis of current research evidence, this expert consensus outlines risk factors, clinical manifestations, clinical grading, ancillary examinations, diagnostic basis, prevention and treatment strategies and efficacy indicators for OM. In addition to strategies such as basic oral care, anti-inflammatory and analgesic agents, anti-infective agents, pro-healing agents, and photobiotherapy recommended in previous guidelines, we also emphasize the role of traditional Chinese medicine in OM prevention and treatment. This expert consensus aims to provide references and guidance for dental physicians and oncologists in formulating strategies for OM prevention, diagnosis, and treatment, standardizing clinical practice, reducing OM occurrence, promoting healing, and improving the quality of life of patients.

This research aims to construct a stable epithelial cell senescence model for screening and e-valuation of senolytics. We explored the optimal conditions for doxorubicin-induced senescence of non-transformed epithelial cells MCF 10A,including the optimal induction concentration, the optimal inter-vention time, and the optimal senescence duration, and confirmed the feasibility of MCF 10A as an epi-thelial senescence model by multiple ways. The optimal condition for Doxorubicin-induced senescence of MCF 10A cells was treatment with 0.6 μmol/L Doxorubicin for 16 h to achieve the best senescence state on the 8th day.Under the optimal induction conditions, the positive rate of senescence-associated β-gal-actosidase(SA-β-gal)staining in the treated group reached 97%.At the same time, biochemical results of detecting the expression of mRNA,proteins, and immunofluorescence demonstrated that the expression levels of senescence-associated secretory phenotype(SASP),p16,p21 and p53 in the treated group were significantly higher than those in the control cells, and Lamin B1 was significantly decreased(P<0.001),which were consistent with the specific characteristics of senescence. In summary,an epithelial senescence model was successfully induced in MCF 10A cells by Doxorubicin in this study, which will promote the screening of senolytics for senescent epithelial cells.

Objective:To investigate the clinicopathological features of SMARCA4-deficient uterine sarcoma. Methods:Five cases of SMARCA4-deficient uterine sarcoma at the Department of Pathology, the First Affiliated Hospital of Nanjing Medical University from 2018 to 2024 were collected. The morphological and immunohistochemical features were observed and analyzed. A follow-up study was also carried out. Results:Five female patients, aged 24, 54, 56, 61, and 41 years, respectively, presented with vaginal bleeding or abdominal pain. All patients had imaging findings of intracavitary lesion in the uterus, with tumor sizes ranging from 3.0 cm to 8.8 cm. The patients were followed up for 2 to 14 months. Case 1 died 9 months after surgery, whereas the remaining four patients were still alive. Histologically, the tumor cells exhibited a diffuse growth pattern, with an infiltration depth involving more than half of the myometrium. Portions of the interstitium appeared sclerosed. Benign endometrial glandular structures were observed in a leaf-like or fissured pattern, resembling those of uterine adenosarcoma. The tumor cells were large epithelioid with abundant or faintly eosinophilic cytoplasm, and the nuclei were moderately to markedly atypia with prominent nucleoli and brisk mitosis. Rhabdoid cells were seen. Some areas showed small round blue cells, with occasional spindle cells and myxoid stroma. Additionally, widespread or focal lymphovascular space invasion was observed within the myometrium. All five cases exhibited absence of SMARCA4 (BRG1) expression and retained SMARCB1 (INI1). Claudin4 expression was negative. There was no deficient expression of mismatch repair proteins MLH1, PMS2, MSH2 and MSH6. p53 showed wild-type expression. Ki-67 index ranged from 30% to 60%. CKpan, CK7, ER, PR, and PAX8 were negative.Conclusions:SMARCA4-deficient uterine sarcoma is rare, highly aggressive, and has a poor prognosis. The tumor exhibits a broad morphological spectrum, with rhabdoid cells and adenosarcoma-like structures serving as important diagnostic clues. The absence of BRG1 expression lends support to a definitive diagnosis.
