Hemorrhagic shock is a common clinical emergency that can aggravate cell injury after resuscitation. Astrocytes are crucial for the survival of neurons because they regulate the surrounding ionic microenvironment of neurons. Although hemorrhagic shock and resuscitation (HSR) injury can impair cognition, it remains unclear how this insult directly affects astrocytes. In this study, we established an HSR model by bleeding and re-transfusion in mice. The social interaction test and new object recognition test were applied to evaluate post-operative cognitive changes, and the results suggest that mice experience cognitive impairment following exposure to HSR. In the HSR group, the power spectral density of β and γ oscillations decreased, and the coupling of the θ oscillation phase and γ oscillation amplitude was abnormal, which indicated abnormal neuronal oscillation and cognitive impairment after HSR exposure. In brief, cognitive impairment in mice is strongly correlated with Ca²⁺ signal strength in lateral septum astrocytes following HSR.

Abnormal tumour cell adhesion is a key step in tumour metastasis， in which weakened homologous and enhanced heterologous adhesion of tumour cells is an important cause of tumour metastasis. Chronic stress can activate the sympathetic nervous system to link and regulate the homologous and heterologous adhesion of tumour cells and exacerbate tumour metastasis. Combining the understanding of traditional Chinese medicine （TCM） and Western medicine on tumour metastasis， it is believed that the mechanism of "qi constraint and stagnation， tumor toxin transmission and retention" in TCM theory is highly related to the abnormal adhesion of tumour cells triggered by chronic stress. Qi constraint and stagnation is closely related to chronic stress and its activation of the sympathetic nervous system， and transmission and retention of tumor toxin explained the mechanism of tumour metastasis due to abnormal adhesion of tumour cells from the perspective of TCM. By regulating the key link of sympathetic nervous system-tumour cell adhesion， application of the formulas of regulating qi and resolving toxin can improve chronic stress and inhibit tumour metastasis.

Objective:To investigate the efficacy, safety and surgical experience of middle meningeal artery embolization using polyvinyl alcohol particle in treating chronic subdural hematoma (CSDH).Methods:A total of 51 CSDH patients (25 accepted first treatment, 19 had invalid atorvastatin treatment, and 7 recurred after burr hole drainage) admitted to Department of Neurosurgery, First Affiliated Hospital of Army Medical University from May 2021 to August 2022 were enrolled. The characteristics of middle meningeal artery were evaluated by three-dimensional time-of-flight (3D TOF) MRA before embolization. With confirmation of no surgical contraindications, unilateral or bilateral embolization of the middle meningeal artery with polyvinyl alcohol particle was performed under local anesthesia. The hematoma recurrence rate and proportion of patients with reduction of maximum hematoma thickness>50% at 2 months after embolization were used as primary outcomes to evaluate the efficacy. Complications and embolization-related adverse events were recorded, and the experiences were retrospectively summarized.Results:For all 51 patients, the characteristics of the middle meningeal artery shown in the preoperative 3D TOF MRA were consistent with those in the intraoperative angiography; 16 patients with unilateral hematoma showed thickened bilateral middle meningeal arteries in preoperative 3D TOF MRA, bilateral blood supply was confirmed by intraoperative angiography and bilateral middle meningeal artery embolization was performed. Interventional embolization was successful in all 51 patients, including unilateral embolization in 19 and bilateral embolization in 32, with a total of 83 sides of embolizations. After embolization, hematoma was absorbed and symptoms improved in 48 patients, and hematoma recurrence was observed in 3 patients (5.9%), with an overall success rate of 94.1%. Excluding the 3 patients with hematoma recurrence, 39 patients completed the 2-month follow-up, and these patients had the maximum hematoma thickness reduction>50%. No mortality, complications or surgery-related adverse events were observed in them.Conclusion:Middle meningeal artery embolization with polyvinyl alcohol particle is safe and effective in treating CSDH; once bilateral middle meningeal artery blood supply is indicated by preoperative MRA in patients with unilateral hematoma, bilateral angiography and embolization should be considered during interventional treatment.

During coronavirus disease 2019 epidemic, the treatment of severe trauma has been impacted. The Consensus on emergency surgery and infection prevention and control for severe trauma patients with 2019 novel corona virus pneumonia was published online on February 12, 2020, providing a strong guidance for the emergency treatment of severe trauma and the self-protection of medical staffs in the early stage of the epidemic. With the Joint Prevention and Control Mechanism of the State Council renaming "novel coronavirus pneumonia" to "novel coronavirus infection" and the infection being managed with measures against class B infectious diseases since January 8, 2023, the consensus published in 2020 is no longer applicable to the emergency treatment of severe trauma in the new stage of epidemic prevention and control. In this context, led by the Chinese Traumatology Association, Chinese Trauma Surgeon Association, Trauma Medicine Branch of Chinese International Exchange and Promotive Association for Medical and Health Care, and Editorial Board of Chinese Journal of Traumatology, the Chinese expert consensus on emergency surgery for severe trauma and infection prevention during coronavirus disease 2019 epidemic ( version 2023) is formulated to ensure the effectiveness and safety in the treatment of severe trauma in the new stage. Based on the policy of the Joint Prevention and Control Mechanism of the State Council and by using evidence-based medical evidence as well as Delphi expert consultation and voting, 16 recommendations are put forward from the four aspects of the related definitions, infection prevention, preoperative assessment and preparation, emergency operation and postoperative management, hoping to provide a reference for severe trauma care in the new stage of the epidemic prevention and control.

In recent years, the burden of chronic and non-communicable diseases has become more serious in China, and the area specific heterogeneities are obvious. Southwestern China is a vast area with many ethnic groups and distinctive characteristics. While the local health resources are relatively limited, and the prevention and control of high-risk chronic diseases is challenging. In order to comprehensively analyze the disease distribution and potential exposure risk in populations in different ethnic groups in southwestern China, a natural population cohort study in southwestern China (multi-ethnic cohort study) was launched in 2017 with the support of the "Precision Medicine Research" Key Project, National Key Research and Development Program of China. At present, this cohort consists of 119 556 participants aged 30-79 years (18-79 years in Tibet Autonomous Region) and pregnant women in five provinces (municipality), i.e. Sichuan, Yunnan, Guizhou, Tibet, and Chongqing. A large-scale, multi-ethnic, high-quality natural population cohort with biobank has been established. The study has made some significant progress in the evaluation of the health impact of environmental and lifestyle factors in southwestern China and developed an innovative model for large cohort establishment in less developed areas. The results of this study can provide a reference for the establishment and research of large natural population cohorts in complex geographical scenarios, and serve as high-quality basic resources for China's precision medicine research and disease prevention and control decisions in areas with multi-ethnic groups.

We developed an in-vitro bioassay for determining the bioactivity of human insulin by homogeneous time-resolved fluorescence immunoassay. CHO-INSR B1284 transgenic cells were used as target cells. Key assay parameters, including the cell density, the range of working concentrations, and the stimulation time were optimized. The specificity, relative accuracy, intermediate precision, linearity, and range of the method were validated, as well as the passage stability of the CHO-INSR B1284 cell line. The national standard of recombinant human insulin was used as the benchmark to evaluate the relative potency of insulin analogues and drugs. The drugs and the reference human insulin showed a dose-response relationship, R² > 0.995, which conforms to the four-parameter equation: y = (A - D) / [1 + (x/C)^B] + D. Specificity of the method was good. The geometric mean, relative bias, and geometric coefficient of variation (GCV, %) of the five concentrations (n = 8, 64%, 80%, 100%, 125% and 156%) met the requirements of the General Rules of Chinese Pharmacopoeia, 2020 edition, Volume IV (9401). In summary, a bioassay for determining the in vitro bioactivity of human insulin based on a homogeneous time-resolved fluorescence technique was established; the method was simple, time-saving, accurate and precise, and could be used for the evaluation of biological activity and quality.

Objective: To compare the efficacy and safety of prolonged dual antiplatelet therapy (DAPT>1 year) in patients with stable coronary artery disease (CAD) and diabetes who were event-free at 1 year after percutaneous coronary intervention (PCI) with drug-eluting stent (DES) in a large and contemporary PCI registry. Methods: A total of 1 661 eligible patients were selected from the Fuwai PCI Registry, of which 1 193 received DAPT>1 year and 468 received DAPT ≤1 year. The primary endpoint was major adverse cardiac and cerebrovascular event (MACCE) and Bleeding Academic Research Consortium (BARC) type 2, 3 or 5 bleeding, MACCE was defined as a composite of all-cause death, myocardial infarction or stroke. Multivariate Cox regression analysis and inverse probability of treatment weighting (IPTW) Cox regression analysis were performed. Results: After a median follow-up of 2.5 years, patients who received DAPT>1 year were associated with lower risks of MACCE (1.4% vs. 3.2%; hazard ratio (HR) 0.412, 95% confidence interval (CI) 0.205-0.827) compared with DAPT ≤1 year, which was primarily caused by the lower all-cause mortality (0.1% vs. 2.6%; HR 0.031, 95%CI 0.004-0.236). Risks of cardiac death (0.1% vs. 1.5%; HR 0.051, 95%CI 0.006-0.416) and definite/probable ST (0.3% vs. 1.1%; HR 0.218, 95%CI 0.052-0.917) were also lower in patients received DAPT>1 year than those received DAPT ≤ 1 year. No difference was found between the two groups in terms of BARC type 2, 3, or 5 bleeding (5.3% vs. 4.1%; HR 1.088, 95%CI 0.650-1.821). Conclusions: In patients with stable CAD and diabetes who were event-free at 1 year after PCI with DES, prolonged DAPT (>1 year) provides a substantial reduction in ischemic cardiovascular events, including MACCE, all-cause mortality, cardiac mortality, and definite/probable ST, without increasing the clinically relevant bleeding risk compared with ≤ 1-year DAPT. Further well-designed, large-scale randomized trials are needed to verify the beneficial effect of prolonged DAPT in this population.

Objective: The relationship between serum uric acid (SUA) levels and glycemic indices, including plasma glucose (FPG), 2-hour postload glucose (2h-PG), and glycated hemoglobin (HbA1c), remains inconclusive. We aimed to explore the associations between glycemic indices and SUA levels in the general Chinese population. Methods: The current study was a cross-sectional analysis using the first follow-up survey data from The China Cardiometabolic Disease and Cancer Cohort Study. A total of 105,922 community-dwelling adults aged ≥ 40 years underwent the oral glucose tolerance test and uric acid assessment. The nonlinear relationships between glycemic indices and SUA levels were explored using generalized additive models. Results: A total of 30,941 men and 62,361 women were eligible for the current analysis. Generalized additive models verified the inverted U-shaped association between glycemic indices and SUA levels, but with different inflection points in men and women. The thresholds for FPG, 2h-PG, and HbA1c for men and women were 6.5/8.0 mmol/L, 11.0/14.0 mmol/L, and 6.1/6.5, respectively (SUA levels increased with increasing glycemic indices before the inflection points and then eventually decreased with further increases in the glycemic indices). Conclusion An inverted U-shaped association was observed between major glycemic indices and uric acid levels in both sexes, while the inflection points were reached earlier in men than in women.

BACKGROUND: Prospective analyses have yet to identify a consistent relationship between sleep duration and the incidence of gastrointestinal (GI) cancers. The effect of changes in sleep duration on GI cancer incidence has scarcely been studied. Therefore, we aimed to examine the association between baseline sleep duration and annual changes in sleep duration and GI cancer risk in a large population-based cohort study. METHODS: A total of 123,495 participants with baseline information and 83,511 participants with annual changes in sleep duration information were prospectively observed from 2006 to 2015 for cancer incidence. Cox proportional-hazards models were used to calculate hazard ratios (HRs) and their confidence intervals (CIs) for GI cancers according to sleep duration and annual changes in sleep duration. RESULTS: In baseline sleep duration analyses, short sleep duration (≤5 h) was significantly associated with a lower risk of GI cancer in females (HR: 0.31, 95% CI: 0.10-0.90), and a linear relationship between baseline sleep duration and GI cancer was observed (P = 0.010), especially in males and in the >50-year-old group. In the annual changes in sleep duration analyses, with stable category (0 to -15 min/year) as the control group, decreased sleep duration (≤-15 min/year) was significantly associated with the development of GI cancer (HR: 1.29; 95% CI: 1.04-1.61), especially in the >50-year-old group (HR: 1.32; 95% CI: 1.01-1.71), and increased sleep duration (>0 min/year) was significantly associated with GI cancer in females (HR: 2.89; 95% CI: 1.14-7.30). CONCLUSIONS Both sleep duration and annual changes in sleep duration were associated with the incidence of GI cancer.

Objective To develope a deep learning algorithm for pathological classification of chronic gastritis and assess its performance using whole-slide images (WSIs). Methods We retrospectively collected 1,250 gastric biopsy specimens (1,128 gastritis, 122 normal mucosa) from PLA General Hospital. The deep learning algorithm based on DeepLab v3 (ResNet-50) architecture was trained and validated using 1,008 WSIs and 100 WSIs, respectively. The diagnostic performance of the algorithm was tested on an independent test set of 142 WSIs, with the pathologists' consensus diagnosis as the gold standard. Results The receiver operating characteristic (ROC) curves were generated for chronic superficial gastritis (CSuG), chronic active gastritis (CAcG), and chronic atrophic gastritis (CAtG) in the test set, respectively.The areas under the ROC curves (AUCs) of the algorithm for CSuG, CAcG, and CAtG were 0.882, 0.905 and 0.910, respectively. The sensitivity and specificity of the deep learning algorithm for the classification of CSuG, CAcG, and CAtG were 0.790 and 1.000 (accuracy 0.880), 0.985 and 0.829 (accuracy 0.901), 0.952 and 0.992 (accuracy 0.986), respectively. The overall predicted accuracy for three different types of gastritis was 0.867. By flagging the suspicious regions identified by the algorithm in WSI, a more transparent and interpretable diagnosis can be generated. Conclusion The deep learning algorithm achieved high accuracy for chronic gastritis classification using WSIs. By pre-highlighting the different gastritis regions, it might be used as an auxiliary diagnostic tool to improve the work efficiency of pathologists.
