Gallbladder-duodenal fistula,a severe complication of cholecystitis caused by gallstones,is clinically rare.Its clinical presentation lacks specificity,and conventional preoperative imaging often fails to establish a definitive diagnosis.This report describes a case where a gallbladder-duodenal fistula was diagnosed using oral microbubble ultrasound contrast agent for gastric-duodenal contrast-enhanced ultrasound,providing a novel approach for diagnosing this condition.

Objective To compare the performance of contrast-enhanced ultrasound(CEUS)and ultrasound(US)in the differential diagnosis between cholesterol polyps and gallbladder adenomas. Methods A total of 136 patients with gallbladder polyp lesions(GPLs)and undergoing cholecystectomy in the First Medical Center of Chinese PLA General Hospital from January 2019 to October 2020 were retrospectively analyzed.All the patients underwent US and CEUS examinations before cholecystectomy.US and CEUS images of cholesterol polyps and gallbladder adenomas were compared for the evaluation of the performance of CEUS in the diagnosis of gallbladder adenomas. 3.Incidentally detected gallbladder agenesis in a child: the importance of identifying anatomic structure

The absence of a gallbladder is a very rare anomaly. While it is usually asymptomatic, it can cause biliary colic symptoms. For these reasons, gallbladder agenesis can be misdiagnosed as a hepatobiliary disease and is diagnosed correctly after surgery. This condition may also be detected through an autopsy for other causative diseases. Abdominal ultrasonography is used as a diagnostic method to detect gallbladder agenesis. Hepatobiliary scintigraphy, magnetic resonance cholangiopancreatography, and endoscopic cholangiopancreatography are also used to make a more accurate diagnosis. In the emergency room, however, gallbladder agenesis can still be misdiagnosed as acute or chronic cholecystitis, leading to the detection of gallbladder agenesis in the operating room. Although some cases of gallbladder agenesis detected in adults during surgery have been reported in Korea, there are no reports of gallbladder agenesis in pediatric patients to date. This paper reports a case of gallbladder agenesis in a symptomatic child that was detected incidentally by a radiographic examination.

4.False-negative Hepatobiliary Scintigraphy for Biliary Atresia

We present the case of a patient with biliary and duodenal atresia who showed false-negative hepatobiliary scintigraphy results. The patient was born at 37 weeks and 2 days of gestation. Her mother had undergone amnioreduction after detection of a double-bubble ultrasound sign in the fetal abdomen. At 2 days of age, total serum bilirubin level was elevated. On hepatobiliary scintigraphy 4 days later, the gallbladder was visualized from 30 min and it showed duodeno-gastric reflux at 240 min. After 24 h, the radiotracer was almost washed out in the hepatic parenchyma, but there was retention in the gastroduodenal junction. Because the biliary to duodenal transit was visible, biliary atresia seemed unlikely. Abdominal ultrasonography at 7 days of age showed a small dysmorphic gallbladder, but triangular cord sign was not definite. Magnetic resonance cholangiography revealed atretic gallbladder. Although cystic and common bile ducts were visible, the proximal common hepatic bile duct was not visible. The next day, serum total bilirubin levels remained elevated (17.1 mg/dl) with direct bilirubin level of 1.2 mg/dl. Kasai portoenterostomy with duodeno-duodenostomy was performed at 10 days of age. Histopathological evaluation showed a fibrous obliteration of the common bile duct, consistent with that of biliary atresia.

5.Hepatotoxicity in an Adolescent with Black Iced Tea Overconsumption

Tea is the most widely consumed beverage after water in the world. The consumption of iced tea has increased in Western countries and spiked among teenagers for enjoyment, freshening up and alertness. A teenager presented with symptoms of hepatitis. Liver ultrasound revealed sludge in the gallbladder. Laboratory investigations excluded all known causes of hepatotoxicity. Detail nutritional history revealed that the patient had been drinking 1.5–2 liters of black iced tea per day for the last three months. He was immediately advised to stop drinking any tea. Gradually all symptoms disappeared and two months after discontinuation of the tea, all liver enzymes returned to normal and the sludge in the gallbladder disappeared. This case report underlines the importance of a meticulous assessment of a child's dietary behavior when investigating a case of hepatotoxicity and raises awareness about the potential side effects of tea overconsumption.

6.Comet tail artifact on ultrasonography: is it a reliable finding of benign gallbladder diseases?

PURPOSE: The aim of this study was to evaluate whether the comet tail artifact on ultrasonography can be used to reliably diagnose benign gallbladder diseases. METHODS: This retrospective study reviewed the clinical findings, imaging findings, preoperative ultrasonographic diagnoses, and pathological diagnoses of 150 patients with comet tail artifacts who underwent laparoscopic cholecystectomy with pathologic confirmation. The extent of the involved lesion was classified as localized or diffuse, depending on the degree of involvement and the anatomical section of the gallbladder that was involved. This study evaluated the differences in clinical and imaging findings among pathologic diagnoses. RESULTS: All gallbladder lesions exhibiting the comet tail artifact on ultrasound examination were confirmed as benign gallbladder diseases after cholecystectomy, including 71 cases of adenomyomatosis (47.3%), 74 cases of chronic cholecystitis (49.3%), two cases of xanthogranulomatous cholecystitis (1.3%), and three cases of cholesterolosis (2.0%); there were two cases of coexistent chronic cholecystitis and low-grade dysplasia. There were no statistically significant differences in any of the clinical and ultrasonographic findings, with the exception of gallstones (P=0.007), among the four diseases. There were no significant differences in the average length, thickness, or number of comet tail artifacts among the four diagnoses. No malignancies were detected in any of the 150 thickened gallbladder lesions. CONCLUSION: The ultrasonographic finding of the comet tail artifact in patients with thickened gallbladder lesions is associated with the presence of benign gallbladder diseases, and can be considered a reliable sign of benign gallbladder disease.

7.A Guidewire May Save the Day.

Endoscopic gallbladder drainage (EGBD) has been used to treat acute cholecystitis or to relieve malignant biliary obstruction as an alternative to percutaneous gallbladder drainage and patient's are poor surgical candidates. This is currently being performed by placement of lumen apposing metallic stent (LAMS) with electrocautery mounted tip delivery system also called as “hot” technique. We had reported a case of self-expanding metallic stent (SEMS) within LAMS after stent migration during EGBD using “hot” technique and propose routine use of guidewire in patients undergoing the procedure.

8.A Newly Designed, Fully Covered, Metal Stent with Wide Flanges for EUS-Guided Drainage and Access.

BACKGROUND/AIMS: Endoscopic ultrasound (EUS)-guided drainage of pancreatic fluid collections is a useful and effective procedure. One of the main limitations is the lack of devices tailored specifically for this technique. This study aims to evaluate a newly developed, fully covered, biflanged metal stent customized for EUS-guided transenteric drainage. METHODS: We enrolled one dog in an animal study and six patients with pancreatic fluid (five with pancreatic pseudocysts and one with pancreatic walled-off necrosis) in a clinical study. We performed EUS-guided drainage of the pancreatic fluid or gallbladder using a newly developed metal stent. Study outcomes were technical and clinical success, adverse events, and stent removability. RESULTS: In the animal study, the stent was placed successfully in the gallbladder via the EUS-guided transduodenal approach without complication. The cholecystoduodenal tract was intact on necropsy performed immediately after the procedure. The stents were inserted successfully and deployed in the six patients. There was no adverse event during or after the procedure. CONCLUSIONS: EUS-guided transenteric drainage using a new fully covered, biflanged metal stent was technically feasible and was not associated with any serious adverse event. The tailored design of the flange at the end of the stent facilitated the effectiveness of the procedure.

9.Hepatobiliary diseases in buffalo (Bubalus bubalis): clinical, laboratory, and ultrasonographic findings

This study describes ultrasonographic observations of five hepatobiliary diseases in buffalo (Bubalus bubalis). Fifty buffalo, including 20 clinically normal and 30 hepatobiliary diseased buffalo were enrolled in the study. Complete clinical, radiographic and ultrasonographic examinations and laboratory analyses were conducted. Focal parenchymal lesions including liver abscess (n = 12) and hepatic cyst (n = 6), diffuse parenchymal lesion (hepatobiliary cirrhosis, n = 5) and obstruction of hepatobiliary passages including cholestasis (n = 4), and hepatocholelithiasis (n = 3) were successfully imaged by ultrasonography. Hepatic abscess imaged as a hypoechoic to echogenic circumscribed mass of various diameters with a distinct echogenic capsule. Hepatic cyst imaged as a pear-shaped sac with a bright echogenic margin, anechoic content, and distal acoustic enhancement. In hepatobiliary fibrosis, the liver showed linear bands of increasing echogenicity with less distinct imaging of the portal vasculature. Cholestasis was imaged as dilatation of the gallbladder (GB) with wall thickening and homogeneous or heterogeneous contents. Hepatocholelithiasis imaged as an echoic structure within the hepatic parenchyma, or within and around the GB and bile duct, with more echogenicity of the hepatic parenchyma than normal. Ultrasonography can be an efficient rapid, noninvasive tool for screening of common hepatobiliary diseases in buffalo under field conditions.

10.Evaluation of gallbladder and common bile duct size and appearance by computed tomography in dogs

The feasibility of using computed tomography (CT) to identify the common bile duct (CBD) and comparison with ultrasonography (US) results were evaluated in normal beagle dogs and dogs without hepatobiliary and pancreatic diseases. In addition, CBD diameters were obtained from CT at the level of the porta hepatis and the duodenal papilla level in dogs with underlying diseases that may cause cholestasis. US is a useful modality in the estimation of gallbladder volume because ejection fraction and CBD diameter from US were not significantly different from those of CT. The normal biliary tract was visible on CT images in 68% of the normal dog group. CBD diameter was not over 3 mm and 3.5 mm at the porta hepatis and duodenal papilla levels, respectively in normal dogs weighing less than 15 kg. Dogs suspected to have cholestasis associated with hepatobiliary or pancreatic diseases had significantly larger CBD than that in normal dogs.
