1.Analysis of stroke-related risk factors among urban community residents:A case-control study based on propensity score matching

Objective To analyze stroke-related risk factors and their impact on urban community residents in Beijing,focusing on chronic diseases,psychological health,and quality of life,to provide evidence for community screening and intervention strategies.Methods Data were collected from residents aged 50+in 19 communities by using questionnaires and a WeChat App.Propensity score matching(PSM)balanced baseline characteristics.Logistic regression(univariate and stepwise)and mediation effect analysis were conducted by using SPSS and R.Results After PSM,87 stroke cases and 348 controls showed balanced baseline characteristics.The stroke group had higher chronic disease prevalence and lower psychological and quality of life scores.Hypertension increased stroke risk(P=0.002).Cognitive-12 Scale(Cog-12)was positively,and European Quality of Life-5 Dimensions(EQ-5D)negatively,associated with stroke(P<0.001).Hypertension mediated stroke risk through Cog-12 and EQ-5D,with EQ-5D contributing 26.72%.Conclusion In this urban community study,stroke prevention should focus on managing chronic diseases,improving cognitive health and quality of life,and addressing psychological health.Digital tools can enhance follow-up and assessment,optimizing community-based stroke management strategies.

3.A case report of primary renal fibrosarcoma

A patient with intermittent right lumbar pain for more than one month and aggravating for one week was reported. The right laparoscopic radical nephrectomy was performed, and fibrosarcoma of the right kidney was diagnosed. The patient refused further radiotherapy or chemotherapy and was discharged on the 11th after surgery. One year after operation, the patient underwent chest CT, and subdural tumor of right lobe of liver was detected. A surgery was performed in another hospital, and liver metastasis from right renal fibrosarcoma was diagnosed. He was alive four months after operation.
4.Two cases report of segmental testicular infarction

The segmental testicular infarction rate is extremely low, usually showing acute testicular pain, which needs to be distinguished from testicular tumor, testicular torsion, etc. Only a few cases were reported. In this study, two cases were presented. Case one, a 13-year-old boy, was admitted to hospital due to right testicular swelling and pain for 3 days. Presenting with vomiting once on May 5, 2017.He underwent right scrotum exploration and testicular fixation. Testicular pain was disappeared and discharged on the five days operation. There was no obvious abnormality in color doppler ultrasound after one year after operation. Another patient, a 26-year-old man, was admitted to hospital due to left testicular swelling and pain for 6 on March 15, 2018. He was treated with Xue Saitong, natra heparin calcium and Maizhiling tablets. After seven days of treatment, the testicular pain improved and were discharged. One month after discharge, the patient had no pain or discomfort, and the color Doppler ultrasound showed that the left subtesticular polar-like low-isoecho, and a small amount of star-like blood flow signal around.
5.Early and late outcomes in patients with severe extracranial internal carotid stenosis undergoing carotid endarterectomy.

OBJECTIVETo determine the perioperative and late outcomes for carotid endarterectomy (CEA) in treatment of patients with high-grade stenotic lesions of the extracranial internal carotid artery.
METHODSTwenty patients underwent 21 CEAs at the Zhongshan Hospital between May 1993 and June 2000. They were 19 men and 1 woman, with a mean age of 64 +/- 9 years. Seven patients were performed for stroke, 11 for transient ischaemic attacks (TIAs) and 2 for asymptomatic disease. Duplex scan was the primary tool of evaluation prior to surgery. Perioperative digital subtraction angiography and magnetic resonance angiography were done for 19 and 18 patients, respectively. The percentage of stenosis was calculated using NASCET criteria. Of the 21 lesions operated, 19 had a stenosis of 70% or greater, 2 had ulcerative lesions with a stenosis ranging from 60% to 69%. All CEAs were performed under cervical block anaesthesia with selective intraoperative shunting and patch angioplasty. The patients were followed up regularly with duplex scan surveillance.
RESULTSThere was no mortality or stroke during 30 days postoperatively. A TIA occurred in one patient and cranial nerve injury in 2 patients perioperatively. All patients were followed up for a mean interval of 31 +/- 20 months (range: 1 - 63 months). The 2-year survival rate and risk of stroke were 92.3% and 0%, respectively, and the 5-year survival rate and risk of stroke were 79.1% and 12.5%, respectively. Two asymptomatic recurrent stenoses ranging from 50% to 60% were detected on follow-up duplex scan.
CONCLUSIONSFor the patients in this study, CEA is associated with an acceptable perioperative outcome as well as a satisfactory long-term beneficial effect in stroke prevention.

6.Effects of tumor necrosis factor-? on skeletal muscle ischemia reperfusion injury

Objectives To investigate the effect of tumor necrosis factor ? on skeletal muscle ischemia reperfusion injury (SMIR). Methods Twenty four healthy male Sprague Dawley rats were divided into three groups. Group A underwent anesthesia and external jugular vein cannulation only. Group B underwent 4 hours of left hind limb ischemia followed by 4 hour reperfusion. Group C ischemia and reperfusion were treated with anti TNF ? monoclonal antibody (2?mg/kg). ResultsSMIR significantly increased the transcription of TNF ? mRNA in monocyte.
