Debates persist regarding the efficacy and safety of azvudine, particularly its real-world outcomes. This study involved patients aged ≥60 years who were admitted to 25 hospitals in mainland China with confirmed SARS-CoV-2 infection between December 1, 2022, and February 28, 2023. Efficacy outcomes were all-cause mortality during hospitalization, the proportion of patients discharged with recovery, time to nucleic acid-negative conversion (T NANC), time to symptom improvement (T SI), and time of hospital stay (T HS). Safety was also assessed. Among the 5884 participants identified, 1999 received azvudine, and 1999 matched controls were included after exclusion and propensity score matching. Azvudine recipients exhibited lower all-cause mortality compared with controls in the overall population (13.3% vs. 17.1%, RR, 0.78; 95% CI, 0.67-0.90; P = 0.001) and in the severe subgroup (25.7% vs. 33.7%; RR, 0.76; 95% CI, 0.66-0.88; P < 0.001). A higher proportion of patients discharged with recovery, and a shorter T NANC were associated with azvudine recipients, especially in the severe subgroup. The incidence of adverse events in azvudine recipients was comparable to that in the control group (2.3% vs. 1.7%, P = 0.170). In conclusion, azvudine showed efficacy and safety in older patients hospitalized with COVID-19 during the SARS-CoV-2 omicron wave in China.
2.Free peroneal artery perforator flap designed by vascular localization with three-dimensional CT angiography for repairing soft tissue defects of the dorsal forefoot

Objective:To investigate the surgical method and clinical effect of the free peroneal artery perforator flap designed by vascular localization with three-dimensional CT angiography(3D-CTA)in repairing soft tissue defects of the dorsal forefoot.Methods:A retrospective case series study was conducted to analyze the clinical data of 17 patients with soft tissue defects of the dorsal forefoot admitted to Hospital of the 80th Group Army of PLA from February 2015 to January 2019,including 11 males and 6 females,aged from 16 to 65 years[(39.2±9.7)years]. The area of soft tissue defects ranged from 3.0 cm×2.0 cm to 10.0 cm×7.0 cm,and the size of skin flap was from 3.5 cm×2.5 cm to 10.5 cm×7.5 cm. Preoperative 3D-CTA was performed to select the appropriate perforator vessels,and a personalized skin flap was designed according to the examination results and wound conditions. Of all,10 patients were repaired with the peroneal perforator flap at the first stage,and 7 patients grafted with peroneal perforator flap with wound expansion in the second stage. The perforator diameter,vertical distance from the starting point of the perforator to the tip of the lateral malleolus and horizontal distance from the starting point of the perforator to the outer edge of the lower leg were compared between preoperative 3D-CTA and actual intraoperative measurements. The survival of the flap and occurrence of the vascular crisis were observed after operation. The foot function was assessed with the American Orthopedic Foot and Ankle Society(AOFAS)ankle hindfoot score preoperatively and at postoperative 6 months. Six months after operation,effect of flap repair was evaluated by using flap satisfaction score and flap sensory function measured by the criteria established by British Medical Rresearch Council(BMRC). Incision healing and motor function of the donor area were detected.Results:All patients were followed up for 6-12 months[(9.6±2.3)months]. There was no statistically significant differences in perforator diameter,vertical distance from the starting point of the perforator to the tip of the lateral malleolus and horizontal distance from the starting point of the perforator to the outer edge of the lower leg between preoperative 3D-CTA and actual intraoperative measurements( P>0.05). All flaps survived without vascular crisis. Six months after operation,a higher AOFAS ankle hindfoot score[(84.0±7.9)points]was found when compared to that preoperatively[(51.3±8.2)points](P<0.05),with excellent results in 12 patients and good results in 5 patients. At 6 months after the operation,the thickness of the flap was flush with the surrounding skin,which did not affect the wearing of shoes;the thickness of the flap was level with the surrounding skin but did not affect the wearing of shoes. The repair effect was satisfactory with the flap satisfaction score of(8.7±2.3)points.Based on the criteria established by BMRC,the sensory function of the flap continued to improve,including 10 patients reaching the level of S 3-S 4 with the two-point discrimination of(10.2±2.0)mm and 7 patients reaching the level of S 2. All incisions at the donor area were healed well without obvious scar contracture or motion limitation. Conclusions:For soft tissue defects of the dorsal forefoot,3D-CTA assisted vascular localization is able to have individualized and precise design of peroneal artery perforator flap preoperatively to assist rapid and accurate skin flap cutting and accelerate defect healing and functional recovery in the dorsal foot. Moreover,the free peroneal artery perforator flap has advantages of high patients' satisfaction,with minor trauma on donar site or without damaging the main artery.
3.Repair of heel soft tissue defect in children with free medial plantar flap with nerve

Objective:To explore the method and clinical effect of the free medial plantar flap with the nerve in the reconstruction of skin and soft tissue defect of heel in children.Methods:A series of retrospective analyses was conducted on the cases with heel skin and soft tissue defects admitted to the 89th Hospital of the PLA from July 2014 to February 2019. The medial plantar skin flap with cutaneous branches of the medial plantar nerve was harvested from the healthy side and transferred to the opposite side to repair the soft tissue defect of the heel of children. Then, the free skin graft was taken from the groin area to cover the donor site, and the wound was sutured directly. The appearance, texture, and color of the flap were observed, and the sensory recovery of the flap was evaluated two years after the operation. The functional condition was elevated by the American Orthopedic Foot and Ankle Society (AOFAS) ankle and hindfoot function scoring system.Results:A total of 6 children with heel skin and soft tissue defects were enrolled, including 4 males and 2 females, aged from 3 to 13 years.There were 4 cases on the right, and 2 cases on the left side, the size of skin and soft tissue defect was 2 cm×3 cm-6 cm×7 cm. All 6 flaps survived, the skin graft area healed in one stage, and the size of the flap was 3 cm×4 cm-7 cm×8 cm. The patients were followed up for 2-3 years. The appearance of the flap was not bloated, the color and texture of the flap were similar to the surrounding tissue, and the pedicle was flat. The two-point discrimination of the flap was 5-10 mm, with an average of 7 mm. There were only pigmentation and no obvious scar in the donor site of the uninjured foot. The ankle joint function was evaluated according to the American Association of foot and ankle surgery : 4 cases were excellent and 2 cases were good.Conclusions:The free medial plantar flap with the nerve is similar to the heel tissue, with reliable blood supply, no damage to the main blood vessels, and good sensory recovery after transplantation. It is an ideal flap for repairing heel soft-tissue defects in children.

5. Application of tracheotomy combined with thyrocricocentesis and puncture of front tracheal wall in emergency treatment of laryngeal edema in burn patients

Objective:
To explore the effect of tracheotomy combined with thyrocricocentesis and puncture of front tracheal wall in emergency treatment of laryngeal edema in patients with burns.
Methods:
From November 2000 to August 2018, 22 patients with severe burn or extremely severe burn combined with acute laryngeal edema were rescued in the author′s unit, including 18 males and 4 females, aged 17 to 68 years. All patients were complicated with mild inhalation injury or above and more than deep partial-thickness burn to head, face, and neck. From November 2000 to October 2012, simple emergency tracheotomy was performed for 12 cases. From May 2013 to August 2018, tracheotomy combined with thyrocricocentesis and puncture of front tracheal wall was performed for 10 cases. Rescue effect and complication of the two kinds of tracheotomy were recorded. Data were processed with Fisher′s exact probability test.
Results:
Among the 12 patients treated with simple emergency tracheotomy, 5 cases survived and 7 cases died of suffocation during tracheotomy. Among the 10 patients treated with tracheotomy combined with thyrocricocentesis and puncture of front tracheal wall, 9 cases survived and 1 case died of cardiac arrest caused by arrhythmia. Background:
With the publication of Sepsis-3 definition, epidemiological data based on Sepsis-3 definition from middle-income countries including China are scarce, which prohibits understanding of the disease burden of this newly defined syndrome in these settings. The purpose of this study was to describe incidence and outcome of Sepsis-3 in Yuetan sub-district of Beijing and to estimate the incidence rate of Sepsis-3 in China.

The medical records of all adult residents hospitalized from July 1, 2012 to June 30, 2014 in Yuetan sub-district of Beijing were reviewed. Objective: To evaluate the feasibility and potential value of comprehensive geriatric assessment (CGA) in elderly (≥60 years) patients with newly diagnosed acute myeloid leukemia (AML) in China. Methods: The CGA results of 83 newly diagnosed AML (non-APL) patients from 16 hospitals in Beijing and Tianjin between March 2016 and December 2017 were prospectively collected and analyzed. The clinical data, treatment and follow-up information were also collected. Results: Of 83 newly diagnosed elderly AML patients, 81 patients (97.6%) completed all designated CGA assessment. The median number of impaired scales of the CGA assessment in the studied population was 2(0-6). Sixteen patients (19.3%) showed no impairments according to the geriatric assessment scales implem ented by this study. The distributions of impaired scales were as follows: impairment in ADL, 55.4%; IADL impairment, 42.2%; MNA-SF impairment, 48.2%; cognitive impairment, 15.7%; GDS impairment, 31.7%; HCT-CI impairment, 19.5%, respectively. In patients with "good" ECOG (n=46), the proportion of impairment for each CGA scale ranged from 6.5% to 37.0% and 32 patients (68.9%) had at least one impaired CGA scale. Survival analysis showed that the number of impaired scales of the CGA was significantly correlated with median overall survival (P=0.050). Conclusions: CGA was a tool with feasibility for the comprehensive evaluation in elderly AML patients in China. Combined with age and ECOG, CGA may be more comprehensive in assessing patients' physical condition.

Objective To investigate the effect of long-term high-fat diet on cognitive function and hippocampus neurons ultrastructure in obese rats.Methods Forty SD rats were randomly assigned to a high fat diet (HFD) group and a common diet (CD) group.Meanwhile,HFD-induced obese rat model were established.The spatial learning and memory were measured by the Morris water maze,and the neurons ultrastructural changes in rat hippocampus CA1 region at the corresponding period were observed by transmission electron microscopy.Results The average weight of rats was 25%,28%,and 22% higher in the HFD group than in the CD group at the 12,16,and 20 weeks,respectively;the Lee's indexes were 6%,4%,and 8% higher;the average swimming latency were 52%,44%,and 40% longer;the average swimming distance were 85%,45%,and 51% longer;the average swimming speed were 57%,34%,and 18% higher;the duration of staying in the target quadrant were 32%,54%,and 63% shorter;and the average times of crossing the plate form were 30%,34%,and 34% shorter,respectively (all P <0.001).In comparison of ultrastructure in hippocampus CA1 region of rats at corresponding time points,the amounts of degenerated and necrosis neurons,of the deformed and vacuolar mitochondria,and of the less rough endoplasmic reticulum were significantly more at 12,16,and 20 weeks in the HFD group than in the CD group.Conclusions Long-term HFD-induced obesity damages the structure of neurons in the hippocampus,impairs spatial learning and memory function,and accelerates cognitive aging in rats.
10.Biomechanical research on effects of joint line height on stress of medial and lateral compartment after unicompartmental knee arthroplasty

Objective To investigate biomechanical effects of different joint line height on unicompartmental knee arthroplasty (UKA) and to provide biomechanical evidence for surgical decision during UKA using finite element analysis (FEA).Methods On the basis of knee joint CT data from a healthy volunteer (male,30 years old,165 cm and 60 kg) and UKA prosthesis 3D scanned data,the 3D models of normal knee and UKA were constructed.Subject-specific finite element models with inhomogeneous material property assignment were constructed for normal knee and UKA with 3 different height of joint line (-3 mm,0 mm and +3 mm).Starting from the tibial articular surface,5 sections were extracted with 2 mm interval.30 nodes were selected in medial and lateral area,respectively on each section.The mean stress value of nodes was defined as the stress level of subchondral bone.Results Mean values of stress on polyethylene upper surface of 0,-3 and+3 mm joint line position were 14.84,26.81and 20.86 MPa,and the difference was statistical significant (t0vs-3=4.896,P0vs-3=0.000;t0vs+3=3.455,P0v+3=0.008;t-3vs+3=2.579,P-3vs+3=0.020).Mean values of stress on tibial prosthesis upper surface of 0,-3 and +3 mm joint line position were 29.69,50.49 and 39.99MPa,respectively,and the difference was statistical significant(t0vs-3=5.675,P0vs-3=0.000;t0vs+3=4.755,P0v+3=0.001;t-3vs+3=4.783,P-3vs+3=0.000).When joint line was in 0 mm height,stress level of subchondral bone was similar to that of normal knee.When joint line was in-3 mm height,significantly increased stress was found on polyethylene (increased 81%) and tibial component (increased 70%) surface compared with that of 0 mm height,while stress on trabecular bone under lateral tibial articular surface also increased by 8.7%.When joint line was in +3 mm height,increased stress on polyethylene (increased 41%) and tibial component (increased by 35%) surface were less than that of-3 mm height.Stress on trabecular bone under lateral articular surface decreased by 55.6% compared with 0 mm height.Conclusion During UKA,ensuring a normal height of joint line is benefit to keep the stress path of the medial and lateral tibia similar with normal knee and decrease the risk of premature polyethylene wear,tibial component subsidence and periprosthetic fracture.
