1.Characteristic analysis of nanogel composite system and its application strategies in visualization of diagnostic imaging and therapy

BACKGROUND:Nanogels can combine the properties of nanoparticles and hydrogels,which have multiple advantages and can play a great role in clinical translation.OJECTIVE:To summarize the self-and cross-linking properties of nanogels,the diagnostic and therapeutic integration strategies they played in a variety of imaging processes,in conjunction with their functional visualization and application properties.METHODS:By searching PubMed,Web of Science,CNKI,and Wanfang databases,English and Chinese search terms were"nanogel,nanohydrogel,imaging,magnetic resonance imaging,fluorescence imaging,ultrasound imaging,photoacoustic imaging,multimodal imaging,therapy,treatment."According to the inclusion criteria,78 articles were finally selected for review.RESULTS AND CONCLUSION:The preparation process of nanogels is significantly affected by their multiple properties and cross-linking mechanisms.Optimization of these properties and mechanisms can help to improve their structural performance and accelerate the translation of their clinical applications.With the trend of integrated diagnosis and treatment,functional visualization of nanogels can effectively image tumors with the help of magnetic resonance imaging,fluorescence imaging,ultrasonography,photoacoustic imaging,and multimodal imaging,which can be of key value in the treatment strategies of various diseases.These findings not only pave the way for improving the preparation methods of nanogels,but also create a robust groundwork for hastening their clinical translation.

3.Thrombus Migration After Tenecteplase Versus Alteplase in Acute Large Vessel Occlusion

Background:
and Purpose In patients with large vessel occlusion (LVO), intravenous thrombolysis (IVT) frequently alters thrombus location; however, the clinical impact of this phenomenon remains unclear. We aimed to compare post-IVT thrombus dynamics between tenecteplase and alteplase and to evaluate the association between thrombus dynamics and 3-month outcomes.
Methods:
This retrospective study analyzed prospectively collected, multicenter data from consecutive patients with LVO who underwent bridging therapy between January 2022 and December 2024. Thrombus dynamics were classified as resolution, migration, or stability. Analyses incorporated propensity score matching with weighting to balance baseline characteristics.
Results:
Of the 806 initially included patients, 746 were included after matching (373 treated with tenecteplase and 373 treated with alteplase). The incidence of thrombus migration was significantly higher in the tenecteplase group than in the alteplase group (19.3% vs. 11.3%; odds ratio [OR]: 1.92; 95% confidence interval [CI] 1.27–2.91). The advantage of tenecteplase over alteplase was restricted to patients with an IVT-to-puncture time of <60 minutes (18.6% vs. 6.2%; p=0.001) and was no longer significant when the interval ≥60 minutes (19.7% vs. 15.0%; p=0.204; pinteraction=0.043). Additionally, thrombus migration was associated with a better functional outcome (OR: 1.62; 95% CI 1.04–2.53). Finally, tenecteplase was associated with improved functional independence compared with alteplase (OR: 1.43; 95% CI 1.04–1.95).
Conclusions
Tenecteplase demonstrated superior efficacy in inducing thrombus migration compared with alteplase, particularly within 60 minutes of IVT administration. Thrombus migration independently predicted improved functional independence. These findings support the preferential use of tenecteplase for bridging therapy in patients with LVO.
4.Risk of ovarian cancer in women with pelvic inflammatory disease and homologous recombination repair gene mutations under 55:a population-based cohort study

Objective:
To address the relation among pelvic inflammatory disease (PID), genetic vulnerability and ovarian cancer (OC) risk, we assessed the association between PID and OC risk, alongside the interplay with germline homologous recombination repair (gHR) mutation, utilizing the UK Biobank.
Methods:
We conducted a prospective cohort study in the UK Biobank by tracking OC incidences between individuals with and without a PID history. Identification of gHR mutations (BRCA1, BRCA2, RAD51C, RAD51D, BRIP1) carriers were accomplished through paired whole-exome sequencing data. We used Cox’s regression models to evaluate the hazard ratios (HRs) for OC risks under PID.
Results:
In the large prospective cohort study, the adjusted HR for OC in patients with PID was 1.45 (95% confidence interval [CI]=0.90, 2.32) compared with those with non-PID.Intriguingly, age-stratified analysis unveiled a positive association between PID history and OC risk in those aged under 55 years (HR=1.92; 95% CI=1.02, 3.63). Moreover, individuals aged younger than 55 years harboring both a history of PID and gHR mutations exhibited the highest risk of OC (HR=7.40; 95% CI=1.03, 53.10).
Conclusion
An association between PID and OC risk emerged, notably in the subgroup aged younger than 55 years old. Individuals with both a PID history and gHR mutations exhibited the highest risk of OC. These findings imply PID as a potential precursor for OC, underscoring the importance of early intervention, particularly in the younger population with gHR mutations.
5.Effect of miR-7-5p on Proliferation, Invasion of Non-small Cell Lung Cancer Cells by Targeting POLE4 and Its Underlying Mechanism

Objective To detect the expression levels of miR-7-5p and POLE4 in non-small cell lung cancer cells and their effect on cells proliferation, migration and invasion. Methods qRT-PCR was used to detect the relative expression levels of miR-7-5p and POLE4 mRNA in NSCLC tissues, adjacent tissues, tumor cells and human normal bronchial epithelial cells. Luciferase reporter gene was used for analyzing of the targeting relation between POLE4 and miR-7-5p in NSCLC cells. si-NC and si-POLE4 were transfected into SPC-A-1 cells as the si-NC group and si-POLE4 group, and the control group was set at the same time. Objective:
To investigate the clinical outcomes of selective major aortopulmonary collaterals(MAPCAs) unifocalization and report histopathological findings in patients with pulmonary atresia with ventricular septal defect and major aortopulmonary collaterals(PA/VSD/ MAPCAs).
Methods:
The study enrolled 6 MAPCAs/VSD/PA patients with age ranged from 6 to 96 months and body weight ranged from 5.0 to 23.0 kg. These patients underwent selective MAPCAs unifocalization and primary repairs. Preoperative cardiac catheter, selective arteriography, cardiac CTA and intraoperative pathology were performed to identify different function, anatomic distribution and histopathology of MAPCAs.
Results:
6 MAPCAs/VSD/PA patients underwent selective MAPCAs unifocalization and primary repair. No death occurred after operation and at follow-up which lasted for 1 to 20 months. Preoperative cardiac catheter, selective arteriography and intraoperative histopathology demonstrated distribution of functional MAPCAs similar to native pulmonary artery arborization and participating in arterial gas exchange. Functional MAPCAs were classified into elastic arteries according to histopathology.
Conclusion
There are two histological type of MAPCAs which play different roles. Selective unifocalization to functional MAPCAs which are classified into elastic arteries like native pulmonary artery is a safe and effective treatment approach for PA/VSD/MAPCAs.
7.Relationship between preoperative long-term sleep disorder and postoperative hyperalgesia in patients undergoing cardiac surgery

Objective:To evaluate the relationship between preoperative long-term sleep disorder and postoperative hyperalgesia in the patients undergoing cardiac surgery.Methods:One hundred and eighty-one adult patients of both sexes, aged 18 yr, undergoing elective cardiac valve surgery under cardiopulmonary bypass with general anesthesia, were enrolled in this study.On 1st day before surgery, the Pittsburgh Sleep Quality Index questionnaire was used to assess the patient′s sleep quality in the last month.When Pittsburgh Sleep Quality Index score was more than 5, the patient was considered to have long-term sleep disorder.Postoperative analgesia was performed with sufentanil.Patients were divided into 2 groups according to the numeric rating scale (NRS) score: non-hyperalgesia group (NHA group, NRS score <4) and hyperalgesia group (HA group, NRS score ≥4). A multivariate logistic regression was used to identify the risk factors associated with postoperative hyperalgesia.Results:The results of logistic regression analysis found that smoking and preoperative long-term sleep disorder were independent risk factors for postoperative hyperalgesia.Conclusion:Preoperative long-term sleep disorder may induce hyperalgesia after cardiac surgery in patients.
8.Prognosis of the complete transposition of great arteries with left ventricular outflow tract obstruction after intraventricular repair

Objective To compare the clinical characteristics and prognosis of patients who received two different intraventricular repair. Methods We retrospectively analyzed the clinical data of 24 complete transposition of the great arteries (TGA)/left ventricular outflow tract obstruction (LVOTO) patients who all received intraventricular repair. The patients were allocated into two groups including a REV group and a Rastelli group. There were 13 patients with 9 males and 4 females at median age of 25.2 (6, 72) months in the REV group. There were 11 patients with 10 males and 1 female at median age of 47.9 (14, 144) months in the Rastelli group. Results The age at operation (P=0.041), pulmonary valve Z value (P=0.002), and LVOT gradient (P=0.004), rate of multiphase operation between the REV group and the Rastelli group was statistically different. The mean follow-up time was 17.3 months. And during the follow-up, 1 patient had early mortality, 2 patients had early reintervention, 7 patients had postoperative RVOTO, and received Rastelli and larger VSD inner diameter were associated with postoperative RVOTO. Conclusion As the traditional surgery for TGA/LVOTO patients, the intraventricular repair has a low early mortality and low early reintervention. Modified REV is associated with postoperative peripheral pulmonary vein isolation (PVIS). Patients who received Rastelli operation and with larger VSD inner diameter are more likely to have postoperative RVOTO, but the reintervention for PVI and RVOTO during follow up is very low.

Objective To evaluate the effect of aminophylline on the efficacy of extubation under ultra-fast track anesthesia ( UFTA) in pediatric patients with congenital heart disease ( CHD) undergoing surgical correction of anomaly. Methods Ninety American Society of Anesthesiologists physical status Ⅰor Ⅱ pediatric patients, aged<6 yr, undergoing surgical correction of anomaly, were randomized into 3 groups ( n=30 each) using a random number table method: group Ⅰ, group Ⅱ and group Ⅲ. After the end of surgery, normal saline was given in groupⅠ, 1 mg∕kg aminophylline was given in groupⅡ, and 2 mg∕kg aminophylline was given in groupⅢ. The condition of extubation in the operating room and respirato-ry rate at extubation were recorded. Mean arterial pressure, heart rate and bispectral index value were re-corded at the end of surgery, and at 9 and 15 min after administration. Results Compared with groupⅠ, the rate of successful extubation in the operating room, BIS value at 9 min after administration and respira-tory rate at extubation were significantly increased inⅡand Ⅲ groups ( P<0. 05) . There was no significant difference in the parameters mentioned above between group Ⅱand group Ⅲ ( P>0. 05 ) . Conclusion Aminophylline can increase the efficacy of extubation under UFTA in pediatric patients with CHD undergoing surgical correction of anomaly, and 1 mg is the recommended dose.
