1.Mechanisms of Qiaobai cold compress solution in improving acne vulgaris based on transcriptomics and experiment

OBJECTIVE To investigate the mechanism by which Qiaobai cold compress solution (QBCS) improves acne vulgaris (AV) based on transcriptomics and animal experiments. METHODS Rats were randomly divided into a blank control group ( n =6) and a modeling group ( n =30). AV models were established in the modeling group by topical application of oleic acid to the inner surface of both ears, combined with subcutaneous injection of Cutibacterium acnes suspension into the auricle. Successfully modeled rats were further divided into the model group, positive control group (Tretinoin cream, 0.045 g/kg), and QBCS low-, medium-, high-dose groups [3.55, 7.11, 14.22 g/kg (calculated by the amount of crude drug) ] , with 6 rats in each group. Rats in each d rug group were treated with the corresponding drugs once daily for 14 consecutive days. After the final administration, changes in the appearance of the ears and histopathological changes in the ear tissues were observed, and serum levels of inflammatory factors, including tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and IL-1β, were measured. Auricular tissues from the blank control group, model group and QBCS medium-dose group were collected for transcriptome sequencing. Differential expressed genes (DEGs) were screened and subjected to Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis, followed by validation using real-time quantitative polymerase chain reaction and Western blot assay. RESULTS Compared with the model group, rats in all QBCS groups showed alleviated auricular acne symptoms, with reduced epidermal thickening, sebaceous gland hyperplasia, and inflammatory cell infiltration. Serum levels of TNF-α (except for the QBCS low-dose group), IL-6 (except for the QBCS low-dose group) and IL-1β were significantly decreased ( P <0.05). A total of 590 DEGs were identified (blank control group vs. model group), and 596 DEGs were identified (model group vs. QBCS medium-dose group). Above DEGs (blank control group vs. model group) were mainly enriched in Toll-like receptor (TLR) and nuclear factor-kappa B (NF-κB) signaling pathways, etc. Validation experiments showed that, compared with model group, low-, medium- and high-dose of QBCS reduced, to varying degrees, the mRNA expression of TNF-α, TLR2, interferon-γ and CXC chemokine ligand 8 in the auricular tissues of AV rats, increased the mRNA expression of peroxisome-proliferator-activated receptor gamma and tumor protein 53, and inhibited the phosphorylation of NF-κB p65 protein as well as the expressions of TLR2 and myeloid differentiation primary response protein 88(MyD88) ( P <0.05). CONCLUSIONS QBCS can alleviate auricular inflammation and skin lesions in AV rats. This effect may be related to inhibition of the TLR/MyD88/NF-κB signaling pathway, thereby suppressing the expression of downstream inflammatory factors such as TNF-α.
2. Treatment of pretibial myxedema with local subcutaneous injection of glucocorticoid: Three cases report

To investigate the treatment of hyperthyroidism complicated with pretibial myxedema. Three patients with hyperthyroidism complicated with pretibial myxedema were treated with topical injection of corticosteroid at the skin lesion. All 3 patients were cured with pretibial mucous edema by local injection of corticosteroids. Multi-point injection of glucocorticoid combined with maintaining euthyroid state is effective and has little side effect.
3. The effects of hyperbaric oxygen on the hemodynamics and intracranial pressure of patients with severe brain injury: A systematic review of randomized and controlled trials

Objective:
To systematically evaluate the effects of hyperbaric oxygen on the hemodynamics and intracranial pressure of patients with severe craniocerebral injury (STBI).
Methods:
Reports of randomized and controlled trials applying hyperbaric oxygen in the treatment of STBI were retrieved from the Pubmed, Cochrane Library, Embase, CBM, CNKI, VIP and Wan Fang databases. Each report found was evaluated by two researchers independently applying pre-defined inclusion and exclusion criteria. The data were extracted and combined and a meta-analysis was performed.
Results:
Eight trials involving 725 patients were included in the meta-analysis. They combined to demonstrate that intracranial pressure, oxygen uptake and scores on the Glasgow coma scale improved significantly more in the hyperbaric oxygen group than in the control group after between 3 and 10 days of treatment.
Conclusion
Hyperbaric oxygen therapy is effective in treating severe craniocerebral injury and it is worthy of clinical application.
4.Effect of hyperbaric oxygen in the treatment of radiation encephalopathy: a systematic review of randomized controlled trials

Objective To assess systematically the efficacy and safety of hyperbaric oxygen (HBO)combined with drugs in treatment of radiation encephalopathy (PREP).Methods Computer-aided information retrieval was made by using such databases as Pubmed,Cochrane Library,Embase,CBM,CNKI,VIP and WanFang data,at the same time,manual data retrieval was also made by searching related literature both at home and abroad.The time span for information retrieval was from the inception of the above-mentioned database to March 2018.After 2 reviewers independently screened the retrieved literature in accordance with the included and excluded criteria,and collected the retrieved papers and evaluated the methodological quality of the retrieved literature,meta-analysis was performed by using RevMan 5.2 software.Results Finally,a total of 16 randomized controlled trials (RCTs) involving 990 patients were included in the study.Meta analysis showed that effective rate and cure rate of the HBO combined with drugs group were respectively [OR =5.78,95% CI (4.15,8.06)] (P<0.01) and [OR=4.56,95c%CI (2.88,7.20)] (P<0.01),which were superior to those of the simple drug treatment group.And the number of foci was less than that of the simple drug treatment group [OR =-2.63,95% CI (-3.70,-1.56)] (P < 0.01).Conclusion Current evidence shows that HBO combined with drug treatment could produce good effects on radiation encephalopathy.For this reason,it is worth further clinical popularization.
5.Analysis of the therapeutic efficacy of hyperbaric oxygen combined with Yangxueqingnao granules for the treatment of tinnitus and dizziness in patients with sudden deafness

Objective To investigate the clinical effects of hyperbaric oxygen (HBO) combined with Yangxueqingnao granules on tinnitus and dizziness in patients with sudden deafness.Methods One hundred and twenty-four patients admitted into the Sichuan Provincial People's Hospital from March 2015 to June 2016 were randomly assigned to the observation group (n =61) and the control group (n =63) by using the random digital software to generate random number and opaque envelope hidden test scheme.The control group only received routine therapy,including improvement of circulation,nerve nutrition,hormone and symptomatic treatment,while the observation group was supplemented with HBO + Yangxueqingnao granules in addition to routine therapy.The observation group received HBO therapy one session a day,5 days a week and 10 days a treatment course.In addition,the patients in the observation group had Yangxueqingnao granules orally,one pack once and 3 times a day.After 2 weeks of treatment,therapeutic efficacy of the 2 groups was evaluated by the blinded method,and after 3 and 6 months of treatment,therapeutic effects on tinnitus and dizziness of the 2groups were evaluated through medical follow-ups by telephone.Results All the patients of the 2 groups had tinnitus,the observation group had 42 cases of dizziness,while the control group had 36 cases of dizziness.Following treatment for 2 weeks,3 and 6 months,total effective rates of tinnitus in the observation group were respectively 83.61%,75.41% and 77.05%,which were all higher than those of the control group (65.08%,57.14%,58.73%),and statistical significance could be noted when comparisons were made between the 2 groups (P < 0.05).Total effective rate of dizziness in the observation group after 2 weeks,3 and 6 months of treatment was 95.23%,which was obviously better than those of the control group (75.00%,80.56%,83.33%),and statistical significance could also be seen when comparisons were made between the 2 groups (P < 0.05).Conclusions HBO combined with Yangxueqingnao granules in the treatment of patients with sudden deafness complicated with tinnitus and dizziness could produce good clinical effects.For this reason it was worth further clinical promotion.

8.Hyperbaric oxygen therapy can improve the learning capacity and memory in vascular dementia

Objective To explore the influence of hyperbaric oxygen therapy (HBOT) on the learning and memory ability using a rat model of vascular dementia (VD).Methods Sixty Sprague-Dawley rats were selected and divided into a normal group,a sham operation group,a VD group and an HBOT group,each of 15 rats using a random number table.The improved four-vessel method was used to establish a VD model in all except the rats in the normal group,but for the sham operation group,the vertebral artery was not subjected to firing and the arteria carotis communis was not clipped.The HBOT group was then given HBOT for 7 days.The Morris water maze test was used to evaluate their learning and memory ability of all 4 groups.Results Compared to the normal group,the mean escape latency of the sham operation group,the VD group and the HBOT group was significantly longer in each case (P<0.05).Their platform quadrant crossing times were also significantly less,and their quadrant swimming distance as a proportion of their total swimming distance was significantly smaller (P<0.05).Compared with the sham operation group,the mean escape latency of the VD and HBOT groups was significantly longer (P<0.05),the times of crossing platform quadrant were less and their quadrant swimming distance was a smaller proportion of their total swimming distance.As compared with the VD group,the mean escape latency of the HBOT group was significantly shorter [(25.73±6.20) s],the average platform quadrant crossing times were significantly more [(9.51±2.25) times] and the platform quadrant swimming distance of the HBOT group was significantly longer [(494.72±208.26)],representing a significantly larger proportion of the total swimming distance [(28.71 ± 5.32) %] (P<0.05).Conclusion HBOT can effectively improve learning and memory in vascular dementia,at least in rats.
9.Establishment of a rabbit model of facial skin lifting and application of the absorbable soft tissue plastic belt

Objective To establish a rabbit model of facial skin lifting and to evaluate the skin lifting effects of absorbable soft tissue plastic belt, and to provide experimental basis for clinical application of this plastic belt.Methods 36 Japanese white rabbits were randomly divided into model control group (n=6) and plastic belt lifting group (n=30). The model control group received only facial skin resection surgery, while the plastic belt lifting group had facial skin resection and plastic belt implantation.The changes of rabbit general status, skin lifting effects, performance of plastic belt and pathological changes of subcutaneous tissue after implantation were determined during the 4 weeks after surgery. Results The rabbit skin wound was healed within 3-7 days after surgery without infection.Meanwhile, the plastic belts did not show shifting or rupture, and only whitening around the perforations was observed in the two groups.Compared with the model control group, the MA and MB distances in the plastic belt lifting group were significantly lower ( P<0.01 ) , while the biological tension of plastic belts in the facial skin lifting rabbit models was significantly increased with the extension of time after implantation (P<0.01), and the biological tension was 18.62 N at 4 weeks after transplantation. In addition, the tensile intensities of perforations and plate in the two groups were significantly reduced at 4 weeks postoperatively (P<0.01), the tensile intensity of perforations and plate in the two groups were maintained at 35.07 N and 53.31 N, respectively, and the perforations/plate tensile intensity ratio of the two groups remained unchanged during 4 weeks after transplantation ( P >0.05 ) .Moreover, the molecular weight ( Mw ) , peak molecular weight ( Mp ) , Z molecular weight (Mz) and viscosity were gradually decreased along with the time passing after implantation (P<0.01), and its dispersion Mz/Mw ratio was also gradually decreased from 2 weeks after implantation (P<0.01), and no obvious pathological changes were found after subcutaneous implantation of the plastic belts.Conclusions We have successfully established a facial skin lifting rabbit model, and the plastic belt can obviously lift the facial skin fascia system and keep intact more than 4 weeks in the body.Therefore, this plastic belt can be applied for anti-wrinkle facial soft tissue lifting against the skin damage caused by aging.

Antineutrophil cytoplasmic antibody-associated vasculitis (AAV) refers to the groups of potentially life-threatening autoimmune disease.Antithyroid drug is one of the causes.Usually the involved organs are skin,kidney,and (or) lung.Early diagnosis and treatment of AAV is essential.Timely cessation of antithyroid drugs is the first step.If necessary,glucocorticoids and (or) immunosuppressive agents should be used to delay the progression of the disease.
