OBJECTIVE To optimize the preparation process of Pollen Typhae dripping pills.METHODS Using appearance morphology, roundness, tailing phenomenon, adhesion condition, friability, and total flavonoids content of Pollen Typhae as evaluation indicators, the analytic hierarchy process-entropy weight method (AHP-EWM) hybrid weighting method was employed to determine the weight coefficients of each indicator and calculate the comprehensive score as the response value. The effects of matrix type and ratio, drug-to-matrix ratio, and condensation temperature on the forming process of dripping pills were respectively investigated, through single-factor experiments and Box-Behnken response surface methodology and the optimal process was optimized and validated. The cumulative dissolution rate of Pollen Typhae dripping pills within 150 min was determined and compared with that of total flavonoid extract from Pollen Typhae .RESULTS The optimal preparation process for Pollen Typhae dripping pills was determined as follows: the ratio of PEG4000 to PEG6000 was 2∶1, the drug-to-matrix ratio was 1∶2, and the condensation temperature was 17.5 ℃. The measured average comprehensive score of dripping pills prepared under these conditions was 9.208 (RSD=0.91%, n =3), with a relative error of 0.83% compared to the predicted value of 9.285. The release mechanism of Pollen Typhae dripping pills was Fickian diffusion controlled deceleration release. The initial dissolution rate was fast, and the dissolution rate decreased in the middle and later stages. The total flavonoid extract from Pollen Typhae was released rapidly in the initial dissolution stage and quickly entered the plateau phase. Their maximum dissolution rates were 95.96% and 84.07%, respectively.CONCLUSIONS The optimized preparation process for Pollen Typhae dripping pills was stable and reliable. The dissolution rate of Pollen Typhae dripping pills prepared by optimized process was higher than that of the total flavonoid extract from Pollen Typhae .

Objective::Diverse antithrombotic strategies were applied to patients undergoing aortic valve replacement. However, the optimal therapeutic regimen for patients undergoing transcatheter aortic valve implantation/replacement (TAVI/TAVR) remains unclear. The purpose of this study was to compare the efficacy and safety of various antithrombotic therapies following TAVI/TAVR.Methods::Relevant clinical trials evaluating the effect of anticoagulation or antiplatelet regimens on patients after TAVI/TAVR from inception to September 2020 were identified using the PubMed, EMBASE, and the Cochrane Library databases. The inclusion criteria including (1) all patients underwent TAVI/TAVR; (2) the interventions were antithrombotic strategies that prevent the occurrence of thrombotic events in patients; (3) randomized controlled trials or prospective observational studies; and (4) investigation of at least 1 outcome with a follow-up period of ≥3 months. The exclusion criteria including (1) research content was identical or irrelevant to the purpose of the present study; (2) lack of the required outcome index or availability of fragmentary original information; and (3) the full text is not available. The major outcomes were all-cause mortality, thromboembolic complications, and bleeding events. The Cochrane Collaboration’s tool and the Newcastle-Ottawa Scale were used for assessing the risk of bias in included studies.Results::Thirteen studies (3 randomized controlled trials and 10 non-randomized studies) were identified, with a total of 23,497 patients. Four studies compared direct oral anticoagulants (DOACs) with warfarin, 1 study compared aspirin with warfarin, 6 studies compared aspirin plus clopidogrel (dual antiplatelet therapy (DAPT)) with aspirin monotherapy, and 2 studies compared DAPT and aspirin monotherapy with warfarin concurrently. There were no significant differences found between the DOAC and warfarin groups regarding all-cause mortality (risk ratio (RR): 1.03; 95% confidence interval (CI): 0.65-1.64; P = 0.909; Phet = 0.105), clinical adverse events (RR: 1.59; 95% CI: 0.99-2.58; P = 0.057; Phet = 0.738), or bleeding events (RR: 0.93; 95% CI: 0.78-1.11; P = 0.437; Phet = 0.338). The rates of all-cause mortality (RR: 0.71; 95% CI: 0.54-0.93; P = 0.012; Phet = 0.845) and bleeding events (RR: 0.43; 95% CI: 0.22-0.83; P = 0.012; Phet = 0.569) were lower in the aspirin group versus the warfarin group; however, there was no difference in the rate of clinical adverse events (RR: 0.38; 95% CI: 0.14-1.07; P = 0.068; Phet = 0.593). The DAPT group had an advantage versus the aspirin group in all-cause mortality (RR: 0.89; 95% CI: 0.82-0.98; P = 0.013; Phet = 0.299); however, the incidence of bleeding events (RR: 2.06; 95% CI: 1.39-3.07; P < 0.001; Phet = 0.001) exhibited an increasing trend. Notably, there was a slight decrease in the incidence of clinical adverse events (RR: 1.09; 95% CI: 0.94-1.26; P = 0.268; Phet = 0.554). Conclusion::The present meta-analysis integrates the latest published results on antithrombotic strategies in patients after TAVI/TAVR. Aspirin showed a favorable risk-benefit profile versus warfarin, with lower rates of all-cause mortality and bleeding events. Although DAPT was also associated with a significantly lower rate of all-cause mortality, it was linked to a higher incidence of bleeding events. The DOACs did not show significant benefits compared with warfarin. Some certain limitations should be noted, such as different types of trails produce heterogeneity and finite inclusion of TAVI/TAVR patients increased selection bias.

Journal of Apoplexy and Nervous Diseases 2020;37(10):886-890
Objective To explore the risk factors and risk mapping model of post-stroke cognitive dysfunction (PSCD) in patients with Acute stroke (AI).Methods Two hundred and sixty cases of AI patients were divided into PSCD group and PSCD group according to whether the patients had PSCD.Independent risk factors affecting the occurrence of PSCD in AI patients were screened by Logistic regression analysis.R software was used to establish a risk prediction model for the screened independent risk factors, and the predictive and accuracy of the model were verified. Results Logistic regression analysis showed that age, education level, hypertension, diabetes, physical exercise and cerebrovascular stenosis were independent risk factors for PSCD in AI patients (P<0.05),which were all highly correlated with PSCD.Based on the above 6 influencing factors,a nomogram model was established to predict PSCD in AI patients. The verification results show that the model has a high predictive performance with a C-index of 0.808 (95%CI 0.747~0.869).Conclusion Comprehensive factors such as age, education level, hypertension, diabetes, physical exercise and cerebral vascular stenosis should be taken into account in time for AI patients, and the probability of PSCD occurrence in AI patients should be predicted through the individual line map model, and intervention measures should be taken as soon as possible to improve the prognosis of the patients.

Objective To investigate the expression and the early diagnostic significance of heat shock protein 70 in acute allograft rejection of liver-transplamed rats. Methods The model of rat orthotopic liver transplantation was made by using a modified "two-cuff technique". The rats were randomly divided into 3 groups. For each group, donors and receptors all included 15 rats respectively. The control group: Wistar to Wistar liver transplantation; The untreated group: SD to Wistar liver transplantation, not receiving any immunosuppressant after liver transplantation; The treatment group: SD to Wistar liver transplantation, receiving intramuscular injection of tacrolimus (FKS06, 2mg kg-1. day-1) after operation. Five rats were executed randomly in every group on the post-transplantation day 3, 5 and 7 and the graft samples were obtained for optical microscopic observation. The expression of HSPT0 in grafts was detected by using immunohistochemical method and RT-PCR. The correlation between acute rejection following liver transplantation and the expression of HSP70 in grafted liver was studied. Results There was no acute rejection examined in the control group. The untreated group showed typical allograft rejection and the rejection activity index (RIA) went up gradually after the operation (P<0.01). The treatment group showed no rejection or borderline allograft rejection. The level of HSP70 was increased transiently after operation, then reduced in the control group (P<0.05). The level of HSP70 in the untreated group was higher than in the control groupand gradually increased with the prolongation of time after transplantation (P<0.01). A significant correlation was found between HSP70 and pathological score in the untreated group (P<0.01). The treatment group showed low levels of HSP70 of all the time. Conclusions The expression of HSP70 in grafts is closely related to the occurrence and development of the acute rejection and can be useful for early diagnosis of acute allograft rejection following liver transplantation.
