Objective To investigate the correlation between serum levels of phosphoglycerate mutase family member 5(PGAM5)and YT521-B homology N6-methyladenosine RNA binding protein 2(YTHDF2)with ventricular remodeling and prognosis in patients with heart failure with preserved e-jection fraction(HFpEF).Methods A total of 175 HFpEF patients(HFpEF group)were selected as subjects,and 90 healthy volunteers undergoing physical examinations were chosen as the control group.The HFpEF patients were divided into adverse prognosis group(n=61)and favorable progno-sis group(n=114)based on their prognosis status.Serum PGAM5 and YTHDF2 levels were meas-ured using enzyme-linked immunosorbent assay.Ventricular remodeling indicators[left ventricular posterior wall thickness(LVPWT),left ventricular end-diastolic diameter(LVEDD),interventricular septal thickness at end-diastole(IVST)and left ventricular mass index(LVMI)]were assessed by transthoracic echocardiography.Pearson correlationanalysis was used to analyze the relationship be-tween serum PGAM5 and YTHDF2 levels and ventricular remodeling indicators.The relationships of serum PGAM5 and YTHDF2 levels with the prognosis of HFpEF patients,as well as predictive effi-cacy,were evaluated using multivariate unconditional logistic regression analysis and receiver oper-ating characteristic curve analysis,respectively.Results Levels of LVPWT,LVEDD,IVST,LVMI and YTHDF2 in the HFpEF group were significantly higher than those in the control group,while PGAM5 levels were significantly lower(P<0.05).Serum PGAM5 levels in HFpEF patients showed negative correlations with LVPWT,LVEDD,IVST and LVMI,whereas YTHDF2 levels showed positive correlation(P<0.05).The incidence of adverse prognosis among 175 HFpEF pa-tients was 34.86%(61/175).Significant differences were observed in age,New York Heart Asso-ciation(NYHA)heart function classification,N-terminal pro-B-type natriuretic peptide(NT-proBNP),LVPWT,LVEDD,IVST,LVMI,PGAM5 and YTHDF2 between the adverse prognosis group and favorable prognosis group.High levels of PGAM5 and YTHDF2 were independent protective and risk factors for adverse prognosis in HFpEF patients(P<0.05).The area under the curve for predic-ting adverse prognosis in HFpEF patients using combined serum PGAM5 and YTHDF2 levels was 0.884,which was greater than the values obtained from individual predictions of 0.790 and 0.791(Z=3.722,3.373;P<0.001).Conclusion Decreased serum PGAM5 levels and increased YTHDF2 levels are associated with worsening ventricular remodeling and poor prognosis in HFpEF patients.Combined measurement of serum PGAM5 and YTHDF2 levels shows high predictive effica-cy for the prognosis of HFpEF patients.
2.Clinical study of intracranial hypotension targeted body posture combined with pharmacotherapy in the treatment of chronic subdural hematoma

Objective:To compare the efficacy of body posture combined with pharmacotherapy and pharmacotherapy alone in the treatment of chronic subdural hematoma(CSDH).Methods:Firstly, retrospective case series study was conducted. Thirty cases of CSDH that had received body posture combined with pharmacotherapy at Department of Neurosurgery, Huashan Hospital Affiliated to Fudan University from December 2016 to October 2020 were studied retrospectively. Twenty-seven patients were male, and 3 patients were female. The age of patients ( M(IQR)) was 66(16) years (range:28 to 84). Nineteen patients had unilateral hematoma, and 11 patients had bilateral hematoma. All patients received pharmacotherapy and body posture therapy that was to raise their lower limbs 20 to 30 cm with leg lift pad and get abdominal compressed with customized abdominal belt in supine position. Patients were required to maintain the body posture as much as possible, with the maximum to 16 to 18 hours per day. Patients with unilateral hematoma should tilt the head to the affected side and avoid tilting it to the opposite side. For patients with bilateral hematoma, there was no need for head lateralization. Patient were treated with oral dexamethasone and atorvastatin simultaneously. The preliminary efficacy of body posture combined with pharmacotherapy was determined by hematoma improvement rate which was analyzed by Clopper-Pearson method. Then, the multi-center, prospective, randomized controlled trial had carried out in 9 medical centers from August 2020 to November 2021. The stratified block randomization method was adopted. Patients were randomized in a ratio of 1∶1 to either receive pharmacotherapy alone(the control group) or body posture combined with pharmacotherapy(the experiment group) for 3 months and followed up for 6 months. Effective treatment was defined as complete absorption of hematoma, or the hematoma volume decreased by more than 10 ml and Markwalder grading scale score had improved by more than 1 point compared to the baseline. The efficacy rate and surgery conversion rate at 3 months and recurrence at 6 months were observed. Comparison between groups was performed with paired sample t test, Mann-Whitney U test, χ2 test, corrected χ2 test, or Fisher exact probability method. Logistic regression was used to compare the effective rate and operation rate between the two groups. Results:In the respective study, 30 patients completed follow-up 13 to 353 days after treatment. At the last follow-up, the incidence of almost complete absorption or significantly absorption of hematoma (hematoma volume was significantly reduced accompanied by symptom improvement) was 93.3%. The 95% CI for the incidence that analyzed by the Clopper-Pearson method was 77.9% to 99.2%. One hundred and six patients were enrolled in the multicenter study. Fifty-five patients underwent body posture combined with pharmacotherapy. The age was 74(17) years (range:26 to 92). Thirty-nine patients were males and 16 were females. Fifty-one patients underwent pharmacotherapy alone. The age was 69(12) years (range:48 to 84). Thirty-seven patients were males and 14 were females. The length of body posture recorded in diary card was (15.7±2.3) hours(range:7.6 to 19.3 hours). The efficacy rate in the body posture combined with pharmacotherapy group and pharmacotherapy alone group were 83.6% (46/55) and 56.9% (29/51), respectively at 3 months. The result of the logistic regression analysis showed that the efficacy of body posture combined with pharmacotherapy group was better than that of pharmacotherapy alone group ( OR=3.88,95% CI:1.57 to 9.58, P=0.003). Surgery rate in the body posture combined with pharmacotherapy group and pharmacotherapy alone group were 5.5% (3/55) and 21.6% (11/51) respectively. The result of Logistic regression showed that the pharmacotherapy alone group was more likely to be converted to surgery ( OR=0.21,95% CI:0.05 to 0.80, P=0.023). At the 6 months, no recurrence of cases was found in the body posture combined with pharmacotherapy group. However, the recurrence rate of pharmacotherapy alone group was 6.3% (3/48), there was no significant difference between the two groups ( P>0.05). Conclusion:The effect of body posture combined with pharmacotherapy for chronic subdural hematoma is better than that of pharmacotherapy alone.

Objective To investigate the impacts of ultrasound-guided stellate ganglion block(SGB)on hemodynamics and postoperative cognitive function in patients undergoing shoulder arthroscopic surgery.Methods From January 2024 to June 2024,120 patients undergoing shoulder arthroscopic surgery in our hospital were randomly assigned into general anesthesia group(n=60,implementing general anesthesia)and assisted SGB group(n=60,implementing ultrasound-guided SGB combined with general anesthesia).The intraoperative hemodynamics,postoperative stress status[serum cortisol(Cor)and interleukin-6(IL-6)],postoperative pain level[evaluated by visual analogue scale(VAS)score],postoperative biomarkers[serum matrix metalloproteinase-9(MMP-9)and neurospecific protein S-100β(S-100β)],and postoperative cognitive function[evaluated using the mini-mental state examination(MMSE)]were compared between the two groups.Results There was no statistically significant difference in intraoperative blood loss and surgical time between the two groups of patients(P>0.05).After induction of anesthesia(T1),the mean arterial pressure(MAP)and heart rate(HR)of the two groups of patients were significantly lower than when they entered the operating room(T0),the differences were statistically significant(P<0.05).The MAP and HR during the beginning of the surgery(T2),30 min after the start of the surgery(T3),and at the end of the surgery(T4)were higher than those at T0,the differences were statistically significant(P<0.05).While the MAP and HR in the assisted SGB group during T1,T2,T3 and T4 time points were lower than those in the general anesthesia group,the differences were statistically significant(P<0.05).The VAS scores of the assisted SGB group were significantly lower than those of the general anesthesia group at 12 and 24 h after surgery,and the differences were statistically significant(P<0.05).The levels of serum Cor and IL-6 in the two groups at 12 and 24 h after surgery were higher than those at 1 d before surgery,but the levels of serum Cor and IL-6 in the assisted SGB group were lower than those in the general anesthesia group,the differences were statistically significant(P<0.05).The levels of serum MMP-9 and S-100β in the two groups at 24 and 72 h after surgery were higher than those at 1 d before surgery(P<0.05),but the levels of serum MMP-9 and S-100β in the assisted SGB group were lower than those in the general anesthesia group,the differences were statistically significant(P<0.05).The MMSE score of the two groups at 3 and 5 d after surgery were lower than those at 1 d before surgery,but the MMSE score of the assisted SGB group was higher than that of the general anesthesia group,the difference was statistically significant(P<0.05).Conclusion The implementation of ultrasound-guided SGB during shoulder arthroscopic surgery can maintain intraoperative hemodynamic stability,obviously alleviate postoperative stress and pain,obviously reduce serum MMP-9 and serum S-100β levels,and help alleviate postoperative cognitive dysfunction.It is worthy clinical application.

6.Study on efficacy and safety of sequential rivaroxaban use in reducing blood loss after applying tranexamic acid in total hip arthroplasty

Objective To research the efficacy and safety of sequential rivaroxaban use in reducing blood loss after applying tranexamic acid(TXA)in total hip arthroplasty(THA).Methods According to the design by the random control principle,150 pa tients undergoing unilateral primary THA from September 2012 to June 2015 were selected and randomly divided into the group A,B,C,D and E (n=30).The group A did not use TXA,the group B received intravenous drip of 10 mg/kg TXA at 10 min before skin incision,the group Creceived intravenous drip of 15 mg/kg TXA at 10 min before skin incision,the group D respectively received intravenous drip of 15mL/kg TXA at 10 min before skin incision and after 3 h,the group E received intravenous drip of 15 mL/kg TXA at 10 min before skin incision and articular cavity use of 1 g TXA before closing the incision.Oral 10 mg rivaroxaban was given at postoperative 6-12 h when the drainage volume was less than 30 mL/h and then the conventional dose was used until postoperative 35 d.The intraoperative blood loss,postoperative drainage volume,hidden blood loss,blood transfusion rate,postoperative anticoagulation time,time of removing drainage tube,postoperative prothrombin time on postoperative 1 d,activated partial thromboplastin time,descend value of hemoglobin,and occurrence rates of postoperative deep vein thrombosis (DVT) and pulmonary embolism (PE) were observed in the group A,B,C,D and E.Results The intraoperative blood loss,postoperative drainage volume,hidden blood loss,blood transfusion rate and descend value of hemoglobin on postoperative 1 d had statistical differences among 5 groups(P<0.05).The are significant differences between the group D and A in the intraoperative blood loss,postoperative drainage volume,hidden blood loss,blood transfusion rate,descend value of hemoglobin on postoperative 1 d,postoperative anticoagulation time and removal drainage tube time(P<0.05).All cases had no symptomatic DVT and PE during the perioperative period and postoperative 3-month outpatient or telephone follow-up.Conclusion Sequential rivaroxaban use after applying TXA during THA perioperative period is safe and effective.Moreover intravenous drip of 15 mL/kg TXA at 10 min before skin incision and after 3 h has most significant effect in reducing bleeding volume during THA perioperative period.
7.Effects of Hedan tablet on cytokins and oxidative stress factors in APOE-/-mouse

Objective To observe the effects of Hedan tablet on cytokines and oxidation factors in APOE-/-mouse, and to explore its effect on atherosclerosis and to explore its behind mechanism. Methods APOE-/-mice (n=50) were randomly divided into control group, model group, low dose Hedan tablet treatment group, high dose Hedan tablet treatment group and simvastatin treatment group. Mice in control group were given normal feed while mice in other groups were fed with high cho?lesterol diet. Hedan or Simvastatin was administrated intra-gastrically while normal saline was given to model group in the same route. After 12 weeks, mice were sacrificed to observe the mRNA level of tumor necrosis factor-α(TNF-αmRNA) in aorta by RT-PCR. Mean while, serum levels of interleukin-1 (IL-1), interleukin-10 (IL-10), malonaldehyde (MDA) and su?peroxide dismutase (SOD) were determined in different groups. Results Compared with control group, TNF-αmRNA tran?scription level as well as serum levels of IL-1 and MDA significantly increase while serum levels of IL-10 and SOD de?creased remarkably in model group, (P<0.01). Compared with model group, mRNA levels of TNF-αas well as serum levels of IL-1 and MDA were significantly decreased while serum levels of IL-10, SOD were greatly increased in low dose and high dose Hedan tablet treatment groups as well as in simvastatin treatment group (P<0.01). Conclusion Hedan tablet inhibit the formation of atherosclerosis through its anti-oxidation role and anti-inflammation role.
8.Meta-analysis of the efficacy of combined therapy vitamin D with calcium in nutritional rickets

Objective To assess the efficacy of combined vitamin D with calcium and vitamin D or cal-cium alone in rickets of vitamin D deficiency by meta-analysis method. Methods Searches were made in Cochrance Library , Pubmed, Web of science, Scirus, CNKI, Chinese Biological Medical Literature Database, (CBM),Wangfang from the establishment of the data base till March 2013. All randomized controlled trials about combined with vitamin D and calcium in rickets of vitamin D deficiency were eligible. Serum 25-( OH) ritamin D,phosphate ,ALP,calcium,PTH,phosphate,albumin ,radiographic score were chosen as evaluation in-dex to evaluate the weighted mean diffreence ( WMD) and 95% confidence interval ( CI) for continuous data. RevMan 5. 0. 2 software was used to make meta-analysis. Results 437 literatures were reviewed. Three eligible trials were used for meta-analysis. Meta-analysis showed that:(1)the increase of Serum 25-(OH)vitamin D:Combined therapy group and vitamin D group (MD= -7. 88,95%CI:-12. 24~ -3. 52);Combined therapy group and calcium group (MD= -18. 32,95%CI:-22. 61~ -14. 04). (2)the increase of serum phosphate:Combined therapy group and vitamin D group ( MD= -0. 64 ( 95%CI:-0. 86 ~ -0. 42 );Combined therapy group and calcium group MD= -0. 16 ( 95%CI:-0. 84 ~0. 51 ) . ( 3 ) the decrease of Serum ALP:Combined therapy group and vitamin D group (MD= 109. 99,95%CI:20. 40 ~199. 58);Combined therapy group and calcium group (MD=59. 89,95%CI:10. 09~109. 59 ). (4)the increase of serum calcium :Combined therapy group and vitamin D group (MD= -0. 71,95%CI:-0. 91,-0. 52);the decrease of Radiographic score:Com-bined therapy group and vitamin D group( MD=0. 68,95%CI:0. 42~ 0. 95). Except that the increase of serum phosphate between combined therapy group and calcium group had no significant difference,the rest had signifi-cant difference. Conclusion The long term efficacy in combined therapy group is much effective than vitamin D group or calcium group.
9.Study of focal cerebral ischemia on cell proliferation and expression of glial fibrillary acidic protein in caveolin-1 knockout mice

Objective To investigate focal cerebral ischemia on cell proliferation and expression of glial fibrillary acidic protein(GFAP) in caveolin-1 knockout mice.Methods Focal cerebral ischemia was induced by middle cerebral artery occlusion(MCAO) to caveolin-1-knockout mice and homologous C57 wild-mice.Expression of BrdU and GFAP in the brain were detected using immunohistochemistry in the postoperative 7d.Results The number of BrdU( (29.04 ± 1.68 )/mm2 ; (24.13 ±- 1.57 )/mm2 ) and GFAP-positive cells ( ( 10.75 ± 2.32 )/mm2 ; ( 18.14 ± 1.95 )/mm2 ) were increased in wild-type and caveolin-1 -knockout mice after ischemia,and there was significant difference compared with sham group (BRDU( 6.15 ± 1.09 )/mm2,(6.23 ± 1.05 )/mm2 ; GFAP (2.31 ±0.98)/mm2,(2.07 ± 1.01 )/mm2 ) (P<0.05).Expression of BMU in knockout mice was less than that in wild mice,but expression GFAP in knockout mice was stronger than that in wild mice,and the difference was statistically significant (P < 0.05 ).Conclusion Cerebral ischemia can promote cell proliferation and glial activation in brain,loss of caveolin-1 reduced cell proliferation and enhanced glial activation early stage of brain ischemia.
10.Short term therapeutic effect on treatment of postoperational large intestine carcinoma by Fupiyiwei decoction combined with chemotherapy and it's effect on immune function.

OBJECTIVETo investigate short term clinical therapeutic effect of fupiyiwei decoction (FPYWD) combined with chemotherapy in treating postoperational large intestine carcinoma and the effect on immune function.
METHODThe 58 cases of postoperational large intestine carcinoma were randomly divided into the traditional Chinese medicine (TCM) group (33 cases) and the control group (25 cases). The toxic and adverse reaction, Karnofsky score and clinical therapeutic effect during chemotherapy were observed, and the level of subgroup of T cells CD3, CD4, CD8 and NK cells were detected. Before and after treatment 28 healthy adult were regarded as normal group.
RESULTThere was no significant difference in clinic therapeutic effect between the TCM group and the control group after 12 chemotherapy treatment courses. But the quality of life the TCM group remarkably better than in the control group (P < 0.05), while the incidence rate of toxic and adverse reaction of chemotherapy in the former group was lower than in the latter (P < 0.05). The level of CD3+, CD4+ T cell and the viability of NK cells in all patients decreased during the chemotherapy treatment course while the level of CD8+ T cell increased, the level of CD3+ T cell and NK cells in peripheral blood in patients increased after treatment, but was lower than in normal group. The level of CD8+ T cell was decreased and was lower than in normal group. There was no significant difference (P < 0.05). The level of CD4 T cell after TCM group, there was significant different in CD4+ T cell between TCM group and control group, normal group (P < 0.05). The viability of NK cell increased but there was no significant difference between the two group.
CONCLUSIONFupiyiwei decoction (FPYWD) is effective in prevention and treatment of the toxic and adverse effects of chemotherapy of postoperational large intestine carcinoma, and can improve quality of life and immune function of patients. increase the effective of chemotherapy.
