1.Risk factors for postoperative premature epiphyseal closure in pediatric patients with distal tibial epiphyseal fractures and their predictive efficacy

Objective:To investigate the risk factors and their predictive efficacy for postoperative premature epiphyseal closure in pediatric patients with distal tibial epiphyseal fractures.Methods:A retrospective cohort study was conducted to analyze the clinical data of 216 pediatric patients with distal tibial epiphyseal fractures admitted to Zhengzhou Orthopedics Hospital from January 2018 to December 2023, including 136 males and 80 females, aged 2-16 years [11.8(9.8, 13.3)years]. Among them, 112 patients were affected on the left side and 104 on the right. According to the Salter-Harris fracture classification, the fracture was classified as type I in 14 patients, type II in 97, type III in 38, type IV in 64 and type VI in 3. According to the presence of premature epiphyseal closure after surgery, the patients were divided into premature epiphyseal closure group ( n=38) and normal epiphyseal group ( n=178). Age, gender, body mass index (BMI), injury mechanism, side of injury, Salter-Harris classification of fracture, initial displacement distance of the fracture end, medial malleolar involvement by the fracture line surgical fixation method, operation duration, reduction method, and reduction quality were recorded in the two groups. Univariate analysis and binary Logistic regression analysis were used to evaluate and determine the independent risk factors for postoperative premature physeal closure in pediatric patients with distal tibial epiphyseal fracture. The receiver operating characteristic (ROC) curve was plotted and area under the curve (AUC) was calculated to evaluate the predictive efficacy of each risk factor for the occurrence of premature physeal closure in pediatric patients with distal tibial epiphyseal fractures. Results:Univariate analysis showed the occurrence of postoperative premature epiphyseal closure of the distal tibia was associated with age, Salter-Harris fracture classification, medial malleolar involvement by the fracture line, surgical fixation method, reduction method, and reduction quality ( P<0.05), while it was not correlated with gender, BMI, injury mechanism, side of injury, initial displacement distance of the fracture end, and operation duration ( P>0.05). Multivariate binary Logistic regression analysis showed that medial malleolar involvement by the fracture line ( OR=0.18, 95% CI 0.04, 0.76, P<0.05) and reduction quality ( OR=43.18,95% CI 10.71, 174.05, P<0.01) were significantly correlated with the occurrence of postoperative premature epiphyseal closure of the distal tibia. The results of ROC curve analysis showed that medial malleolar involvement by the fracture line had limited predictive efficiency (AUC=0.53, 95% CI 0.43, 0.63), reduction quality had moderate predictive efficacy (AUC=0.81, 95% CI 0.72, 0.91), while their combination demonstrated even higher predictive efficacy (AUC=0.83, 95% CI 0.74, 0.91). Conclusions:Medial malleolar involvement by the fracture line and reduction quality are independent risk factors for postoperative premature epiphyseal closure in pediatric patients with distal tibial epiphyseal fractures. Reduction quality demonstrates good predictive efficacy, while medial malleolar involvement by the fracture line shows limited predictive value. The combination of both factors achieves even better predictive performance.
2.Mechanism of tetrahydrocurcumin in improving kidney injury in mice with type 2 diabetes mellitus based on transcriptomics

Objective To investigate how dietary supplementation with tetrahydrocurcumin(THC)improves kidney injury in type 2 dia-betic mellitus(T2DM)and its mechanism of action using transcriptome sequencing(RNA-seq).Methods C57BL/6J mice were randomly assigned to the control,T2DM,and T2DM+THC groups.After a high-fat meal and streptozotocin injection,the body weights and fasting blood glucose levels of each mouse with T2DM were measured.Hematoxylin and eosin staining,Oil red O staining,and RNA-seq were performed to examine kidney pathology,lipid deposition,and differentially expressed genes,respectively,in the mice.Results Mice in T2DM group exhibited significantly higher fasting blood glucose levels(P<0.001),renal tubule degeneration,glomeruli enlargement,disordered epithelial cells,and increased kidney lipid deposition after 12 weeks compared with those of the control group.THC adminis-tration alleviated all these conditions(P<0.001).RNA-seq analysis revealed significant gene expression variations among the control,T2DM,and T2DM+THC groups.THC may protect against T2DM-induced kidney injury and lipid deposition by regulating the cell cycle(apoptosis),P53 signaling pathway,and PPARγ signaling path way,as indicated by Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analyses.In mice with T2DM,THC intervention may upregulate the expression of Cd36,Lpl,PPARγ,and Plin4 genes in renal tissues,while downregulating Ccnb1,Ccnb2,Cdk1,Bub1,and Cdc25c gene expressions.The proteins encoded by the four upregulated genes interact,as do those encoded by the five downregulated genes.Conclusion THC administration improves fasting blood glucose levels,reduces renal damage,and decreases fat deposition in mice with T2DM.The processes may involve decreasing apoptosis,blocking the P53 signaling pathway,and activating the PPARγ signaling pathway.
3.Association between cancer-related fatigue and PD-1 inhibitors in patients with malignant melanoma and its influencing factors

Objective:To explore the relationship between programmed death-1(PD-1)inhibitors and cancer-related fatigue(CRF)in patients with malignant melanoma,and to identify associated influencing factors.Methods:A total of 100 patients with malignant melanoma treated at Jinan People's Hospital between April 2019 and April 2024 were included as study subjects.The Chinese version of the Piper Fatigue Scale was used to evaluate patients'fatigue levels within three months before and after the initiation of PD-1 inhibitor therapy.Results:There was a significant difference in CRF score before and after PD-1 inhibitor treatment(P<0.001).Univariate analysis showed no significant association between fatigue severity and factors such as gender,smoking history,tumor site,or PD-1 inhibitor type(all P>0.05).However,age,tumor stage,anemia,leukopenia,secondary hypothyroidism,secondary adrenal insufficiency(AI),and secondary adrenocorticotropic hormone deficiency(P<0.05 or P<0.01 or P<0.001)were significantly associated with CRF.Multivariate regression analysis identified secondary hypothyroidism,secondary AI,anemia,and leukopenia as independent risk factors for severe CRF in patients with malignant melanoma(all P<0.05).Conclusion:Adverse reactions of PD-1 inhibitors,including secondary hypothyroidism,secondary AI,anemia,and leukopenia,are independent risk factors for CRF in patients with malignant melanoma.
4.Treatment of femoral head chondroblastoma in children and adolescents by curettage and bone grafting via the fovea of femoral head through the surgical hip dislocation approach

Objective:To investigate the clinical effectiveness of tumor lesion curettage and allogeneic bone grafting via the fovea of femoral head through the surgical hip dislocation approach in children and adolescents with femoral head chondroblastoma (FHCB).Methods:Case series study.The clinical data of 7 FHCB patients [3 males and 4 females with a mean age of 13.7 years (11-19 years)] treated at the Department of Pediatric Orthopedics, Zhengzhou Orthopedic Hospital from January 2016 to March 2023 were retrospectively analyzed.All patients underwent lesion curettage and allogeneic bone grafting via the fovea of femoral head through the surgical hip dislocation approach.The healing, local recurrence and complications were assessed via clinical and radiological tests.Functional outcomes were evaluated using the Musculoskeletal Tumour Society (MSTS) scoring system.A paired t-test was used to compare the MSTS scores before the operation and at the final follow-up. Results:All patients were followed up for 30 months on average (18-48 months).The average healing time for bone grafting was 4.9 (3-8) months.Local recurrence and serious postoperative complications such as avascular necrosis of femoral head, secondary osteoarthritis, ectopic ossification and nonunion of greater trochanter osteotomy end were not observed in all patients up to the last follow-up.The average MSTS score at the last follow-up was (29.14±0.45) points.Conclusions:Curettage and bone grafting via the fovea of femoral head through the surgical hip dislocation approach is effective and safe in the treatment of FHCB in children and adolescents.This method can maintain the integrity of the articular surface and growth plate, and also preserve the blood supply of the femoral head and the normal function of the hip joint, with satisfactory short-term effects.

8.Analysis of Life Expectancy and Potential Years of Life Lost of Residents in Tianzhu Tibetan Autonomous County of Wuwei in 2021

Objective To understand the composition of death causes of residents in Tianzhu Tibetan Autonomous County,Wuwei City,and to analyze the current situation of life expectancy and life loss of residents,so as to provide evidence for disease control and health promotion.Methods According to the international classification of diseases(ICD-10),the population data and death cause monitoring data of Tianzhu Tibetan Autonomous County in 2021 were analyzed,standardized with the data of the seventh national census,and analyzed with SPSS 21.0 software and life expectancy statistics.Results The crude mortality rate of residents in Tianzhu Tibetan Autonomous County was 642.25/100 000,The male crude mortality(727.12/100 000)rate was higher than the female(553.29/100 000).The overall life expectancy was 79.07 years.The life expectancy in males was 76.77 years and the life expectancy in females was 81.47 years.The first five causes of death were circulatory system(mortality 270.14 per 100,000),nervous system(mortality 159.57 per 100,000),malignant tumor(mortality 101.97 per 100,000),respiratory system(mortality 27.81 per 100,000)and gastrointestinal disease disease(mortality 26.48 per 100,000).The first five places of AYLL were injury and poisoning(24.81 years/person),spirit and blood(15.42 Years/person),infectious diseases(13.06 years/person),malignant tumor(11.78 Years/person)and urogenital diseases(9.94 years/person).Conclusion The highest mortality rate in the 2021 Bairi Tibetan Autonomous County was circulatory system disease,and the major causes of premature death were injury and poisoning.
9.The protective effect and mechanism of Taraxasterol on Erastin induced ferroptosis in chondrocytes

Objective To investigate the role of Taraxasterol(TAR)on ferroptosis in chondrocytes induced by Erastin.Methods The C28/I2 chondrocyte line was treated with Erastin to construct the ferroptosis model of chon-drocytes in vitro and the experiments were divided into Control,Erastin,TAR,and TAR+Erastin groups.Cell via-bility was detected by the CCK-8 assay.Cytotoxicity was detected by the lactate dehydrogenase(LDH)kit and the Calcein/PI cytokinesis kit.Flow cytometry was used to detect lipid reactive oxygen species(ROS).The intracellular glutathione(GSH)content was detected by GSH kit.Mitochondrial membrane potential was detected by JC-1 stai-ning and RH123 staining.ACSL4 and GPX4 protein expression and the key indicators of ferroptosis were detected by Western blot.Results TAR restored the decreased cell viability of C28/I2 chondrocytes induced by Erastin treatment as well as reduced Erastin-induced cytotoxicity(P<0.01).Compared with the control group,the level of intracellular lipid ROS increased(P<0.01)and the content of GSH decreased(P<0.01)after treatment with Erastin,while TAR could reduce the production of lipid ROS(P<0.01)and increase the content of GSH(P<0.01).TAR restored mitochondrial membrane potential in C28/I2 chondrocytes ferroptosis,decreased ACSL4 pro-tein expression(P<0.01)and increased GPX4 protein expression(P<0.01).In addition,TAR restored the re-duced cell viability caused by IL-1 β treatment.Conclusion TAR can inhibit Erastin induced ferroptosis in C28/I2 chondrocytes,which may be related to the regulation of ACSL4 and GPX4 protein expression.
10.Genetic modification of acetogens and optimization of fermentation process in C1-gas bioconversion.

The current linear economy model relies on fossil energy and increases CO2 emissions, which contributes to global warming and environmental pollution. Therefore, there is an urgent need to develop and deploy technologies for carbon capture and utilization to establish a circular economy. The use of acetogens for C1-gas (CO and CO2) conversion is a promising technology due to high metabolic flexibility, product selectivity, and diversity of the products including chemicals and fuels. This review focuses on the physiological and metabolic mechanisms, genetic and metabolic engineering modifications, fermentation process optimization, and carbon atom economy in the process of C1-gas conversion by acetogens, with the aim to facilitate the industrial scale-up and carbon negative production through acetogen gas fermentation.
