1.Effectiveness of arthroscopic release assisted with medial small incision in treatment of non-traumatic elbow stiffness.

OBJECTIVE:
To explore the effectiveness of arthroscopic release of elbow joint assisted by medial small incision ulnar nerve release in the treatment of non-traumatic elbow stiffness.
METHODS:
The clinical data of 15 patients with non-traumatic elbow stiffness treated with arthroscopic release of elbow joint assisted by medial small incision ulnar nerve release between April 2019 and September 2023 were retrospectively analyzed. There were 6 males and 9 females with an average age of 46 years ranging from 34 to 56 years. The causes included rheumatoid arthritis in 3 cases, gouty arthritis in 2 cases, loose bodies in 3 cases, and elbow osteoarthritis in 7 cases. There were 4 cases with ulnar neuritis and 3 cases with synovial osteochondromatosis. The duration of elbow stiffness ranged from 6 to 18 months, with an average of 10 months. The operation time and intraoperative blood loss were recorded. The effectiveness was evaluated by visual analogue scale (VAS) score, range of elbow motion (maximum flexion, maximum extension, and total flexion and extension), Mayo score, and Hospital for Special Surgery (HSS) elbow score.
RESULTS:
The operation time was 60-90 minutes, with an average of 65 minutes, and the intraoperative blood loss was 40-100 mL, with an average of 62 mL. All patients were followed up 13-18 months, with an average of 14 months. There was no complication such as vascular and nerve injury, poor wound healing, collateral ligament injury, elbow joint space narrowing, osteophyte proliferation, or loose body formation around the joint. At last follow-up, the elbow range of motion (maximum flexion, maximum extension, and total flexion and extension), VAS score, and Mayo score significantly improved when compared with those before operation ( P<0.05). The HSS elbow score was 85-95, with an average of 92; 12 cases were excellent, 3 cases were good, and the excellent and good rate was 100%.
CONCLUSION
Arthroscopic release of elbow joint assisted by medial small incision ulnar nerve release is an effective way to treat non-traumatic elbow stiffness, which has the advantages of small trauma, short operation time, and good effectiveness. It can carry out early elbow rehabilitation training and significantly improve elbow function.

2.Clinical study of sphincter-preserving surgery combined with cutting seton and loose seton in the treatment of ischiorectal fossa abscess

Objective To evaluate the clinical efficacy of a sphincter-preserving technique combining cutting seton and loose seton drainage for the treatment of ischiorectal abscess.Methods A prospective ran-domized controlled trial was conducted involving 184 patients with ischiorectal abscess,who were randomly di-vided into an experimental group(n=92)and a control group(n=92).The experimental group underwent sphincter-preserving surgery combining cutting seton and loose seton drainage,while the control group re-ceived single-stage incision and seton drainage.Clinical outcomes,anal function,operative time,postoperative pain intensity,wound healing time,and pruritus ani were compared between the two groups.Results The sur-gical time of the experimental group was longer than that of the control group[31.50(25.00,40.00)min vs.20.00(20.00,30.00)min],and the difference was statistically significant(P<0.05).On postoperative days 1,3,and 7,the NRS scores for pain were lower than those in the control group,and the wound healing time was shorter than that in the control group[24.00(20.00,25.75)days vs.29.00(26.00,32.00)days],with statistically significant differences(P<0.05).The recent cure rate of the experimental group was lower than that of the control group(88.04%vs.94.57%),and the difference was not statistically significant(P>0.05).After follow-up,the long-term cure rate of the experimental group was lower than that of the control group(84.78%vs.93.48%),and the Wexner score for anal incontinence was lower than that of the control group[1.00(0.00,1.00)vs.1.00(0.00,2.00)],with statistically significant differences(P<0.05).Conclu-sion The sphincter-preserving technique combining cutting seton and loose seton drainage for ischiorectal ab-scess reduces postoperative pain,shortens wound healing time,and effectively protects anal function with reli-able short-term efficacy.However,further improvements are needed to enhance long-term clinical outcomes.

Recent studies have shown that the physiological homeostasis of oral mucosal cells is regulated by the circadian clock.Dis-ruption or dysfunction of the circadian clock is closely associated with the development of oral squamous cell carcinoma(OSCC).Research based on the circadian clock offers a novel perspective on the pathogenesis and therapeutic strategies for OSCC.However,there is current-ly limited research on this topic,and people generally have insufficient understanding and recognition of the circadian clock.Given the complexity and challenges of circadian clock which is the fourth dimension of medical research,we organize relevant experts based on summarizing the current research results of circadian clock in the pathogenesis and precision diagnosis and treatment of OSCC,combining the scientific principles of the circadian clock's role and their long-term research experience,then summarizes and recommends the con-sensus opinions for the research of circadian clock in the pathogenesis mechanism and precision diagnosis and treatment of human OSCC,with the hope of providing guidance for the basic research and clinical application of circadian clock or circadian rhythm in the pathogene-sis mechanism and precision diagnosis and treatment of oral and maxillofacial squamous cell carcinoma.

5.Study on the effect and safety of ciprofol combined with local anesthesia on elderly patients undergoing hemorrhoidectomy

OBJECTIVE To evaluate the effect and safety of ciprofol combined with local anesthesia on elderly patients undergoing hemorrhoidectomy.METHODS A total of 108 elderly patients who underwent hemorrhoidectomy at the Department of Proctology,Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine from February 2023 to June 2024,were included.The patients were randomly divided into the etomidate group(54 cases)and the ciprofol group(54 cases)based on a computer-generated random sequence.One patient with intraoperative bleeding and one who refused postoperative examination were excluded,resulting in a final total of 106 patients completing this trial(52 in the etomidate group and 54 in the ciprofol group).The two groups of patients underwent hemorrhoidectomy under local anesthesia with sedative assistance,receiving either etomidate at 0.2 mg/kg or ciprofol at 0.3 mg/kg combined with sufentanil at 0.1 μg/kg.The sedation success rate,induction time,recovery time,operation time and hemodynamic parameters[heart rate(HR),mean arterial pressure(MAP),pulse oxygen saturation(SpO2)and respiratory rate(RR)]were observed at baseline(T0),after successful induction of anesthesia(T1),10 minutes after the start of surgery(T2),and awakening(T3).Additionally,the levels of oxidative stress markers[malondialdehyde(MDA)and superoxide dismutase(SOD)]were measured at T0,T2 and T3.The occurrence of adverse reactions was also recorded.RESULTS The induction time of the ciprofol group was significantly shorter than that of the etomidate group(P<0.05),while the recovery time was significantly longer in the ciprofol group compared to the etomidate group(P<0.05).At T1 and T2,HR,MAP and RR of two groups were significantly lower compared to the same group at T0(P<0.05).However,no significant differences were observed in the hemodynamic parameters at each time point between the two groups(P>0.05).At T2 and T3,the MDA levels in both groups were significantly higher than at T0,while the SOD levels were significantly lower than the same group at T0;the ciprofol group showed significantly better outcomes than the etomidate group at the same time points(P<0.05).The proportion of patients with intraoperative somatic movements was significantly lower in the ciprofol group compared to the etomidate group(P<0.05).CONCLUSIONS When ciprofol is used in elderly patients undergoing hemorrhoidectomy,it demonstrates certain advantages over etomidate in maintaining hemodynamic stability and reducing oxidative stress.Additionally,ciprofol has higher safety.

7.Role of UBC9-mediated SUMO modification in homocysteine-induced pyroptosis of macrophages

Objective To study the role of ubiquitin-conjugating enzyme 9(UBC9)in the pyroptosis of homocysteine-induced macrophages mediated by small ubiquitin-like modifier(SUMO)modification.Methods First,the effects of homocysteine at different concentrations(0 μmol/L,50 μ.mol/L,100 μmol/L,150 μmol/L and 200 μmol/L)on the viability and pyrodeath of mouse macrophages(RAW264.7)were detected by CCK-8 and Western blot.Western blot was used to detect the expression levels of UBC9,SUMO-1,and the inflammatory cytokine IL-1β in different groups of cells.qRT-PCR was used to detect the mRNA expression of UBC9 before and after RNA interference and the expression of UBC9,pyrogen-related protein,and SUMO-1 after RNA interference.Results After stimulation with 100 μmol/L homocysteine,the effect of macrophage activity was minimal,and NLRP3 and Caspase-1 were the proteins with the most obvious increase in expression(P<0.05).Compared with the Control group,the Hcy group's expression of IL-1β and SUMO-1 was increased(P<0.01).Compared with the Control group,the Hcy group's UBC9 protein and mRNA levels were increased(P<0.05).The expression of NLRP3,Caspase-1,IL-1β,UBC9,and SUMO-1 was decreased in the si-UBC9+Hcy group compared with the si-NC+Hcy group(P<0.01).Conclusions Homocysteine induces pyroptosis in macrophages,and its mechanism of action is related to the up-regulation of UBC9 to induce SUMO modification.
8.Protective effect of human amniotic mesenchymal stem cells on pulmonary microvascular endothelial cell injury induced by pine sawdust smoke solution in rats

Objective To investigate the protective effect of human amniotic mesenchymal stem cells(hAMSCs)on pulmonary microvascular endothelial cell(PMVECs)through observing the effects of prolifer-ation,apoptosis and inflammatory response after PMVECs injury induced by pine sawdust smoke solution.Methods HAMSCs and rat PMVECs were isolated and cultured.The flow cytometry and immunofluores-cence were used to identify hAMSCs and PMVECs respectively.The experimental grouping:control group(normal cultured PMVECs),smoke group(PMVECs injury induced by pine sawdust smoke solution),smoke+hAMSCs group(after PMVECs was injured by smoke solution,hAMSCs and PMVECs were co-cul-tured in Transwell culture system).The proliferative activity of PMVECs after co-culture for 12,24 h was measured by cell counting kit-8(CCK-8),the apoptosis of PMVECs was measured by flow cytometry,and the ex-pression levels of TNF-α and IL-6 were detected by enzyme-linked immunosorbent assay(ELISA).Results hAMSCs and PMVECs were successfully isolated and cultured,and the hAMSCs surface markers CD105(95.4%),CD73(99.8%)and CD90(99.8%)were identified as strongly positive expression,while CD34,CD45,CD14,CD19 and HLA-DR were weakly expressed(1.96%in total).The vascular endothelial cell mark-er CD34 in PMVECs was positively expressed,moreover its combination with aggulutinin BSI was also posi-tive.At the observation time point of 12,24 h co-culture,compared with the Control group,the proliferation activity of PMVECs in the Smoke group was inhibited(P<0.05),and the cellular apoptosis was increased(P<0.05),the TNF-α and IL-6 expression levels were up-regulated(P<0.05);the phenomena of PMVECs proliferation activity inhibition,apoptosis increase and inflammatory factor expression level up-regulation in the Smoke+hAMSCs group were reversed compared with the Smoke group(P<0.05).Conclusion After PMVECs are injured by smoke solution,hAMSCs could decrease the PMVECs inflammatory factors expres-sion,promote its proliferation activity and inhibit its apoptosis,thus play the protective effect on PMVECs.

10.Resveratrol and Sir2 Reverse Sleep and Memory Defects Induced by Amyloid Precursor Protein.

Resveratrol (RES), a natural polyphenolic phytochemical, has been suggested as a putative anti-aging molecule for the prevention and treatment of Alzheimer's disease (AD) by the activation of sirtuin 1 (Sirt1/Sir2). In this study, we tested the effects of RES and Sirt1/Sir2 on sleep and courtship memory in a Drosophila model by overexpression of amyloid precursor protein (APP), whose duplications and mutations cause familial AD. We found a mild but significant transcriptional increase of Drosophila Sir2 (dSir2) by RES supplementation for up to 17 days in APP flies, but not for 7 days. RES and dSir2 almost completely reversed the sleep and memory deficits in APP flies. We further demonstrated that dSir2 acts as a sleep promotor in Drosophila neurons. Interestingly, RES increased sleep in the absence of dSir2 in dSir2-null mutants, and RES further enhanced sleep when dSir2 was either overexpressed or knocked down in APP flies. Finally, we showed that Aβ aggregates in APP flies were reduced by RES and dSir2, probably via inhibiting Drosophila β-secretase (dBACE). Our data suggest that RES rescues the APP-induced behavioral deficits and Aβ burden largely, but not exclusively, via dSir2.
