By analyzing the meridian related diseases of the heart in the Lingshu · Meridians chapter, this study summarizes the characteristics of heart diseases caused by the Heart Channel of Hand-Shaoyin, Pericardium Channel of Hand-Jueyin, and other meridians from the perspectives of pathogenic meridians, lesion sites, and disease nature. The pathogenesis and connotation of these diseases are analyzed, providing new ideas for the prevention and treatment of heart related diseases. Among the twelve meridians, besides the Heart Channel of Hand-Shaoyin and Pericardium Channel of Hand-Jueyin, there are five meridians that cause heart diseases, namely the Lung Channel of Hand-Taiyin, Stomach Channel of Foot-Yangming, Spleen Channel of Foot-Taiyin, Kidney Channel of Foot-Shaoyin, and Gallbladder Channel of Foot-Shaoyang. Diseases are more common in the yin meridian, with pain and heat as the main symptoms. They are related to the fact that the heart is a yang zang-organ, and evil qi is easily converted from yang to heat, causing diseases through the transmission of heart fire. There are three mechanisms by which other meridians can affect the Heart Channel of Hand-Shaoyin and Pericardium Channel of Hand-Jueyin, leading to symptoms such as heart pain and palpitations. Firstly, pathogenic factors can be transmitted along the meridians, causing discomfort and heartache to the heart; secondly, due to the abnormal pathogenic behavior of the Five Elements, where the gallbladder wood do not generate heart fire, and the heart yang is weak and painful; kidney water does not control heart fire, and the heat disturbs the mind and causes palpitations; thirdly, qi function is abnormal, which leads to abnormal opening and closing of the Yangming and Shaoyang channels, causing palpitations due to insufficient yin qi in the pericardium meridian or pain due to yang deficiency in the heart channel. External pathogens enter the lung through the channels, causing obstacles in the generation and distribution of pectoral qi, affecting the qi function of the heart channel and causing distress. The three mechanisms can exist independently or simultaneously as diseases.

Objective To investigate the relationship between serum levels of angiotensin-converting en-zyme 2(ACE2),β2-microglobulin(β2-MG),angiopoietin like protein 3(ANGPTL3)and glucose and lipid me-tabolism in patients with primary aldosteronism(PA),and their diagnostic value for PA.Methods From May 2022 to May 2024,a total of 115 PA patients admitted to the hospital were regarded as the study group,and another 115 healthy individuals who underwent physical examinations were included as the control group.En-zyme linked immunosorbent assay(ELISA)was applied to detect the levels of ACE2,β2-MG,and ANGPTL3.Pearson correlation was applied to analyze the correlation between serum ACE2,β2-MG,ANGPTL3 levels and glucose and lipid metabolism indicators in PA patients.Multivariate Logistic regression was applied to analyze the influencing factors of PA occurrence.Receiver operating characteristic(ROC)curve was applied to analyze the diagnostic value of ACE2,β2-MG,and ANGPTL3 for PA.Results Compared with the control group,the levels of serum β2-MG,ANGPTL3,fasting plasma glucose(FPG),glycated hemoglobin(HbA1c),fasting in-sulin(FINS),triglycerides(TG),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C)in the study group were obviously increased(P＜0.05),while the levels of ACE2 and high-density lipoprotein cholesterol(HDL-C)were greatly decreased(P＜0.05).According to Pearson correlation,ACE2,β2-MG,ANGPTL3 were associated with FPG,HbA1c,FINS,TG,TC,LDL-C,and HDL-C(P＜0.05).According to multivariate Logistic analysis,β2-MG,ANGPTL3,FPG,HbA1c,FINS,TG,TC,and LDL-C were risk factors affecting the occurrence of PA(P＜0.05),while ACE2 and HDL-C were protective factors affecting the oc-currence of PA(P＜0.05).According to the ROC curve,the area under the curve(AUC)of serum ACE2,β2-MG,and ANGPTL3 levels for diagnosing PA was 0.742,0.772,and 0.806,respectively.The AUC of the com-bined diagnosis of the three for PA was 0.920,which was larger than the single detection(Z combination vs.ACE2=4.795,Zcombination vs.β2-MG=4.292,Zcombination vs.ANGPTL3=3.391,P＜0.05).Conclusion The levels of serum β2-MG and ANGPTL3 are elevated in PA patients,the level of ACE2 is greatly reduced.ACE2,β2-MG and ANGPTL3 are associated with glucose and lipid metabolism,and their combined diagnostic value for PA is relatively high.

Objective To evaluate the clinical efficacy of invisible orthodontic appliances without brackets for the distal movement of maxillary molars to improve the ability of orthodontists to predict treatment outcomes.Methods Web of Science,Cochrane Library,Embase,PubMed,Wanfang Database,CNKI Database,and VIP Database were searched for studies investigating the efficacy of invisible orthodontic appliances for distal movement of maxillary molars in adult patients and published from database inception to August 1,2023.A total of three researchers screened the studies and evaluated their quality and conducted a meta-analysis of those that met quality standards.Results This study included 13 pre-and postcontrol trials with a total sample size of 281 patients.The meta-analysis revealed no sig-nificant differences in the sagittal or vertical parameters of the jawbone after treatment when compared with those before treatment(P＞0.05).The displacement of the first molar was MD=-2.34,95％CI(-2.83,-1.85);the displacement was MD=-0.95,95％CI(-1.34,-0.56);and the inclination was MD=-2.51,95％CI(-3.56,-1.46).There was a statistically significant difference in the change in sagittal,vertical,and axial tilt of the first molar before and after treatment.After treatment,the average adduction distance of the incisors was MD=-0.82,95％CI(-1.54,-0.09),and the decrease in lip inclination was MD=-1.61,95％CI(-2.86,-0.36);these values were significantly different from those before treat-ment(P＜0.05).Conclusion Invisible orthodontic appliances can effectively move the upper molars in a distal direc-tion and control the vertical position of the molars.When the molars move further away,there is some degree of com-pression and distal tilt movement,which is beneficial for patients with high angles.The sagittal movement of incisors is beneficial for improving the patient's profile.

Objective To investigate the association of serum neurofilament light chain(NFL)and glial fibrillary acidic protein(GFAP)with Parkinson disease(PD)with anxiety and depression.Methods A total of 96 PD patients and 38 healthy controls were enrolled in this study.Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)were used to evaluate anxiety and depression in the PD patients,and the patients were divided into groups based on their diseases.Ultrasensitive single molecule array(Simoa)was used to quantitatively measure the serum levels of NFL and GFAP.The levels of NFL and GFAP were compared between groups;Spearman correlation analysis and logistic re-gression analysis were used to investigate the association of NFL and GFAP with anxiety and depression in PD patients;the receiver operating characteristic(ROC)curve was used to access the value of GFAP level in the diagnosis of PD with de-pression.Results Compared with the healthy control group,the PD patients with or without anxiety and depression had a significant increase in the level of NFL(P＜0.001),and compared with the PD patients without depression or anxiety,the PD patients with depression or anxiety had a significant increase in the level of GFAP(P＜0.05).In the PD patients,the serum level of GFAP was positively correlated with the degree of anxiety(r=0.271,P=0.008)and depression(r=0.302,P=0.003).After adjustment for confounding factors,the logistic regression analysis showed that GFAP remained an inde-pendent risk factor for depression in PD patients(P=0.028).The ROC curve analysis showed that GFAP had an area un-der the ROC curve of 0.660 in the diagnosis of PD with depression.Conclusion Serum GFAP level is associated with anxiety and depression in PD and is an independent risk factor for PD with depression.

OBJECTIVES: This study aimed to construct the finite element model of the mandibular first molar with the invisible appliance and explore the dentition movement characteristics of the mandibular first molar when using micro-implant anchorage and different initial positions of the first molar. METHODS: Models of the mandible, tooth, periodontal membrane, and invisible appliance were constructed using cone beam computed tomography (CBCT) data. The two groups were divided into the non-anchorage group and the micro-implant group (between the roots of the first molar and the second molar) based on whether the elastic traction of the micro-implant was assisted or not. The two groups were divided into the following conditions based on the starting position of the first molar: Working condition 1: the distance between the first molar and the second premolar was 0 mm; working condition 2: the distance between the first molar and the second premolar was 1 mm; working condition 3: the distance between the first molar and the second premolar was 2 mm; working condition 4: the distance between the first molar and the second premolar was 3 mm. The data characte-ristics of total displacement and displacement in each direction of dentition were analyzed. RESULTS: In the non-ancho-rage group, all the other teeth showed reverse movement except for the first molar which was moved distally. Meanwhile, in the micro-implant group, except for a small amount of mesial movement of the second molar in wor-king condition 1, the whole dentition in other working conditions presented distal movement and anterior teeth showed lingual movement, among which the distal displacement of the first molar in working condition 4 was the largest. With the change of the initial position of the first molar to the distal, the movement of the first molar to the distal, the premolar to the mesial, and the anterior to the lip increased, while the movement of the second molar to the mesial decreased. CONCLUSIONS The micro-implant can effectively protect the anterior anchorage, increase the expression rate of molar distancing, and avoid the round-trip movement of the second molar. The initial position of the first molar movement is related to the amount of distancing and the remaining tooth movement.

OBJECTIVES: This study aimed to conduct a meta-analysis of the efficacy of mandibular advance clear alig-ners with traditional functional appliances as the control group. METHODS: PubMed, Web of Science, Embase, Cochrane Library, China Biomedical Abstracts Database, China Knowledge Network Database, Wanfang Database, and Weipu Database were used in this study. The two groups of researchers screened the literature and extracted data based on the inclusion and exclusion criteria established by PICOS entries, and used the ROBINS-I scale for quality evaluation. Revman 5.4 and Stata 17.0 software were used for meta-analysis. RESULTS: Nine clinical controlled trials were included in this study with a total sample size of 283 cases. No significant difference was found in SNA, SNB, ANB, Go-Pog, U1-SN, Overjet, and other aspects between the invisible group and the traditional group in the treatment of skeletal class Ⅱ ma-locclusion patients; there was a 0.90° difference in mandibular plane angle between the two groups; the growth of the mandibular ramus (Co-Go) in the traditional group was 1.10 mm more than that in the invisible group; the lip inclination of the lower teeth in the invisible group was better controlled, 1.94° less than that in the control group. CONCLUSIONS The invisible group can better control the lip inclination of the mandibular anterior teeth when guiding the mandible. Furthermore, the mandibular plane angle (MP-SN) can remain unchanged, but the growth of the mandibular ramus is not as good as the traditional group, and auxiliary measures should be taken to improve it in clinical practice.

Objective To explore the differential expression of protein in the serum of individuals susceptible to noise induced hearing loss (NIHL) susceptible individuals working in the military tunnel.Methods A total of 40 soldiers from one tunnel construction troop were divided into the susceptible group and the nonsusceptible group.Twenty soldiers were selected for each group.The average age of the susceptible group was 24.79±2.03 years old and their thresholds of the speech and high frequencies were 22.43±8.31 dB HL and 48.55± 11.54 dB HL,respectively.The average age of the nonsusceptible group was 23.67 ± 3.56 years old and their thresholds of the speech and high frequencies were 13.40±4.13 dB HL and 9.40±2.54 dB HL,respectively.Five microliter peripheral venous blood samples were collected from each individual Two-dimensional electrophoresis (2-DE) and MALDI-TOF-MS were used to separate and identify the differentially expressed proteins.Results Thirty-seven protein spots differentially expressing between the NIHL susceptible and nonsusceptible were found after 2 DE.Compared by mascort score,10 differential proteins were harvested.Among these,5 peptides including proteasome subunit alpha-5,complement C4-A,haptoglobin,apolipoprotein A-I and vitronectin were upregulated,and other 5 ones,including Lysozyme C,beta-2 glycoprotein-1,pigment epithelium derived factor,35 kDa trypsin inhibitor heavy chain H and transthyretin were downregulated in NIHL susceptible individuals.The differences were statistically significant(P＜0.05).Conclusion The differentially expressed proteins were closely related to oxidative stress responses in susceptible individuals,including proteasome subunit alpha-5,complement C4A,haptoglobin,apolipoprotein A-I,beta-2 glycoprotein-1,pigment epithelium derived factor,35 kDa trypsin inhibitor heavy chain H and transthyretin.They might participate in the occurrence of NIHL through this way.The proteins harvested from this study were expected to be specific candidate serum NIHL susceptibility biomarkers in blood to help screen susceptible individuals.

Objective To explore the diagnostic value of prostate specific antigen (PSA),compound prostate specific antigen(cPSA),and cPSA / tPSA ratio in prostatic cancer(PCa).Methods Two hundred nineteen people,aged 62-68,were divided into three groups according to the clinical diagnosis,including PCa group(n=73),benign prostatic hyperplasia(BPH) group(n=76) and control group(n=70) composed of healthy people.The various indicators,including tPSA,cPSA and cPSA/tPSA were measured by Advia Centaur CP chemiluminescent analyzer.Results tPSA,cPSA and c/t significantly increased in PCa group compared with those in BPH or control group (P<0.05).ROC analysis indicated that if the tPSA cut-off values was19.89 μg/L,the sensitivity,specificity,the area under the curve (AUC) and Youden index would be 94.5%,97.9%,0.96 and 0.924 respectively.If the cut-off value of cPSA was 13.14 μg/L,the sensitivity,specificity,the area under the curve(AUC) and Youden index would be 95.9%,91.8%,0.98 and 0.877 respectively.The chi-square test showed that there is no correlation between tPSA,cPSA and pathology diagnosis(χ2=102.036、151.409,P<0.05).Conclusion PCa or PCa treatment relapse should be highly suspected when the concentration of tPSA or cPSA in patients reached 19.89 μg/L and 13.14 μg/L,the patients should be reminded for treatment timely.

Objective To investigate the feasibility,safety and effectiveness of endovascular angioplasty with covered stent and embolization with spring coil in treating ruptured femoral artery pseudoaneurysm (FAP) associated with bleeding caused by injection of addictive drug.Methods The clinical data of 32 patients with ruptured FAP complicated by bleeding caused by injection of addictive drug,who were admitted to authors' hospital during the period from July 2012 to December 2015,were retrospectively analyzed.The average age of the patients was 36.5 years old,among them 25 patients were male (78.1％).Results Successful hemostasis was achieved in all 32 patients.The technical success rate of endovascular therapy was 100％,and no death occurred during perioperative period.Endovascular repair with covered stent was carried out in 25 patients (78.1％),embolization of femoral profound artery with spring coil in 9 patients (9.4％),and covered stent implantation together with embolization of femoral profound artery in 4 patients (12.5％).The patients were followed up for a mean of (17.5±11.6) months,with a follow-up rate being 93.8％ (30/32).The 3-year cumulative stent patency rate was 90.9％,and the 3-year overall survival rate was 91.3％.Conclusion For the treatment of ruptured FAP complicated by bleeding caused by injection of addictive drug,endovascular treatment is safe and effective with satisfactory mid-term clinical effect.This technique helps win the valuable time for critically ill patients to receive two-stage surgical debridement and vascular repair,as two-stage thorough debridement is an important means to control infection.
