1.Clinical and molecular characteristics of myeloproliferative neoplasms patients with NFE2 gene mutations

Objective:To explore the clinical features and molecular characteristics of myeloproliferative neoplasms (MPNs) patients with NFE2 gene mutations.Methods:Gene targeted sequencing was used to detect NFE2 gene mutation in 723 patients diagnosed with MPNs who were admitted to Institute of Hematology & Blood Diseases Hospital, Chinese Academy of Medical Sciences & Peking Union Medical College between April 2021 and June 2023. The association between NFE2 gene mutations and clinical features and molecular characteristics of MPNs patients were retrospectively analyzed.Results:Among 723 patients with MPNs, NFE2 gene mutations were found in 41 cases (5.7%) . NFE2 gene mutations were predominantly frameshift mutations (44.4%) , followed by nonsense mutations (33.3%) . The median number of mutations in patients with NFE2 gene mutations (4 [2,5]) was higher compared to the group without NFE2 gene mutations (2, [1,3]) ( P<0.001) . NFE2 gene mutations frequently co-occurred with mutations in MPL, ATM, PPM1D, and TET1. NFE2 gene mutations were mostly sub-clonal events, with 80.5% occurring after MPNs driver mutations (JAK2, CALR, or MPL) . NFE2 mutations were correlated with older age [median age: 60 (54, 67) years vs 54 (41, 63) years, P=0.001]. Patients with NFE2 gene mutations had a higher incidence of pre-diagnosis thrombosis (39.0% vs 22.0%, P=0.012) and pre-diagnosis arterial thrombosis (36.6% vs 20.4%, P=0.014) . Using a logistic regression analysis model adjusting for age and comorbidities (including chronic infections, malignancies, and autoimmune diseases) , NFE2 gene mutation was identified as an independent determinant of elevated tumor necrosis factor-alpha (TNF-α) ( OR=2.747, 95% CI: 1.143-6.605, P=0.024) , interferon-gamma (IFN-γ) ( OR=2.689, 95% CI: 1.191-6.076, P=0.017) , IL-10 ( OR=3.219, 95% CI: 1.343-7.717, P=0.009) , IL-12P70 ( OR=3.397, 95% CI:1.003-11.508, P=0.049) , IL-17 ( OR=2.284, 95% CI: 1.017-5.127, P=0.045) . In polycythaemia vera (PV) patients with the NFE2 gene mutation, the proportion of those classified as high-risk is notably higher in both the IWG-PV and mutation-enhanced international prognostic systems for PV (MIPSS-PV) (66.7% vs 25.3% for IWG-PV, P=0.033; 22.2% vs 2.0% for MIPSS-PV, P=0.013) . Similarly, for essential thrombocythaemia (ET) patients, the proportion in the high-risk group of the mutation-enhanced international prognostic systems for ET (MIPSS-ET) is significantly higher (15.4% vs 6.1%, P=0.021) . No statistically significant differences were observed in overall survival or cumulative incidence of thrombosis between NFE2-mutated (38 cases) and non-mutated MPNs patients (671 cases, P>0.05) . Conclusion:NFE2 gene mutations in MPNs were predominantly frameshift mutations. NFE2 gene mutations were correlated with older age, elevated levels of several inflammatory factors (including TNF-α、IFN-γ、IL-10、IL-12P70、IL-17) , and they mostly occurred in late-stage of MPNs.

This paper summarizes the current situation of the cultivation of specialized Traditional Chinese Medicine (TCM) geriatric nursing talents under the background of the integration of medical care and elderly care, including the cultivation status, necessity, feasibility, and existing problems. Based on this, reflections and prospects are put forward, aiming to further promote the cultivation of specialized TCM nursing talents, facilitate the in-depth integration of TCM nursing and geriatric nursing, and drive the high-quality development of nursing.
3.Construction of an exercise management program for inpatients with postmenopausal osteoporosis based on the health action process approach

Objective:To construct an exercise management program for inpatients with postmenopausal osteoporosis (PMOP) based on the health action process approach (HAPA) .Methods:Using HAPA as a theoretical basis, the literature on exercise management in PMOP inpatients was systematically searched and relevant evidence was extracted. On this basis, the first draft of an exercise management program for PMOP inpatients was constructed by combining the results of semi-structured interviews with 16 PMOP inpatients. The first draft of the exercise management program for PMOP inpatients was revised after two rounds of Delphi expert consultation.Results:A total of 15 experts were invited for two rounds of expert consultation. In the two rounds of consultation, the effective recovery rate of the questionnaire was 100.0% (15/15), the expert authority coefficient was 0.880, and Kendall's harmony coefficient was 0.156 and 0.208, respectively ( P<0.01). The final exercise management program for PMOP inpatients consisted of three first-level items, nine second-level items, and 48 third-level items. Conclusions:The exercise management program for PMOP inpatients is scientific and comprehensive, which can provide a reference for nursing staff.

7.Study on protective effects of twin drugs of tetramethylpyrazine-scutellarein on cerebral ischemia-reperfusion injury model rats and its mechanism

OBJECTIVE To study the protective effects of twin drugs of tetramethylpyrazine-scutellarein (TMSC4) on cerebral ischemia-reperfusion injury (CIRI) model rats and its mechanism. METHODS One hundred and five SD rats were randomly divided into sham operation group, model group, scutellarein group (0.7 mmol/kg), tetramethylpyrazine group (0.7 mmol/kg), and TMSC4 low-dose, medium-dose and high-dose groups (0.35, 0.7, 1.4 mmol/kg), with 15 rats in each group. Sham operation group and model group were given constant volume of normal saline intragastrically, and other groups were given relevant drug intragastrically, once a day, for consecutive 14 d. Except for sham operation group, all other groups were treated to establish the CIRI model using the thread occlusion method. After 2 hours of ischemia and 22 hours of reperfusion, the brain index and brain water content of the rats were measured. Serum levels of interleukin 1β (IL-1β), IL-6 and tumor necrosis factor α (TNF-α), the levels of superoxide dismutase (SOD), malondialdehyde (MDA), glutathione peroxidase (GSH-Px) and catalase (CAT) in brain tissues, the situation of neuronal cell apoptosis, and the protein expressions of B-cell lymphoma 2 (Bcl-2), Bcl-2-associated X protein (Bax) and cleaved-caspase-3 were evaluated. RESULTS Compared with sham operation group, the brain index, brain water content, the serum levels of IL-1β, IL-6 and TNF-α, the levels of MDA in brain tissues, the brain cell apoptosis and the protein expressions of Bax and cleaved-caspase-3 in model group were significantly increased (P<0.05); the levels of SOD, GSH- Px and CAT and the protein expression of Bcl-2 in brain tissues were significantly decreased (P<0.05). Compared with model group, the above indexes of rats were reversed significantly in administration groups (P<0.05), while the reverse effects of TMSC4 medium-dose and high-dose groups were significantly better than those of scutellarein group and tetramethylpyrazine group (P<0.05). CONCLUSIONS TMSC4 has a certain protective effect in CIRI model rats, the mechanism of which may be related to relieving inflammatory reaction and oxidative stress, inhibiting cell apoptosis.
8.Investigation and analysis of iodine nutritional statusin healthy adults in Urumqi of Xinjiang Uyghur Autonomous Region

Objective To investigate iodine nutrition status in healthy adults in Xinjiang Urumqi city and their relationship to ethnicity, gender and age. Method A cross-sectional survey was performed in 2 100 residents of Xinjiang Urumqi 2 communities in May 2013, of which 1 835 healthy adults aged 18-84 years, mean age 46.3 ± 14.2 years were enrolled. Urine iodine with arsenic-cerium catalytic spectrophotometry and salt iodine and water iodine of the residents were measured. Result The water iodine content was 3.14 μg/L, salt iodine was 27.75 mg/kg, 1 835 urine samples were collected, the urinary iodine median(MUI) was133.4 μg/L, iodine deficiency accounted for 32.37% (595 cases), iodine sufficient 42.94% (788 cases), iodine super-sufficient 14.44%(265 cases), iodine excess 10.19%(187 cases). The urine iodine level in Han ethnic group was higher than those in Uygur adults, no significant difference was observed between men and women (P>0.05). Urinary iodine levels among different age groups had no statistical difference, but in the older groups the level was lower than that in young or middle age adults. The urinary iodine level decreased gradually with the age increasing. Conclusion There was a sufficient pattern of iodine nutrition levels in healthy adults in Xinjiang Urumqi; there was a correlation between the level of iodine nutrition and the ethnicity but not with gender;iodine nutrition level in older age group was lower than those in younger age groups. Monitoring the urinary iodine continually and decreasing iodine intake may be necessary for part of iodine excess population.

In China Medical University,20-teaching-hour electron microscope technique course has been arranged for the undergraduates of Laboratory Medicine.The purpose is to let the students master the specialized skill and essential scientific research ability through studying the relative basic knowledge and operation of the electron microscope.The questionnaire results show that the attitude of students is positive and 83% of them are satisfied in general.This paper summarizes the experiences of the practice.
