1.Construction of a nomogram prediction model for Alzheimer's disease among the elderly in community

Objective:
To establish a nomogram prediction model for Alzheimer's disease (AD) among the elderly in community, so as to provide the evidence for early screening and prevention of AD.
Methods:
Based on the Zhejiang Healthy Aging Cohort Study, the elderly aged 60-90 years who completed the baseline survey were selected as the study subjects. Follow-up surveys were conducted from 2015 to 2016 and from 2019 to 2021. Sociodemographic characteristics, lifestyle factors, medical history, and waist circumference were collected through questionnaire surveys and physical examinations. Cognitive function was assessed using the Mini-Mental State Examination (MMSE), and a diagnosis of AD was made based on the Alzheimer's Disease Assessment Scale-Cognitive Subscale and medical history. The participants were randomly divided into training and validation sets at 8∶2 ratio. LASSO regression was used to screen for predictive factors. Multivariable logistic regression model was used to analyze predictive factors and construct a nomogram. The model was analyzed and evaluated using the receiver operating characteristic (ROC) curve and decision curve analysis (DCA).
Results:
A total of 6 988 elderly were included at baseline, with a mean age of (68.19±6.63) years. There were 3 438 males (49.20%), and 3 550 females (50.80%). The median follow-up duration was 4.90 (interquartile range, 3.80) years, with 817 new cases of AD were identified, yielding an incidence of 11.69%. LASSO regression and multivariable logistic regression showed that age (OR=1.017, 95%CI: 1.005-1.030), gender (female, OR=1.820, 95%CI: 1.533-2.165), educational level (primary school, OR=0.813, 95%CI: 0.673-0.980), physical exercise (not active, OR=1.572, 95%CI: 1.260-1.980), dining companions (spouse and children, OR=0.771, 95%CI: 0.598-0.995), baseline MMSE score (OR=0.843, 95%CI: 0.821-0.866), and waist circumference (OR=0.981, 95%CI: 0.973-0.989) were risk predictors for AD among the elderly in community. The prediction model demonstrated an area under the ROC curve of 0.740 (95%CI: 0.698-0.783) in the validation set, with a sensitivity of 0.731 and a specificity of 0.667. DCA indicated that when the probability threshold was 0.060 to 0.325, the clinical net benefit was relatively high.
Conclusion
The AD risk prediction model constructed in this study has good discrimination and clinical practicability, can be used for early screening of AD among the elderly in the community.
2.Clinical Study on Qishao Huoxue Prescription for the Treatment of Lumbar Disc Herniation in Qi Stagnation and Blood Stasis Syndrome

Objective To observe the clinical efficacy of Qishao Huoxue Prescription in treating low back pain caused by lumbar disc herniation with qi stagnation and blood stasis syndrome.Methods Totally 80 patients of lumbar disc herniation with qi stagnation and blood stasis syndrome were divided into observation group and control group according to random number table method,with 40 cases in each group.The observation group received oral administration of Qishao Huoxue Prescription,while the control group received oral celecoxib capsules for 3 weeks.Follow-ups were conducted after 1 month and 3 month of treatment,and the clinical efficacy of two groups after 3 months of treatment was evaluated.The Visual Analog Scale(VAS)scores,Japanese Orthopaedic Association Assessment of Treatment Score(JOA),Oswestry Disability Index(ODI)and Quality of Life(SF-36)scores for pain before treatment,1 and 3 weeks after treatment,and 1 and 3 months after treatment in two groups were observed;the blood flow changes of two groups before and after 3 weeks of treatment were observed,including whole blood high shear,medium shear,and low shear viscosity,plasma viscosity,and fibrinogen(FIB);adverse reactions in both group were recorded.Results The total effective rate in the observation group was 97.5%(39/40),significantly higher than 87.5%(35/40)in the control group(P<0.05).At 1 and 3 weeks of treatment and 1 and 3 months after treatment,VAS scores and ODI scores significantly decreased compared to before treatment,while JOA scores and SF-36 scores significantly increased in both groups.The improvement in scores in the observation group was better than that in the control group(P<0.01).The observation group showed a significant decrease in blood high,medium,and low shear viscosity,PV and FIB levels after 3 weeks of treatment compared to before treatment(P<0.05),and all levels were lower than those in the control group(P<0.05).No significant adverse reactions were observed in both groups.Conclusion Qishao Huoxue Prescription can significantly improve the clinical symptoms,hemorheological indicators,and quality of life of low back pain caused by lumbar disc herniation with qi stagnation and blood stasis syndrome,with good safety.
3.Comprehensive clinical evaluation of empagliflozin in the treatment of type 2 diabetes mellitus

Objective To conduct a multidimensional and multi-level evaluation of the comprehensive clinical value of empagliflozin in the treatment of type 2 diabetes mellitus.Methods Based on the National Essential Medicines List(2018 Edition),dapagliflozin was selected as the control.A comprehensive clinical evaluation index system was established through literature review,focus group interviews and in-depth expert interviews,encompassing six dimensions:safety,efficacy,economy,suitability,innovation,and accessibility.The Delphi method and hierarchical direct weighting method were used to screen indicators and determine their weights.Evidence for each indicator was collected and integrated both qualitatively and quantitatively through literature research,real-world studies,and pharmacoeconomic evaluations.Experts scored the indicators based on the collected evidence,and a total score for the comprehensive clinical evaluation of empagliflozin was calculated by combining these scores with indicator weights,followed by a comparative analysis with dapagliflozin.Results A comprehensive clinical evaluation of empagliflozin in the treatment of type 2 diabetes mellitus was successfully established,consisting of 6 primary indicators,14 secondary indicators,and 41 tertiary indicators.The overall evaluation score for empagliflozin was 90.35,and 89.47 for dapagliflozin.Conclusion The comprehensive clinical value of empagliflozin in the treatment of type 2 diabetes mellitus is slightly higher than that of dapagliflozin.This finding can serve as a reference for rational clinical drug use and related decision-making.

6.Safety of teriflunomide in Chinese adult patients with relapsing multiple sclerosis: A phase IV, 24-week multicenter study.

BACKGROUND:
Disease-modifying therapies have been approved for the treatment of relapsing multiple sclerosis (RMS). The present study aims to examine the safety of teriflunomide in Chinese patients with RMS.
METHODS:
This non-randomized, multi-center, 24-week, prospective study enrolled RMS patients with variant (c.421C>A) or wild type ABCG2 who received once-daily oral teriflunomide 14 mg. The primary endpoint was the relationship between ABCG2 polymorphisms and teriflunomide exposure over 24 weeks. Safety was assessed over the 24-week treatment with teriflunomide.
RESULTS:
Eighty-two patients were assigned to variant ( n = 42) and wild type groups ( n = 40), respectively. Geometric mean and geometric standard deviation (SD) of pre-dose concentration (variant, 54.9 [38.0] μg/mL; wild type, 49.1 [32.0] μg/mL) and area under plasma concentration-time curve over a dosing interval (AUC tau ) (variant, 1731.3 [769.0] μg∙h/mL; wild type, 1564.5 [1053.0] μg∙h/mL) values at steady state were approximately similar between the two groups. Safety profile was similar and well tolerated across variant and wild type groups in terms of rates of treatment emergent adverse events (TEAE), treatment-related TEAE, grade ≥3 TEAE, and serious adverse events (AEs). No new specific safety concerns or deaths were reported in the study.
CONCLUSION:
ABCG2 polymorphisms did not affect the steady-state exposure of teriflunomide, suggesting a similar efficacy and safety profile between variant and wild type RMS patients.

Alzheimer's disease(AD)presents a notable gender disparity in prevalence among elderly individuals, with elderly women being a high-risk group for AD onset.Researches have confirmed the effects of estrogen on the nervous system and cognitive function through in vitro and animal models.Throughout their reproductive years, women undergo various life events such as menarche, menopause, pregnancy, childbirth, miscarriage, relevant surgeries, and medication, all of which can impact their endocrine status and subsequently influence brain function.As a result, there is a growing body of epidemiological evidence investigating the relationship between women's menstrual and reproductive factors and AD.Despite some controversies, this article offers a thorough review of current epidemiological research on the link between different menstrual and fertility factors in women and AD.
8.Scale-invariant feature-enhanced deep learning framework for oral mucosal lesion segmentation

Objective:To develop PixelSIFT-UNet, a novel semantic segmentation model that integrates deep learning with scale-invariant feature transform (SIFT) algorithm to improve the segmentation accuracy of oral mucosal lesions.Methods:This investigation utilized 838 standard clinical white light images of oral mucosal diseases acquired from January 2020 to December 2022 at the Stomatology Hospital Zhejiang University School of Medicine. Randomization was achieved through Python′s random.seed function implementation. The random sample function was subsequently applied for sampling distribution. The dataset was stratified into three subsets with a 6∶2∶2 ratio: training ( n=506), validation ( n=166), and testing ( n=166). Lesion boundaries were annotated using Labelme software, and a PixelSIFT-UNet-based deep learning model was developed with VGG-16 and ResNet-50 backbone networks. Model parameters were optimized using the validation set, and performance metrics [including Dice coefficient, mean intersection over union (mIoU), mean pixel accuracy (mPA), and Precision] were assessed on the test set. The model′s performance was benchmarked against conventional semantic segmentation frameworks (U-Net and PSPNet). Results:The developed PixelSIFT-UNet model could achieve precise segmentation of three common oral mucosal lesions: oral lichen planus, oral leukoplakia, and oral submucous fibrosis. Utilizing VGG-16 as the backbone network, the model achieved Dice coefficient, mIoU, mPA, and Precision values of 0.642, 0.699, 0.836, and 0.792, respectively. Implementation with ResNet-50 backbone network yielded metrics of 0.668, 0.733, 0.872 and 0.817, demonstrating significant improvements across all performance indicators compared to conventional U-Net model (relevant metrics: 0.662, 0.717, 0.861 and 0.809) and PSPNet model (relevant metrics: 0.671, 0.721, 0.858 and 0.813).Conclusions:The proposed PixelSIFT-UNet architecture demonstrates superior performance in oral mucosal lesion segmentation tasks, surpassing conventional semantic segmentation models and providing robust quantitative improvements in segmentation accuracy.
