Yttrium-90(90Y)selective internal radiation therapy(SIRT)is an emerging modality for the treatment of hepatocellular carcinoma(HCC),leveraging the nuclide 90Y to deliver targeted radiation therapy.90Y has a long half-life and can be used to selectively ablate tumor cells by high-energy beta rays.It has high biological effectiveness and robust local control capabilities. In recent years, with the continuous advancement of basic and clinical research, the application of 90Y-SIRT in the conversion treatment of unresectable HCC(uHCC)has made significant progress. However, challenges remain in the clinical application of 90Y-SIRT,including how to improve the efficacy of conversion therapy and how to optimize therapy regimens. This review aims to summarize the research progress of90Y-SIRT in the conversion therapy of uHCC.

The morphological characteristics of hepatocellular carcinoma(HCC)tumor margins are pivotal in influencing patient's prognosis and the selection of therapeutic strategies. This paper re-viewed the classification methods of HCC tumor margins, ranging from traditional macroscopic classifi-cations to refined classification systems based on multi-omics analysis, and analyzed the role of these classification methods in guiding the formulation of personalized treatment plans.Additionally, this paper emphasized the crucial role of three-dimensional imaging techniques in assessing tumor margin morphology and outlined future research directions, including validating the effectiveness of multi-omics classification systems and developing new imaging and molecular biomarkers to achieve more precise treatment plans and prolong patient survival.

Hepatocellular carcinoma(HCC)is one of the most common malignant tumors in China. When HCC is diagnosed, most patients have been at the middle or advanced stage and have lost the chance of radical resection. In recent years, the roles of targeted therapy, immunotherapy, and radiotherapy have become increasingly prominent in the treatment of unresectable HCC.Although HCC is sensitive to radiation, the clinical value of radiotherapy is often underestimated due to the low tolerance of liver to radiation. With the development of radiotherapy technology, radiotherapy has been widely used in unresectable HCC.Patients can gain more survival benefits whether treated with radiotherapy alone or in combination with other treatments. This review focuses on the application and progress of radiotherapy and its combination therapy in unresectable HCC.

Chinese Journal of Neurology 2023;56(12):1325-1332
Muscular dystrophy is a group of progressive muscle diseases caused by inherited genetic mutations. Muscular weakness and atrophy are the most common presentations. Muscle biopsies show obvious muscular necrosis, regeneration and abnormal hyperplasia of fibrous connective tissues. In addition to the manifestations of the skeletal muscle, many patients also exhibit symptoms due to the involvement of brain, eye, heart, or endocrine organs. Brain involvement can manifest as neurological and cognitive abnormalities, such as intellectual disability, seizures, drowsiness, or autism spectrum disorders. Cranial imaging can be normal or shows gray matter atrophy and abnormal white matter signals. Clinicians should pay attention to the diagnoses and studies of brain abnormalities in patients with muscular dystrophies. The progression of the brain abnormalities should be explored, and effective intervention measures should be designed based on different ages. Since muscular dystrophies are rare diseases with a very low incidence, national collaboration with sophisticated neuromuscular centers is absolutely essential to support clinical care and basic research for such disorders. Basic research scientists should also seek techniques of modern molecular biology such as induced pluripotent stem cells-derived neuronal cells and multiple omics techniques to identify the pathological mechanisms, analyze the phenotypic characteristics, and prioritize candidate therapeutic targets.

This cross-sectional study aimed to investigate the quality of care of diabetes in Shanghai, China. A total of 173 235 patients with type 2 diabetes in 2017 were included in the analysis. Profiles of risk factors and intermediate outcomes were determined. The patients had a mean age of 66.43 ± 8.12 (standard deviation (SD)) years and a mean diabetes duration of 7.95 ± 5.53 (SD) years. The percentage of patients who achieved the target level for HbA1c (< 7.0%) was 48.6%. Patients who achieved the target levels for blood pressure (BP) < 130/80 mmHg and low-density lipoprotein-cholesterol (LDL-c) < 2.6 mmol/L reached 17.5% and 34.0%, respectively. A total of 3.8% achieved all three target levels, and the value increased to 6.8% with an adaptation of the BP target level (< 140/90 mmHg) for those over 65 years. Multivariable analysis identified the factors associated with a great likelihood of achieving all three target levels: male, young age, short diabetes duration, low body mass index, macrovascular complications, no microvascular complications, prescribed with lipid-lowering medication, and no prescription of antihypertensive medication. In conclusion, nearly 50% and one-third of the patients with diabetes met the target levels for HbA1c and LDL-c, respectively, with a low percentage achieving the BP target level. The percentage of patients who achieved all three target levels needs significant improvement.

BACKGROUND AND PURPOSE: The wnt/β-catenin signaling pathway plays a critical role in embryonic development and adult-tissue homeostasis. Recent investigations implicate the importance of wnt/β-catenin signaling in normal wound healing and its sustained activation being associated with fibrogenesis. We investigated the immunolocalization and activation of wnt/β-catenin in polymyositis (PM), dermatomyositis (DM), and Duchenne muscular dystrophy (DMD). METHODS: Immunofluorescence staining and Western blot analysis of β-catenin were performed in muscle specimens from 6 PM, 8 DM, and 6 DMD subjects. The β-catenin/Tcf4 DNA-binding activity in muscle was studied using an electrophoretic mobility shift assay (EMSA), and serum wnt/β-catenin/Tcf transcriptional activity was measured using a luciferase reporter gene assay. RESULTS: Immunoreactivity for β-catenin was found in the cytoplasm and nuclei of muscle fibers in PM, DM, and DMD. The protein level of β-catenin was elevated, and EMSA analysis confirmed the activation of wnt/β-catenin signaling. The transcriptional activities of β-catenin/Tcf in the circulation were increased in patients with PM, DM, and DMD, especially in those with interstitial lung disease, and these transcriptional activities decreased when PM or DM patients exhibited obvious clinical improvements. CONCLUSIONS: Our findings indicate that wnt/β-catenin signaling is activated in PM, DM, and DMD. Its activation in muscle tissue and the circulation may play a role in modulating muscle regeneration and be at least partly involved in the process of muscle and pulmonary fibrosis.
