1.Gene-predicted associations between 731 immune cell phenotypes and rheumatoid arthritis

BACKGROUND:Rheumatoid arthritis is widely prevalent worldwide,with its high incidence and universality that considerably affects patients' quality of life.Previous studies have focused on a few immune cells or cytokines,whereas this study comprehensively provides a more complete view of the immune mechanisms in rheumatoid arthritis.OBJECTIVE:To explore the causal relationship between 731 immune cell phenotypes and rheumatoid arthritis using the Mendelian randomization method,thereby providing evidence of causality.METHODS:The 731 immune cell phenotypes used in this study were sourced from the GWAScatalog database,jointly developed by the National Human Genome Research Institute(NHGRI)and the European Bioinformatics Institute(EBI).The rheumatoid arthritis data were from the Finngen database,developed by the Finnish Institute for Molecular Medicine(FIMM).The inverse variance weighting method was employed as the primary analytical approach.Additionally,multiple analytical methods,including MR-Egger,weighted mode,simple mode,and weighted median,were concurrently utilized to complement the final results.Sensitivity analyses(Cochran's Q test,MR-Egger regression,and MR-presso analysis)were also conducted to verify the stability and feasibility of the data.RESULTS AND CONCLUSION:(1)After excluding results through heterogeneity testing,the inverse variance weighting analysis indicated that 10 absolute cell counts,15 median fluorescence intensities of surface antigen levels,1 morphological characteristic,and 9 relative cell counts had a causal relationship with the occurrence of rheumatoid arthritis.(2)According to cell classification,this study found that seven types of B cells,seven types of classical dendritic cells,six types of mature T cells,four types of monocytes,three types of myeloid cells,three types of TBNK cells(lymphocyte subset T cells,B cells and natural killer cells),and five types of Tregs had a causal association with the occurrence of rheumatoid arthritis.(3)Through comprehensive bidirectional two-sample MR analysis,we demonstrated the complex causal relationships between multiple immune phenotypes and rheumatoid arthritis,highlighting the intricate interaction patterns between the immune system and rheumatoid arthritis.These results provide new biomarkers for the early screening and diagnosis of rheumatoid arthritis in China,and help to improve the diagnostic accuracy and sensitivity.

4.Differential diagnosis model construction of invasive degree of lung adenocarcinoma manifesting as ground-glass nodules with no or little solid component based on energy spectrum CT features

Objective:To construct the differential diagnosis model of invasive degree of lung adenocarcinoma manifesting as ground-glass nodules (GGNs) with no or little solid component based on energy spectrum CT features, and to provide reference for the follow-up clinical diagnosis and treatment.Methods:A retrospective study was conducted on 145 patients who underwent surgical treatment for lung adenocarcinoma at the 969th Hospital of Joint Logistics Support Force of Chinese People's Liberation Army and Inner Mongolia Hospital of Peking University Cancer Hospital from January 2019 to December 2022, presenting with CT findings of no or little solid component GGNs. The patients were divided into invasive group (51 cases) and microinvasive group (94 cases) based on the invasive degree. Logistic regression was used to conduct a multivariate analysis of factors affecting the differential diagnosis of invasive degree of lung adenocarcinoma manifesting as GGNs with no or little solid component, and construct a logistic regression model. Receiver operator characteristic (ROC) curve was used to analyze the predictive efficiency of each index.Results:Univariate analysis showed that the maximum diameter of nodules ( t=-6.30, P<0.001) , average CT value of nodules ( t=-5.43, P<0.001) , air bronchial sign ( χ2=23.21, P<0.001) , microvascular CT imaging type ( χ2=27.94, P<0.001) were predictors of invasive degree of lung adenocarcinoma manifesting as GGNs with no or little solid component. Multivariate logistic regression analysis showed that the maximum diameter of nodules ( OR=1.72, 95% CI: 1.33-2.23, P<0.001) , average CT value of nodules ( OR=1.01, 95% CI: 1.01-1.02, P<0.001) , air bronchial sign ( OR=4.92, 95% CI: 1.59-15.21, P=0.006) and microvascular CT imaging type Ⅲ ( OR=14.01, 95% CI: 2.97-66.06, P=0.001) were independent predictors of invasive degree of lung adenocarcinoma manifesting as GGNs with no or little solid component. A logistic regression model was constructed based on the results of the above multiple factor analysis: logit ( P) =0.54×maximum diameter of the nodule+0.01×average CT value of the nodule+1.59×air bronchogram sign+2.64×microvascular CT imaging type (type Ⅲ) -3.33. ROC curve analysis showed that the areas under the curve for differential diagnosis of invasive degree of lung adenocarcinoma manifesting as GGNs with no or little solid component based on the maximum diameter of nodules, average CT value of nodules, air bronchogram sign, microvascular CT imaging type, and logistic regression model P-value were 0.759, 0.751, 0.686, 0.741, and 0.918, respectively. Conclusions:The energy spectrum CT features, including the maximum diameter of nodules, average CT value of nodules, air bronchial sign, and microvascular CT imaging type, can be used for differential diagnosis of invasive degree of lung adenocarcinoma manifesting as GGNs with no or little solid component. The logistic regression model constructed using the above four factors has shown good performance in predicting the invasive degree of patient.
5.Clinical Observation on the Shengqing Jiangzhuo Acupuncture Method Combined with Western Medical Treatment in Treating Gouty Nephropathy with Damp-Heat Accumulation Syndrome

Objective To evaluate the clinical efficacy of Shengqing Jiangzhuo(lifting lucid yang and lowering turbid yin)Acupuncture Method,based on the"turbidity-clearness interaction"theory,combined with western medicine in treating gouty nephropathy(GN)patients with damp-heat accumulation syndrome.Methods A total of 117 patients diagnosed with GN from September 2023 to September 2024 at the First Affiliated Hospital of Guangzhou University of Chinese Medicine Chongqing Hospital were enrolled and randomly divided into three groups,with 39 cases in each group.The western medicine group received Febuxostat Tablets,the control group was given western medicine+conventional acupuncture,and observation group was given western medicine+Shengqing Jiangzhuo Acupuncture Method.All groups received 4 weeks of treatment.Clinical efficacy,traditional Chinese medicine(TCM)syndrome scores,visual analogue scale(VAS)of pain scores,and laboratory parameters—including serum uric acid(UA),blood urea nitrogen(BUN),creatinine(Scr),estimated glomerular filtration rate(eGFR),β2-microglobulin(β2-MG),24-hour urinary protein(24h-UTP),cystatin C(CysC)were assessed.The erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)were compared before and after treatment.Results(1)The total effective rate was 71.79%(28/39)in the western medicine group,84.62%(33/39)in the control group,and 92.31%(36/39)in the observation group.The observation group demonstrated statistically significant superiority over the western medicine group(P<0.05),while showing marginally better efficacy than the control group without statistical significance(P>0.05).(2)After treatment,all three groups exhibited significant improvements in ESR,CRP,and UA levels(P<0.05).The observation group significantly outperformed the other two groups in reducing ESR and CRP levels(P<0.05),and was statistically superior to the western medicine group in lowering UA levels(P<0.05),though no significant difference was observed versus the control group for UA improvement(P>0.05).The control group showed significantly greater reductions in ESR,CRP,and UA levels than the western medicine group(P<0.05).(3)After treatment,Scr,BUN,and eGFR levels were significantly improved in all groups(P<0.05).The observation group achieved significantly better improvements in Scr and eGFR than the other two groups(P<0.05).However,no statistical difference existed between the control and western medicine groups for Scr or eGFR enhancement(P>0.05).No intergroup differences were detected in BUN improvement(P>0.05).(4)Significant differences emerged in β2-MG and CysC levels among all groups after treatment(P<0.05).Both observation and control groups demonstrated statistically significant improvements in 24-hour 24h-UTP(P<0.05),whereas the western medicine group showed no significant change(P>0.05).The observation group significantly outperformed the other groups in reducing 24h-UTP and β2-MG levels(P<0.05),and was superior to the western medicine group in lowering CysC(P<0.05),though no difference versus the control group was observed for CysC reduction(P>0.05).The control group showed no statistical advantage over the western medicine group in improving 24h-UTP,β 2-MG,or CysC(P>0.05).(5)After treatment,TCM syndrome scores and VAS scores were significantly improved in all groups(P<0.05).The observation group achieved significantly greater reductions in TCM syndrome scores and VAS scores than the western medicine group(P<0.05).The control group showed statistically superior VAS score improvement versus the western medicine group(P<0.05),but no significant differences emerged in TCM syndrome score improvement compared to either group(P>0.05).No statistical difference existed between observation and control groups for VAS score reduction(P>0.05).Conclusion Shengqing Jiangzhuo Acupuncture Method,grounded in the"turbidity-clearness interaction"theory,effectively reduces serum UA,controls inflammation,and preserves renal function in damp-heat accumulation type of GN,demonstrating significant therapeutic benefits.
6.Investigation on the current nursing practice status of prone position ventilation in patients with moderate to severe acute respiratory distress syndrome among intensive care unit nurses in Shandong province

Objective To understand the nursing practice of prone position ventilation for patients with moderate to severe acute respiratory distress syndrome(ARDS)in intensive care unit(ICU)in Shandong province,so as to provide basis for standardizing the nursing practice process of prone position ventilation and carrying out training for hospitals.Methods A self-made questionnaire was used,and convenience sampling was adopted.From September 15th to November 5th,2023,ICU nurses were selected from various hospital levels in Shandong province to investigate the obstructive factors of prone ventilation implementation,the weak links in nursing practice and status,and the occurrence of complications.Results A total of 1 188 questionnaires were collected,of which 991 were valid.92.8%(920/991)of nurses had performed prone position ventilation.The biggest obstacle to the implementation of prone position ventilation was the complexity of patient treatments and multiple devices involved[74.6%(686/920)].Regarding the status of training,90.5%(897/991)of nurses had received training on prone position ventilation and 77.0%(763/991)of nurses felt that training was needed.As for pre-operation assessment,more than 80.0%of nurses evaluated patients'vital signs,airway and secretions and so on,among which the evaluation awareness of analgesia was the worst[81.6%(751/920)].As for the main points of implementation,only 14.0%(129/920)of nurses chose the opposite side of the most important pipeline as the turning direction;48.6%(447/920)of nurses chose the anti-Trendelenburg position;36.3%(334/920)of nurses chose to ventilate≥12 hours.Facial edema[81.7%(752/920)],skin pressure injury[78.9%(726/920)]and eye complication[75.8%(697/920)]were the top 3 most frequent complications.Conclusions ICU nurses'prone position ventilation practices were generally line with the nursing team standard for prone position of adult mechanically ventilated patients and the best evidence recommendation,and needs to be further standardized in aspects of turning direction,position management,ventilation duration,and enteral nutrition management.It is recommended that nursing managers at all levels of hospitals further improve the quality of nursing practice of prone position ventilation according to relevant evidence-based evidence and the actual situation of hospitals.

10.The effect of pressure controlled ventilation-volume guaranteed combined with personalized positive end expiratory pressure ventilation on oxygenation function and postoperative pulmonary complications in patients undergoing brain tumor surgery

Objective:To explore the ventilation strategy of using pressure controlled ventilation-volume guaranteed (PCV-VG) mode combined with personalized positive end expiratory pressure (PEEP) during surgery, and its impact on oxygenation function and incidence of postoperative pulmonary complications (PPCs) in patients undergoing brain tumor surgery.Methods:Sixty patients who underwent elective brain tumor surgery at the Linyi People′s Hospital from January 2023 to June 2023 were selected. The patients were randomly divided into PCV-VG ventilation mode group (T group) and volume controlled ventilation (VCV) mode group (C group) using a random number table method, with 30 patients in each group. One patient was excluded from group T due to changes in the condition, and 29 patients were actually included. Group T adopted PCV-VG ventilation mode and searched for the maximum dynamic lung compliance (Cdyn) by titrating PEEP. The PEEP corresponding to the maximum Cdyn value was the optimal PEEP, which was maintained until the end of mechanical ventilation. Group C adopted a ventilation mode of VCV plus 5 cmH 2O fixed value PEEP. All patients underwent arterial blood gas analysis before anesthesia induction (T 1), 15 minutes after setting ventilation mode (T 2), and 15 minutes after extubation (T 3), recording arterial oxygen partial pressure (PaO 2), carbon dioxide partial pressure (PaCO 2), lactate (Lac), and blood glucose (Glu), and calculating oxygenation index (PaO 2/FiO 2). The levels of white blood cells (WBC), C-reactive protein (CRP), and procalcitonin (PCT) were recorded before and 72 hours after surgery. The incidence of postoperative PPCs at 72 hours was observed. Results:There was a statistically significant difference in the grouping effect and time effect of PaO 2 and PaCO 2 between group C and group T (all P<0.05), while there was no statistically significant difference in the interaction effect (all P>0.05); The differences in grouping effects, time effects, and interaction effects of PaO 2/FiO 2 between group C and group T were statistically significant (all P<0.05); The average value of the optimized PEEP obtained by T-group titration was 7.48 cmH 2O, corresponding to an average platform pressure (Pplat) of 14.90 cmH 2O, and an average value of 53.37 ml/cmH 2O corresponding to the maximum Cdyn value. Compared with the Pplat and Cdyn corresponding to the fixed value of 5 cmH 2O PEEP in Group C, the Pplat corresponding to the optimized PEEP in Group T was lower than that in Group C ( P<0.05), and the Cdyn was higher than that in group C ( P<0.05). The CRP level and incidence of PPCs in group T after 72 hours of surgery were significantly lower than those in group C (all P<0.05). Conclusions:Compared with VCV, the ventilation mode of PCV-VG combined with personalized PEEP can improve intraoperative lung ventilation and oxygenation function in patients with brain tumors, reduce the occurrence of pulmonary inflammation 72 hours after surgery, and lower the incidence of PPCs.
