1.Clinical study on modified Sanren Decoction combined with triple therapy in treating Helicobacter pylori-related chronic atrophic gastritis

Objective:To evaluate the clinical efficacy of Sanren Decoction combined with triple therapy in the treatment of Hp-related chronic atrophic gastritis (CAG).Methods:A randomized controlled trial was conducted. A total of 118 patients with Hp-related CAG admitted to Department of Spleen and Stomach Diseases of Xi'an Affiliated Hospital of Shaanxi University of Chinese Medicine from February 2021 to February 2023 were selected as observation subjects, and were divided into two groups according to random number table method, with 59 cases in each group. The control group received triple therapy (amoxicillin+clarithromycin+omeprazole), whereas the observation group was combined with modified Sanren Decoction on the basis of the control group. Both groups were continuously treated for 10 days. TCM symptoms were scored before and after treatment. Updated Sydney System (UAS) was applied to evaluate the gastric mucosal lesions. ELISA was applied to detect serum levels of cluster of differentiation 47 (CD47), VEGF and pepsinogenⅠ/pepsinogen Ⅱ (PGⅠ/PGⅡ). The Hp infection status was evaluated by Urea Breath Test (UBT). In addition, the adverse reactions were observed and recorded during treatment.Results:After treatment, the scores of gastric fullness, stomachache, acid reflux and bitter taste in observation group were lower than those in the control group ( t=4.52, 3.46, 4.34, 4.83, P<0.01), and the scores of gastric mucosal inflammation, atrophy, intestinal metaplasia and dysplasia were also lower compared to control group ( t=3.68, 2.88, 3.73, 4.45, P<0.01 or P<0.05). After treatment, the levels of serum CD47 [(4.46±1.29) μg/L vs. (5.01±1.37) μg/L, t=2.25] and VEGF [(70.53±10.47) ng/L vs. (79.34±10.56) ng/L, t=4.55] in observation group were lower ( P<0.05 or P<0.01), while the level of PGⅠ/PGⅡ [(12.45±1.78) vs. (10.82±1.62), t=5.20] was higher compared with that of the control group ( P<0.01). After treatment, the negative conversion rate of Hp was 96.61% (57/59) in observation group and that in control group was 81.36% (48/59) ( χ2=7.00, P=0.008). During treatment, the incidence of adverse reactions was 23.73% (14/59) in observation group and 15.25% (9/59) in control group, revealing no statistical significance between the two groups ( χ2=1.35, P=0.245). Conclusion:Modified Sanren Decoction combined with triple therapy can improve gastric mucosal lesions, reduce serum CD47 and VEGF levels, and increase PGⅠ/PGⅡ ratio and Hp negative conversion rate in patients with Hp-related CAG, with good safety.
2.Efficacy comparison of robot-assisted percutaneous screw implantation and free-hand open screw implantation by Wiltse approach in the treatment of thoracolumbar fracture

Objective:To compare the clinical efficacy of robot-assisted percutaneous screw implantation and free-hand open screw implantation by Wiltse approach in the treatment of thoracolumbar fracture.Methods:A retrospective cohort study was performed to analyze the clinical data of 71 patients with thoracolumbar fracture admitted to Second Affiliated Hospital of Soochow University from May 2018 to May 2020. There were 52 males and 19 females, with age range of 22-54 years[(41.0±7.8)years]. Of all, 33 patients were treated with robot-assisted percutaneous screw implantation (Group A) and 38 patients were treated with free-hand open screw implantation by Wiltse approach (Group B). Following parameters were measured, including frequency of radiation exposure, operation time, intraoperative blood loss, length of hospital stay, incidence of complications, rate of fracture healing at 3 months and 6 months postoperatively, visual analogue scale (VAS) and Oswestry dysfunction index (ODI) at 3 days, 3 months, 6 months postoperatively and at the last follow-up, anterior vertebral body height ratio and sagittal Cobb angle preoperatively, at 3 days postoperatively and at the last follow-up, and rate of screw implantation of grade A and B and rate of facet joint violation at 3 days postoperatively.Results:All patients were followed up for 10-24 months[(15.2±4.4)months]. Frequency of radiation exposure and operation time showed no statistical differences between the two groups (both P>0.05). Intraoperative blood loss was 100(100, 135)ml in Group A, less than 160(120, 200)ml in Group B ( P<0.01). Length of hospital stay was 8(7, 11) days in Group A, shorter than 12(10, 16)days in Group B ( P<0.01). There were no complications such as infection, spinal nerve injury or cerebrospinal fluid leakage in both group. There were no significant differences between the two groups in the rate of fracture healing at 3 and 6 months postoperatively (all P>0.05). VAS and ODI in Group A was 3(2, 4)points and 21(18, 23)points at 3 days postoperatively, lower than 4 (3, 5)points and 27(20, 32)points in Group B ( P<0.05 or 0.01), and the two groups showed no significant differences in VAS and ODI at other time points (all P>0.05). There were no significant difference in the anterior vertebral body height ratio or sagittal Cobb angle between the two groups at 3 days postoperatively and at the last follow-up (all P>0.05). Rate of screw implantation of grade A and B was 96.5% (191/198) in Group A, higher than 90.4% (206/228) in Group B ( P<0.05). Rate of facet joint violation was 4.0%(8/198) in Group A, lower than 11.8% (27/228) in Group B ( P<0.01). Conclusion:For thoracolumbar fracture, robot-assisted percutaneous screw implantation is superior to free-hand open screw implantation by Wiltse approach in terms of less bleeding, shorter hospitalization, earlier pain alleviation, higher accuracy of screw implantation and lower risk of facet joint violation.
3.Investigation of leprosy survivors in Fengxian District, Shanghai

Objective:To understand the current living and health status of leprosy survivors in Fengxian District, Shanghai, and to provide scientific evidence for improving their quality of life. Methods:In January 2018-June 2020, professionally trained CDC staff performed a household investigation on 41 patients with leprosy (case group) and 82 non-leprosy persons (control group) every year. Living and health status, and routine laboratory examinations such as blood pressure, blood glucose, and blood lipids were monitored. Results:Majority of the patients with leprosy had low educational level (68.29% being primary school) and were mostly farmers (51.22%).The patients had abnormal residual Ⅱ level 5 (12.20%) and mostly were tb-like (TT) (53.66%). Moreover, some patients were not incapacitated (46.34%), had no family financial difficulties (29.27%), did not acquire the national subsistence allowance policy (58.54%), and asked for "regular physical examination" (68.29%).Compared to the 82 control persons, the patients with leprosy had significantly different body mass index (BMI) and blood pressure (
4.Fallopian tube perfusion with ozone water for recanalization of obstructed fallopian tube in pluripara:observation of curative effect

Objective To evaluate the curative effect of fallopian tube perfusion with ozone water in treating obstructed fallopian tube in pluripara.Methods A total of 116 patients with distal fallopian tube obstruction were included in this study.The diagnosis was confirmed by clinical and laboratory materials.The patients were randomly divided into the study group and the control group with 58 patients in each group.During hospitalization period all patients received interventional recanalization therapy for obstructed fallopian tube.For the patients of the study group,ozone water perfusion of fallopian tube was employed,while for the patients of the control group,fallopian tube perfusion with conventional anti-inflammatory drug and antiadhesion solution was adopted.All the patients were followed up for 6 months,the pregnancy rate and the fallopian tube re-occlusion rate of both groups were calculated,and 2 weeks after the treatment the patients'discomfort symptoms were evaluated.Results The success rate of fallopian tube recanalization,the pregnancy rate and the re-obstruction rate in the study group were 93.1% (54/58),79.3% (46/58) and 5.2%(3/58) respectively,which in the control group were 91.4% (53/58),60.3% (35/58) and 172% (10/58) respectively.Statistical analysis indicated that no significant difference in the success rate of fallopian tube recanalization existed between the two groups (P>0.05).The pregnancy rate of the study group was higher than that of the control group,while the re-obstruction rate in the study group was lower than that in the control group;the differences between the two groups were statistically significant (P<0.05).No significant differences in discomfort symptom scores existed between the two groups (P>0.05).Conclusion Fallopian tube perfusion with ozone water can be adopted for tubal recanalization,this technique can effectively improve the postoperative pregnancy rate and reduce the tubal re-obstruction rate.
5.Bibliometric analysis of advances in clinical nutrition in Chinese Journals

Objective To reveal the tendency in clinical nutrition research in chinese journals by bibliometric analysis.Methods Literature was searched in Chinese Biomedical Databases (CBM) of SinoMed.By using Noteexpress software,the year distribution,document cartier classifications,research areas,cited frequency of articles and first author distribution were analyzed.Results The number of Chinese literature on clinical nutrition had increased steadily from 1 paper in 1974 to 1980 papers in 2011.The journals with more clinical nutrition articles are mainly professional journals,accounted for 9.21%.Research in the recent 5 years was mainly focused on the modes and timing of nutrition support.The proportions of studies on parenteral nutrition and enternal nutrition were close.Conclusion Clinical nutrition has became one of the hot research topics in China in recent years,although the appropriate and reasonable use of clinical nutrition remain challenging.
6.Effect of Compoud Qingqin Liquids on Renal Function of Uric Acid Nephropathy Rats

Objective To observe the effect of Compoud Qingqin Liquids on renal function of rat model of uric acid nephropathy, and to discuss its protection of renal function. Methods The rat model was induced by gavaging adenine and feeding yeast. SD rats were randomly divided into control group, model group, positive group, and high-, medium-, low-dose groups of Chinese medicine. Blank control group and model group were daily gavaged with distilled water, positive control group was daily gavaged with allopurinol by 9.33 mg/kg, and high-, medium-, low-dose group of Chinese medicine was daily gavaged with Compound Qinggin Liguids by 3.77, 1.89, 0.09 g/(kg·d) respectively for 6 weeks. General condition of rats were observed, renal pathological changes were observed with light and electron microscope. Urine protein concentration, blood uric acid, blood urea nitrogen, creatinine and kidney weight index were respectively tested before and after treatment. Results There were no significant differences in eating, drinking and body weight between before and after modeling. Compoud Qingqin Liquids can obviously decrease the concentration of urine protein, blood uric acid, serum creatinine, blood urea nitrogen, and kidney weight index (P<0.05) of rats with uric acid nephropathy. Renal tubular epithelial cells atrophy and renal interstitial fibrosis of high-dose group of Chinese medicine were not evident. Conclusion Compoud Qingqin Liquids can protect the rats renal function against uric acid renal injury.
