1.Analysis of symptomatic differences in 391 patients with Brucella infections in different age groups

In order to investigate the clinical manifestations and laboratory findings associated with brucellosis for the purpose of facilitating prompt clinical diagnosis and effective treatment, as well as to offer a laboratory reference for the prevention of brucellosis outbreaks. In this study, a retrospective cohort design was employed to gather epidemiological characteristics, clinical symptoms, and associated laboratory data from 391 patients diagnosed with bacterial culture-positive brucellosis at the People′s Hospital of Ningxia Hui Autonomous Region between 2019 and 2023. The patients were categorized into four age groups, with each group representing a 20 years age interval. Statistical analyses were conducted to examine the differences between these age groups using R version 4.3.2 and SPSS version 24.0 software. The results showed that among the 391 cases of brucellosis patients, fever (241 cases, 61.64%), lumbar and leg joint pain (225 cases, 57.54%), abnormal liver function (124 cases, 31.71%), spleen enlargement (78 cases, 19.95%), and cholecystitis (75 cases, 19.18%) were identified as the most prevalent clinical symptoms. Furthermore, the presence of clinical symptoms such as fever, lumbar and leg joint pain, abdominal pain and distension, headache, dizziness, rash, and complications including abnormal liver function, cholecystitis, spleen enlargement, exhibited statistical significance across four distinct age groups ( P<0.05). The initial presentation of acute fever and rash was more pronounced in the pediatric cohort. In contrast, the youth group exhibited a higher prevalence of acute fever, liver function abnormalities, dizziness, headache, and splenomegaly. Furthermore, the strong-age group and the older age group more frequently experienced lumbar and leg joint pain, and symptoms related to the liver, gallbladder, spleen, and digestive system. In terms of laboratory examination, the rates of decreased white blood cell count (WBC), red blood cell count (RBC) and platelet count (PLT) were 18.91% (73 cases), 28.94% (112 cases) and 22.34% (86 cases), respectively. C-reactive protein (CRP), alanine aminotransferase (ALT), aspartate aminotransferase (AST), total biluric acid (TBA), adenosine deaminase (ADA), alkaline phosphatase (ALP), glutamyl transpeptidase (GGT), procalcitonin (PCT), and D-Dimer (D-Dimer). The proportion of patients with elevated quantitative test results was 88.56% (271 cases), 50.90% (197 cases), 52.20% (202 cases), 23.82% (91 cases), 75.72% (290 cases), 23.06% (89 cases), 40.89% (157 cases), 68.34% (218 cases) and 73.33% (209 cases), respectively. Among them, the quantitative detection results of CPR, ALP and D-Dimer had statistical significance among the 4 age groups ( χ2=16.366,28.089,7.880, P<0.05). Abnormally elevated laboratory parameters, including C-reactive protein (CRP), alkaline phosphatase (ALP), and D-dimer, were more pronounced in middle-aged and older cohorts, with a positive correlation to advancing age. Among the 391 brucella culture positive specimens, the top three departments with the highest number of isolations were infectious disease (52.4%, 204 cases), emergency (9.5%, 37 cases) and orthopedics (8.4%, 33 cases). In conclusion, the clinical manifestations of brucellosis are multifaceted and heterogeneous, frequently involving multiple organ systems, and the laboratory test results exhibit considerable variability. These findings indicate that clinicians should integrate clinical features of brucellosis with laboratory test results, considering the patient′s age group, to enhance diagnostic accuracy and triage efficiency in suspected cases. Improving relevant diagnostic examinations can thereby aid in the effective control and prevention of the disease.

3.Clinical study of Kinesio tape to improve pain and function in patients with knee osteoarthritis

Objective To investigate the efficacy of kinesio taping(KT)in the treatment of pain,function,range of motion and muscle strength in patients with knee articular osteoarthritis(OA).Method Forty patients with knee articular OA diagnosed in our hospital from January 2021 to January 2023 were selected.We randomly divided them into two groups,each with 20 cases.The observation group received KT therapy,while the control group received sham bandage therapy.Visual Analogue Scale/Score(VAS)was used to assess the degree of activity and nocturnal pain,KOOS score was used to assess the functional status of patients,digital goniometer was used to measure the range of activity and hand-held force meter was used to assess muscle strength.The evaluation time includes before treatment and on the 12th day of treatment.Result Compared with before treatment,the pain,function,range of motion,and muscle strength of patients in the control group and KT group were significantly improved after treatment.And compared with the control group,the KT group also significantly improved the above indicators after treatment.Conclusion KT therapy has significant efficacy in relieving short-term pain and improving functional limitations in knee osteoarthritis.
4.Phenotypic and genomic characterization for Salmonella isolates recovered from meat products in Beijing wholesale markets, 2014-2015

Objective:To analyze the phenotypic and genomic characteristics of Salmonella isolates recovered from meat products in Beijing wholesale markets. Methods:A total of 336 Salmonella strains from meat products collected from wholesale markets in Beijing were tested for antimicrobial resistance to 25 antimicrobial compounds by micro-broth dilution method; whole genome data were sequenced, followed by the serotype and ST type prediction by Seqsero2 and SISTR software, and the drug resistance genes and virulence factors were also predicted with CARD and VFDB databases of Abricate software; Salmonella serotyping assay kit and serum agglutination method were used for serotype confirmation of some isolates with different genome prediction results. Results:The resistance rates to Nalidixic acid and Ampicillin were 62.5% (210/336) and 55.1% (185/336), respectively, and all isolates were susceptible to Tigecyclin, Cefoxitin and Carbapenem antimicrobial compounds; 207 isolates (61.6%, 207/336) were multi-drug resistant, some could even be resistant to ten categories of drugs at the same time, and the most common antimicrobial resistance spectrum was NAL-AMP-SAM. A total of 24 serotypes were detected with predominant serotypes of Enteritidis (34.5%, 116/336), Derby (17.3%, 58/336) and Indiana (10.4%, 35/336). A total of 27 ST types were detected, the dominant type was ST11; ST types were in good consistency with serotypes; The detection rates of resistant genes referred to aminoglycosides, fluoroquinolones, β-lactams, sulfonamides and tetracyclines are more than 48%, and the first two reached 100%. The prediction of drug resistance genes was consistent with the results of antimicrobial resistance phenotype. A total of 122 virulence genes were predicted, 74 of which existing among all isolates.Conclusion:Salmonella in meat from the wholesale markets of Beijing has a high proportion of multiple drug resistance, a complex drug resistance spectrum, a variety of serotypes and ST types, and a high carrying rate of drug resistance gene and virulence gene; drug resistance phenotype and genotype are relatively consistent.
5.Gene mutation types and clinical characteristics of children with G6PD deficiency in Zunyi area

Objective:To study the types of pathogenic gene mutations and their main clinical characteristics in children with glucose-6-phosphate dehydrogenase (G6PD) deficiency in Zunyi area.Methods:Children with clinical manifestations of "yellow staining" or "suspected yellow staining" who were admitted to Guizhou Children's Hospital, Affiliated Hospital of Zunyi Medical University, from September 13, 2018 to September 13, 2020 were selected for G6PD gene mutation detection by multicolor probe melting curve analysis, and the pathogenic gene mutation types and clinical characteristics of children with G6PD deficiency were analyzed.Results:The results of G6PD gene mutation detection showed that among the 1 740 children tested, 119 were positive for gene mutation, and the positive detection rate was 6.84%. The proportion of male infants was higher than that of female infants, and the difference was statistically significant (91 males and 28 females, χ 2 = 15.10, P < 0.001); infancy accounted for 63.87% (76/119), and early childhood accounted for 18.49% (22/119). A total of 11 known pathogenic gene mutation types and 1 unknown mutation were detected. Among the top 4 pathogenic gene mutations, the overall was c.1024 C>T, c.1376 G>T, c.1388 G>A and c.95 A>G, male was c.1376 G>T, c.1388 G>A, c.1024 C>T and c.95 A>G; female was c.1024 C>T, c.95 A>G, c.1388 G>A and c.519 C>T. Among the 119 children with G6PD gene mutation, 90 cases had varying degrees of jaundice, including 36 cases of severe and more severe jaundice (including 2 cases of extremely severe neonatal bilirubin encephalopathy), and 54 cases of mild to moderate jaundice; 37 cases had anemia of different degrees, including 6 cases of mild anemia, 12 cases of moderate anemia, and 19 cases of severe or more severe anemia (including 1 case of extremely severe anemia). Conclusions:There are 12 types of gene mutations in children with G6PD deficiency in Zunyi area, and the most common mutation types are c.1024 C>T, c.1376 G>T, c.1388 G>A and c.95 A>G. Children with G6PD deficiency are often accompanied by varying degrees of jaundice and anemia.

7.The follow-up observation on the abnormal elevation of fetal pulmonary artery systolic pressure at middle and late stages by Doppler echocardiography

Objective To investigate the outcome of fetus with abnormal increase of pulmonary artery systolic pressure at second and third trimester by color Doppler ultrasound . Methods Ninety-five fetuses with a little or mild tricuspid regurgitation ( control group) and 60 fetuses with moderate and severe tricuspid regurgitation (observation group) were included . The degree ,velocity ,and differential pressure of tricuspid regurgitation were measured and the variations of baseline information and the measured value of pulmonary systolic pressure between the two groups were compared . As for the follow -up on observation group ,the pressure of fetus with high pulmonary systolic pressure ( > 20 mmHg) was repeatedly measured every 4 weeks until it return to normal . Results There were significant differences in terms of gestational weeks ,velocity and pressure of tricuspid regurgitation ,as well as pulmonary systolic pressure between the two groups ( P < 0 .001) . Pulmonary systolic pressure was positively correlated with gestational weeks , velocity and pressure of tricuspid regurgitation ( r = 0 .442 ,0 .998 ,0 .999 ;all P < 0 .001 ) ,but had no correlations with the age of pregnant women ( r = - 0 .001 , P = 0 .674) . The follow-up revealed that ,in observation group , 47 cases ( 78 .3% , systolic pressure < 50 mmHg ) presented with the decreased pulmonary systolic pressure ,the disappeared or the slight appeared regurgitation before birth ,meanwhile , 13 ( 21 .7% ,systolic pressure ≥ 50 mmHg) exhibited severe tricuspid regurgitation and persistent pulmonary elevation ,with the highest of more than 70 mmHg accompanying the varying degrees of right heart failure . Only one of 13 fetuses died due to persistent pulmonary hypertension and hypoxia ( oxygen saturation <45% ) . The fetal pulmonary artery systolic pressure of the remaining 12 cases recovered from 5 to 105 days after birth ,with normal heart function . Conclusions The majority of fetal pulmonary arterial hypertension complicated with obvious tricuspid regurgitation is reversible functional alteration , which can restore normality in most cases before or after birth .

Objective:
To investigate the clinical effect and safety of long-acting pegylated interferon-α-2b (Peg-IFN-α-2b) (Y shape, 40 kD) injection (180 μg/week) in the treatment of HBeAg-positive chronic hepatitis B (CHB) patients, with standard-dose Peg-IFN-α-2a as positive control.
Methods:
This study was a multicenter, randomized, open-label, and positive-controlled phase III clinical trial. Eligible HBeAg-positive CHB patients were screened out and randomized to Peg-IFN-α-2b (Y shape, 40 kD) trial group and Peg-IFN-α-2a control group at a ratio of 2:1. The course of treatment was 48 weeks and the patients were followed up for 24 weeks after drug withdrawal. Plasma samples were collected at screening, baseline, and 12, 24, 36, 48, 60, and 72 weeks for centralized detection. COBAS® Ampliprep/COBAS® TaqMan® HBV Test was used to measure HBV DNA level by quantitative real-time PCR. Electrochemiluminescence immunoassay with Elecsys kit was used to measure HBV markers (HBsAg, anti-HBs, HBeAg, anti-HBe). Adverse events were recorded in detail. The primary outcome measure was HBeAg seroconversion rate after the 24-week follow-up, and non-inferiority was also tested. 9.Effect of postoperative analgesia nursing on the comfort degree of laparotomy patients in hepatobiliary surgery department

Objective To investigate the effect of postoperative analgesia nursing on the comfort degree of laparotomy patients in hepatobiliary surgery department.Methods A total of 86 laparotomy patients in our department were randomly divided into observation group and control group,receiving postoperative analgesia nursing and routine nursing,respectively.And postoperative analgesia and postoperative comfort were compared.Results The postoperative pain degree in the observation group was less than that in the control group,the incidence of incomplete analgesia was lower than that in the control group,pain call frequency in the operation day,1 day after operation,and 2 day after operation was lower than that in the control group,the difference was statistically significant (P <0.05).Comfort of physiological,psychological,social and cultural aspects in the observation group were higher than that in the control group,nursing satisfaction was higher than the control group,the difference was statistically significant (P < 0.05).Conclusion Postoperative analgesia nursing can significantly reduce degree of pain of laparotomy patients in hepatobiliary surgery department,and improve the postoperative comfort and nursing satisfaction.
10.Application of rapid rehabilitation surgery nursing in the postoperative rehabilitation of patients in the department of hepatobiliary surgery

Objective To investigate the effect of rapid rehabilitation surgery nursing in patients in the department of hepatobiliary surgery.Methods A total of 160 patients in our department were randomly divided into observation group and control group, with 80 cases in each group.The control group was given routine nursing during perioperative period, and the observation group was given rapid rehabilitation surgery nursing, and surgical stress, postoperative rehabilitation and hospital costs of the two groups were compared.Results The levels of cortisol, epinephrine, CRP, HAMA after 1 d of the operation in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05).Postoperative bed-off time, anal exhaust time, defecation time, hospitalization time in the observation group were shorter than that in the control group, complications, hospitalization costs were less than that in the control group, the differences were statistically significant(P<0.05).Conclusion Rapid rehabilitation surgical nursing can reduce the surgical stress of patients in department of hepatobiliary surgery, accelerate the postoperative recovery, reduce the treatment cost, so it is worth promoting.
